1.Inflammatory response in cardiac remodeling after myocardial infarction

The remodeling and reparative process post myocardial infarction (MI) can be divided into three phases:the inflammatory phase,the proliferative phase and the stable phase.The inflammatory immune response plays an important part in the process of cardiac remodeling.First of all,the initiation of inflammatory response relies on the activation of innate immunity with a group of pro-inflammatory cytokines,chemokines and adhesion molecules.These molecules lead to the infiltration of the infarct area with neutrophils and mononuclear cells,further clearing the wound from dead cardiomyocytes and matrix debris.After resolution of inflammatory response,reparative cells and cytokines infiltrate into the heart and promote the differentiation and growth of myofibroblasts and endothelial cells,contributing to wound contraction as well as producing fiber tissue to form a scar.Moreover,overactive immune responses could accentuate infarct injury and dilative remodeling while deficiency of inflammation leads to insufficient repair,which highlights the dual function of the immune response in myocardial injury and repair post MI.Also the intense immune response along with fibrosis in non-infarct area is also closely associated with adverse remodeling.Thus,targeting specific factors in the inflammatory reaction may hold promise in patients with MI.
2.Biological characterization and literature review of Robinsoniella peoriensis from bloodstream infection

Abstract: Objective To analyze the phenotype and drug resistance of Robinsoniella peoriensis strains isolated from the blood of patients with prostate cancer and to learn the epidemiological characteristics of the strains. Methods Culture medium growth characteristics analysis, Gram staining, VITEK MS mass spectrometry identification, in vitro drug susceptibility test, 16S rRNA gene sequencing were performed on the strains, and case summary analysis, historical drug sensitivity results comparison and phylogenetic tree construction were carried out. Results Four of the repeatability tests of mass spectrometry identification were R. peoriensis, and the identification accuracy was 99.9%, which was the first time that mass spectrometry analysis in China accurately detected this strain. The 16S rRNA gene sequencing confirmed that the strain was R. peoriensis, and GenBank accession number is OL826796. There are currently 18 cases of R. peoriensis related to human infection in the world, mainly including bloodstream infection, prosthetic joint infection, and postoperative wound infection. The homology of OL826796 in this case with HGUE-09/943 (GU322806.1) isolated in Spanish was 99.58%; in vitro drug susceptibility showed that OL826796 was resistant to penicillin and clindamycin, and sensitive to vancomycin, imipenem, tetracycline and metronidazole. Statistical analysis of drug susceptibility of 18 cases found that R. peoriensis could be tested for drug susceptibility by E-test method: penicillin 100% (7/7), clindamycin 70% (7/10), ampenem 0% (0/4), metronidazole 0% (0/9), meropenem 0% (0/4), vancomycin 0% (0/3). Conclusion R. peoriensis is a rare anaerobic-positive bacillus. When sterile site infection occurs, attention should be paid to timely communication with clinical reports, and penicillin and clindamycin should be used cautiously to fight infection, so as to improve the cure rate of postoperative immunocompromised patients.
3.Analysis of ADR/ADE Reports of Respiratory Department in a Class Three Grade A Hospital during 2011 to 2015

OBJECTIVE:To provide reference for rational drug use in respiratory department. METHODS:ADR/ADE report-ed by respiratory department of a class three grade A hospital during Jan. 2011 to Dec. 2015 from national ADR monitoring system were collected. 2011-2015 ADR/ADE database of respiratory department in a hospital was established and analyzed statistically. RE-SULTS:202 records were gotten,among which 89 cases were male(44.06%)and 113 cases were female(55.94%);the patients’ age approximated normal distribution,the incidence of ADR/ADE was the highest in 61-80 age group (55.94%);the occurrence time of ADR/ADE was mostly lower than 1 h (47.21%);the main route of administration was intravenous drip (71.29%);22 kinds of drugs were involved,and antibiotics took up the lead(43.11%);84 patients(41.58%)used 2 and more kinds of drugs before the occarrence of ADR/ADE;organs/systems involved in ADR/ADE were skin and its appendants (32.86%) and digestive system(26.19%). Most of the ADR/ADE could be cured or recovered after related treatment. CONCLUSIONS:The occurrence of ADR/ADE in respiratory department is related to various factors,of which advanced age,intravenous administration and antibiotics use are important ones. Great importance should be attached to rational clinical drug use as well as ADR/ADE monitoring and re-porting. Clinical also should pay attention to the safety of drug use in elderly patients.
4.Food intolerance and human immune function

