1.Effect of Wenxiao Granule on the Expression of MCP-1 and TNF-? in Experimental Atherosclerosis Rabbits

Objective To observe the effect of Wenxiao Granule on the expression of MCP-1 and TNF-? in experimental atherosclerosis rabbit, and explore its mechanism in treating atherosclerosis. Methods Thirty-six New Zealand rabbits were randomized to four groups: control group, Wenxiao granule group, Simvastatin group. They were fed a high-fat diet except normal group until the mature atherosclerotic plaques formed. The rabbits in two drug-treated groups were treated with respective drug for 8 weeks. All rabbits were sacrificed. The morphology and composition of atherosclerotic plaques in aortic roots were examined. The expression of MCP-1 and TNF-? were determined by immunohistochemical staining. Results The expression of MCP-1 and TNF-? in plaque of Wenxiao Granule group and simvastatin group were significantly dereased compared with the model group (P 0.05). Conclusion Wenxiao Granule can inhibit the expression of MCP-1 and TNF-?, which may stabilize plaque of atherosclerosis.

3.Transfacet approach decompression for thoracic spinal stenosis caused by anterior compression

Objective To study the efficiacy and safety of transfacet approach decompression to treat thoracic spinal stenosis caused by anterior compression.Methods Thirty-three patients with thoracic spinal stenosis caused by anterior compression were treated in our institution from April 2005 to April 2009.Nineteen patients with more than 12 months follow-up were included in this study.Among of them,10 were male and 9 were female,with the age ranged from 33 to 77 years(mean,55.9 years).The causes of compression for spinal stenosis included ossification of posterior longitudinal ligament(OPLL)in 5 cases,thoracic disc herniation(TDH)in 11 cases,OPLL with ossification of ligamentum flavum(OLF)in 2 cases and TDH with OLF in 1 case.All patients underwent anterior decompression via a transfacet approach combined with anterior fusion and posterior fixation.The modified Japanese Orthopaedic Association(JOA)score system and Nurick Myelopathy grade were used to evaluate the outcomes.Results The operation time ranged from 180 to 480 min,with an average of 299.5 min and the blood loss was varied from 250 to 2200 ml,with an average of 918.5 ml.Among 7 cases with OPLL(including combined with OLF),2 patients developed neurologic deterioration and 1 patient developed cerebrospinal fluid leakage.There were no neurologic deterioration,cerebrospinal fluid leakage and other complications occurred in 12 cases with TDH(including combined with OLF).The follow-up ranged from 12 to 54 months(mean,28.6 months).The preoperative JOA score ranged from 2 to 11(mean,6.3).The JOA score in the last follow-up ranged from 5 to 11(mean,8.6).According to Nurick Myelopathy grade,the preoperative grade was 0 in 2 cases,1 in 2,2 in 4,3 in 5,4 in 2,and 5 in 4.The number of postoperative grade was 6,6,3,3,1 and 0 respectively.Conclusion Satisfactory decompression could be achieved by using transfacet approach for thoracic spinal stenosis caused by anterior compression.The approach is a safe and promising alternative for thoracic spinal anterior decompression.
4.Clinical Research on Wenxiao Decoction in Treatment of Carotid Atherosclerostic Plaque

Objective To observe the effect of Wenxiao Decoction in treating the patients with carotid atherosclerostic (CAS) plaque and assess the safety. Methods Sixty patients with CAS plaque were randomly allocated to treatment group (33 cases) and control group (27 cases). The control group was given routine therapy, and the treatment group was given Wenxiao Decoction additionally. And they were further divided into half-year treatment group and half-year control group, 1-year treatment group and 1-year control group. Carotid artery color Doppler ultrasound and hepatorenal function were examined before and after treatment. Results Compared with the control groups after therapy, the average numbers of plaques, maximal plaque thickness, Crouse plaque integral were decreased both in 1-year and half-year treatment groups (P<0.05, P<0.01). The plaque stability of 1-year treatment group was better than that of half-year treatment group. The average numbers of plaques, plaques size and Crouse plaque integral increased in control groups after routine therapy (P<0.05, P<0.01). Bilateral common carotid artery diameter, intima-medial thickness (IMT) and hepatorenal function had no statistical difference between before and after treatment both in treatment and control groups (P>0.05). Conclusion Wenxiao Decoction is effective and safe in treatment of CAS plaque, and plaque stability is more obvious with the extension of treatment time.
5.Application of percutaneous epididymal sperm aspiration and percutaneous testicle sperm aspiration in the differentiating diagnosis of obstructive and nonobstructive azoospermia

