2.Gait behavior analysis in rat models of diabetic peripheral neuropathy

Objective To study the real-time gait behavioral changes in rat models of experimental diabetic peripheral neuropathy.Methods Twenty-five SPF Sprague-Dawley rats were randomly divided into experimental group ( 6 males and 7 females) and control group (6 males and 6 females).Diabetes was induced in rats by intraperitoneal injection of streptozotocin ( STZ ) in a dose of 45 mg/kg.The gait behavior in all rats was tested at 12 weeks after diabetes modelling.Results Compared with the control group, the rats with diabetic peripheral neuropathy showed statistically significant different walk cycle extension, walk speed, average print intensity, balance and coordination.The abnormal gait behavior of the rat models was mainly reflected in the increased average and each foot walk cycle extension ( P<0.01 ) , average intensity (P<0.05), absolute average body rotation (P<0.01),and shortened both homologous coupling and homolateral coupling( P <0.05 ) .Conclusions Experimental rat models of diabetic peripheral neuropathy can exhibit obvious changes of gait behavior, and may provide a reference for related clinical and basic research.
3.Adherence Condition of Mycobacterium Tuberculosis on 2 Kinds of Artificial Joint Interface

Objective To compare the adherence condition of mycobacterium tuberculosis on 2 kinds of artificial joint interface. Methods The Chromium-Cobalt-Alloy with hydroxyapatite ceramic coating and porous coating were cocultured with the bacterial liquid of mycobacterium tuberculosis. The optical density of mycobacterium tuberculosis in different interfaces were detected by turbidimetry. Adherence conditions were observed by electron microscope scanning. Results There was no significant difference between two groups in the optical density value (P>0.05). The electron microscope scanning showed that more mycobacterium tuberculosis adhered to the porous coating than hydroxyapatite ceramic coating (P<0.05). Conclusion The hydroxyapatite ceramic coating has more advantages to the porous coating in prosthetic replacement for patients with joint tuberculosis.
4.Clinical studying of thoracic endovascular aortic repair for treating multiple tears Stanford type B thoracic aortic dissection

Objective This study aims to analysis in the clinical features of Stanford type B thoracic aortic dissection with multiple tears,and to explore the clinical methods of thoracic endovascular aortic repair (TEVAR) treating this disease.Methods From February 2011 to May 2015,the cases that diagnosed with multiple tears (≥ two tears) Stanford type B thoracic aortic dissection and accepted operations with TEVAR at department of Thoracic Cardiovascular Surgery,Wuhan General Hospital of Guangzhou Command,were retrospective analyzed their clinical data.Except the first tear of proximal,cases were named for the treatment group whom used surgical methods treating distal aortic dissection tears.Otherwise,the cases were named non-treatment group.We compared the incidence of chest and back pain,the progress of distal aortic dissection,the changes of false lumen with aortic dissection,and the benefit of distal tears in two groups after 6 months after TEVAR.Results A total of 67 cases were recruited this retrospective study,successful operations with TEVAR were conducted in all patients,and no serious complications occurred postoperatively in all patients.7 cases with treatment group,60 cases with non-treatment group,no deaths within two groups in 6 months after TEVAR.There were no significance statistically in complained of the incidence of chest and back pain,and the rate of progress with distal aortic dissection (P > 0.05).There was statistically significant in the rate of changes with false lumen with aortic dissection false lumen (P < 0.05).Some cases with abdominal visceral artery blood flow from the distal tears of aortic dissection in the non-treatment (n =19).Conclusions TEVAR is an effective method for treating multiple tears Stanford type B thoracic aortic dissection.Distal tears should be individualized treatment according to the characteristics of Stanford type B thoracic aortic dissection or try not to deal with.
5.Clinical Efficacy of Thoracic Endovascular Aortic Repair for Treating the Patients With Complicated Thoracic Aortic Dissection

Objective: To investigate the clinical efifcacy of thoracic endovascular aortic repair (TEVAR) for treating the patients with complicated thoracic aortic dissection and to report the relevant clinical experiences. Methods: We retrospectively analyzed the records of 56 patients with complicated thoracic aortic dissection who received the operation of TEVAR in our hospital from 2011-02 to 2015-10 in order to analyze their operation methods with reasons. Results: There were 21 patients with complex anatomic conditions for aortic arch, 7 of them received TEVAR and 14 received TEVAR with covered left subclavian artery; 16 patients with complex shape of aortic dissection, 5 of them received TEVAR and 11 received TEVAR with covered left subclavian artery; 19 patients with aortic dissection involving aortic arch, 17 of them received hybrid procedures of aortic arch branch bypass surgery + TEVAR and 2 received TEVAR with fenestrated stent grafts. All operations were successful and no severe complications occurred. Conclusion: TEVAR is a fast and effective method for treating complicated thoracic aortic dissection, which may expand the application ranges as covering left subclavian artery, branch vessel bypass and modiifed stent grafting.
6.Impact of surface physical properties of artificial joint prosthesis materials on adhesion of Mycobacterium tuberculosis

