1.Research and study on health of young generation in rural communities in Nagano prefecture.

A check of the health of residents in their twenties and thirties in Nagano Prefecture's rural communities indicates that the rate of hypertensives and the incidence of cholesterolemia were low in those age groups. There were signs that the prevalence of obesity is somewhat high among males but low among females. Virtually no incidence of anemia was observed among men but the incidence was fairly high among women. The number of persons with hepatic dysfunction was relatively large among men. In terms of the intake of nutrients, there were not a few men for whom the meals were imbalanced, and the intake of alcoholic beverages was high.
It is necessary that women should refrain from taking in confectionary and taken in fishmeat, meat, milk, egg, soybeans and other proteins. In Nagano Prefecture's rural communities, there remain signs that the intake of salt is high even in young generation, so that it is important to take measures for the prevention of hypertension.
2.Skin Cancer Screening Project in Saku District, Nagano Prefecture. Effects of Ultraviolet Radiation on Health.

In recent years, an increasing number of researchers have warned that depltion of the Earth's ozone layer by chlorofluorocarbons allows extra ultraviolet radiation from space to strike our planet, causing a rise in the incidence of skin cancers and precancerous dermatoses worldwide. This motivated us to perform a series of screening for skin cancers on the inhabitants at age 40 and above of the Saku District, Nagano Prefecture, where the intensity of the sun's harmful ultraviolet radiation is thought to be relatively high.
The first screening was carried out from 1993 through 1995. The number of subjects totaled 7, 528-2, 658 men and 4, 870 women-which represents 7.4% of the populace. Of the total, 67 people had a histopathological examination. The result revealed that there were four cases of malignancy and 45 cases of precancerous dermatosis. The malignant cases were made up of one case of malignant melanoma and three cases of basal-cell carcinoma, while the precancerous cases were one case of Bowen's disease and 44 cases of solar keratosis.
The follow-up examination was conducted in 1996 on 93 subjects (40.6%) of the 229 examiness who had been found to need to be watched in the first screening. Biopsy was indicated in five subjects. Treatment was suggested to 11 subjects. A visit to the department of dermatology at Sake Central Hospital was recommended to 10 people. Further observation was needed in 24 cases.
The screening project for skin cancers under a long-range plan would raise awareness of the diseases among the people and contribute to the early detection, treatment and prevention of various forms of skin cancer. It could also serve the purpose of protecting the environment from deterioration on n global srnle.

In concequence of the rapid extension of the average life expectancy of the Japanese after World War II, this country has realized a society of longevity. Although longevity is in itself something to be celebrated, many people have misgivings about health in their old age. Advances in medical technology, one of the factors which promoted the society of longevity, continued overcoming various diseases while bringing about many patients troubled with after effects. Also the number of old people suffered from chronic diseases has been increasing gradually.Today, people wish healthy long life from the heart, not simply longevity. Nagano Prefecture has come to be known as the area eminent for long healthy life expectancy in recent years. Behined this, there are various activities aimed at healthier life by citizens. In this paper, we report one of the activities carried out at “Yachiho village” (as a result of the affiliation with Saku machi in 2005 renamed Sakuho machi) located in the eastern part of Nagano Prefecture.The health care program for all villagers, which the government of Yachiho village started in 1959 with cooperation of Saku Central Hospital, produced fruit while breeding many voluntary activities by citizens. We explored the practices of the group worked on the improvement of nutrition, and studied about the essential conditions which have enabled the formation of health promotion activities by citizens.

4.Urinary Incontinence Among the Middle-Aged and Senior Members of a Rural Community.

We investigated the prevalence of, and factors in, urinary incontinence among the people of middle to advanced age, using a yes/no questionnaire. The total number of respondents was 4, 020 (1, 520 males and 2, 500 females). Seventy-five percent of them were over 65 years old (median age: 67 years). The prevalence of urinary incontinence was 14% for the males and 30% for the females. The data were analyzed by standard statistical tests, such as χ2 tests. The factors closely associated with urinary incontinence were aging, the high urinary frequency in a day, a history of cerebrovascular accidents, difficulty of urination in the males and UTI in the females. Urinary incontinence of the urge type account for 64% of the males and that of the stress type 81% of the females. Fifteen percent of the males and 6% of the females had consulted with physicians or urologists about urinary incontinence. These percentages were lower than those in European countries. However, we suspect that there may be much more latent patients who suffer from urinary incontinence in their daily community life.
6.Steroid-refractory extensive enteritis complicated by ulcerative colitis successfully treated with adalimumab.

Extracolonic involvement of the gastrointestinal tract is extremely uncommon in ulcerative colitis (UC) and rarely found in the upper gastrointestinal tract or in postoperative cases since it typically responds to steroids. Here we report a case of UC complicated by extensive ileal inflammation that was refractory to steroids. A 20-year-old man was diagnosed with UC of typical pancolitis without ileal involvement and started treatment with pH-dependent mesalazine and oral prednisolone. Although his symptoms transiently resolved, the condition flared when the steroid dose was tapered down. Computed tomography revealed marked thickening of the ileal wall, and capsule endoscopy and balloon-assisted enteroscopy found diffuse mucosal inflammation with ulcers in the ileum. On the contrary, the inflammation in the colon and rectum was improving. Since the response to the second steroid course was inadequate, treatment with adalimumab and 6-mercaptopurine was initiated and finally achieved clinical and endoscopic remission. The investigation of small intestinal lesions is necessary in patients with UC whose clinical deterioration cannot be explained by colonic lesions.

