1.Clinical value of TNF-α in serum of patients with colorectal cancer

Objective To investigate the application value of serum tumor necrosis factor alpha (TNF-α) expression in diagnosing colorectal cancer (CRC).Methods One hundred and thirty inpatients with CRC in our hospital from February 2015 to November 2016 were selected and divided into 90 cases of CRC confirmed by operation and histopathology(CRC group),including intestinal cancer with and without metastasis,and 40 cases of benign colorectal diseases(colonrectal benign disease group),at the same time 40 persons undergoing physical examination were selected as the healthy control group.The levels of TNF-α,carcinoembryonic antigen (CEA) and carbohydrate antigen (CA) 724 were collected in all subjects.The data of various groups were not normally distributed,and the nonparametric statistical method was used to analyze and compare.Results Serum TNF-α level in clinical TNM stage Ⅱ(median 9.47 pg/mL),Ⅲ(9.95 pg/mL) and Ⅳ(9.67 pg/mL) of the CRC group was significantly higher than that in TNM stage Ⅰ(6.59 pg/mL);which in the CRC with metastasis group (9.95 pg/mL) was higher than 8.03 pg/mL in CRC without metastasis group,the difference was statistically significant (P<0.05);the TNF-α expression had no obvious difference among different ages and between different genders(P>0.05).The area under the ROC curve in each test index:TNF-α was 0.868,CEA was 0.912,CA724 was 0.523,and which of their joint detection was 0.940.Conclusion The expression of TNF-α is increased in the occurrence and development process of CRC,its serum level detection has certain clinical application value,the TNF-α expression has no obvious difference in the subjects with different ages and different sexes and has wide suitability.The combine detection of serum TNF-α,CEA and CA724 in the patients with CRC significantly increases the level,the combined detection of 3-index can have complementary advantages to increase the sensitivity of CRC diagnosis,which serve as the significant indexes in CRC early diagnosis,therapeutic effect evaluation and prognosis monitoring.
2.Value of combined detection of serum DCD, CA199, CA724, and CEA in diagnosis of colorectal cancer

Objective To investigate the possibility of early diagnosis of colorectal cancer by combined detec-tion of tumor markers dermcidin(DCD),CA199,CA724 and CEA. Methods We selected 81 colorectal cancer patients and 30 healthy individuals. Serum levels of DCD were detected by ELISA,and serum levels of CA199, CA724 and CEA were detected by electrochemical luminescent immunoassay,and the expression of DCD in colorectal cancer tissue and para-carcinoma tissue was measured by Western blot. Results Serum levels of CA199,CA724,and CEA were significantly higher in colorectal cancer patients than in healthy individuals(P<0.001). Combined detection of DCD,CA199,CA724 and CEA had a greater AUC(0.942)for identifying colorec-tal cancer from the controls. The expression of DCD was significantly higher in colorectal tissue than in para-carci-noma tissue(P<0.05). Conclusions Level of DCD expression was significantly higher in colorectal tissue than in para-carcinoma tissue. Expressions of serum DCD,CA199,CA724,and CEA may be a marker in diagnosis of can-cer,with higher clinical value for the adjuvant diagnosis of colorectal cancer.
3.The leukocyte classification comparison and clinical features analysis between patients with influenza A and B

Objective To investigate the clinical features and value of white blood cell(WBC) count during influenza diagnosis.Methods Compare with leukocyte count and its classification,clinical features between 38 cases of influenza A and 55 cases of influenza B patients.Results The follow results have were significant difference between these two group(P<0.05):The WBC content (WBC),neutrophil count (NEUT),lymphocyte(LYMPH) count and platelet (PLT).In both group,WBC were mostly in normal range.However,WBC and NEUT% in the group A were 15.79% and 34.21%,higher than the B group 7.27%,25.45%.Additionaly,WBC and NEUT% in upper respiratory tract infection group were 59.26% and 66.67%,higher than the normal ranger,and group influenza A flow patients with higher proportion for headache and crackles,were 47.36% and 52.63%,respectively(P<0.05).Conclusion The WBC count and its classification have significant difference between influenza A and B.Furthermore,patients with influenza A virus are more likely to have co-infection with bacteria.
4.Clinical value of combined detection of C-reactive protein and white blood cell in children with anemopyretic tonsillitis

