1.Effect of Shugan Jiannao Tiaoyu Tablets on expression of hippocamp C-FOS and C-JUN protein in post-stroke depression rat

Objective:To study the anti-depression mechanism of Shugan Jiannao Tiaoyu Tablets(SJTT) and provide evidene for clinical application. Methods: Depressive rat model were established by unpredictable stress with raising alone. Model rats were randomly divided into 5 groups, each 12, then each group was administrated through intragastric perfusion daily with the rate of 1.0ml/100g according to their weight before stimulation: fluoxetine group with 0.75 mg/kg, low-dose group of SJTT with 1.8g/kg, high-dose group of SJTT with 3.6g/kg, normal group and model group with 1.0ml/100g of isotonic Na chloride. The whole course lasted for 21 days from the first day of model making to the day of death. The pathologic character of hippocampus was observed by light microscopic examination. The change of cell ultramicrostructure was observed by transmission electron microscope. The expression of C-FOS and C-JUN were detected by immunohistochemistry. Results: SJTT could reduce the neuron damage of hippocam. Compared with model group, the gray scale of immunoreaction positve cells of C-FOS and C-JUN increased in high dose group of SJTT. Conclusion:SJTT can decrease the neuron damage induced by stress in hippacam and had anti-depression effect.
2.Feasibility of intentional occlusion of the left subclavian artery in endovascular aortic repair of thoracic aortic disease

Objective To investigate the feasibility and effect of intentional occlusion of the left subclavian artery (LSA) in endovascular aortic repair EVAR of thoracic aortic disease.Methods The cases,who needed occlusion of the LSA in EVAR of thoracic aortic disease,were examined cerebral circulation,carotid artery,vertebral basilar artery and circle of Willis.If the vertebral artery had a good blood pathway,and carotid artery and circle of Willis had no stenotic,we occluded the LSA directly in EVAR,and observed the brain and upper limb ischemia complications postoperation.Results Among 40 cases who were occluded the LSA directly in EVAR,28 cases were successful with no neurological complications and left upper limb ischemia symptoms postoperation,12 patients with mild symptoms of subclavian steal syndrome and neurdogical complications,but none required and secondary surgical intervention.Conclusions Prior to intentional LSA occlusion,attention must be paid to potential supraaortic variants and pathologies.Only in this way,it was safe and effective to occlude the LSA in EVAR of thoracic aortic disease.
3.CT-study on branches artery thoracic aorta

Objective To provide vessel anatomical materials guidance for endovascular aortic repair,the branches artery of thoracic aortic was studied by CT angiography (CTA).Methods From January 2008 to February 2012,739 adult cases' CTA data were collected,all cases performing thoracic CTA in Wuhan General Hospital of Guangzhou Command.We measured the diameter and/or leugth of the ascending aorta,aortic arch and branches artery of aortic arch,and made an analysis.Results The aortic arch includes standard and variant types.Standard type is common,which accounted for 91.1% of the total number,while variant type accounted for 8.9%.In the standard aortic arch of patients,the diameter of aortic arch above the opening of coronary artery (CA) was (35.7 ±4.3) mm,the diameter of ascending aortic arch at the opening of brachiocephalic trunk (BCT) was (33.6 ±4.2) mm,the diameter of aortic arch between the BCT and the left common carotid artery (LCCA) was (29.4 ± 5.7) mm,the diameter of aortic arch between the LCCA and the left subclavian artery (LSA) was (27.6 ± 4.2) mm,the diameter of descending aortic at the opening of the LSA was (25.4 ± 4.5) mm,the diameter of the head BCT from aortic arch was (12.9 ±0.9) mm,the diameter of the head LCCA from aortic arch was (8.5 ± 0.7) mm,the diameter of the head LSA from aortic arch was (10.4 ± 1.1) mm,the length of aorta between the CA and the BCT was (53.3 ±12.5) mm,the length of aortic between the BCT and the LCCA was (4.7 ± 1.5) mm,the length of aortic between the LCCA and the LSA was (7.9 ± 2.6) mm,the length between the opening of BCT and the right subclavian artery (RSA) was (41.1 ± 8.2) mm,the length between the opening of LSA and the opening of left vertebral artery was (38.5 ±5.7) mm,the angle between the horizontal of BCT and the LCCA and the sagittal plane was (71.2 ± 7.2) °,the angle between the plane of LCCA and the LSA and the plane of sagittal was (31.1 ± 2.9)°.Conclusions The CT data of the thoracic aorta can be used as reference for production of stents and guide releasing the stents in endovascular repair.
4.Clinical Efficacy of Thoracic Endovascular Aortic Repair for Treating the Patients With Complicated Thoracic Aortic Dissection

