1.Clinical efficacy of four kinds drug oral medicine for 10 days sequential therapy for eradication of helicobacter pylori infection of children

Objective To investigate the clinical efficacy and safety of four kinds of drug oral medicine for 10 days sequential therapy for eradication of helicobacter pylori (Hp) infection of children.Methods One hundred and thirty children with Hp infection in the First Hospital of Datong from Apr.2013 to Apr.2014 were randomly divided into two groups with 65 cases in each group.Patients in the observation group were treated with four kinds of oral drug 10 d sequential therapy with omeprazole and amoxicillin-clavulanate potassium at first 5 d and then omeprazole,clarithromycin and metronidazole for the late 5 d.Patients in the control group were received standard triple therapy with omeprazole,clarithromycin and amoxicillin clavulanate potassium for 10 d.After treatment for 1 month,the Hp eradication rate,treatment rate,adverse reaction were evaluated and compared by 14C urea breathe test.Result The clinical efficacy rate of observation group was 93.8 % (61/65),significantly higher than that in control group (75.4%,49/65).Meanwhile,Hp eradication rate in observation group was 90.8% (59/65),significantly higher than that in control group (67.7%,44/65).The differences were significant(x2 =8.51,10.52; P < 0.05).The incidence of adverse reaction of observation group was 10.8% (7/65),lower than that of the control group 15.4% (10/65),and the differences was significant(x2 =0.61,P > 0.05).Conclusion There are high Hp eradication and relatively safe of four kinds of oral drug 10 d sequential therapy clinical in treatment of Hp infection of children.It is worthy of clinical promotion.
2.Efficacy of combined metformin and other hypoglycemic drugs

Metformin was widely recommended as a first‐line drug for patients with type 2 diabetes mellitus(T2DM ). However ,studies have shown that patients who start metformin as monotherapy often required additional agents to maintain glycaemic control with the natural progression of T 2DM. Anti‐diabetic agents may be used in combination with metformin ,including sulfonylureas ,glinides , thiazolidinediones(TZDs) ,α‐glucosidase inhibitor (AGDI) ,dipeptidyl peptidase‐4 inhibitor (DPP‐4) and glucagon‐like peptide‐1 (GLP‐1 ) or insulin. This paper mainly reviews the recent studies about the therapeutic effect of metformin in combination with other anti‐diabetic agents.
3.EFFECT OF HYPERBARIC OXYGEN ON ACTIVITY OF SUPEROXYGEN DISMUTASE AND CATALASE IN SPINAL CORD TISSUES AFTER SPINAL CORD INJURY

To study the effect of hyperbaric oxygen (HBO) on the activity of superoxygen dismutase (SOD) and catalase (CAT) in spinal cord tissue, a rat spinal cord injury (SCI) model was established. The results showed that the activity of SOD and CAT in the spinal cord decreased significantly after injury, bat recovered after the treatment with 0 25MPa HBO and 0 25MPa HBO+hormone. These results suggest that HBO can increase the activity of anti oxidase in spinal cord,and prevent or relieve the injury by free radicals. Thus it can recover the function of spinal cord.
4.EFFECT OF HYPERBARIC OXYGEN ON CONCENTRATION OF EXCITATORY AMINO ACIDS IN SPINAL CORD TISSUES OF RATS WITH SPINAL CORD INJURY

To study the mechanism of hyperbaric oxygen (HBO) on spinal cord injury (SCI), we duplicated a SCI model and determined the concentrations of excitatory amino acids [EAA, including aspartic acid (Asp) and glutamic acid (Glu)] in spinal cord tissue after treated with 0 1 MPa HBO, 0 25 MPa HBO, 0 25 MPa N 2 O 2 and hormone, respectively. The results showed that the concentration of EAA (Glu) in the spinal cord significantly increased after injury, whereas after treatment it decreased in all but the N 2 O 2 group. It decreased most significantly in 0 25MPa HBO and 0 25MPa HBO+hormone groups. There were significant differences between 0 25MPa HBO group and 0 1MPa HBO group, 0 25MPa HBO+hormone group and pure hormone group ( P
5.Effect of the morphology of S-shaped root canals with reciprocating single file system on root canal irrigation