Objective To explore the clinical significance of combined detection of C‐reactive protein (CRP)and white blood cells (WBC)in children with anemopyretic tonsillitis .Methods A total of 284 patients with anemopyretic tonsillitis were divided into ex‐cessive pattern group (131 cases) ,wind‐heat invasion group (153 cases) ,other 120 healthy children were recruited into control group .The WBC ,percentage of neutrophils (NEU % ) ,percentage of lymphocytes (LYM % ) and CRP were analyzed among the three groups .Results WBC and NEU% in excessive pattern group and wind‐heat invasion groups were higher than those in the control group (P< 0 .05) ,CRP in wind‐heat invasion group was higher than that in excessive pattern group and the control group (P< 0 .05) .CRP in the excessive pattern group were higher than that in the control group (P< 0 .05) .When CRP cut‐off value in wind‐heat invasion group was 12 .65 mg/L ,the sensitivity was 90 .6% ,specificity was 86 .3% .Conclusion The combined detection of WBC and CRP have clinical value in differential diagnosis of children with anemopyretic tonsillitis .
5.Analysis on characteristics and drug resistance of pathogens in ICU elderly patients with respiratory tract infection

Objective To investigate the distribution characteristic and drug resistance of respiratory tract pathogens in ICU eld‐erly patients to provide the basis for clinical medication and control of nosocomial infection .Methods The isolation situation and drug resistance of pathogens in ICU elderly patients with respiratory tract infection from January 2012 to December 2014 were ret‐rospectively analyzed .Results Among 501 cases of respiratory tract infection ,350 cases were Gram‐negative bacilli infection ,which were mainly P .aeruginosa and A .baumannii;50 cases were Gram‐positive coccus infection ,which was mainly S .aureus ;101 cases were complicating fungal infection ,which was mainly C .albicans .The resistance of P .aeruginosa to imipenem showed upward trend (P<0 .05) ,A .baumannii had higher resistance to commonly used antimicrobial drugs ,but the drug resistance trend had no obvious change(P>0 .05) .Imipenem‐resistant A .bauman(IRAB) ,ESBLs‐producing E .coli and methicillin‐resistant S .aureus (MRSA) in the elderly patients with respiratory tract infection all exceeded 50% of each constitution ratio .Conclusion Multi‐drug resistant bacteria are usually isolated from ICU elderly patients ,their drug resistance rates maintain a higher level .Therefore clinicians should rationally select antibacterial drugs by combining with the laboratory reports ,increase the prevention and management of multi‐drug resistant bacteria and reduce the nosocomial infection occurrence .
6.Effect of Numb gene on tubular epithelial-to-mesenchymal transition in rat proximal epithelial cells

Objective To explore the effect of Numb on tubular epithelial-to-mesenchymal transition (EMT) in rat proximal epithelial cells. Methods NRK52E cells were treated with different concentrations of recombinant human transforming growth factor-β1 (TGF-β1) (0, 1, 5, 10, 15, 20 μg/L) for 48 h or 10 μg/L TGF-β1 for different times (0, 24, 48, 72 h) in vitro. The expressions of E-cadherin, a-smooth muscle actin(α-SMA) and Numb in NRK 52E cells were detected by RT-PCR, Western blot and immunofluorescence staining. Meanwhile Numb siRNA oligo was transfected into NRK 52E cells with lipofectamine before TGF-β1 treatment, then Western blot was applied to detect the protein expression of E-cadherin, α-SMA and Numb in NRK52E cells. Results TGF-β1 could induce EMT in NRK52E cells in dose- and time-dependent manner. During the progress of TGF-β1-induced EMT, the protein expression of Numb in 5, 10, 15, 20 μg/L group was 1.33 folds (P=0.024), 1.39 folds (P=0.035), 1.45 folds (P=0.025), 1.51 folds (P=0.000) respectively as compared to 0 μg/L group. Likewise, the protein and mRNA expression of Numb in 24 h, 48 h, 72 h group was 1.48 folds (P=0.046) and 1.56 folds (P=0.012), 1.54 folds (P=0.011) and 1.82 folds (P=0.008), 1.79 folds (P=0.028) and 1.82 folds (P=0.002) respectively as compared to 0 h group. Moreover, large amount of Numb was accumulated in the cytoplasm. Down-regulation of Numb expression by siRNA transfection did not influence the basal expression of E-cadherin and α-SMA in NRK 52E cells, but attenuated the progression of EMT in NRK52E cells induced by TGF-β1. The up-regulation of α-SMA protein was reduced to 18.1% (P=0.004) while the down-regulation of E-cadherin protein was reversed to 2.19 folds (P=0.004). Conclusion Numb can promote EMT in rat proximal epithelial cells.
7.Notch activity is increased in a rat peritoneal fibrosis model induced by high glucose dialysate

