Promoted vigorously by state and government , it is a strategic emphasis of a new round of medical reform that public hospitals combine with social capital .In order to expect more attention to the hot issue from wheth-er theoretical or practical views , this article draws lessons from the experiences that foreign hospitals have managed to combine with social capital , to analyze the necessity of the new medical mode under the background of the medical re -form policies implementation and the enlarged health industry flourishing development , and meanwhile to discuss the value of the new medical mode for the present medical situation .

An 11 year old girl presented with dyspnea and a rough cough, after having mistakenly swallowed a steel ball and unable to relieve the symptoms. Chest X-ray showed an image consistent with a 10 mm diameter circular object, embedded in the right mainstem bronchi near the fifth thoracic level and the medicastinal moving to the right. Aim: To establish an analytical method to study the chemical components in Zhizi-Gancao-Chi(ZGC) decoction, and investigate its anti-depressant effect. Methods: An LC-PDA-MS/MS method was established and applied. The separation was performed on an ODS C_(18) column (250 mm ×4. 6 mm, 5 μm) with a gradient elu-tion. The mobile phase consisted of water/acetic acid( 1 %, v/v) and methanol. The main components were identi-fied according to the UV and MS. The anti-depressant effect was evaluated with mice tail-suspension test and forced swimming test. Results: The components of the ZGC decoction were separated isolated and ten of them were identified. These compounds are classified to iridoid glycosides, flavonoids and triterpene saponins. In addi-tion, its anti-depressant effect was observed for the first time. Conclusion: The LC-PDA-MS/MS method can ef-fectively separate and identify the components of ZGC decoction, which has significant anti-depressant effect. The relationship between the chemical component and its anti-depressant effect needs further study.

To observe the clinical therapeutic effect and safety of local encircled acupuncture plus valaciclovir in treating senile herpes zoster.Methods:Sixty senile patients with herpes zoster were divided into two groups.In acupuncture and medicine group,the patients were treated by encircled acupuncture plus valaciclovir.The needle was inserted about 0.8 cun away from herpes and to form an angle of 15°with the skin around the skin lesion.During treatment,valaciclovir was taken orally 300 mg every time,twice every day for successive 10 days.In westem medicine group,valaciclovir was taken orally 300 mg every time,twice every day for successive 10 days.Results:The time of stopping herpes,relieving pain andscabbing in acupuncture and medicine group was significantly lower than that in western group.Conclusion:Local encircled acupuncture plus valaciclovir in treating senile herpes zoster got effects quickly and could effectively shorten the course of disease and reduce the incidence rateofresidual neuralgia.

Objective To study whether vancomycin dosage 2 g daily in Chinese adult patients would achieve the target serum trough concentration of 10-20 mg/L.Methods The data of 122 adult inpatients under therapeutic drug monitoring while receiving vancomycin were collected from September 2011 to December 2012.The correlation between vancomycin trough concentration and dosage,age,sex,body weight,concomitant medications were analyzed retrospectively.Results Vancomycin trough concentration was below the target concentration in 62.1% of the included patients.Age is an independent risk factor for attainting target concentration.Serum concentration was below target concentration in 70.0% of the patients below 60 years of age.Serum concentration was above target concentration in 62.5% of the patients above 60 years of age.Conclusions Clinicians and pharmacists should assess the dosage regimen of vancomycin cautiously,and age should be considered especially as an independent risk factor.

OBJECTIVE: To verify the biocompatibility between diamond-like carbon (DLC) film and human vascular endothelial cells and to provide evidences for construction of artificial mechanical valve prosthesis. METHODS: Nanophase DLC film was deposited using pulse laser deposition, while vascular endothelial cells derived from human umbilical vein was cultured with nanophase DLC film in vitro. Cell growth and adhesion were observed under inverted microscope, and cell proliferation was measured with MIT method. In addition, levels of nitric oxide (NO) and prostacyclin (PGI2) were measured in the DLC group and blank control group in order to evaluate their activities. RESULTS: Adhesion, proliferation, and growth of vascular endothelial cells derived from human umbilical vein were great on the surface of nanophase DLC film. There were no significant differences in the levels of NO and PGI2 between DLC group and blank control group (P > 0.05), showing that nanophase DLC film had no effect on activity of vascular endothelial cells derived from human umbilical vein. CONCLUSION: Nanophase DLC film has a good biocompatibility, and it can become an ideal material of tissue-engineering artificial mechanical valve prosthesis.

