1.The clinical application of standard craniotomy with large bone flap for frontal-temporal severe craniocerebral in jury patients

Objective To explore the curative effect of standard craniotomy with large bone flap of the treatment for frontal-temporal severe craniocerebral injury patients.Methods Standard craniotomy with large bone flap was performed on 63 patients with frontal-temporal severe craniocerebral injury.All postoperative patients were followed up.The curative effect was assessed according to GOS.Results All patients were followed up for 1 year.According to the GOS,28 cases recovered well,12 cases became moderately disabled,4 cases were severely disabled,3 cases was in vegetative state and 16 cases died.The mortality rate was 25.4%.Conclusions Standard craniotomy with large bone flap is the best method of the treatment for frontal-temporal severe craniocerebral injury.Timely and thoroughly standard craniotomy with large bone flap for frontal-temporal severe craniocerebral injury patients can significantly improve the the curative effect of the patients and reduce the rate of mortality.
3.Forearm free skin flap to repair the whole defect of the lip

Objective To sum up transplantation of forearm skin flap to repair the whole defect of the lip.Method Forearm free skin flap was used to repair the whole defect of the lip in 5 cases. Results The treatment was completed in one stage and the results were satisfactory in 5 cases.And the reconstructed lip showed good shape.Conclusions Forearm skin flap has many strong points:the skin flap has big area and wide blood vessel diameter.It is successed easy to be identical,operation can be completed once, and don't suffered harm to face or neighbour supply area.The skin flap has safe grain,more thinner, good springy and similar colour.It plays an important role in the repairs of the whole defect of the lip with nearby skin soft tissue defect.

Objective: To investigate the expression of heat shock response protein(HSP_ 70 ) in human squamous tongue cancer Tca8113 cells during the recovery periods of heat shock.Methods:Tca8113 cells were subjected to heat shock at 43 ℃ for 30 min, then the cells were cultured for 2,4,6,8,12,24 and 48 h respectively. 5.Effect of topically applied recombinant adenovirus of adiponectin on bone defect repair.

OBJECTIVETo investigate the effect of local administration of recombinant adenovirus of human adiponectin (hAPN) Ad-hAPN-EGFP on tibial defect repair of SD rats.
METHODSTibial defect (2 mm) models of 36 SD rats (72 sides) were randomly divided into three groups (A, B, and C; n=24). The three groups were injected with Ad-hAPN-EGFP, Ad-EGFP, and normal saline, respectively, during operation and the day after operation. A week after the operation, the expression of hAPN and osteogenesis-related factors were detected by real-time polymerase chain reaction. Three weeks after operation, the tibias were examined by micro-computed tomography, hematoxylin-eosin staining, and Masson staining to evaluate the restoration of bone defects.
RESULTS1) The hAPN expression was detected in group A but not in groups B and C. Osteogenesis-related factors expression of group A was significantly higher than that of the other groups (P<0.05). 2) Osteogenesis (including bone mineral density, relative bone volume, trabecular number and trabecular thickness) in group A were more evident than those in groups B and C (P<0.05). No significant differences were found between groups B and C (P>0.05).
CONCLUSIONLocal administration of recombinant adenovirus Ad-hAPN-EGFP may be an effective strategy to improve the restoration of bone defects in vivo.

6.Recombinant adenovirus Ad-APN-EGFP constructing and promoting osteogenesis around implants.

OBJECTIVEWe aimed to construct a recombinant adenovirus Ad-APN-EGFP and to evaluate the transfection efficiency and effect of the adenovirus promoting osteogenesis around implants.
METHODSRecombinant adenovirus Ad-APN-EGFP containing adiponectin (APN) was constructed by DNA sequencing and restriction enzyme digestion. Rat bone marrow stem cells (BMSCs) were transfected in vitro. Transfection efficiency and APN mRNA expression were tested. Animal models of rat femoral epiphysis and hydroxyapatite (HA)-coated implants were established. The Ad-APN-EGFP adenovirus at 10 μL was injected into the defect around HA-coated implants in the experimental group, whereas the same volume of phosphate-buffered saline was injected into the defect in the control group. Osteogenesis around implants was observed by hematoxylin-eosin staining four weeks after implantation.
RESULTSAd-APN-EGFP was successfully constructed by DNA sequencing and restriction enzyme digestion. The transfection efficiency of Ad-APN-EGFP to BMSCs was up to ≥ 90%. APN mRNA expression of BMSCs transfected with Ad-APN-EGFP was higher than that of the control group. Osteogenesis in the experimental group was more evident than that in the control group in vivo.
CONCLUSIONAd-APN-EGFP could be transfected into BMSCs and express APN mRNA at a high level. Ad-APN-EGFP could improve osteogenesis around implants in vivo.