Objective To investigate the role of Notch signaling in the progression of peritoneal fibrosis in a rat model induced by high glucose dialysate. Methods Male Sprague Dawley rats were subjected to daily peritoneal dialysis (PD) with a lactate-buffered solution containing 4.25% glucose. They were sacrificed at 2 and 4 weeks after PD. The parietal thickness was measured with Masson staining. The expression of TGF-β1, E-cadherin, α-SMA and collagen Ⅰ was examined by immunoblotting. The expression of Notch ligand Jagged-1 and the negative Notch signaling regulato--Numb was analyzed by both immunoblotting and RT-PCR. The expression of a Notch nuclear target gene Hcs-1 was examined by RT-PCR. Results Both HE and Masson trichrome staining revealed an increase in peritoneal thickness with a loss of mesothelial cells and a rich of collagen matrix deposition in the submesothelial zone was evident at 4 weeks after PD. Meanwhile, compared to healthy rats, the expression of TGF-β1, ct-SMA and collagen Ⅰ was significantly increased, but the expression of E-cadherin was decreased in peritoneum after PD treatment. It was difficult to detect the Jagged-1 and Hes-1 expression in normal peritoneum, but their expression was graduaUy increased after PD. In contrast, the expression level of Numb, a negative regulator of Notch signaling, was dramatically decreased after PD. Conclusions Notch signaling is activated during the process of PD-induced peritoneal fibrosis and the activation of Notch signaling is associated with the loss of negative regulation of Notch signaling via decreased expression of Numb. Inhibition of Notch signaling via overexpression of its negative regulators such as Numb may be a novel therapeutic approach for peritoneal fibrosis in PD patients.
8.Efficacy and safety evaluation for 28 Chinese non-Hodgkin lymphoma patients treated with Hyper-CVAD/MA regimen

Objective To evaluate the efficacy and safety of the hyper-CVAD/MA regimen as an intensified treatment option for 28 T cell and aggressive/highly aggressive B cell NHL in Chinese patients. Methods Clinical data of 28 NHL patients treated with hyper-CVAD/MA regimen from Jan 2005 to Sep 2008 were retrospectively analyzed. Results 27 NHL patients were available for the efficacy analysis, with a response rate of 70.4 %. For the 13 B cell lymphoma cases, the response rate was 84.6 %. The main toxicity was Grade Ⅲ or Grade Ⅳ myelosuppression in all cases and 2 treatment related deaths. Conclusion Hyper-CVAD/MA regimen had a high response rate in T cell and aggressive /highly aggressive B cell NHL lymphoma, companied by significant toxicity when treating Chinese patients. Further clinical practices are needed to pick up a suitable dose which can balance efficacy and safety.

Contracture and spasticity of ankle joints were major sources of disability in neurological impairment including stroke and cerebral palsy, etc. The manual stretching used in physical therapy might be laborious and time-consuming to the therapists and the outcome was dependent on the experience and the subjectiveend feelingof the therapists. A device was developed that could safely stretch the an-kle joint to its extreme positions with quantitative control of the resistance torque and stretching velocity. Furthermore, it could satisfy a strong need for quantitative and objective measures of the impairment and rehabilitation outcome. This was just the meaning intelligent stretching referred to. This article described the origin of the concept of intelligent stretching and its definition, operational principle, and su-periority and weakness, as well as its application in ankle joint spasticity and contracture in patients with stroke and cerebral palsy.