ObjectiveTo study the relationship between the susceptibility of mutiple sclerosis (MS) and single nucleotide polymorphisms (SNP) rs6897932 of interleukin-7 receptor alpha (IL-7RA) in Asian.MethodsThe SNP rs6897932 in the IL-7RA gene was genotyped by real time polymerase chain reaction (PCR) using TaqMan SNP Genotyping Assays. 78 cases with MS and neuromyelitis optica (NMO), 187 patients with non-NMO MS and 158 healthy controls were enrolled.ResultsThe frequencies of both the C allele and the CC genotype of SNP rs6897932 in the IL-7RA gene in non-NMO MS patients were higher than those of healthy controls(89.3％ vs. 79.8％, OR=2.12, 95％CI:1.38-3.25 P＜0.01; 78.6％ vs. 63.3％, OR=2.13,95％CI:1.32-3.43, P＜0.01).However, there was no significant difference in the frequency of either the C allele or the CC genotype between control group and NMO patients.ConclusionsIL-7RA gene is one of susceptibility gene for MS in Asian. C allele was presumed as a risk factor of MS while T allele might be a protective factor.

Objective To investigate the diagnostic value of microscopic observation drug susceptibility assay (MODS) in extrapulmonary tuberculosis. Methods MODS technology was constructed by using 24-well cell culture plate and liquid culture.Ziehl-Neelsen smear,Lowenstein-Jensen culture and MODS were used to detect Mycobacterium tuberculosis in 74 pleural fluid samples collected from patients with tuberculous pleurisy,63 cerebrospinal fluid samples collected from patients with tuberculous meningitis and 18 samples collected from non-tuberculosis suspects.The immunochromatography was used to distinguish Mycobacterium tuberculosis from nontuberculosis mycobacteria. The results of Ziehl-Neelsen smear, Lowenstein-Jensen culture and MODS were compared by x2 test.Results The positive rates of MODS,Lowenstein-Jensen culture and Ziehl-Neelsen smear were 58.1 ％ (43/74),18.9 ％ (14/74 ) and 6.8％ (5/74),respectively in tuberculous pleurisy patients; 54.0％(34/63),20.6％ (13/63) and 4.8％ (3/63),respectively in tuberculous meningitis patients.The positive rate of MODS technology was significantly higher than that of Lowenstein Jensen culture in tuberculous pleurisy patients (x2 =24.00,P＜0.01) and tuberculous meningitis patients (x2 =14.97,P ＜ 0.01). Each Mycobacterium obtained from MODS and Lowenstein-Jensen culture was identified as Mycobacterium tuberculosis by immunochromatography.All of the 18 pleural fluid and cerebrospinal fluid samples which collected from non-tuberculosis suspects were all negative detected by Ziehl-Neelsen smear,Lowenstein-Jensen culture and MODS.The median time to culture positive of MODS was 9 days in cerebrospinal fluid and 14 days in pleural fluid samples,which were both significantly shorter than that of Lowenstein-Jensen culture (31 days in both cerebrospinal fluid and pleural fluid samples). Conclusion Compared to conventional microbiological diagnosis methods,MODS is a rapid detection method of Mycobacterium tuberculosis with a higher positive detection rate,which is suitable for rapid diagnosis of extrapulmonary tuberculosis.

Objective To determine the expression of human protection of telomeres 1 (hPOT1) and its relationship with Helicobacter pylori (HP) infection in gastric carcinoma.Methods The expressions of hPOT1 protein and hPOT mRNA were detected in 53 gastric carcinoma specimens (observed group) and 20 normal gastric mucosa tissues (control group) by SP immunohistochemical method and in situ hybridization (ISH), respectively.HP infection was examined by Warthin-Starry method in observed group.Results The positive expression rate of hPOT1 protein was 84.91％ (45/53) in observed group, higher than that in control group [30.00 ％ (6/20)] (P ＜ 0.01).The positive expression rate of hPOT1 mRNA was 58.49 ％ (31/53) in observed group, higher than that in control group [10.00 ％ (2/20)] (P ＜ 0.05).The positive co-expression rate of hPOT1 protein and mRNA was 56.60 ％ (30/53), both had positive relationship in gastric carcinoma (r =0.394, P ＜ 0.05).The rate of HP infection in 53 cases of gastric carcinoma was 52.83 ％ (28/53).The positive expression rates of hPOTI protein and mRNA in observed group with HP infection were significantly higher than those in observed group without HP infection[protein: 96.43 ％ (27/28) vs 72.00 ％ (18/25), P ＜0.05;mRNA: 85.75 ％ (24/28) vs 28.00 ％ (7/25), P ＜ 0.01].Conclusions hPOT1 may be associated with occurrence of gastric carcinoma.Combined detection of hPOT1 protein and mRNA can be used for the diagnosis of gastric carcinoma.HP infection may be associated with abnormal expression of hPOT1 in occurrence of gastric carcinoma.
