Background: VHWs play an important role in primary health care. The Chuongmy district, Hatay province had only 50% of VHWs compared to the national rate of 90%. \r\n', u'Objectives: This study aims to describe the current activities of VHWs in Chuongmy district and identify the related factors. \r\n', u'Subjects and methods: The study adopted the cross sectional design, combined qualitative and quantitative data collection techniques with VHWs and the leaders of local authority and health facilities. \r\n', u'Results: VHWs play an important role in implementing health programs and health care activities in the villages. Almost all tasks and functions of VHWs were carried out in the villages but with different rate and quality. The major activities that VHWs carried out were preventive medicine and treatment of common diseases. Nevertheless, the quality of many activities should be improved. The performance and stability of VHWs depend on different factors such as training, income, qualification, support monitoring and supervision of local authority and health sectors. \r\n', u'Conclusions: The local authority and health sectors should have policies to provide update training to VHWs, incentives to retain them to work and enhance the supportive supervision of VHWs activities. \r\n', u'

2.Home delivery in Luong Son, Hoa Binh

Background: There are about 600.000 deaths related to pregnancy and delivery in the world every year. In Vietnam, the rate of maternal death is 165/100.00. The majority of maternity and new born deaths occurred in the first week after delivery, especially 24 hours after delivery. Objective: To investigate the status of delivery and various factors related to home delivery and propose recommendations to reduce the rate of home delivery in Luong Son, Hoa Binh. Subject and method: The cross sectional study using a structured questionnaire with women who gave birth in 2003 was conducted in two communes of Luong Son district. Result: The home delivery rate was high (31.5%) and only 36.1% women decided themselves where to deliver. The main reasons for home delivery was perception related to last home delivery was normal, therefore this delivery should be at home (53.8%). Among 108 women interviewed, only 72% gave birth with the assistance of health professionals and 28% delivered with none health professional assistance. The rate of obstetric complications and new born death was relatively high among those that had homebirths. Conclusion: In order to increase the delivery rate at health care facilities and with the assistance of health professionals, there is required the cooperation of stakeholders for improving infrastructure, economic development, health education for increasing awareness of people on safe delivery. The public should change their perception, custom and behavior toward homebirths. Besides, the health sector should better manage the maternal health care program, monitor the pregnant women, plan the delivery dates and advocate their families (husbands and parent in laws) to take the women to deliver at the health care facilities.

3.Anticancer effects of saponin and saponin-phospholipid complex of Panax notoginseng grown in Vietnam

Objective: To evaluate the antitumor activity both in vitro and in vivo of saponin–phospholipid complex of Panax notoginseng. Methods: The in vitro cytotoxic effect of saponins extract and saponin–phospholipid complex against human lung cancer NCI-H460 and breast cancer cell lines BT474 was examined using MTS assay. For in vivo evaluation of antitumor potential, saponin and saponin–phospholipid complex were administered orally in rats induced mammary carcinogenesis by 7,12-dimethylbenz(a)anthracene, for 30 days. Results: Our data showed that saponin–phospholipid complex had stronger anticancer effect compared to saponin extract. The IC50 values of saponin–phospholipid complex and saponin extract for NCI-H460 cell lines were 28.47μg/mL and 47.97μg/mL, respectively and these values for BT474 cells were 53.18μg/mL and 86.24μg/mL, respectively. In vivo experiments, administration of saponin, saponin–phospholipid complex and paclitaxel (positive control) effectively suppressed 7,12-dimethylbenz(a) anthracene-induced breast cancer evidenced by a decrease in tumor volume, the reduction of lipid peroxidation level and increase in the body weight, and elevated the enzymatic antioxidant activities of su-peroxide dismutase, catalase, glutathione peroxidase in rat breast tissue. Conclusions: Our study suggests that saponin extract from Panax notoginseng and saponin–phospholipid complex have potential to prevent cancer, especially breast cancer.
4.The impact of hyperandrogenism on the outcomes of ovulation induction using gonadotropin and intrauterine insemination in women with polycystic ovary syndrome

