AlM:To observe the clinical efficacy of two types of scleral-fixated posterior chamber intraocular lens ( lOL ) combined intra-capsule lens extraction to treat cataract dislocation.
METHODS: After intra - capsule lens extraction of dislocated cataract, two types of lOL were used as scleral-fixated posterior chamber lOL . First group was with four seal loops, the second group was with two open loops. All 21 patients (23 eyes) took the examination of the best corrected visual acuity, intraocular pressure, fundus, and lOL decentration using camera system attached to slit - lamp ophthalmoscope and lOL tilted using ultrasound biomicroscopy (UBM) at 6mo after surgery.RESULTS: After 6mo, all patients had improved eye sight. There was significant difference in the mean lOL decentration between the two groups ( 0. 57mm vs 0. 79mm, P<0. 05). There was significant difference in the mean lOL tilted degree between the two groups (6° vs 11°, P<0. 05) .
CONCLUSlON: lOL with four loops are more appropriate as scleral-fixated posterior chamber lOL with less tiltness and dicentration.

?AlM: To observe the situations of different surgical methods on dry eyes in patients with pterygium excision combined transplantation.
?METHODS: Seventy-eight cases ofpterygium patients (81 eyes ) were randomly divided into three groups. Group A underwent pterygium excision combined large autologous conjunctival flap transplantation; group B underwent pterygium excision combined with small conjunctival flap;group C underwent pterygium excision combined with small conjunctival flap with autologous limbal stem cell. Repair of postoperative corneal epithelium, 1, 3mo preoperation and postoperation tear film break up time ( BUT ) and questionnaire of ocular surface disease index ( OSDl ) were observed among three groups, which caused the situation of dry eyes by pterygium and pterygium excision were evaluated.
?RESULTS: BUT: Group A was shorter than that in groups B and C at 15d postoperation (P<0. 05); at 1mo postoperation, group A was no statistical difference with groups B and C (P>0. 05). Postoperative dry eye ratio of group A was higher than that in groups B and C at 1mo postoperation ( P < 0. 05 ). There was no statistical difference between group B and group C ( P>0. 05 ) , but group C showed lower postoperative dry eye ratio. Corneal epithelium recover time of group A was longer than that in groups B and C (P<0. 05). There was no statistical difference between group B and group C ( P>0. 05), but group C showed a tendency to be shorter recover time.
? CONCLUSlON: Pterygium excision combined with small conjunctival flap and autologous limbal stem cell shows guickly corneal epithelium recover and low dry eye ratio and deserve to recommended.

Objective　To evaluate the effectiveness of intratracheal tube suctioning by respiratory mechanics measurement.Methods　The indexes of respiratory mechanics,including respiratory rate,expiratory tidal volume,% leak around the tube,dynamic respiratory compliance and mean airway resistance were measured just before and 20 minutes after intraendotracheal tube suction in mechanical ventilated children with respiratory failure.Results　Fifty-two measurements was carried out in 11 patie nts.The mean values of respiratory risistance was (116.73±27.12)cmH2O/L.s before suctioning and (93.38±26.64)cmH2O/L.s after suctioning,with significant differrence(P＜0.01)；The mean values of % leak around the tacheal tube dereased from(18.12±4.12)% before suctioning to (8.71±3.76)% after suctioning (P＜0.05),The mean values of expiratory tidal valume was markedly increased from(7.31±2.12)ml/kg before suctioning to(5.72±1.2)ml/kg after suctioning(P＜0.01),The total respiraory rate markedly decreased after suctioning.Conclusion　The respiratory mechanics measurement is very useful to evaluate the airway patient.The removal of secretions is crucial in respiratory managemet.