Objective: To evaluate effect of the morphology of simulated S-shaped root canals with Reciproc or Mtwo instruments on root canal irrigation. Methods: A total of 40 simulated S-shaped resin blocks were randomly divided into 2 groups (n=20), which was prepared by Reciproc or Mtwo. Blue ink was injected with constant speed and volume into the canals using 30G lateral opening syringe. hTe distance from needle tip to apex and vapor lock length were measured. A type of 25# 0.04 gutta-percha point was used to stir and remove the vapor lock. hTe time for removal of the vapor lock was recorded. Another syringe was used to inject saline into the canals and to measure the length of stagnant water. Results: hTe distance from needle tip to apex in the Reciproc group was signiifcantly longer than that in the Mtwo group [(4.276 ± 0.221) mm vs (3.459 ± 0.205) mm,P<0.05]. The vapor lock length in the Reciproc group was signiifcantly longer than that in the Mtwo group [(4.472 ± 0.230) mm vs (3.668±0.217) mm,P<0.05]. hTe time to remove vapor lock was signiifcantly shorter in the Reciproc group than that in the Mtwo group [(10.13 ± 1.79) s vs (15.29 ± 2.15) s,P<0.05]. The length of stagnant water was also shorter in the Reciproc group than that in the Mtwo group[(1.351 ± 0.142) mm vs (2.245 ± 0.206) mm,P<0.05]. Conclusion: hTe morphology of S-shaped root canal affects the root canal irrigation. hTe effect of root canal irrigated by Reciproc is better than that by Mtwo.
6. Reversal of drug resistance in A549/DDP cells by silencing MT1H gene with MT1H siRNA

Objective: To explore the feasibility of using siRNA targeting metallothionein 1H (MT1H) gene to reverse the drug resistance of A549/ DDP cells to cisplatin (DDP). Methods: The expressions of MT1H mRNA in A549 and A549/DDP cells were detected by reverse transcriptase polymerase chain reaction (RT-PCR). MT1H siRNA was transfected into A549/DDP cells with high expression of MT1H. The mRNA and protein levels of MT1H were detected by RT-PCR and Dot blotting, respectively. The chemosensitivity of A549/DDP cells to cisplatin was assessed by MMT assay. The apoptotic ratio induced by DDP was determined by TUNEL and flow cytometry analysis (FCM). The apoptosis-related protein levels of Bcl-2 and Bax were determined by immunohistochemistry. Results: MT1H mRNA was highly expressed in A549/DDP cells but not in A549 cells. The mRNA and protein levels of MT1H were significantly down-regulated at 48 h after transfection in MT1H siRNA group compared with the control group. The chemosensitivity of A549/DDP cells to DDP was significantly elevated. The apoptotic ratio induced by DDP was significantly increased. Expression of Bcl-2 was greatly down-regulated but the expression of Bax had little change. Conclusion: Silencing MT1H gene with MT1H siRNA reduces the expression of Bcl-2, enhanced the apoptosis-inducing effect of DDP on A549/DDP cells, and effectively reverses the drug resistance of A549/DDP cells.
7.Antimicrobial resistance of gram-positive pathogenic bacteria isolated from children's respiratory tract infection

Objective To investigate antirnierobial resistance of comlnon pathogenic bacteria isolated from children's respiratory tract in Beijng friendship Hospital during the period between January 2004 and December 2006.Methods Forty-nine strains of Streptococcus pneumoniae,fifty-five strains of Staphylococcus aureus and fifteen strains of Group A β-hemolytic streptococcus were isolated from children with community-acquired respiratory tract infeetion in pediatric department of Beijng Friendship Hospital.Kirby-Bauer agar dilution method Was used to determine the susceptibility to antimicrobial agents.Results Ninety percent of Streptococcus pneumoniae strains were resistant to erythromycin.Penicillin-non-susceptible streptococcus pneumoniae accounted for 10%,while 90% of which were multi-drug resistant.For Staphylococcus aureu strains,the resistant rates of ampicillin,penicillin and oxacfllin were 86%,82%,and 9% respectively,and serere multi-drug resistance was found.Eighty percent of the group A β-hemolytic streptococcus strains were resistant to erythromycin,however all remained sensitive to penicillin and ampicillin.Conclusion The antimicrobials resistant pathogenic bacteria isolated from children with respiratory tract infection have become a severe prdblem.The resistant rates of macrolides are high for Streptococcus and Group A β-hemolytic streptococcus.MeticiUin resistant staphylococcus aureus(MRSA)strains were found in the study.Severe multi-drug resistance was found.Non β-lactam antibiotics should be avoided when respiratory tract bacterial infection occurred in children.
8.Repair of compound defect in the hand by compound or combined flap transfer

Objective To introduce the outcome of compound defect in the hand treated by compound flap transfer or combined flaps transfer. Method From Dec.1983 to June 2001,thirty-five cases with five kinds of compound defect in the hand were treated surgically.Among them,four cases with defect of skin and extensor tendon in the dorsum of hand were repaired by dorsalis pedis flap with tendons;Three cases with longitudinal subtotal loss of distal thumb and one case with longitudinal subtotal loss of distal middle finger were reconstructed with fibular segmental osteo-onychocutaneous flap from great toe or tibial segmental osteo-onychocutaneous flap from second toe;Eight cases with total or subtotal defect at middle part of thumb or fingers were reconstructed with segmental second toe; seventeen cases with defect of thumb and hand skin were reconstructed with combined transplantation of second toe and free skin flap;Two cases with middle finger defect at the base of proximal phalanx were reconstructed with bridging transplantation of bilateral second toe. Result Thirty-three cases were successful.Follow-up period ranged from 0.5 to 5 years postoperatively.The excellent and good rate was 82.9% assessed with provisional functional assessment criterion for upper limbs issued by Chinese Society of Hand Surgery. Conclusion The compound flap transfer or combined flaps transfer was an ideal method in repair of compound defect in the hand.
9.Clinical study of comprehensive intervention on bone mass reduction in postmenopausal women