7.Randomized, crossover questionnaire survey of acceptabilities of controlled-release mesalazine tablets and granules in ulcerative colitis patients

BACKGROUND/AIMS: Oral mesalazine is an important treatment for ulcerative colitis (UC), and non-adherence to mesalazine increases the risk of relapse. Controlled-release (CR) mesalazine has 2 formulations: tablets and granules. The relative acceptabilities of these formulations may influence patient adherence; however, they have not been compared to date. This study aimed to evaluate the acceptabilities of the 2 formulations of CR mesalazine in relation to patient adherence using a crossover questionnaire survey. METHODS: UC patients were randomly assigned to 2 groups in a 1:1 ratio. Patients in each group took either 4 g of CR mesalazine tablets or granules for 6 to 9 weeks, and then switched to 4 g of the other formulation for a further 6 to 9 weeks. The acceptability and efficacy were evaluated by questionnaires, and adherence was assessed using a visual analog scale. The difference in acceptabilities between the 2 formulations and its impact on adherence were assessed. RESULTS: A total of 49 patients were prospectively enrolled and 33 patients were included in the analysis. Significantly more patients found the tablets to be less acceptable than the granules (76% vs. 33%, P=0.0005). The granules were preferable to the tablets when the 2 formulations were compared directly (73% vs. 21%, P=0.004), for their portability, size, and numbers of pills. The adherence rate was slightly better among patients taking the granules (94% vs. 91%) during the observation period, but the difference was not significant (P=0.139). CONCLUSIONS: CR mesalazine granules are more acceptable than tablets, and may therefore be a better option for long-term medication.

9.Individualized treatment based on CYP3A5 single-nucleotide polymorphisms with tacrolimus in ulcerative colitis

BACKGROUND/AIMS: The pharmacokinetics of tacrolimus (TAC) is known to be largely influenced by single-nucleotide polymorphisms (SNPs) in CYP3A5. Patients starting TAC require careful dose adjustment, owing to the wide range of optimal dosages, depending on their CYP3A5 expression status. Here, we evaluated whether individualization of TAC dosages based on CYP3A5 SNPs would improve its therapeutic efficacy in ulcerative colitis. METHODS: Twenty-one patients were prospectively treated, with their initial dosage adjusted according to their CYP3A5 status (0.1, 0.15, and 0.2 mg/kg/day for CYP3A5*3/*3, CYP3A5*1/*3, and CYP3A5*1/*1, respectively). Their clinical outcomes were compared with those of patients treated with a fixed dose (0.1 mg/kg/day). RESULTS: The first blood trough level of CYP3A5 expressors, CYP3A5*1/*3 or CYP3A5*1/*1, and the overall rate in achieving the target blood trough level within a week in the individualized-dose group were significantly higher than those in the fixed-dose group (5.15±2.33 ng/mL vs. 9.63±0.79 ng/mL, P=0.035 and 12.5% vs. 66.7%, P=0.01). The remission rate at 2 weeks in the expressors was as high as that in the nonexpressors, CYP3A5*3/*3, in the individualized-dose group. CONCLUSIONS: Individualized TAC treatment is effective against ulcerative colitis regardless of the CYP3A5 genotype.

10.Effect of Severe Hemolysis in Blood Samples on Laboratory Results

In this study, we investigated the effects of severe hemolysis (hemoglobin [Hb] > 500 mg/dL) in blood specimens by classifying them into non-hemolysis, hemolysis (Hb ≤ 500 mg/dL), and severe hemolysis. Investigated items were total protein (TP), albumin (ALB), total bilirubin (T-Bil), direct bilirubin (D-Bil), aspartate transaminase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LD), alkaline phosphatase (ALP), gamma-glutamyl transferase (γ-GT), creatine kinase (CK), amylase (AMY), cholinesterase (ChE), blood urea nitrogen (BUN), creatinine (Cre), sodium (Na), potassium (K), chloride (Cl), calcium (Ca), uric acid (UA), triglycerides (TG), total cholesterol (T-Cho), high-density lipoprotein cholesterol (HDL-C), lowdensity lipoprotein cholesterol (LDL-C), and C-reactive protein (CRP), and actual clinical test results were used. Based on the results, we were able to classify the error relationships into three groups according to hemolysis status. Group A shows an error between non-hemolysis and hemolysis, and an even stronger error in severe hemolysis (T-Bil, AST, LD, Na, K, Ca, and UA). Group B showed no error between non-hemolysis and hemolysis, but errors in strong hemolysis (ALB, D-Bil, ALT, γ-GT, CK, AMY, TG, T-Cho, HDL-C, and LDL-C). Group C shows no errors in either hemolysis or strong hemolysis (TP, ALP, ChE, BUN, Cre, Cl, and CRP). Among these, the Group B classification was a new finding. In situations where the measurement of hemolyzed specimens is unavoidable, it is important that clinical laboratory technologists be aware of its impact and provide the results in a way that can be used in clinical practice.