ObjectiveTo investigate the effect of immunoglobulin G (IgG) extravasated from blood circulation on the expression of toll-like receptor 4 (TLR4) induced by peripheral lipopolysaccharide (LPS) in rat brain. Methods The rats were divided into four groups in random, 5 rats in each. Group one received LPS 100μg/kg by intraperitoneal administration, normal saline was given by intravenous injection 6 hours later; group two was injected with adrenalin (AD) 15μg/kg intravenously; group three was treated with LPS intraperitoneally, AD was injected 6 hours later; group four was injected normal saline intravenously as control. For all groups, the animals were sacrificed 30 min after the last injection, and the brains were taken for investigation of the TLR4 expressions by immunofluorescence staining and RT-PCR. Result Immunofluorescence staining showed that IgG immunoreactive product was patch-like, distributed in the brain parenchyma in all the animals that received AD. In the LPS+normal saline group, IgG was found merely around the blood vessels. Meanwhile, in LPS+AD animals, TLR4 immunoreactive product coexisted with microglia marker Iba-1 within the IgG extravasated area. The double-labeled cells dispersed in the brain parenchyma and near to the cerebral vessels. In the LPS+saline group, TLR4 positive cells were endothelial-like. RT-PCR results indicated that the expression level of TLR4 in the LPS+AD group were significantly higher than that in the LPS+saline group or AD group or the saline control (P<0.01). Conclusion Extravasated circulating IgG may enhance the TLR4 expression in the rat brain induced by peripheral LPS.

BACKGROUND: Studies have confirmed that Atorvastatin drugs can increase the number of endothelial progenitor cells significantly in vitro, as well as the content of vascular endothelial growth factor(VEGF). OBJECTIVE: To investigate the effect of Atorvastatin on VEGF expression in necrotic femoral heads of rabbits. DESIGN, TIME AND SETTING: The randomized controlled animal experiment was performed at the Department of Orthopedic Surgery, Renmin Hospital of Wuhan University, from September 2007 to November 2008. MATERIALS: Forty-five male and female healthy New Zealand white rabbits weighing 2.5-3.5 kg were randomly divided into normal control group, model control group and AtorvastaUn group, 15 rabbits in each group. METHODS: Nitrogen refrigeration was used to develop femoral head necrosis models of rabbits in the model control and Atorvastatin groups. Two weeks after modeling, the animals in the Atorvastatin group were administered intragastically with Atorvastatin, normal control and model control group were treated with the same volume of normal saline. MAIN OUTCOME MEASURES: Each five rabbits were sacrificed at the 4th, 8th, and 12th weeks respectively for general observation, X-ray and histological observation. VEGF protein expression was assessed by immunohistochemistry method and VEGF mRNA level was assessed by reverse transcription - polymerase chain reaction method. RESULTS: The VEGF protein and mRNA levels in the model control and Atorvastatin groups were obviously lower than those in the normal control group, while the VEGF protein and mRNA levels in the Atorvastatin group were much higher than those in the model control group at the 8th and 12th weeks alter the treatment with Atorvastatin (P < 0.05). CONCLUSION: Atorvastatin can significantly upregulate the expression of VEGF, which is probably an effective clinical treatment to avascular necrosis of the femoral head.