Objective:
This study aimed to investigate the impact of hyperandrogenism (HA) on the outcomes of ovulation induction (OI) using gonadotropin and intrauterine insemination (IUI) in patients with polycystic ovary syndrome (PCOS).
Methods:
This was a retrospective cohort study including 415 patients undergoing OI using gonadotropin and IUI treatment between January 2018 and December 2020 at a single infertility center. Baseline characteristics, clinical and laboratory parameters, and pregnancy outcomes were investigated.
Results:
Among the study population, there were 105 hyperandrogenic (25.3%) and 310 non-hyperandrogenic patients (74.7%). The live birth rate was lower in the HA group than in the non-HA group, but this difference did not reach statistical significance due to the limited sample size (14.3% vs. 21.0%, relative risk=0.68; 95% CI, 0.41–1.14, p=0.153). No predictive factors for live birth were identified through logistic regression analysis.
Conclusion
HA did not negatively affect the outcomes of OI using gonadotropin and IUI cycles in Vietnamese women with PCOS. The result may not be applicable elsewhere due to the large variation in the characteristics of women with PCOS across races and populations.
5.Predictors of twin pregnancy in in vitro fertilization with intracytoplasmic sperm injection cycles with day 3 double embryo transfer

Objective:
The purpose of this study was to identify factors associated with twin pregnancy following day 3 double embryo transfer (DET).
Methods:
This retrospective cohort study incorporated data from 16,972 day 3 DET cycles. The participants were women aged between 18 and 45 years who underwent in vitro fertilization with intracytoplasmic sperm injection (IVF/ICSI) at My Duc Assisted Reproduction Technique Unit (IVFMD), My Duc Hospital, located in Ho Chi Minh City, Vietnam.
Results:
Of the 16,972 day 3 DET cycles investigated, 8,812 (51.9%) resulted in pregnancy. Of these, 6,108 cycles led to clinical pregnancy, with 1,543 (25.3% of clinical pregnancies) being twin pregnancies. Factors associated with twin pregnancy included age under 35 years (odds ratio [OR], 1.5; 95% confidence interval [CI], 1.32 to 1.71; p<0.001) and cycles involving the transfer of at least one grade I embryo. Relative to the transfer of two grade III embryos, the risk of twin pregnancy was significantly elevated following the transfer of two grade I embryos (OR, 1.40; 95% CI, 1.16 to 1.69; p<0.001) or a combination of one grade I and one grade II embryo (OR, 1.27; 95% CI, 1.05 to 1.55; p=0.001).
Conclusion
By analyzing a large number of IVF/ICSI cycles, we identified several predictors of twin pregnancy. These findings can assist medical professionals in tailoring treatment strategies for couples with infertility.
6.Management of Antibiotic-Resistant Helicobacter pylori Infection: Perspectives from Vietnam

Antibiotic resistance is the most important factor leading to the failure of eradication regimens. This review focuses on the prevalence of Helicobacter pylori primary and secondary resistance to clarithromycin, metronidazole, amoxicillin, levofloxacin, tetracycline, and multidrug in Vietnam. We searched the PubMed, EMBASE, Vietnamese National Knowledge Infrastructure, and Vietnamese Biomedical databases from January 2000 to December 2016. The search terms included the following: H. pylori infection, antibiotic (including clarithromycin, metronidazole, amoxicillin, levofloxacin, tetracycline, and multidrug) resistance in Vietnam. The data were summarized in an extraction table and analyzed manually. Finally, Excel 2007 software was used to create charts. Ten studies (three studies in English and seven in Vietnamese) were included in this review. A total of 308, 412, 523, 408, 399, and 268 H. pylori strains were included in this review to evaluate the prevalence of H. pylori primary resistance to amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance, respectively. Overall, the primary resistance rates of amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance were 15.0%, 34.1%, 69.4%, 27.9%, 17.9% and 48.8%, respectively. Secondary resistance rates of amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance were 9.5%, 74.9%, 61.5%, 45.7%, 23.5% and 62.3%, respectively. In Vietnam, primary and secondary resistance to H. pylori is increasing over time and affects the effectiveness of H. pylori eradication.