Objective To discuss current detection of food intolerance in health check-up populations and the influence of food intolerance on immune system and inflammation parameters.Methods Serum levels of 14 kinds of food-specific IgG antibodies were analyzed by using enzyme linked immunosorbent assay (ELISA) among 257 healthy check-up adults.White blood cell count,eosinophils,globulin,erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),rheumatoid factor (RF),antinuclear antibodies (ANA) and T cell function were compared between the positive group and the negative group.Student's t test and Chi-square test were used for data analysis.Results Food-specific IgG antibodies were found in 134 participants (52.14%).The most common intolerant foods were egg,crab and milk.Moderate to severe food intolerance was caused by egg or milk.There was significant difference in eosinophils and serum globulin between the positive and the negative group (t =-0.07,x2 =8.91,both P <0.05).However,no difference was observed in white blood cell count,ESR,CRP,RF,ANA and T cell function between two groups (all P > 0.05).Conclusion Eosinophils and serum globulin may significantly increased in people with food intolerance,although white blood cell count,ESR,CRP,RF,ANA and T cell function could be normal.
5.Bioethnopharmaceutical analytical pharmacology:a strategy for elucidating ethnoherbal absorbed bioactive compounds

To review the background and advance of a strategy of bioethnopharmaceutical analytical pharmacology(BAP).BAP is an idea for elucidating absorbed bioactive compound(ABC)in traditional Chinese medicine(TCM)formula or herbal remedy.It is also a mode of active comparison between absorbed compounds and the parent formula including determination of ABC in blood,the ABC dosage identical to its content in formula,and the active comparison of ABC and the parent formula.The ABC of Guan-Xin-Er-Hao formula elucidated by BAP strategy has been reviewed and the significance of BAP strategy will be further discussed.
6.Function disorder assessment on patients with mild psychiatric impairment due to road traffic accidents.

OBJECTIVE:
To explore the methods to assess the function disorder of patients with mild psychiatric impairment due to road traffic accidents.
METHODS:
In contrast to that of the patient with physical impairment, the function disorder of the patient with mild psychiatric impairment was assessed by the standard assessment instruments, such as the Barthel index of ADL (BI), Functional Activities Questionnaire (FAQ), Social Disability Screening Schedule (SDSS), Activity of Daily Living Scale (ADL), Physical Self-maintenance Scale (PSMS), and Instrumental Activities of Daily Living Scale (IADL).
RESULTS:
Except BI, the each total score of FAQ, SDSS, ADL, PSMS, and IADL correlated well with the rank of psychiatric impairment. The difference of each total score of rating scales among different physical impairment rank was not statistically significant. At the impairment rank of level 10, the difference of each total score of FAQ and ADL was not significant between the two groups; each total score of BI, SDSS, and IADL in psychiatric impairment was higher than that of physical impairment, while the total score of PSMS in psychiatric impairment was lower than that of physical impairment. At the level 9, except PSMS, the each total score of BI, FAQ, SDSS, IADL and ADL in psychiatric impairment was higher than that of physical impairment. At the level 8, except BI and PSMS, the each total score of FAQ, SDSS, IADL and ADL in psychiatric impairment was higher than that of physical impairment.
CONCLUSION
The different criteria for assessment of mild psychiatric and physical impairment may exist, and appropriate indexes for assessment between physical and psychiatric impairment need to be further elucidated.