7.Two-stage treatment protocol for the management of temporomandibular joint ankylosis with secondary deformities in adults

Objective:To introduce a 2-stage treatment protocol for the management of temporomandibular joint ankylosis with sec-ondary deformities in adults.Methods:24 adult patients (9 males and 15 female)(30 joints)at the average age of 26.1 years un-derwent TMJ reconstruction as the initial surgery,followed by orthodontic treatment and correction of secondary deformities as the sec-ond surgery.Clinical outcome was assessed based on maximal incisal opening,radiography and medical photography.Results:Skele-tal deformities were significantly improved in all patients,satisfactory occlusion was achieved with the orthodontic treatment,average maximal incisal opening increased from 3.4 mm to 32.5 mm(P <0.05).Conclusion:The 2-stage treatment protocol is an effective approach for management of TMJ ankylosis with secondary deformities in adult patients.
8.Primary study on change of STAT3-TLR2 signaling axis in Chlamydia trachomatis persistent infection

Objective:To explore in state of Chlamydia trachomatis persistent infection,the STAT3-TLR2 axis may be activated and mediating abnormal secretion of inflammatory cytokines.Methods: We established acute infection and IFN-γinduced persistent infection model of Ct in HeLa cell.Gene transcription, cytokine secretion and protein expression were detected by using qRT-PCR, ELISA and Western blot respectively in STAT3-TLR2 signaling axis in each Ct infection model.Results: Persistent Ct infections upregulated the transcription of TLR2,significantly increased both the secretion of inflammatory cytokine IL-1αand the expression of STAT3 and TLR2,moreover,enhanced the activation of STAT3 simultaneously.Conclusion: In the Ct persistent infection induced by IFN-γ,the STAT3-TLR2 signaling axis activated significantly in HeLa cell.
9.Surveillance for Respiratory Viruses in Children with Acute Lower Respiratory Infections in Chongqing between 2003 and 2007

Objective To investigate the clinical spectrum of respiratory viruses in infants and young children with acute lower respiratory infection(ALRI) in Chongqing area from 2003-2007.And to assess the clinical diagnostic value of virus detection in nasopharyngeal secretions(NPS) and serum viral antibody detection for ALRI.Methods Cases of 2 529 specimens of NPS in hospitalized children with ALRI from Apr.2003 to Oct.2007 were taken for detecting 7 common respiratory virus antigens by immunofluorescence assay including respiratory syncytial virus (RSV),adenovirus(ADV),influenza A(IA),influenza B (IB),parainfluenza virus1-3 (PIV1,PIV2,PIV3).Fifty-five thousand eight hundred and eighty-seven samples were tested for ADV-IgM by ELISA.Among those,45 159 cases were further tested for RSV-IgM by ELISA.Results Respiratory virus pathogens were detected in 778 samples out of 2 529(30.76%) including RSV positive in 668 samples (85.86%),PIV3 positive in 75 samples (9.64%),IA positive in 22 samples (2.57%),ADV positive in 15 samples ( 1.93%),only 1 sample ( 0.13%) positive for both PIV1 and RSV. And the positive rate of RSV-IgM was 0.9%-15.2%,and the positive rate for ADV-IgM was about 0.6%-10.6%.RSV infection occured mainly in winter and spring.Conclusions Respiratory virus is the most common pathogen in children with ALRI during the survey period in Chongqing area,especially for RSV infection.The pattern of RSV circulation varied every year with seasonality.It is suggest that this year is peak one for RSV infection from the monthly positive results,especially in Feburary(50%) in 2007.But the infection rate of PIV3,IA,ADV and PIV1 are lower,particularly IB and PIV2 infection have not been seen for the last 5 years.It is fast and accurate to detect RSV antigen and suit to clinical diagnosis by using immunofluorescence assay than other antibody detection.
10.Distribution of Staphylococcus aureus strains colonized in healthy community population and molecular epidemiological characteristics for MRSA strains

Objective To investigate the nasal colonization of Staphylococcus (S.) aureus strains among medical university students in Shenyang and to study the molecular epidemiological characteristics of methicillin resistant S. aureus (MRSA) strains. Methods Sterilized nasal swabs were used to collect nasal bacteria from both nares of the students. Nasal specimens were further identified as S. aureus strains, sensitive or resistant to methicillin through a series of tests. Molecular related methods including staphylococcal cassette chromosome mec (SCCmec) typing, pulsed- field gel electrophoresis (PFGE) , coagulase isotyping and minimum inhibitory concentration (MIC) determination etc. were used to characterize the isolates. Prevalence of the panton-valentine leukocidin (pvl) genes (lukS and F-PV) among the isolates was also assessed. Results Staphylococci were found in 488 specimens from 977 participants through the surveillance program, conducted in 2009. Of the 488 specimens being tested, 364 were identified as coagulase-negative staphylococci (CoNS) and 124 as S. aureus. MRSA strain among the S. aureus isolates was accounted for 3.4%. In the surveillance program conducted in 2010, staphylococci grew in 310 specimens fiom 657 participants. Of the 310 specimens tested, 195 were identified as CoNS and 115 as S. aureus. The percentage of MRSA strains among the S. aureus isolates was 7.7%. In total, 239 students carried S.aureus, and the percentage of MRSA carriers among the total specimens tested in this study was 5.1%.Most of the MRSA strains could be classified into one of the five types of SCCmec elements. Type Ⅳ a SCCmec strains were most frequent seen overall (10 isolates). A total of 11 pulsotypes were identified among the MRSA strains and were classified into 7 major groups (A to G) by the mutual correlations of their banding patterns. Ten MRSA strains were identified as pvl positive strains. Conclusion An MRSA clone (Ⅳ a SCCmec pulsotype A) carrying pvl toxin gene was found to be prevalent in the nares of the healthy university students.
