1.Protective effects of curcumin on LPS-induced acute renal injury in mice

Objective To study the protective effects of curcumin on LPS-induced acute renal injury to offer the clinical evidence of treatment in mice. Methods Fifty-two mice were randomly divided into 3 groups, the control group (n=12), the LPC group (n=20) and the treated group (LPS + curcumin group, n=20). LPS (5 mg/kg) was injected intraperitoneally into the mice of the LPS group. Curcumin (20 mg/kg) was injected intraperitoneally after an intraperitoneal injection of LPS (5 mg/kg) to the mice of the treated group. The TNF-? level of the renal tissue, the serum level of urea nitrogen (BUN) and serum creatinine (Cr) were determined in 3, 6, 12 and 24 h after injection. The expression of TLR2 mRNA was detected with semi-quantitative RT-PCR and the histopathological changes of the kidneys were studied. Results The TNF-? level of renal tissue and the serum levels of BUN and Cr were significantly increased in 3 h after injection in the LPS group and reached the peak at 6 h. The significant increase of the parameters lasted for 24 h. The changes of the expression of TLR2 mRNA were similar to those of TNF-?, BUN and Cr. The histopathological changes of the kidneys included renal tubular necrosis and detachment, formation of casts, glomerular ischemia and shrinkage and infiltration of large amount of PMN in LPS group. In the theated group, the tissue level of TNF-?, the serum levels of BUN and Cr and the expression of TLR2 mRNA were significantly decreased in 3, 6 and 12 h after injection and the histopathological changes were obviously alleviated. Conclusion Curcumin can alleviate LPS-induced acute renal injury and exerts good protection on the functions of the kidneys.
2.Study on CsA Therapeutic Window Concentration in Renal Transplant Recipients

OBJECTIVE:To establish an optimal therapeutic window concentration of CsA trough levels in renal transplant recipients on triple immunosuppressants regimen METHODS:A total 1 874 samples from 268 renal transplant recipients were measured by fluorescence polarization immunoassay(FPIA) According to the duration after operation and clinical diagnoses ,the whole blood CsA trough levels were compared among subgroups RESULTS:The optimal therapeutic window concentration of CsA was 300~400?g/L(within 1 month after operation),250~350?g/L(2nd~3rd month),150~250?g/L(4th~6th month),100~200?g/L(7th~12th month)and 100~150?g/L(more than 12 months) CONCLUSION:The above mentioned therapeutic window concentratin of CsA trough levels was ideal for renal transplant recipients with no marked acute toxic effects and rejection reaction
3.Retroperitoneoscopic renal pedicle lymphatic disconnection for chyluria.

OBJECTIVETo report our experiences in retroperitoneoscopic renal pedicle lymphatic stripping for chyluria.
METHODSSix cases of filarial chyluria were admitted to our hospital from November 2001 to June 2002. Of these cases, 4 were men and 2 women, with age ranging from 34 to 52 years (mean, 42 years). Diagnosis was made by using urine test for the presence of chyle and fat globule, cystoscopy, excretory urogram and retrograde pyelography. Chyluria was found on the left renal unit in 2 cases and on the right side in 4 cases. The technique of retroperitoneoscopic management of chyluria consisted of nephrolympholysis, hilar vessel stripping and ureterolympholysis.
RESULTSOperative time ranged from 69 to 120 minutes (mean, 95 minutes). Intraoperative blood loss was 50-180 ml (mean, 85 ml). Chyluria disappeared in all patients immediately after operation. Mild hematuria occurred in 4 cases within 12 hours and disappeared at 24 hours. Subcutaneous emphysema around the lesions was found in 2 cases and was spontaneously absorbed 3 days after the treatment. There was no lymphatic leak at the lesions. The patients were discharged from the hospital 5-9 days after the treatment. All patients gained weight and their haemoglobin and serum protein increased by 13.5 g/L and 3.66 g/L respectively. No chyluria recurrence was reported during 1-1.6 years follow-up.
CONCLUSIONSRetroperitoneoscopic renal pedicle lymphatic disconnection for chyluria is a safe, effective and efficient surgical procedure with minimal invasion, less pain, lower morbidity, short hospital stay and rapid recovery.

5.C-TIRADS combined with contrast-enhanced ultrasound for evaluation of category 4 nodules in Hashimoto's thyroiditis

Objective Evaluation of Chinese-thyroid imaging reporting and data system(C-TIRADS)combined with contrast-enhanced ultrasound(CEUS)for the assessment of category 4 nodules in the setting of Hashimoto's thyroiditis.Methods Retrospective analysis of 120 C-TIRADS category 4 thyroid nodules from 79 patients with confirmed Hashimoto's thyroiditis who attended the Yiyang Central Hospital from June to December 2022.Thyroid nodules exhibiting one or more benign or malignant features that were suspicious on CEUS were treated as downgraded or upgraded one level.Using the final surgical pathology results as the gold standard,working characteristic(ROC)curves of subjects based on C-TIRADS grading before and after CEUS adjustment were plotted to compare diagnostic efficacy.Results The sensitivity,specificity,and accuracy of the CEUS-adjusted C-TIRADS were 93.0%,87.8%and 90.8%,respectively(P<0.05).The area under the ROC curve was 0.811 and 0.904,respectively(P<0.05).Conclusion C-TIRADS combined with CEUS has better diagnostic efficacy in evaluating category 4 nodules in Hashimoto's thyroiditis.
