2.Diagnostic value of serum HE4 and YKL-40 for the ovarian cancer

Objective To study the significance of serum human epididymis protein 4 ( HE4 ) and human cartilage glycoprotein-39(YKL-40) in the diagnosis of ovarian cancer .Methods 100 patients with ovarian cancer , 90 patients with benign tumor of ovary and 50 healthy people were enrolled .Serum HE4 and YKL-40 were detected . Results The positive rates of serum HE 4 in ovarian cancer group , ovarian benign tumor group and healthy group were 86.0%,5.5%,2.0%.The positive rate of serum HE4 in ovarian carcinoma was significantly higher than that in benign ovarian tumor group and the healthy group (χ2 =31.06,30.82,all P<0.01).The positive rates of serum YKL-40 in ovarian cancer group ,ovarian benign tumor group and healthy group were 82.0%,3.3%,2.0%.The positive rate of serum YKL-40 in ovarian carcinoma was significantly higher than that in benign ovarian tumor group and the healthy group(χ2 =30.92,32.06,all P<0.01).Serum HE4 in the diagnosis of ovarian cancer ,the sensitivity was 86.0%,specificity of 95.7%,accuracy of 91.7%.Serum YKL-40 in the diagnosis of ovarian cancer ,the sensitivity was 82.0%,specificity of 97.1%,accuracy of 90.8%.For the joint detection,the sensitivity for diagnosis of ovarian cancer was 92.0%,and the specificity was 93.6%,accuracy was 95.0%,higher than those of the single detection . Conclusion Combined with serum HE 4 and YKL-40 detection can significantly improve the diagnostic accuracy of ovarian cancer patients .
3.Study of informing cancer patients of the disease with the optimized security strategy based on AQ score

ObjectiveTo study the optimized security strategy for informing cancer patients with the disease,and let patients be fully informed and even more actively cooperate with doctors while they are under treatment.MethodsGrouped with adversity quotient (AQ) score,these cancer patients were in comparison with those who had been informed by other ways without support strategy,and different groups of patients were estimated in different strategies in accordance with self-rating anxiety scale (SAS) and self-rating depression scale (SDS).Results With certain strategies,high and low AQ score groups' SAS and SDS scores stably restored in 1 day to 1 week,and the differences had statistical significance(P < 0.01 ).The patients were well-informed without any difficulty and actively cooperated with treatment.No related factors led to self-mutilation,suicide and other serious incidents.While the SAS and SDS scores of the control group who were not informed by the strategy were not satisfactorily recovered,and the patients could not actively cooperate with the doctors.Especially being notified after 1 d to 1 week,the SAS and SDS scores had no significant improvement,and the differences had no statistical significance compared with themselves ( P > 0.05 ).It is difficult to avoid the occurrence of serious incidents.Conclusion Based on AQ score,it is safe and effective to inform cancer patients of their disease condition by individual strategies.It not only respects patients rights,but also gains the active cooperation of the patients,and is helpful for treatment with minimizing adverse effects.
4.Oxidative stress response in diabetic nephropathy rats following injection of embryonic stem cells via the tail vein

BACKGROUND:Occurrence and development of diabetic complications is closely related to the severity of oxidative stress imbalance in the body. OBJECTIVE:To investigate the effect of embryonic stem cel s on oxidative stress response of rats with diabetic nephropathy. METHODS:Primarily cultured rat embryonic stem cel s were observed for cel morphology and surface antigen detection. Sprague-Dawley rats were divided into experimental group (two injections of embryonic stem cel s via the tail vein), model group (injection of the same volume of PBS), and normal control group (with no modeling, intraperitoneal injection of sodium citrate-citrate buffer). In the former two groups, the rats were intraperitoneal y injected sodium citrate-citrate buffer diluted streptozotocin to establish diabetic nephropathy models before treatment. At 5 weeks after the last injection, blood glucose level, renal function indicators (urine protein/urine creatinine, blood urea nitrogen and serum creatinine) were tested in each group;contents of malondialdehyde and protein carbonyl were detected in the kidney;the expression level of superoxide dismutase was detected by western blot assay. RESULTS AND CONCLUSION:The embryonic stem cel s were oval or round, with clear boundary and good refraction, and highly expressed Oct-4 and SSEA-1. Compared with the control group, renal biochemical indicators, malondialdehyde and protein carbonyl contents were significantly increased, while the expression level of superoxide dismutase was decreased dramatical y in the model group and experimental group (P<0.05);compared with the model group, the renal biochemical indicators, malondialdehyde and protein carbonyl contents were dropped significantly in the experimental group, but the expression of superoxide dismutase was significantly rebounded (P<0.05). Taken together, embryonic stem cel s can reverse the occurrence and development of diabetic nephropathy by inhibiting oxidative stress in progress.