Objective To obtain information on the application value of percutaneous epididymal sperm aspiration(PESA) and percutaneous testicle sperm aspiration(PTSA) in the differentiating diagnosis of obstructive and nonobstructive azoospermia.Methods Sperm recovery procedures were done in infertile men with obstructive azoospermia(OA)(n=37) and nonobstructive azoospermia(NOA)(n=28) by PESA or PTSA.Cytological smears were analysed.Results Sperm was found in the 32 epididymides and 5 testicles of OA group and in the 7 epididymides and 11 testicles of NOA group.Sperm counts were significantly different in two groups.Conclusion PESA and PTSA are efficient methods in differentiating OA and NOA.
6.Value of 18 F-FDG PET/CT for prognosis evaluation in patients with small cell lung cancer

Objective To evaluate the prognostic value of 18 F?FDG PET/CT in untreated small cell lung cancer ( SCLC) . Methods A total of 49 SCLC patients( 35 males, 14 females, median age 61 years) with pathologically confirmed SCLC had underwent 18 F?FDG PET/CT scan before treatment from January 2008 to December 2013 and were retrospectively analyzed in this study. Clinical staging were mainly deter?mined by 18 F?FDG PET/CT scan. Performance status ( PS) was acquired according to the clinical symptoms before PET examination. Patients were followed up for at least 6 months and the survival interval was recor?ded. Two?sample t test, Kaplan?Merier, log?rank and Cox regression analysis were used for statistical analy?sis. Results (1)Among 49 patients, 20 died during the follow?up, 1 patient was lost to follow?up, and 45 patients(91.84%) had metastasis. Patients with limited disease (LD) and extensive disease (ED) were 27 and 22 respectively according to 18F?FDG PET/CT imaging. (2)The median OS was 23.68 months and me?dian PFS was 19.93 months. LD patients had significantly longer OS and PFS than ED patients (35.30 months vs 12.57 months, 28.87 months vs 11.30 months;χ2=18.810, 13.647, both P<0.05). (3)SUVmax of primary tumor ranged from 1.97 to 21.50. The SUVmax of primary foci had no difference between LD(8.27± 3.14) and ED(9.68±5.36)patients(P>0.05). SUVmax of primary tumor showed no significant correlation with OS and PFS (both P>0.05).There were no significant differences in OS and PFS between low SUVmax group and high SUVmax group in both LD and ED patients (χ2=0.001-0.565, all P>0.05). (4) Staging based on 18 F?FDG PET/CT results and PS score were independent prognostic factors ( hazard ratios:3.93, 5?00, both P<0.05) , while SUVmax showed no significant prognostic value. Conclusions PS score before PET scan and imaging staging based on 18 F?FDG PET/CT results are independent prognostic factors. The predictive value of primary foci SUVmax needs further investigation.
7.Prokaryotic Expression and Antifungal Activity of HumanαDefensin-5 Protein

Objective To construct the prokaryotic expression vector for HD-5 and purify the recombinant HD-5 protein then analyze its antifungal activity. Methods The HD-5 gene was cloned by PCR, then was inserted into prokary-otic expression plasmid pQE-30Xa to construct pQE-30Xa/HD-5. After sequencing, pQE-30Xa/HD-5 was transformed in-to E.coli M15 cells. Its expression was induced by IPTG and confirmed by SDS-PAGE. The recombinant protein was purified through Ni-NTA affinity purification system. The antifungal activity was tested by disk diffusion method. Results HD-5 gene and pQE-30Xa/HD-5 vector were obtained successfully. E.coli M15 strains was used to express HD-5 fusion protein. After purification, the fusion protein was confirmed by Western blot. The disk diffusion test confirmed that the fusion pro-tein can inhibit Candida albicans. Conclusion Expression vector pQE-30Xa/HD-5 was successfully constructed. The HD-5 fusion protein was expressed in E.coli successfully, which showed a certain degree of antifungal activity.
8.Analysis of correlation between sperm indicators and early spontaneous abortion after in vitro fertilization

Objective To explore the correlation between the sperm indicators and early spontaneous abortion after in vitro ferti ‐lization(IVF) .Methods 251 couples receiving IVF ,were retrospectively analyzed and divided into the research group (98 couples) and control group (153 couples) according to whether spontaneous abortion occurring .The male′s indicators of sperm DNA frag‐mentation index (DFI) ,sperm nucleoprotein immaturity rate ,percentage of normally morphologic sperms ,etc .in the two groups were analyzed .Results DFI ,percentage of normally morphologic sperms ,sperm concentration ,motility and forward motile sperm had no statistical differences between the research group and the control group (P> 0 .05) ;while the sperm nucleoprotein immaturi‐ty rate of the research group was significantly higher than that of control group ,the difference was statistically significant (P <0 .05) ;the sperm nucleoprotein immaturity rate had weak correlation with spontaneous abortion in early pregnancy (P < 0 .05) . Conclusion The sperm nucleoprotein dysmaturity is correlated with the spontaneous abortion in early pregnancy .The sperm nucle‐oprotein immaturity rate could be used to evaluate the risk of spontaneous abortion in early pregnancy after IVF .
9.Effects of antimicrobial peptide LL-37 expressed and purified from prokaryotes in the murine model of vaginal candidiasis