ObjectiveTo study the correlation of personality dysfunction with resilience in secondary vocational students.MethodsBy adopting the questionnaire of Personality Dysfunction Inventory for Middle School Students (MPDI) and Healthy Kids Resilience Assessment scale (HKRA) to assess 358 secondary vocational students.ResultsThe secondary vocational student's scored lower on relationship dysfunction (4.15 ±2.67) than ordinary high school students'( 5.18 ± 3.37 ),and higher on behavioral deviation (6.07 ± 3.20) than ordinary high school students (5.18 ± 3.37 ).There existed great capable difference of total and every fact of personality dysfunction according to grade (P ＜ 0.05,P ＜ 0.01 ),and the quo of personality dysfunction on second year in high vocational school students was better than high and senior high vocational school students.Both parents of secondary vocational student's scored lower on relationship dysfunction( 3.96 ± 2.36),emotional deviation (3.96 ± 2.36 )and personality dysfunction( 15.50 ± 6.27 ) than Single-parent family of secondary vocational students ( 5.73 ±4.12,6.51 ± 3.72,19.78 ± 10.44).There existed great capable difference of total and every fact of personality dysfunction according to relationship with parents (P ＜ 0.05,P ＜ 0.01 ),and the more harmonious in parental relationship,the better in personality.There also existed great capable difference of total and every fact of personality dysfunction according to educational background of parents (P ＜ 0.05,P ＜ 0.01 ),and the higher degree,the better in personality.Personality dysfunction of Students of secondary vocational was negatives correlated with their resilience (Pearson correlation coefficient was between -0.064 and -0.345 for each pair).The linear regression showed thatthepeerrelationshippredictedpersonalitydysfunctionofsecondaryvocationalstudents.ConclusionThe personality dysfunction of secondary vocational students is affected by their parental relationships,educational background of parents,grade and resilience.

ObjectiveTo study the expression of telomerase of fibroblasts in the formation and development of scar,and to investgate the relationship between the telomerase activity and the formation and development of scar.MethodsExpression of telomerase was detected by immunohistochemical techniques in 18 specimens from granulation tissue,17 keloid,16 specimens from hypertrophic scar,28 specimens from mature scar,32 specimens from normal skin,and SPSS16.0 statistics software was used to analyze the relationship between telomerase and scar.Results It was shown that the positive expression rate of telomerase in the granulation tissue group was 94.4 ％,that in the keloid group was 58.8 ％,that in the hypertrophic scar group was 18.8 ％, and that in the mature scar and in the normal skin was zero. comparisons between groups,in addition to the normal scar group and normal skin group,other groups were statistically significant differences.ConclnsionsScar formation is a process with multi-factor participation,while telomerase activation is an important factor. Detecting telomerase activity in the development process of scar formation may determine the prognosis.Through the control of telomerase activity in the pathogenesis of scar may become a new approach of scar treatment.

Objective:To investigate the dynamic changes of intestinal flora in septic model mice.Methods:Forty-two male SPF C57BL/6 mice were selected, the sepsis model was reproduced by cecal ligation and puncture (CLP), and the experimental mice were divided into CLP 6-12 hours group ( n = 9) and 1, 2, 3 days group (all n = 10) and Sham group ( n = 3) according to the time points after modeling. Intestinal flora 16S rRNA sequencing was carried out within feces from the colonic lumen of mice, and the effective sequences were clustered to obtain an operational classification unit (OTU) for statistical analysis of biological information, including Alpha diversity analysis, species composition analysis, principal coordinate analysis (PCoA analysis) and species difference analysis (LEfSe analysis), the dynamic changes of intestinal flora after CLP modeling were analyzed. Results:Compared with Sham group, with the prolongation of modeling time, the Alpha diversity of intestinal flora decreased, showing a decrease in community richness index (3 days after CLP: Chao index 1 was 367.9±162.6 vs. 508.3±105.9, Ace index was 372.5±151.9 vs. 498.8±104.2), Shannon index decreased in community diversity index (3 days after CLP: 2.57±1.06 vs. 4.30±0.57, Simpson index increased (3 days after CLP: 0.26±0.19 vs. 0.04±0.03), suggesting that with the progress of CLP modeling time, the richness and diversity of intestinal flora decreased. According to species composition analysis, in OTU level, OTU 633 accounted for the highest proportion in CLP 1 day group (24.79%), OTU 1016 was dominant in CLP 2 days and 3 days groups, and the highest proportion was 61.75% in CLP 3 days group; in genus level, the abundance of norank_f_Muribaculaceae showed a slight increase and then a significant decrease with time, the abundance of Escherichia-Shigella increased significantly in CLP 2 days and 3 days groups, the abundance of Lactobacillus increased first and then decreased with time, the abundance of Bacteroides showed a trend of gradual increase with time. PCoA analysis suggested that CLP 6-12 hours group had a higher structural similarity with Sham group. The flora structure changed gradually with the time after modeling, and the change was significant after 3 days of CLP. LEfSe analysis indicated that the main components that caused the differences among the groups were g _norank_f_Muribaculaceae, g _Prevotellaceae_UCG-001, s_uncultured_Bacteroidales_bacterium_g_norank_f_Muribaculaceae, g_Parabacteroides, Escherichia-Shigella and OTU 1016. The differences in abundance of Escherichia-Shigella and OTU 1016 among the five groups ranked first in the genus level and OTU level respectively; the abundance of Escherichia-Shigella gradually increased from 0.01% (0%, 0.02%) in 6-12 hours after CLP to 44.79% (3.71%, 53.75%) in 3 days with time, and the abundance of OTU 1016 increased from 0.01% (0%, 0.02%) in 6-12 hours after CLP to 44.69% (3.66%, 53.64%) in 3 days with time. Conclusion:Intestinal dysbiosis occurred in all CLP model groups, the diversity of the flora gradually decreased with the progress of modeling time, Escherichia-Shigella gradually became the dominant bacteria.