7.Clinical Observation of Small-dose Argatroban vs. Aspirin in the Treatment of Acute Cerebral Infarction

OBJECTIVE:To observe the effect and safety of small-dose argatroban vs. aspirin in the treatment of acute cerebral infarction. METHODS:136 patients with acute cerebral infarction were randomly divided into observation group and control group. All patients were given routine treatment,such as anti-intracranial pressure,oxidative stress,brain protection,oxygen,blood pres-sure,blood sugar control,anti-infective,water and electrolyte acid-base balance,etc. Based on it,control group was treated with Arginine aspirin for injection 100 mg,adding into 0.9% Sodium chloride injection 250 ml,iv,once a day. Observation group was treated with Argatroban injection 40 mg,adding into 0.9% Sodium chloride injection 500 ml,24 h continuous infusion for continu-ous 2 d,iv;then dose was decreased to 10 mg,adding into 0.9% Sodium chloride injection 100 ml,iv,once a day,for continu-ous 5 d. The course of both was 7 d. The clinic data was observed,including clinical efficacy,NIHSS(National Institutes of Health Stroke Scale)score,Bathel index scores before and after treatment,and incidence of adverse reactions. The recurrence rate of cere-bral infraction during the 2-year follow-up period was observed. RESULTS:The total effective rate in observation group was signifi-cantly higher than control group,the recurrence rates of cerebral infarction in observation group within 1 and 2 year(s)were signifi-cantly lower than control group,with significant difference(P<0.05). After treatment,the NIHSS score in 2 groups were signifi-cantly lower than before,observation group was lower than control group;Bathel score was significantly higher than before,and observation group was higher than control group,with significant differences(P<0.05). There were no significant differences in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Based on the conventional treatment,compared with aspirin, small-dose argatroban can significantly commute the nerve function of acute cerebral infarction,and reduce the recurrence rate of cerebral infarction,with good safety.
8.Analysis of 80 Cases of Dezocine-induced ADR in Orthopedics Department of Our Hospital

OBJECTIVE:To provide reference for rational use of Dezocine injection in the clinic. METHODS:80 cases of dezo-cine-induced ADR reported by orthopedics department of our hospital during Jan.-Dec. in 2014 were analyzed statistically in terms of age,gender,clinical manifestations,etc. RESULTS:80 cases of dezocine-induced ADR showed that the incidence of middle-aged and elderly patients aged 40 years above was higher(58.75%),especially women. Organs or systems involved in ADR mainly were diges-tive system(57.50%),followed by nervous system(16.25%)and systemic damage reaction(13.75%). 67 cases of ADR occurred within 2 h after medication;78 cases were improved and 2 cases were cured. CONCLUSIONS:Reasonable and safe use of dezocine is main method to prevent and reduce ADR in medical institutions,and is also the important guarantee of patient's physical and men-tal health and life safety.
9.Interpretation of disinfection procedures based on common hemodialysis machines

Objective:In order to improve operator’s cognitive level for standardized disinfection, this paper will focus on analyzing and interpreting the disinfection procedures for some common dialysis machines.Methods: This paper will interpret and compare the disinfection procedures for three brands of dialysis machines in the following aspects: the choice of the disinfectant, the disinfection method, the concentration of the disinfectant and the injected volume of the disinfectant, etc.Results: The procedures of hot disinfection with citric acid were used completely in those three brands of dialysis machine, and the procedure of sodium hypochlorite need been used periodically to remove the proteins and other organic matter.Conclusion:Although the dialysis machines provide almost complete and efficient disinfection procedures, but the operator should need to select the most suitable procedures according to their practical situation. If the operators cannot master the disinfection procedures and have not noticed the importance of it, the wrong operator will lead to incomplete disinfection, affect the normal operation of the dialysis machines and increase the risk of cross-infection.
10.Application of low dose CT colonography combined with enhanced scan in the diagnosis of colorectal neoplasms

Objective To investigate the clinical value of low dose CT colonography (CTC)combined application of enhanced scanning in the diagnosis of colorectal neoplasms,thus to provide a reference for clinical treatment.Methods 64 patients with suspected colorectal cancer were studied retrospectively,they were divided into two groups according to the diagnostic methods.The observation group(21cases)underwent low -dose CTC joint enhanced scan,the control group(43 cases)were taken usual routine dose CTC joint enhanced scan.The scanning image quality and radiation dose of two groups were compared.Took colonoscopy or pathological examination as diagnostic standard,the accuracy of two groups was compared.Results The excellent image rate of the observation group was 95.24%,which was lower than that of the control group,but the difference between the two groups was not statistically significant(χ2 =0.947,P =0.217).The radiation doses of the observation group were (1024.48 ± 98.64)mGy/cm,(15.37 ±1.48)mSv,which were significantly lower than those of the control group[(1411.28 ± 159.57)mGy/cm and (21.17 ±2.39)mSv],the differences were statistically significant (t =15.447,P =0.000;t =15.447,P =0.000).The diagnostic sensitivity,specificity,positive rate,negative rate of the observation group were 100.00%,85.71%,93.33%,100.00%,there were no statistically significant differences compared with the control group (χ2 =0.000,1.014,0.957 and 0.000,P =1.000,0.098,0.211 and 1.000).Youden index of the observation group and the control group were 0.86 and 0.92.Conclusion CTC combined with low -dose enhanced CT diagnosis for colorectal cancer has similar accuracy with conventional dose,and patients will take less radiation dose,it is worthy of clinical application.
