1.Prevalence and correlation factors of cardiovascular damage in patients with diabetic nephropathy and non-diabetic nephropathy

Objective To investigate the prevalence and correlation factors of cardiovascular damage in patients with diabetic nephropathy (DN) and non-diabetic nephropathy (NDN).Methods A total of 278 chronic kidney disease (CKD) patients admitted to the First Affiliated Hospital of Jinan University from January 2014 to May 2016 were enrolled,including 78 case of DN and 200 case of NDN.Patients had cardiac and carotid ultrasonography test by colour doppler ultrasonography,and their clinical and biochemical data were collected.Multiple linear regression analysis and multivariable logistic regression analysis were applied to study the correlation factors of cardiovascular damage in CKD patients.Results Mean age was 48.22 years in the 278-patient cohort,which included 178(64.03%) men.Compared with NDN group,DN patients had higher left atrial dimension,interventricular septal thickness,left ventricular end-diastolic dimension,left ventricular posterior wall thickness,left ventricular mass index (LVMI),carotid intima-media thickness (cIMT) and carotid plaques ratio.Their estimated glomerular filtration rate (eGFR) and the ratio between the peak speed of the early filling wave and that of the atrial contraction wave (E/A ratio) were however lower (all P < 0.05).Prevalence of left ventricular hypertrophy (LVH),left ventricular relaxant function reduction and cIMT thickening in DN group were 67.95%,70.27% and 57.14%,higher than those in NDN group (40.00%,42.31% and 17.39%,respectively) (all P< 0.05).Along with the progress of CKD,LVMI and LVH proportion in patients with DN and NDN increased gradually.LVMI and LVH proportion in DN patients in CKD 1-2 phase and CKD 3-4 phase were higher than those in NDN patients (all P < 0.05).In all CKD phases,cIMT and cIMT thickening proportion in DN group were higher than those in NDN group (all P < 0.05).Just in CKD 1-2 phase,DN group had lower E/A ratio and higher proportion of left ventricular relaxant function reduction than NDN group (all P < 0.05).After multiple linear regression analysis,gender,BMI,hemoglobin,eGFR and DN were related with LVMI;age,serum calcium and DN were related with E/A ratio;age and DN were related with cIMT (all P < 0.05).In multivariate logistic regression,DN,hemoglobin and eGFR decrease were independently associated with LVH;age and BMI were independently associated with reduction of left ventricular relaxant function;age and DN were independently associated with cIMT thickening in all CKD patients (all P < 0.05).Conclusions DN patients have more severe cardiovascular damage than NDN patients,and DN may be associated with LVMI,E/A ratio,cIMT,LVH and cIMT thickening in all CKD patients.
2.Application of assessment system based on key points in clinical skills training among nursing interns

Objective To explore the effects of assessment system based on key points in clinical skills training among nursing interns.Methods Nursing interns of the Affiliated Cancer Hospital of Zhengzhou University were selected as study subjects from January to December 2016 by the method of convenience sampling. They were divided into experimental group (n=50, practice started from January 2016) and control group (n=50, practice started from June 2016) according to the order of internship. Students in control group adopted the conventional skill training and assessment, while students in experimental group received the assessment system based on key points. Critical thinking skills of nursing interns of both groups were compared with the critical thinking disposition inventory (CTDI).Results After training, the total score of CTDI of students (299.82±26.95) in experimental group was higher than that (283.41±24.24) in control group with a significant difference (t=3.201,P=0.002). The scores of 7 dimensions in CTDI were (41.18±4.12), (40.27±3.16), (46.10±2.00), (40.11±5.31), (42.97±5.05), (43.96±3.88), (45.23±3.08) significantly higher than those in control group[(39.62±3.09), (38.62±4.15), (42.04±2.95), (37.92±3.65), (39.50±3.57), (41.82±5.17), (43.51±2.64)] (t=2.142, 2.237, 8.074, 2.402, 3.967, 2.341, 2.981;P<0.05).Conclusions The assessment system based on key points can effectively improve the critical thinking skills of nursing interns. It is worth to be used widely.
3.Association of frailty with anxiety and depression in patients on maintenance hemodialysis

Objective To investigate the current status of frailty in patients on maintenance hemodialysis(MHD),and explore the correlation of frailty with anxiety and depression.Methods General information,clinical data and blood biochemical data of 101 cases who underwent MHD in Department of Nephrology,Shunde Hospital Affiliated to Jinan University from January 2023 to January 2024 were collected.FRAIL scale was applied to evaluate the frailty of the patients,and they were accordingly classified into frailty group and non-frailty group(including pre-frail and non-frail participants).Anxiety and depression were evaluated by GAD-7 and PHQ-9 scale.Univariate analysis and logistic regression were used to explore the association of frailty with anxiety,depression and other possible influencing factors.Results Among the 101 cases,29 cases(28.71%)were includedin frailty group and 72 cases(71.29%)in non-frailty group.There were 42 patients with depression(41.58%)and 25 with anxiety(24.75%).In the frailty group,the prevalence of depression was 65.52%and that of anxiety 55.17%.There were significant differences in age,grip strength,exercise,stroke and coronary heart disease,anxiety and depression,ferritin and CRP between the two groups(P<0.01).Multivariate regression analysis showed that depression,anxi-ety,no exercise,stroke and high ferritin concentration were independent risk factors for frailty in MHD patients(P<0.05).Conclusion In patients on MHD,frailty is closely associated with depression,anxiety,and lack of exercise,and stroke as well as high ferritin concentration are independent risk factors for frailty.