Objective To study the effect of surface physical properties of different materials for the artificial joint pros -thesis interface on Mycobacterium tuberculosis adhesion .Methods The surface polishing coating , titanium coating and hydroxyapatite coating were chosen as the experimental materials to analyze the surface topography and measure the surface roughness, contact angle and surface energy .The M.tuberculosis strains were used by in vitro cultivation method and ob-served by scanning electron microscopy (SEM) to study the morphology of M.tuberculosis and the material surface adhe-sion.The influence of surface physical properties of the interface of the artificial joint prostheses on the proliferation and ad -hesion of bacteria was evaluatd .Results The titanium coating and hydroxyapatite coating materials were hydrophobic , while the surface polishing coating was hydrophilic .The contact angle and surface energy were significantly different ( P<0.05).In the process of bacterial culture of the three types of materials , there was no significant difference (P>0.05)in the OD value of bacterial solution .The titanium coating had the largest amount of interface adhesion for M.tuberculosis, fol-lowed by the hydroxyapatite coating and the polishing coating .The difference between various materials was statistically sig-nificant (P<0.05).Conclusion The adhesion of M.tuberculosis in the surface of artificial joint prosthesis materials is closely related to the surface physical properties .A adhesiveness improves with surface roughness .
7.Variety and clinical significance of T lymphocyte subsets and cytokines in the peripheral blood of patients with avian influenza H7N9 virus infection

Objective To investigate the changes and clinical significance of peripheral blood T lymphocyte subsets and cytokines in influenza A (H7N9) virus infection patients.Methods Twenty-three patients with H7N9 virus infection who received treatment at the First Affiliated Hospital of Soochow University from April 2013 to April 2015 were enrolled as case group.Twenty healthy subjects in the outpatient clinic during the same period were selected as control group.The percentages of T lymphocyte subsets in the peripheral blood including CD3+CD4+ T cells, CD3+CD8+ T cells, regulatory T lymphocytes (Treg), Th17, CD3+CD8-interferon (IFN)-γ+ (Th1) cells and CD3+CD8-IL-4+ (Th2) cells were detected by flow cytometry.The changes of plasma cytokines including interleukin (IL)-8, IL-18, interferon-inducible protein (IP)-10, macrophage inflammatory protein (MIP)-1b were quantified by Bio-Plex Pro human cytokine Group Ⅰ 27-plex panel and Group Ⅱ 21-plex panel.The normality test was performed by Kolmogorov-Smirnov test.Two independent samples t test were used to compare the two groups.Results The percentages of CD3+CD4+ and CD3+CD8+ T cells before treatment in case group were significantly lower than control group ([27.90±10.19]% vs [38.75±6.78]%, t=-2.726, P=0.012;[14.82±7.72]% vs [22.79±6.12]%, t=-2.556, P=0.018), while the percentages of Th17 and Th2 before treatment in case group were significantly higher than control group ([2.64±1.40]% vs [0.29±0.21]%, t=4.668, P<0.001;[2.24±2.00]% vs [0.35±0.25]%, t=2.626, P=0.014).After treatment, 15 cases achieved improvement, among which the percentages of CD3+CD4+ T cells, CD3+CD8+ T cells and Treg after treatment were significantly higher than those before treatment ([36.54±9.20]% vs [25.86±7.22]%, t=-3.339, P=0.007;[22.70±5.68]% vs [15.08±8.80]%, t=-2.811, P=0.017;[6.08±1.70]% vs [3.26±1.64]%, t=-6.670, P<0.001), while the percentages of Th17 and Th1 cells after treatment were significantly lower than those before treatment ([2.12±1.28]% vs [2.76±1.33]%, t=2.220, P=0.048;[4.00±2.13]% vs [6.97±1.81]%, t=4.407, P=0.001).A total of 8 patients died with no significance differences of all the above mentioned immunological parameters after treatment compared with before treatment (all P>0.05).Before treatment, levels of IL-8, IL-18, IP-10 and MIP-1b of case group were significantly higher than those in control group (IL-8: [23.19±14.35] vs [12.78±6.76] ng/L, t=2.277, P=0.035;IL-18:[230.55±230.18] vs [72.80±27.91] ng/L, t=2.348, P=0.036;IP-10:[28 870.55±41 815.22] vs [1 356.13±1 093.10] ng/L, t=2.371, P=0.035;MIP-1b: [197.74±119.87] vs [118.51±41.86] ng/L, t=2.198, P=0.043).Conclusions Patients with H7N9 virus infection exhibit an imbalance of T lymphocyte subsets.It is very important to monitor the changes of T lymphocyte subsets in those patients for clinical prognosis.A storm of cytokines could exist during H7N9 virus infection, which may be the main reason for multiple organ failure.
8.Development and Application of Thyrocricoid Puncture Device in War and Peace