7.Agmatine inhibits the afferent activity of carotid baroreceptor in rats

The effect of agmatine (Agm) on the carotid baroreceptor activity was examined in 24 anesthetized rats with perfused isolated carotid sinus by recording sinus nerve afferent discharges. The results are as follows. (1) By perfusing with 1 mmol/L Agm, the functional curve for the intrasinus pressure (ISP)integral of sinus nerve activiy (ISNA) relation was shifted to the right and downward with decreases in peak slope (PS) and peak integral value of carotid sinus afferent discharge (PIV). By perfusing with high concentrations of Agm (5 and 10 mmol/L), the curves were shifted to the right and downward further with marked decreases of PS and PIV. These results showed that Agm exerted an inhibitory action on the baroreceptor activity in a dose-dependent manner. (2) The Agm-induced decrease in sinus nerve afferent activity was eliminated by pretreatment with IR and α2-AR blocker idazoxan (0.1 mmol/L). (3) Selective α2-AR blocker yohimbine (15 μmol/L) partly abolished the inhibitory effect of Agm on baroreceptor. (4) Preperfusing with Bay K 8644 (500 nmol/L), an agonist of calcium channels, also eliminated the effect of Agm on carotid baroreceptor activity. These results indicate that Agm exerts an inhibitory action on carotid baroreceptor and such an action may be attributed to the reduction in calcium influx in carotid baroreceptor, which is mediated by IR and α2-AR.
8.Clinical significance of direct antiglobulin testing in anemia in patients with severe chronic hepatitis B

Objective To evaluate clinical significance of direct antiglobulin testing(DAT)in anemia in patients with severe chronic hepatitis B(CHB).Methods Red blood cell(RBC)-related parameters detection and DAT were performed among 30 healthy persons,30 CHB patients,and 50 severe CHB patients,clinical factors related to posi-tive DAT were analyzed.Results RBC count,hemoglobin (Hb)concentration,and hematocrit(HCT)level in severe CHB patients were all lower than CHB patients and healthy group(P <0.05),RBC distribution width(RDW)in severe CHB patients were all higher than CHB patients and healthy group(P<0.05);the positive rate of DAT in patients with se-vere CHB,CHB,and healthy group were 62.82%,13.33% and 0 respectively.RBC count,Hb concentration,and HCT level in severe CHB patients with positive DAT were all lower than severe CHB patients with negative DAT (all P <0.05),while RDW was higher than the latter (P=0.001);after RBC was separated through capillary,positive intensity of DAT of aged RBCs was higher than young RBCs in severe CHB patients (P <0.001);among severe CHB patients, DAT-positive and-negative patients differed in gender,age,alanine aminotransferase,total bilirubin,complement C3, C-reactive protein,and complication of diabetes(all P≤0.05).Conclusion Anemia in severe CHB patients may be re-lated to immune hemolysis of aged RBCs induced by antibody adsorption.
9.Routine Examination of Aconitum sinomontanum Nakai and Content Determination of Lappacontine and Ranaconitine

Objective To study the routine examination of Aconitum sinomontanum Nakai and determine the contents of lappacontine and ranaconitine; To provide basis for establishing the quality standard of Aconitum sinomontanum Nakai.Methods Aconitum sinomontanum Nakai were collected from different areas.A method of TLC was used for qualitative discrimination. The methods in the Chinese Pharmacopoeia were adopted for the determination of moisture content, ash content and extractives. Determination of lappacontine and ranaconitine were performed by HPLC. Results The TLC showed that the spots were clear and the separation was good. Individual provisional standards:the moisture,total ash and acid-insoluble ash content of Aconitum sinomontanum Nakai were not more than 11.0%, 12.0%, and 7.0%, respectively; water soluble and alcohol soluble extractives were not less than 18.2% and 10.6%,respectively.The content of ranaconitine and lappacontine in Aconitum sinomontanum Nakai were not less than 0.125% and 0.815%, respectively. Conclusion The method established by the study is accurate and reliable,and can be used for quality evaluation of Aconitum sinomontanum Nakai.