Objective To study the effects of antimicrobial peptide LL-37 expressed and purified from prokaryotes on candida albicans growth. Methods (1) Thirty female Kunming mice were treated with estrogen and white candida yeast suspension were poured into vagina to establish a vulvovaginal candidiasis (VVC) murine model. After successful establishing the VVC mouse model, mice were randomly sorted into test group (n=15) and control group (n=15) . Suspension(30μl, 100μg/ml)of recombinant peptide LL-37 expressed and purified in Prokaryotes was given by intravaginal administration to the test group for 5 days, while the same amount of phosphate buffered saline (PBS) was given to the control group. (2) Tweenty-four hours after treatment, the fungal burden and colony-forming unit (CFU) of vaginal fluids were evaluated. All mice were subsequently sacrificed and vaginal tissues were harvested for tissue homogenate preparation. ELISA was used to determine the levels of nterleukin-10(IL-10)and interferon-γ(IFN-γ)in the isolated vaginal tissues. Results (1) VVC mouse model was established successfully in this study. Vaginal mucosa congestion, edema, vaginal plica disappearing were obviously observed in the control group. After treatment with recombinant protein LL-37 vaginal mucosa has no obvious change in the test group. (2) Fungal burden and CFU of vaginal fluids were significantly lower in the test group [(4.8±1.0)×104 CFU/ml] than that in the control group [(8.5 ± 2.1) × 104 CFU/ml, P=0.017]. IFN-γlevel of the test group was increased [(257 ± 11) vs (197 ± 4) pg/ml, P=0.000], while the level of IL-10 was reduced [ (287 ± 15) vs (379 ± 17) pg/ml P=0.000] resulting in a the ratio of IFN-γ/IL-10 was in significantly higher in test group (0.892±0.008 vs 0.496±0.013, P=0.000). Conclusion Recombinant protein LL-37 expressed and purified from prokaryotes inhibits the growth candida albicans and improves vaginal immunity by adjusting IFN-γand IL-10 secretion in the VVC mouse model, highlighting the therapeutic potential of LL-37 for VVC.
10.Influence of different administration regimens for treatment of bacterial vaginosis on vaginal microflora and the expression of immune factors

Objective Tracking of the vaginal microflora recovery and the expression of immune factors from untreated and treated patients with bacterial vaginosis (BV) by using different administration regimens and studying the relationship of treatment results and regimen selections. Methods 25 healthy females were selected as a control group and 100 BV patients were randomly divided into 4 groups (n=25/group). Group A: Intravaginal administration of metronidazole (× 7 d), Group B:Continuous intravaginal administration of metronidazole (× 7 d) and then live Lactobacillus Capsule (× 7 d) , Group C: Intravaginal administration of nifuratel (× 7 d), Group D: Continuous intravaginal administration of nifuratel (× 7 d) and then live Lactobacillus Capsule (×7 d). The microecological assessment system and EILSA were used to compare the clinical efficacy, vaginal microflora recovery and the changes in IL-8, TLR2 and TNF-αof the vaginal lavage fluid in healthy women or patients with bacterial vaginosis before and after treatments by four treatment strategies. Results ① The vaginal microflora imbalance, flora disturbance, pH value increased were presented in BV group compared with the control group.②Compared to the median of IL-8, TLR2 and TNF-α in vaginal lavage fluids of control group, there was no significant difference in IL-8 level but both TLR2 and TNF-αwere significantly increased (P<0.05) in BV group. The immune factors had no significantly difference in all BV groups.③The therapeutic effect in each BV groups was compared after stopping treatment for 7 days. The cure rate and the vaginal microflora recovery rate were significant higher in group B and D than group A and C (P<0.05). ④ After treatment there was no significant change in IL-8 level but there was an obviouslydecrease in TLR2 and TNF-α(P<0.05). The decreased levels are more significant in groups B and D than groups A and C (P<0.05). Conclusion By combining with the microecological assessment system to evaluate the therapeutic effect of BV, our research suggests that the sequence schemes of nifuratel plus live Lactobacillus Capsule is more effective in therapy effect, restoring normal vaginal micro-ecological environment and vaginal local immunity than metronidazole used alone.