Objective To analyze the clinical effect of comprehensive intervention on bone loss in postmenopausal women, and to provide reference for the prevention of osteoporosis in postmenopausal women .Methods 152 cases of postmenopausal bone mass in our hospital from March 2014 to March 2015 were selected as the research object, all patients were randomly divided into intervention group and control group,76 cases in each group.The control group was treated with HRT hormone replacement therapy and calcium supplement treatment , the intervention group based on the treatment in control group by alendronate treatment,supplemented by health guidance, two patients were treated for 1 years.Comparison of two groups of patients before and after treatment, N (N-MID), osteocalcin, procollagen type I amino terminal peptide (PINP), beta crosslinking degradation products (β-CTX), alkaline phosphatase (ALP), bone mineral density L2-L4 (BMD), E2, Serum Osteocalcin (sOC) and calcitonin (CT), parathyroid hormone (PTH), the incidence rate of fracture, osteoporosis, adverse drug reaction.Results There were no significant differences in BMD (L2-L4),β-CTX, N-MID, PINP, ALP and sOC between the two groups; After one year of treatment, BMD and sOC of the two groups were significantly higher than those before treatment (P<0.05), and the levels of β-CTX, N-MID, PINP and ALP were significantly decreased (P<0.05), and the difference between the two groups was statistically significant (P<0.05).There was no significant difference in E2, PTH and CT between the two groups before treatment.After one year of treatment, the above indexes of the two groups were significantly higher than those before treatment (P<0.05), and there was no statistically significant difference between the two groups.In the comprehensive intervention group, one patient ( 1.32%) was diagnosed as osteoporosis and two cases (2.63%) fractured after one year of treatment,In the control group, 11 cases (14.47%) were osteoporosis, 9 cases%). The fracture rate and osteoporosis rate in the intervention group were significantly lower than those in the control group (P<0.05).In the course of treatment,six patients in the comprehensive intervention group had adverse drug reactions, and the alendronate dosage decreased by half,and no adverse drug reaction was found in the control group.Conclusion hormone,Caltrate,alendronate and health guidance comprehensive intervention program to improve the hormone level in patients with osteopenia, bone reconstruction of women physical level and promote the bone tissue of patients with anti absorption to enhance the role of postmenopausal bone balance favorable development, for postmenopausal bone loss in women with a high clinical value.
10.Crab-shaped hand reconstruction: bilateral second toes transfer for reconstruction of total hand or total digits defect

Objective To report the result of bilateral second toes transfer for reconstruction of total hand or total digits defect. Methods A series of 14 total hand or total digits defect were reconstructed by transplantation of 2nd toes from both feet forming a crab-shaped hand. Among 14 cases, 11 cases were males and 3 females. The patients were aged from 4-44 years, with an average of 22.7 years. One of the 14 cases was of congenital deformity and the other 13 were caused by trauma. The 14 cases were classified into three types according to the level of amputation; Type Ⅰ: defect at the distal end of the metacarpal bone with good function of abductor pollicis in 6 cases; Type Ⅱ: at the proximal end of carpus or the level of wrist joint in 5 cases; and type Ⅲ: at the forearm end stump in 3 cases. All the operations were successful. Results All 28 transferred toes survived uneventfully with good blood supply and pulp fullness and elasticity. Follow-up examination made at mean 6.6 years postoperative period (2-15 years) showed that mean total active motion (TAM) of IP joints of reconstructed digit was 57.5? (35?-105?). All the 14 cases had restored digital function, such as opposition of the fingers, pinching a pencil, a key, a needle or holding an ink-bottle. The mean distance between the two reconstructed digital pulps was 4.3 cm (2.6-6.5 cm). Judging by the evaluation standard of British Medical Institute, sensation recovery to S3+ was obtained in 20 digits of 10 cases. Based on the trial criteria of the function of hand reconstruction of hand surgery society of Chinese Medical Association, 2 were assessed as excellent, 7 good, 4 fair and 1 poor. Conclusion Reconstruction of crab-shaped hand for total hand or total digits defect by transfer of bilateral 2nd toes with metatarso-phalangeal joint can give the patient reasonable prehensile function without much impairment to foot function.