Objective: To investigate the clinical efifcacy of thoracic endovascular aortic repair (TEVAR) for treating the patients with complicated thoracic aortic dissection and to report the relevant clinical experiences. Methods: We retrospectively analyzed the records of 56 patients with complicated thoracic aortic dissection who received the operation of TEVAR in our hospital from 2011-02 to 2015-10 in order to analyze their operation methods with reasons. Results: There were 21 patients with complex anatomic conditions for aortic arch, 7 of them received TEVAR and 14 received TEVAR with covered left subclavian artery; 16 patients with complex shape of aortic dissection, 5 of them received TEVAR and 11 received TEVAR with covered left subclavian artery; 19 patients with aortic dissection involving aortic arch, 17 of them received hybrid procedures of aortic arch branch bypass surgery + TEVAR and 2 received TEVAR with fenestrated stent grafts. All operations were successful and no severe complications occurred. Conclusion: TEVAR is a fast and effective method for treating complicated thoracic aortic dissection, which may expand the application ranges as covering left subclavian artery, branch vessel bypass and modiifed stent grafting.
5.Clinical studying of thoracic endovascular aortic repair for treating multiple tears Stanford type B thoracic aortic dissection

Objective This study aims to analysis in the clinical features of Stanford type B thoracic aortic dissection with multiple tears,and to explore the clinical methods of thoracic endovascular aortic repair (TEVAR) treating this disease.Methods From February 2011 to May 2015,the cases that diagnosed with multiple tears (≥ two tears) Stanford type B thoracic aortic dissection and accepted operations with TEVAR at department of Thoracic Cardiovascular Surgery,Wuhan General Hospital of Guangzhou Command,were retrospective analyzed their clinical data.Except the first tear of proximal,cases were named for the treatment group whom used surgical methods treating distal aortic dissection tears.Otherwise,the cases were named non-treatment group.We compared the incidence of chest and back pain,the progress of distal aortic dissection,the changes of false lumen with aortic dissection,and the benefit of distal tears in two groups after 6 months after TEVAR.Results A total of 67 cases were recruited this retrospective study,successful operations with TEVAR were conducted in all patients,and no serious complications occurred postoperatively in all patients.7 cases with treatment group,60 cases with non-treatment group,no deaths within two groups in 6 months after TEVAR.There were no significance statistically in complained of the incidence of chest and back pain,and the rate of progress with distal aortic dissection (P > 0.05).There was statistically significant in the rate of changes with false lumen with aortic dissection false lumen (P < 0.05).Some cases with abdominal visceral artery blood flow from the distal tears of aortic dissection in the non-treatment (n =19).Conclusions TEVAR is an effective method for treating multiple tears Stanford type B thoracic aortic dissection.Distal tears should be individualized treatment according to the characteristics of Stanford type B thoracic aortic dissection or try not to deal with.

7.Effects of the PEMFs of different intensity on BMD and biomechanical properties of rabbits' femur.

The effects of the pulsed electromagnetic fields (PEMFs) of different intensity on bone mineral density (BMD) and biomechanical properties of rabbits' femur had been studied. Twenty-seven female white big ear rabbits were randomly divided into three groups. The magnetic groups were fed in 15 Hz PEMFs, which pulse duration was set to be 5 ms (6 h x d(-1)), the magnetic intensity was 10 x 10(-4) T and the other was 20 x 10(-4) T. Control group were just fed in coils, and the instrument of PEMFs was powered off. After six weeks, by examine BMD and biomechanical properties of the rabbits' femur, the effects of these PEMFs were studied. Compared with control group, the values of BMD, maximum load and structural rigidity of magnetic group were significantly increased (P < 0.05). In addition, there was significant increase in values of BMD and structural rigidity in group 10 x 10(-4) T in comparison with group 20 x 10(-4) T (P < 0.05). PEMFs is effective in improving BMD and biomechanical properties. The experiment indicated that there was evident "window-effect" during the treatment by PEMFs. It is favorable to the treatment and prevention of osteoporosis.

