Objective To investigate the effect of different anesthesia methods on perioperative body temperature. Methods Eighty patients undergoing esophagus cancer resection were randomized into two groups of combined general and epidural anesthesia(GEA, group A,42 cases) and general anesthesia(GA, group B, 38 cases). Epipharynx and peripheral temperatures were recorded at the same time every 30 minutes for 3 hours after induction. Epipharynx temperature was recorded than after when circulating water mattresses and fluid warming were used to rewarming. The incidence of postoperative shivering was recorded. Results The peripheral temperature was gradually decreased after induction in both groups. The core temperature was decreased quickly within the first 1 hour in both groups, which was reduced slowly within the next two hours with a greater magnitude of temperature reduction in group A than that in group B(P<0.05). The incidence of shivering was higher in group A than that in group B during warming up. Conclusion The patients undergoing long procedures such as esophagus cancer recection under GEA is at more risk for hypothermia than those under GA.

Objective To evaluate the neurotoxic effects of intrathecal (IT) different concentrations of ethanesulfonic acid ropivacaine on spinal cord in rats. Methods Sixty healthy Wistar rats of both sexes weighing 210-220 g in which IT catheters were successfully placed according to Yaksh et al. were randomly divided into 5 groups (n= 12 each). The animals received 0.9% NaCl solution 0.4 ml (group C); 0.224%, 0.447%,0.671%, 0.894% ethanesulfonic acid ropivacaine 0.4 ml (group R1-4 ). The onset time and duration of the block were recorded. The animals were killed on 7th day after IT administration. The L4,5 segment of the spinal cord were removed for neuropathologic examination with electron microscope. The spinal cord injury was scored.Neurotoxicity was defined as the spinal cord injury score ≥ 2 and the spinal neurotoxicity was recorded. Results Onset time was shorter and duration of the block was prolonged with increasing concentrations of ethanesulfonic acid ropivacaine. The incidence of the spinal neurotoxicity was 0, 0, 17%, 42% and 100% in group C, R1, R2, R3 and R4 respectively. The incidence of the spinal neurotoxicity was gradually increased with increasing concentrations of ethanesulfonic acid ropivacaine. Conclusion IT ethanesulfonic acid ropivacaine can produce neurotoxicity to the spinal cord and it depends on the concentration.

OBJECTIVE: To discuss the expression and the significance of Jab1 p27kip1 in laryngeal squamous cell carcinoma and Hep-2 cells. METHOD: Immunohistochemical method was used to examine the expressions of Jab1 and p27kip1 proteins in 50 cases laryngeal squamous cell carcinomas and 10 cases normal laryngeal tissues adjacent to laryngeal squamous cell carcinoma. Hep-2 cells were transfected with synthetic Jab1 siRNA by Lipofectamine 2000. RT-PCR method was adopted to examine the mRNA expression of the Jab1 and p27kip1 gene in Hep-2 cells which was treated with Jab1 siRNA II. RESULT: Clearly brown staining restricted to nucleus was considered as positive expression of Jab1 and p27kip1 protein. The expression rate of Jab1 protein in laryngeal squamous cell carcinoma was significantly higher than that of normal laryngeal mucosa, and the expression rate of protein p27kip1 in laryngeal squamous cell carcinoma was significantly lower than that of normal laryngeal mucosa. There was a negative relationship between Jab1 and p27kip1 protein in laryngeal squamous cell carcinoma. The expression of Jab1 mRNA was suppressed markedly after transfected by Jab1 siRNA II. As the reaction time increased, the expression of Jab1 mRNA of Hep-2 cells decreased significantly, and the expression of p27kip1 mRNA remained unchanged. CONCLUSION The expression rate of Jab1 protein in laryngeal squamous cell carcinoma is significantly higher than that of normal laryngeal mucosa. There is a negative relationship between Jab1 and p27kiPl protein in laryngeal squamous cell carcinoma. After transfected by Jab1 siRNA II in the Hep-2 cells, the expression of Jab1 mRNA is suppressed markedly. Jab1 siRNA would be a good methodology for the further study.
