127 cirrhotic patients in Hospital No 198 from 1996 to 2002 were enrolled in this study. Analysing the rate and degree of gastrointestinal bleeding in accordance with risk factors, the results showed that 28% of cirrhotic patients had experienced gastrointestinal bleeding. The more severe cirrhosis was, the higher rate of gastrointestinal bleeding. Alcohol addiction increased the rate of gastrointestinal bleeding rather than hepatitis B virus and hepatitis C virus. When combined 2 factors, the rate of gastrointestinal bleeding increased 1.5 times. The rate of gastrointestinal bleeding increased according to degree of esophageal varices

Aims: Esophageal replacement by gastric tube is present the procedure of choice in surgical treatment of carcinoma of esophagus. We would like to present our experiences in esophageal replacement by gastric tube without pyloroplasty in the Gastrointestinal Surgical Division at Binh Dan Hospital. Results: During 22 years (1979-2000), we performed 80 cases of esophagoplasty by gastric tube without pyloroplasty for 3 females and 77 males. The patient mean age was 61 years old. There were 51 cases of cancer of middle 1/3, 27 cases of cancer of the lower 1/3 and 2 cases of cancer of the cardio with esophageal extension. We had 24 cases of total esophagectomy, 29 cases of blunt esophagestomy and 27 cases of esophageal reconstruction by gastric tube. In 53 cases, gastric tube was in posterior mediastinum and 27 cases in substernal position. There were 15 patients presented postoperative pneumonia, 8 cases of anastomotic leakage in the neck, 4 cases of pneumothorax, 1 case with splenic rupture, 1 case of jejunal perforation, 2 cases of leakage of abdominal esophageal stump. The mean hospital stay was 20 days. No gastric emptying trouble was noted in this study. The functional results were good in patients survived this operation. Conclusion: The vascularization of the stomach was very good. In our experiences, esophageal reconstruction by gastric tube foiling Akiyama technique without pyloroplasty was suitable for all cases of esophageal cancer.

6500 patients were performed gastroscopy at Hospital No 198 from 1997 to 2002. The results showed that 19 patients (0.29%) experienced gastric cancer. Common clinical symptoms are belch, difficulty of swallow, choking, pain under breastbone. 42.1% of patients had gastric cancer on middle third, 36.8% on lower third, and 21% on upper third. Funggosity is most common. Carcinoma accounts for 68.4%. Only 3/19 cases could be operated. So it's important to screen for endoscopy in order to find early gastric cancer

The study carried out 28 patients whose endoscopic images were doubted esophageal carcinoma. These patients were diagnosing and staging by endoscopic ultrasonography. The results show that 24 cases were diagnosed esophageal carcinoma by endoscopic ultrasonography in which 23 cases were reidentified by biopsy or surgery. One case was not reidentified because this patient has no surgery and biopsy (-). 4 cases were diagnosed non esophageal carcinoma by endoscopic ultrasonography and reidentified by surgery, biopsy and treatment. 12 cases were operated to evaluated the efficacy of endoscopic ultrasonography in infiltrative stage and local lymph node. This data pointed out that the true endoscopic of ultrasonography was 10/12 (84%) in T stage and 9/11 (81%) in N stage. The study showed primarily that endoscopic ultrasonography was a quanlitative method in diagnosis of esophageal carcinoma.

Esophageal perforation is relatively uncommon but it often cause fatal if not properly treated, and it is associated with high morbidity and mortality. We report here on two cases of esophageal perforation caused by Boerhaave syndrome or pneumatic dilatation for treating achalasia. The patients were successfully treated with thoracoscopic primary repair and esophagomyotomy.

Killian-Jamieson diverticulum is a rare disease that is seen at the cervical esophagus. It has quite a different pathogenesis and anatomical location compared with that of Zenker's diverticulum. The pathophysiology and strategy for treating Killian-Jamieson diverticulum are not fully understood. We performed surgery using one incision for treating a case of Killian-Jamieson diverticulum and we review the medical literature that's related to this unusual diverticulum.

Killian-Jamieson diverticulum is a rare diverticular disease. This disease differs from Zenker's diverticulum in its location and mechanism. Various treatment modality have been attempted, but traditional surgical treatment has been recommended for a symptomatic Killian-Jamieson diverticulum due to the concern of possible nerve injury. We performed surgical treatment by cervical incision. We report here on a case of Killian-Jamieson diverticulum and we briefly review the relevant literature.

119 patient with the peptic cancer received the total gastrectomy, participated to a study after 6-12 months of operation, were recovered the digestive functions as 2 different methods. The results of esophageal endoscopy and biosy only allowed to determine the levels of lesion but not allow affirm that these lesions due to reflu. However, there were the close relation between the general picture according to the description of boyce and microscopic picture according to the description of Beigi I with the clinical symptoms of the reflux esophagitis. Therefore, they also can be used as a standard of diagnosis. The method of the digestive functional recovering by making the distribution bag DD type Lygidakis showed the obvious efficacy in the limitation of the esophageal mucosal lesion due to the postoperative reflux
