PURPOSE: To analyze the morphologic changes of the eyelids among different age groups in Korean, and also th find out factors that influence there changes. METHODS: Tow hundred thirty six subjects without any ocular diseases were selected, and sorted by the age (ranging from teenage to the sixties and above), the gender, and according to the existence of the double fold. The pictures of frontal and lateral side of the subjects were taken. After scanning the pictures, MRD1 (marginal reflex distance 1), MRD2 (marginal reflex distance 2), the vertical distance from upper eyelid to eyebrow, the vertical and horizontal distance from medial to lateral canthus, and the horizontal distance between lateral canthus and corneal center were measured using computer-based AutoCAD R14. The measured distances were compared in each subjects. RESULTS: MRD1, MRD2, the vertical distance between medial and lateral canthus, and the horizontal distance between lateral canthus and corneal center were longest in people of the 3rd and 4th decade, and decreased. The horizontal distance between medial and lateral canthus was longest in people of the 5th decade, and decreased thereafter. The vertical distance between upper eyelid and eyebrow was shortest in people of the 4th decade and increased with aging. CONCLUSIONS: It is essential to acknowledge and understand the morphologic changes of the eyelids according to aging so that more satisfactory surgical outcomes may be achieved in cosmetic eyelid operation.

Widened palpebral fissure has been regarded as one of the hallmarks of Bell's palsy. However, the palpebral fissure on the affected side may be narrower than that of the unaffected side. The narrowing of the palpebral fissure has been often thought as a sign caused by weakness of the levator palpebrae and Muller's muscles. However, weakness of the frontal muscle may cause drooping of the eyebrow, resulting in the narrowing of the palpebral fissure. We describe a patient of Bell's palsy with a narrowing of the palpebral fissure.

Difficulties with intraoperative fixation of hair or eyebrow sometimes occur because of displacement or dislodgment of hair during surgery. So we have found that the simple application of a small transparent adhesive disposable dressing, such as Tegaderm(R) or OpSite Flexigrid(R), can prevent this. One of the advantages of this technique, compared with stapler fixation of the hair, is that it does not cause local trauma. An OpSite Flexigrid(R)or Tegaderm(R) is placed such that it covers both the scar and operation site hair. The hair or eyebrow were easily, quickly, and firmly positioned even on the nonshaved scalp. The technique seems to have no drawbacks, and when the Tegaderm(R) or Opsite Flexigrid(R) is removed, patient experiences little discomfort. We have never seen an allergic or direct traumatic cutaneous response following the use of these materials.

There have been no long-term complications or life-threatening adverse effects related to botulinum toxin treatment for any cosmetic indications. Nevertheless, there are well-known, mild side effects of botulinum toxin treatment on the upper face, though most of them are self limited with time. However, excluding brow ptosis, reports about site specific side effects are few and anecdotal. We experienced cases of exaggeration of wrinkles after botulinum toxin injection for forehead horizontal lines, and report them here. In our cases, new appearance of a noticeable glabellar protrusion following botulinum toxin injection on the forehead was observed in 2 patients. Also, a new deep wrinkle on one side of the forehead just above the eyebrow appeared in another 2 patients. The exaggerated wrinkles nearly disappeared without treatment by week 4 in all subjects. These exaggerations of wrinkles may be caused by hyperactivity and overcompensation of untreated muscles. With the increasing availability of diverse botulinum toxin for cosmetic purposes, physicians and patients should be aware of this temporary change after therapeutic injections. We recommend explaining this possible effect prior to injection, for better understanding of treatment for cosmetic indications.

Scars on hair-bearing skin such as the eyebrow produces an unsightly cosmetic appearance because of the absence of hairs on the area. Micropigmentation can be used to overcome the problem effectively because surgical revision, hair transplantation and cosmetic camouflage have some limitations. The use of micropigmentation for medical purpose in Korea has been growing rapidly, but eyebrow scars treated by micropigmnetation have not yet been described in the Korean literature. In this report, we present three cases of eyebrow scars which were treated successfully with micropigmentation.

Alopecia areata(AA) is a common skin disorder with no uniformly successful form of therapy. We experienced two cases of AA on eyebrow improved by superficial cryotherapy with liquid nitrogen. Superficial cryotherapy can be used for the treatment of AA on eyebrow with excellent results.