Objective To investigate the effects of submucosal resection by guidance under esophageal endoscopic ultrasound. Methods 79 cases ofsuspected upper gastrointestinal submucosal tumors were found by endoscopic ultrasonography,transesophageal endoscopic ultra-sound showed lesions under muscularis mucosa,after informed consent,they were underwent endoscopic mucosal resection. Pathological diag-nose of the lesions were found out after resection. Results Compared with pathological results showed that endoscopic ultrasound accurately determine tumor location in the hierarchy;through endoscopic mucosal resection to remove all lesions,5 patients were found bleeding postop-erative,6 patients were found esophageal stricture,the prognosis was good after treatment. Conclusion Endoscopic ultrasonography can accu-rately determine submucosal myoma,it can be used to guide resection of submucosal myoma.

OBJECTIVETo study features of brain gray matter injury in cerebral infarction patients and intervention of scalp acupuncture by using voxel-based morphology.

METHODSA total of 16 cerebral infarction patients were recruited in this study, and assigned to the scalp acupuncture group and the control group, 8 in each group. Another 16 healthy volunteers were recruited as a normal group. All patients received scanning of T1 structure. Images were managed using VBM8 Software package. Difference of the gray matter structure was compared among the scalp acupuncture group, the control group, and the healthy volunteers.

RESULTSCompared with healthy volunteers, gray matter injury of cerebral infarction patients mainly occurred in 14 brain regions such as cingulate gyrus, precuneus, cuneus, anterior central gyrus, insular lobe, and so on. They were mainly distributed in affected side. Two weeks after treatment when compared with healthy volunteers, gray matter injury of cerebral infarction patients in the scalp acupuncture group still existed in 8 brain regions such as bilateral lingual gyrus, posterior cingulate gyrus, left cuneus, right precuneus, and so on. New gray matter injury occurred in lingual gyrus and posterior cingulate gyrus. Two weeks after treatment when compared with healthy volunteers, gray matter injury of cerebral infarction patients in the control group existed in 23 brain regions: bilateral anterior cingulum, caudate nucleus, cuneate lobe, insular lobe, inferior frontal gyrus, medial frontal gyrus, precuneus, paracentral lobule, superior temporal gyrus, middle temporal gyrus, lingual gyrus, right postcentral gyrus, posterior cingulate gyrus, precentral gyrus, middle frontal gyrus, and so on. New gray matter injury still existed in 9 cerebral regions such as lingual gyrus, posterior cingulate gyrus, postcentral gyrus, and so on.

CONCLUSIONSBrain gray matter structure is widely injured after cerebral infarction. Brain gray matter volume gradually decreased as time went by. Combined use of scalp acupuncture might inhibit the progression of gray matter injury more effectively.
