Objective To investigate the effect of vitamin E supplementation on oxidative stress in hemodialysis patients. Methods Fifty-six uremic patients undergoing hemodialysis (HD) were involved in this self-controlled study. The patients were observed for one month to evaluate the status of oxidative stress and then divided into two groups randomly. Group A patients (n=28) received 200mg/d of vitamin E and Group B patients (n=28) were treated with 400mg/d of vitamin E. Vitamin E was supplied for one month in both groups. The serum levels of advanced oxidation protein products (AOPP), malondialdehyde (MDA), vitamin E and activity of glutathione peroxidase (GSHPx) were measured at the initial and the end of vitamin E supplementation respectively. Results Serum levels of AOPP and MDA were significantly higher in HD patients compared with healthy controls (n=56, P

Objective To investigate the clinical treatment of nonpenetrated cornea trauma caused by crops. Methods Clinical data of 128 cases of nonpenetrated cornea trauma caused by crops were retrospectively analyzed. According to the interval time between occurrence of trauma and clinic visiting, the patients were divided into 3 groups:group A (33 cases,<24 h), group B (72 cases, 24 h≤interval time<1 week) and group C (23 cases, ≥ 1 week). The therapeutic effects and prognosis were analyzed. Results There was statistical difference in the incidence of corneal ulcer among group A, group B and group C: 6.1% (2/33), 62.5% (45/72) and 100.0% (23/23), χ2= 52.32, P<0.01. In group B, 12 cases were treated with conjunctival flap covering, 2 cases received keratoplasty and 2 cases undertook enucleation. In group C, 10 cases were treated with conjunctival flap covering, 4 cases received keratoplasty and 2 patients undertook enucleation finally. All the other patients were cured with local debridement and medical treatment. Conclusions Patients with nonpenetrated cornea trauma caused by crops may develop infectious keratitis, and prompt and proper treatment can avoid the secondary infection and improve the outcome. Local debridement in combination with iodophors disinfection can prevent the incidence of infectious keratitis. Conjunctival flap covering is an effective technique in the treatment of corneal ulcer caused by nonpenetrated cornea trauma.

BACKGROUND:Researches showed that the enriched environment could improve the cognitive dysfunction of rats with vascular dementia. However, there are few reports regarding its mechanism of action.
OBJECTIVE:To explore the effect of enriched environment on the cognitive dysfunction of rats with vascular dementia from the behavioral level.
METHODS:Vascular dementia models weremade by permanent ligation of bilateral common carotid arteries and were divided into vascular dementia group (n=8) and enriched environment group (n=12). Vascular dementia group was taken care under conventional breeding environment for 30 days, while the enriched environment group was subjected to the enriched environment for 30 days. Morris water maze test was adapted to test the cognitive function of rats between two groups. Immunohistochemistry and immunoblotting were applied to observe the number ofDCX+cels and DCX protein level in both groups. The number of DCX-labeled cels co-expressing NeuN was observed using immunofluorescence technique.
RESULTS AND CONCLUSION:(1) The escape latency in the vascular dementia group was longer than that in the enriched environment group (P< 0.05). The times across the platform was less in the vascular dementia group than that in the enriched environment group (P< 0.05). (2) In comparison with the enriched environment group, the number of DCX-positive cels andits protein level in the piriform cortex were significantly decreased in the vascular dementia group (P< 0.05). (3) The number of DCX/NeuN co-labeled cels in the piriform cortex was significantly less in the vascular dementia group than in the enriched environment group (P< 0.05). (4) These findings suggested that enriched environment could improve the cognitive dysfunction of rats with vascular dementia through promoting the expression and differentiation of the immature neurons.

Objective To assess the effects of early correction of anemia with recombinant human erythropoietin (rHuEPO) on the development and progression of left ventricular hypertrophy (LVH) in patients with mild-to-moderate chronic renal insufficiency (CRI) who are not on hemodialysis. Methods A total of 158 patients with serum creatinine from 147μmol/L to 400μmol/L were nrolled in this prospective, multicenter study. Eighty-six patients with hemoglobin (Hb)<110g/L received rHuEPO treatment with a target Hb of ≥110g/L (Group A). Forty patients with comparable Hb concentration (<110g/L) but did not receive rHuEPO (Group B) and 32 patients with Hb≥110g/L and without rHuEPO treatment (Group C) were served as controls. Left ventricular mass index (LVMI) was evaluated by echocardiography at baseline and every 3 months for 2 years. Results There was no difference in age, gender, etiology of renal failure, blood pressure and cardiovascular risk factors among the 3 groups. At baseline, the prevalence of LVH was 72.1% in group A,72.5% in group B and 59.4% in group C. LVMI was inversely correlated with Hb levels (r=0.70, P<0.01). During the 2-year period, the mean LVMI decreased from 142.6±25.7g/m2 to 132.4±18.5 g/m2 in group A, while increased significantly in both group B and group C. The mean Hb concentration increased from 93.8±14.6g/L to 111.2±10.3g/L (P<0.05) in group A, but tended to decrease in group B and group C. There was no significant change of the mean blood pressure, number of anti-hypertensive drugs and serum creatinine concentrations in all 3 groups. However, patients' serum creatinine doubled more often in group B and group C than in group A.Conclusions LVH was common in predialysis CRI patients and was associated with the severity of anemia. Early intervention with rHuEPO may reverse LVH in these patients.

Objective: To investigate the clinical characteristics, treatment regimens, and outcomes of patients with primary breast dif-fuse large B-cell lymphoma (PB-DLBCL). Methods: Between January 2010 and January 2018, 21 patients with PB-DLBCL were diag-nosed, treated, and followed up at the First Affiliated Hospital of Zhengzhou University. All patients were female, with a median age of 49 years (ranging from 21 to 77 years) at presentation. All patients received chemotherapy, of which 17 patients received the CHOP regimen and 4 received the EPOCH regimen. Eight patients received chemotherapy followed by radiotherapy, and 13 received chemo-therapy alone. Six patients received prophylactic intrathecal injections. The incidences of refractory and progressive disease between patients who received different regimens were analyzed using the Chi-square test. The overall survival (OS) and progression-free sur-vival (PFS) rates were calculated using the Kaplan-Meier method, and differences in survival were compared using the Log-rank test. Multivariate analysis was performed with the Cox-regression model for those factors that were confirmed as significant in the univari-ate analysis. Results: The most common presentation was a painless mass. The 5-year OS and PFS rates were 74% and 66%, respective-ly. There was no significant difference in the incidence of refractory or progressive disease between the EPOCH and CHOP groups (P=0.603). Six of those who received prophylactic intrathecal injections had no central nervous system recurrence, and 2 patients who did not receive prophylactic intrathecal injections had central nervous system recurrence. Univariate and multivariate analyses showed that both the level of serum β2 microglobulin [P=0.044, hazard ratio (HR)=0.431, 95% confidence interval (CI): 0.432-0.967] and radio-therapy (P=0.002, HR=0.495, 95% CI: 1.073-2.508) were related to the OS of PB-DLBCL. Conclusions: PB-DLBCL often occurs in women, mostly involving the unilateral breast, which manifests mainly as a painless mass. The level of serum β2 microglobulin is a factor of poor prognosis in PB-DLBCL. The treatment modality of chemotherapy combined with radiotherapy can significantly improve the OS of PB-DLBCL. Prophylactic intrathecal injections may be useful to reduce the incidence of refractory disease or recurrence in the central nervous system.
