1.Effects of extracellular signal-regulated kinase on cell cycle and functions of rat cultured hepatic stellate cells stimulated by acetaldehyde

Objective To investigate the effects of PD98059, the specific blocking agent of mitogen extracellular signal responsive kinase, on the cell cycle, cell proliferation, type Ⅰ collagen's secretion and transforming growth factor(TGF)? 1mRNA expression of rat hepatic stellate cells (HSC) stimulated by acetaldehyde. Methods Rat HSC stimulated by acetaldehyde were incubated with different concentration of PD98059. Cell cycle was analysed by flow cytometry. Cell proliferation was assessed by MTT colorimetric assay. Type Ⅰcollagen of culture medium was detected by enzyme linked immunoadsordent assay (ELISA). TGF? 1mRNA expression were examined by RT PCR. Results 20,50,100 ?mol/L PD98059 could significantly inhibit proliferation and induced G0/G1 phase arrest of HSC stimulated by acetaldehyde in a does dependent manner. 50,100 ?mol/L PD98059 could markedly inhibit type Ⅰcollagen's secretion and TGF? 1mRNA expression of HSC stimulated by acetaldehyde. Conclusion The extracellular signal regulated kinase signal transduction passway regulates the cell proliferation, type Ⅰcollagen's secretion and TGF? 1mRNA expression of rat HSC stimulated by acetaldehyde,which might be partly related to its regulative effect on the cell cycle.
2.Clinical efficacy of patient-controlled intravenous analgesia for intractable cancer pain

Objective:To compare the clinical efficacy of oxycodone hydrochloride controlled-release tablets (OHCT) and pa-tient-controlled intravenous analgesia (PCIA) in the treatment of intractable cancer pain. Methods:Retrospective analysis was conduct-ed to evaluate the intractable cancer pain of 89 elderly patients who were admitted to the medical oncology departments of The Second Affiliated Hospital of Fujian Medical University and the Fuzhou General Hospital of Nanjing Military Command between September 2012 and March 2014. Among the 89 patients, 47 were treated with OHCT, and 42 received PCIA. The total dosage ranged from 60 mg/d to 400 mg/d PO q12h for patients in the OHCT group, whereas abackground dose+patient-controlled dosemode was adopted for patients in the PCIA group. The therapeutic efficacy, presence of adverse reactions, cost of treatment, and degree of patient satisfaction were compared between the two groups. Results:The average dosages of analgesics in the two groups were almost the same (P>0.05). Visual analogue scale (VAS) values and daily average VAS values were both lower in the PCIA group than in the OHCT group at 24 h after analgesia (P<0.05). The incidence of adverse reactions, such as nausea and vomiting, was also lower in the PCIA group than in the OHCT group (P<0.05). The cost of treatment and degree of patient satisfaction were the same in both groups (P>0.05). Conclusion:Pa-tients who received PCIA attained better analgesia and exhibited less adverse reactions than those who received OHCT whereas the treatment cost and patient satisfaction did not differ in both groups.
3.Clinical analysis of 12 cases of invasive Klebsiella pneumoniae liver abscess syndrome

Objective:To analyze the clinical features of patients with invasive Klebsiella pneumoniae liver abscess syndrome (IKLAS). Methods:The clinical data of 12 patients diagnosed as IKLAS in Zhuzhou Central Hospital from January 2020 to January 2023 were retrospectively analyzed.Results:Among 12 patients there were 6 males and 6 females with an mean age of 65.3±12.2 years (49-90). Nine patients were complicated with type 2 diabetes. The main clinical manifestations were fever ( n=9), chill ( n=6), shiver ( n=4), nausea and vomiting ( n=2), upper abdominal pain ( n=2), fatigue and anepithymia ( n=2), cough and expectoration ( n=1), disturbance of consciousness ( n=1) and hemoptysis ( n=1). The leukocyte count was increased in 8 cases, lymphocyte count decreased in 10 cases, and platelets count decreased in 3 cases. C-reactive protein and procalcitonin levels were elevated, while serum albumin levels were lowered in all patients. The alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were increased in 7 cases each. Liver abscess was located in the right lobe in 8 cases, in the left lobe in 1 cases, and in both lobes in 3 cases. There were 7 patients with single abscess, and 5 patients with multiple abscesses. The etiology was confirmed by liver pus culture ( n=10) and blood culture ( n=5), respectively. The main sites of invasion were lung and blood stream ( n=10 and n=5, respectively). The majority of Klebsiella pneumoniae isolates were antibiotic sensitive strains and the overall drug resistance rate was relatively low. All patients were given antibiotics, and 10 of them also received liver abscess puncture drainage. After treatment, 11 patients were discharged, and 1 died of septic shock. Conclusions:Patients with IKLAS exhibit diverse clinical symptoms, most patients are complicated with diabetes, and the main sites of invasion are in the lungs and blood stream. Timely diagnosis, active screening of extrahepatic infection sites, effective drainage of abscess and appropriate antibiotic treatment can improve the survival of patients.