Objective To develop a thyrocricoid puncture device to study its cure effect in rescuing patients who suffer from the obstruction of upper respiratory tract caused by war wounds,traffic accidents and pharyngolaryngeal tumor.Methods The clinical emergency cure effect of 268 cases of upper respiratory tract obstruction and 33 cases of sudden death were taken retrospective analysis,who received puncture and intubation through cricothyriod membrane ventilation and high-frequency oxygen supply.Results The achievement ratios in puncture and catheterization,oxygen supply,sputum suction and intratracheal administration on all the patients were very high.Conclusion The thyrocricoid puncture device is safe,easy to carry and operate,of mini-wound and effective in rescuing patients suffering from serious upper respiratory tract obstruction and cardiopulmonary resuscitation(CPR),can adapt to the scene of the wounded in war and peace and can be applied in clinic.
9.Effect and clinical significance of glucocorticoid on CD4+CD25+ regulatory T cells in patients with hepatitis B virus-related pre-liver failure.

OBJECTIVETo investigate the effect and clinical significance of glucocorticoid on CD4+CD25+ regulatory T cells (Tregs) in patients with hepatitis B virus (HBV)-related pre-liver failure.
METHODSThe subjects of this study included 78 patients with pre-liver failure induced by HBV (cases) and 24 healthy individuals (controls). Among the 78 cases, 42 received glucocorticoid treatment and 36 did not. Between-group differences in Tregs (in peripheral blood) were evaluated by flow cytometry and statistical analysis.
RESULTSTwo weeks of glucocorticoid treatment led to an increase in Treg level compared to baseline (before therapy: 2.76 ± 0.73 vs. 3.88 ± 1.60). In addition, after the two weeks of glucocorticoid treatment, the Treg level of improved patients was significantly higher than that measured at baseline (before therapy: 2.70 ± 0.77 vs 3.97 ± 1.59, P < 0.05).
CONCLUSIONGlucocorticoids up-regulate the expression of Treg cells, which may contribute to the immunological mechanism that protects pre-liver failure patients from deterioration of their condition. Careful inspection and monitoring of Treg levels may help improve prognosis of these patients.

10.Analysis of family management level and its influencing factors of family with bronchiolitis obliterans children

Objective:To investigate the family management level of children with bronchiolitis obliterans and analyze its influencing factors, so as to provide reference for clinical medical staff to adopt targeted nursing and health education programs.Methods:This was a cross-sectional study. From January 1, 2019, to April 30, 2022, 201 families of bronchiolitis obliterans children hospitalized in Hunan Children's Hospital were selected as the research objects, and the General Data Questionnaire, Family Management Scale, Coping Style Scale of Parents, and Chronic Disease-Related Health Literacy Scale was used to investigate. Single-factor analysis and multiple regression analysis were used to analyze the influencing factors of bronchiolitis obliterans children's family management level.Results:The total score of bronchiolitis obliterans children's family management was (179.67 ± 9.92) points, the total score for parents' coping style was (177.14 ± 22.19) points, and the total score for health literacy was (102.95 ± 8.60) points. Multiple regression analysis showed that age, disease course, family residence, parents' education level, family monthly income, parents' coping style, and health literacy level were the influencing factors of family management level (all P<0.05). Conclusions:The family management ability of parents of children with bronchiolitis obliterans needs to be further improved. It is suggested that medical staff should formulate corresponding measures according to the age, course of the disease, family residence, parents' education level, etc., carry out targeted health education and home management training, improve the parents' health literacy level, and guide them to deal with diseases positively, to improve their family management level and promote the recovery of children's diseases.
