1.Application of retrograde intra-enteropexy through appendix stump for adhesive ileus

Objective To investigate operative method for preventing and treatment intestinal obstruction due to relapse adhesion.Methods Sixty patients with recurrent adhesive ileus treated by surgery were randomized into two groups,that was intra-enteropexy group and extra-enteropaxy group.Twenty-three patients in intra-enteropexy group were treated by retrograde intra-enteropaxy through appendix stump.Thirty-seven patients in the extra-enteropexy group were treated by Noble procedure.The effect and complication were compared.Results In the intra-enteropexy group,2 cases (8.70%) were complicated with incision infection,1 case was died (4.35%) and there was no relapsed,abdominal spastic pain and incomplete intestinal obstruction occurred.In the extra-enteropexy group,3 cases (8.11%) were complicated with incision infection,3 cases were died (8.11%),abdominal spastic pain were occurred in 11 cases (29.73%),incomplete intestinal obstruction were occurred in 6 cases(16.22%),and there were 3 cases run into relapse.Conclusion The retrograde intra-enteropexy through appendix stump is an effective method for preventing and treatment to adhesive ileus.
2.Effect of Intensive Insulin Therapy on Inflammatory Level of Biliary Pyemia

Objective To explore the possible anti-inflammatory mechanism of intensive insulin therapy (IIT) by studying the effect of IIT on the levels of TNF-?,IL-6,C-reactive protein (CRP) and APACHE Ⅱ score in biliary pyemia.Methods Twenty eight patients with biliary pyemia who were admitted by our department and given an operation within 24 h form Jan.2005 to Dec.2008 were randomly divided into two groups by using random number table numbers:one group treated with IIT (IIT group,n=14) and another group treated with routine insulin therapy (RIT group,n=14).The inflammatory factors,such as TNF-?,IL-6 and CRP were detected dynamically and the APACHEⅡ score was calculated.Results The level of CRP and APACHEⅡ score on day 5 and 7 and the levels of TNF-? and IL-6 on day 3,5 and 7 after operation in IIT group were significantly lower than those in RIT group (P

Objective To investigate the impact of hepatectomy combined with splenectomy on hepatitis B virus (HBV) reactivation in patients with hepatocellular carcinoma (HCC) ≤5 cm and with hypersplenism.Methods This is a retrospective case-control study on 167 patients with HCC ≤5 cm and with hypersplenism who underwent hepatectomy combined with splenectomy at the Shengli Oilfield Central Hospital between May 2008 and June 2015.64 patients underwent hepatectomy combined with splenectomy,and 103 patients hepatectomy alone.The patients were assigned to the hepatectomy combined with splenectomy group (the combined group,n =61) or the hepatectomy alone group (the control group,n =61) using propensity score matching (PSM).Logistic regression was used to evaluate the relative clinical factors associated with HBV reactivation.The stratified Chi-squared test was utilized to determine the impact of the surgical procedure and preoperative anti-viral therapy on postoperative hepatitis B virus reactivation of these patients.Results The serum PLT level,Child-Pugh grading,tumor diameter and surgical procedures were shown to be independent risk factors associated with postoperative HBV reactivation (P < 0.05).To study the impact of preoperative anti-viral therapy on postoperative HBV reactivation:-the incidence of HBV reactivation was higher in the control group than in the combined group (19.7% vs.6.6%,P < 0.05).In the combined group,there was no significant difference between patients who received anti-viral therapy and those who were treatment-naive (5.3% vs.7.1%,P >0.05).In the control group,a higher incidence of HBV reactivation was found in patients with treatment-na(i)ve than in patients who received anti-viral therapy (26.1 % vs.0,P < 0.05).For the patients who received anti-viral therapy,there was no significant difference between the combined group and the control group (5.3% vs.0,P > 0.05).In patients with treatment-na(i)ve,a higher incidence of HBV reactivation was observed in the control group than the combined group (26.1% vs.7.1%,P < 0.05).Conclusions In patients who were not treated with antiviral therapy,hepatectomy combined with splenectomy decreased the incidence of postoperative HBV reactivation in patient with HCC ≤5 cm and with hypersplenism.For the patients who received preoperative anti-viral therapy,the incidence of postoperative HBV reactivation was not decreased with hepatectomy combined with splenectomy.

Objective To design a bank and hospital one card system to optimize outpatient service process and improve service level of outpatient .Methods On the basis of the existing outpatient service process ,we combine transfer function and payment function of bank card ,thus a bank and hospital one card system was designed to utilize the advantage of City Payment Card .Re‐sults The waiting time of outpatients was shortened through the self service of bank and hospital one card system ,the pressure of outpatient service windows was reduced .Conclusion Bank and hospital one card system can optimize outpatient service process , which can improve hospital efficiency and patient satisfaction .
5.Expression and their significance of Jab1 p27kip1 in laryngeal squamous cell carcinoma and Hep-2 cells.

OBJECTIVE:
To discuss the expression and the significance of Jab1 p27kip1 in laryngeal squamous cell carcinoma and Hep-2 cells.
METHOD:
Immunohistochemical method was used to examine the expressions of Jab1 and p27kip1 proteins in 50 cases laryngeal squamous cell carcinomas and 10 cases normal laryngeal tissues adjacent to laryngeal squamous cell carcinoma. Hep-2 cells were transfected with synthetic Jab1 siRNA by Lipofectamine 2000. RT-PCR method was adopted to examine the mRNA expression of the Jab1 and p27kip1 gene in Hep-2 cells which was treated with Jab1 siRNA II.
RESULT:
Clearly brown staining restricted to nucleus was considered as positive expression of Jab1 and p27kip1 protein. The expression rate of Jab1 protein in laryngeal squamous cell carcinoma was significantly higher than that of normal laryngeal mucosa, and the expression rate of protein p27kip1 in laryngeal squamous cell carcinoma was significantly lower than that of normal laryngeal mucosa. There was a negative relationship between Jab1 and p27kip1 protein in laryngeal squamous cell carcinoma. The expression of Jab1 mRNA was suppressed markedly after transfected by Jab1 siRNA II. As the reaction time increased, the expression of Jab1 mRNA of Hep-2 cells decreased significantly, and the expression of p27kip1 mRNA remained unchanged.
CONCLUSION
The expression rate of Jab1 protein in laryngeal squamous cell carcinoma is significantly higher than that of normal laryngeal mucosa. There is a negative relationship between Jab1 and p27kiPl protein in laryngeal squamous cell carcinoma. After transfected by Jab1 siRNA II in the Hep-2 cells, the expression of Jab1 mRNA is suppressed markedly. Jab1 siRNA would be a good methodology for the further study.