8.Relationship Between NLRP3 Inflammasome and Depression and Intervention Effect of Traditional Chinese Medicine: A Review

Depression, a mental disorder with high incidence, threatens people's health and causes heavy economic burden. In recent years, the research on the pathogenesis of depression is no longer limited to neurotransmitters, hypothalamic-pituitary-adrenal axis, and oxidative stress, and neuroinflammation draws increasing attention. Studies have demonstrated that the NOD-like receptor family, pyrin domain-containing 3 (NLRP3) inflammasome as a bridge connecting psychological stress and depression can initiate inflammatory cascade to induce depression. This article expounds the relationship between NLRP3 inflammasome and depression and introduces the mechanisms and targets of NLRP3 inflammasome in regulating depression. Further, we summarize the research progress in the treatment of depression with traditional Chinese medicine, which regulates NLRP3 inflammasomes to reduce the release of inflammatory cytokines. This review is expected to provide new ideas for the prevention and treatment of depression.
9.Immunogenicity and safety of Sabin strain-based inactivated poliovirus vaccine (sIPV) and typeⅠ+ Ⅲ bivalent oral poliovirus vaccine (bOPV) administered in different schedules

Objective:To compare the safety and immunogenicity of Sabin strain-based inactivated poliovirus vaccine (sIPV) and the liquid form of typeⅠ+ Ⅲ bivalent oral poliovirus vaccine (bOPV) administered to infants aged ≥2 months in different schedules.Methods:A randomized, blinded, single-center, parallel-group controlled trial was conducted in Hangzhou from 2017 to 2018. Healthy infants aged ≥2 months were enrolled and randomized to receive the vaccines on a schedule of 2, 3, 4 months. Group 1sIPV+ 2bOPV was given one dose of sIPV and two doses of bOPV; group 2sIPV+ 1bOPV was administrated two doses of sIPV and one dose of bOPV; group 3sIPV received three doses of sIPV. Adverse events (AEs) following vaccination were recorded. Blood samples were collected from the subjects (excluding the quitters or subjects against the trial plan) 28-35 d after the full-course immunization. A microneutralization assay was performed to detect the geometric mean titers (GMTs) of neutralizing antibodies against polio virus of Ⅰ, Ⅱ and Ⅲ types. The seroconversion rates of neutralizing antibodies were also calculated.Results:The overall incidence of AEs following vaccination was 3.57% in 1sIPV+ 2bOPV group, 3.61% in 2sIPV+ 1bOPV group and 1.19% in 3sIPV group (χ 2=1.190, P=0.552) and no severe AEs were reported. The antibody seroconversion rates in 1sIPV+ 2bOPV, 2sIPV+ 1bOPV and 3sIPV groups were respectively 100% (84/84), 100% (83/83) and 100% (84/84) against type Ⅰ poliovirus, 81% (68/84), 96% (80/83) and 99% (83/84) against type Ⅱ poliovirus(χ 2=21.469, P<0.001), and 100% (84/84), 100% (83/84) and 100% (84/84) against type Ⅲ poliovirus. In 1sIPV+ 2bOPV, 2sIPV+ 1bOPV and 3sIPV groups, the GMTs of antibody were 1 024.00, 1 015.48 and 982.61 against type Ⅰ poliovirus ( F=2.742, P=0.066), 16.81, 107.94 and 218.85 against type Ⅱ poliovirus ( F=33.570, P<0.001), and 990.75, 990.36 and 613.92 against type Ⅲ poliovirus ( F=37.886, P<0.001). Conclusions:sIPV and bOPV administered in different schedules showed good safety and immunogenicity in infants aged≥2 months. The GMT and the seroconversion rate of neutralizing antibody against type Ⅱ poliovirus after vaccination were higher in 2sIPV+ 1bOPV and 3sIPV group than in 1sIPV+ 2bOPV group. Higher GMT of neutralizing antibody against type Ⅲ poliovirus was induced in 1sIPV+ 2bOPV and 2sIPV+ 1bOPV groups than in 3sIPV group.

This paper summarized professor ZHANG Lei′s experience in treating scleroderma from “extraordinary pathogens entering collaterals”. The basic pathogenesis of scleroderma is “extraordinary pathogens entering the collaterals”, and extraordinary pathogens can be divided into external and internal categories. External extraordinary pathogens are mostly exogenous wind, cold and damp pathogens, and the pathogenesis is wind, cold and damp invading skin stria and retaining collaterals. Most of the endogenous extraordinary pathogens are turbid phlegm and blood stasis, and the pathogenesis is endogenous phlegm and stasis leaving the channels and overflowing the collaterals, and blocking the collaterals. Blocked by extraordinary pathogens for a long time, the long illness will lead to deficiency and develop into a syndrome of collaterals excess and channels deficiency. Therefore, professor ZHANG creats Tengluo Beverage (藤络饮) as the basic formula to unblock collaterals and dispel pathogens, and recommends to add or subtract it according to the different syndrome and pathogenic characteristics of the edema stage, sclerosis stage, and atrophy stage. In the edema stage, it is advised to expel wind, remove dampness and unblock the collaterals, while in the sclerosis stage, the method of dissolving phlegm, expelling stasis and unblocking collaterals should be used; in the atrophic stage, it is suggested to differentiate the deficiency of qi, blood, yin and yang, and eliminate extraordinary pathogens on the basis of reinforcing healthy qi .
