1.Interpretation and Thinking about Global Vaccine Safety Blueprint

OBJECTIVE:To improve the ability of vaccine post-marketing surveillance,and clearly define the development di-rection of it in China. METHODS:Though introducing the background,strategic objective and operation objective of Global Vac-cine Safety Blueprint,the situation of vaccine post-marketing surveillance in China was analyzed. RESULTS&CONCLUSIONS:Vaccine post-marketing surveillance have developed fast in China recent years and get generally confirmation. However,compared with the goals in the blueprint,there are some activities should be strengthened,such as active surveillance,signal analysis and da-ta standardization,etc. We should take use of the opportunity offered by WHO and other international organizations to shrink the gap between our country and the advanced countries. Drawing support from WHO and other international institutions,vaccine post-marketing surveillance can step on the new level in China.
2.Detection of abnormalities in digital mammograms based on Support Vector Machines

Objective To search an approach based on Support Vector Machine (SVM) for detection of different abnormalities including micro-calcifications and masses from digital mammograms. Methods Such detections were formulated as supervised-learning problems and SVM was applied to the detection algorithm. After the regions of interest were pre-processed by specific rectangular windows, three kinds of parameters were extracted, including the direct pixel value parameter, the parameters from Spatial Grey Level Dependency (SGLD) matrices and from Discrete Cosine Transform (DCT). At first, each kind of parameter was taken as the input of SVM to train and test the machine respectively. Then all the parameters were incorporated into the input of SVM. Results the classification accuracy is 92.28%, 90.35% and 91.12% respectively when only one parameter input. The classification accuracy reaches 99.23% when all the parameter incorporated. Conclusion The parameters extracted from the regions of interest in digital mammograms can reflect the characteristics of different regions and SVM is a powerful tool for the detection of abnormalities from digital mammograms.
3.Comparative study on endoscopic thyroidectomy versus conventional thyroidectomy in thyroid disease

ObjectiveThe aim of this study was to evaluate and compare the surgical outcomes of endoscop ic and conventional open thyroidectomies in patients with thyroid disease.Methods116 patients with tyroid tumor were enrolled.56 patients underwent endoscopic thyroidectomy ( endoscopic group ),and 60 patients underwent conventional open thyroidectomy( conventional group).We analyzed the patients' clinic characteristics,surgical outcomes and complications between the two groups.ResultsThe blood loss was less in the endoscopic group than the open group[( 16.8 ± 9.6) ml vs ( 24.9 ± 14.2 ) ml,t =- 2.427,P < 0.05].The degree of satisfaction for cosmetic outcome in endoscopy group( 96.4% ) was higher than that in conventional group ( 16.7% ) ( x2 =74.508,P < 0.01 ).There was no significant difference in the operating time,volume of drainage and postoperative hospital stay between two groups,and there was no significant difference in the skin ecchymosis,redness and swelling and postoperative pain between two groups( all P > 0.05).No severe postoperative complication was encountered,such as injuries of the re current or superior laryngeal nerve,parathyroid gland injury or massive hemorrhage.ConclusionEndoscopic thyroidectomy has less blood loss,mini-open and excellent cosmetic benefits compared with conventional open thyroidectomy.
4.Drug Quality Representation Analysis of Preparations of Aurea Helianthus based on Anti-myocardial Ischemia Effect

This study was aimed to select the best preparation form of Aurea Helianthus in order to provide a scientific basis for research and development of anti-myocardial ischemia drugs.The content of effective index components of preparation of Aurea Helianthus was detected using HPLC-PDA.Acute myocardial ischemia rat model introduced by injecting pituitrin was used to investigate the effect of anti-myocardial ischemia.The quality of preparation from Aurea Helianthus was characterized based on pharmacy and efficacy.And the characterized results were given correlation analysis.The results showed that the HPLC method for quantitative determination was verified by methodology.The average recoveries were in the range of 97.04%-102.44%.All three preparations of Aurea Helianthus had the efficacy of anti-myocardial ischemia in varying degrees.The enrichment had the highest effect with lowest extraction rate and the lowest dosage,which was the best form in medicinal-preparation application of Aurea Helianthus.It was concluded that the content of related drug components was higher with unique proportion in enrichment of Aurea Helianthus which was prepared based on drug system proved a high quality medicine.Therefore,the key to the improvement of efficacy was to explore the fittest proportion and improve the content of related pharmaceutical ingredients pertinently according to drug system.
5.Preservation of the remaining pancreatic body and tail in pancreatic operations

Objective The purpose of this study was to determine how to preserve the remaining pancreatic body and tail in the pancreatectomy. Methods In seven cases of pancreatectomy, three of them were the rupture of pancreatojejunal anastomosis, and four of them were the pancreatectomy for tumor in the pancreatic neck or body. During operations, a bridge internal drainages was used to drain the pancreatic juice into the adjacent jejunum. After the operations, the supportive treatment, continuous irrigation of peritoneal cavity and pancreatic enzyme inhibition were used. Results In all seven patients, the remaining pancreatic body and tail were successfully preserved. The endocrine functions of these patients recovered to nearly normal level and patients were discharged. Conclusions In preserving the remaining pancreatic body or tail, the bridge internal drainage has its advantage of convenience. It effectively preserves the exocrine of pancreas as well as its endocrine
6.Classifications and Manipulation of the Abnormality of Cervical Curvature

ObjectiveTo analyze the classifications of the abnormalities of cervical curvature and its treatments with manipulation. Methods209 patients with cervical lordosis were analyzed. The abnormalities of the cervical curvature were classified as over-curvature (>45°), hypo-curvature (0°~30°), the straightened cervix (0°), backward curvature(0°~―20°), multiple curvatures (the cervical curvature showed "S" shaped). The C_5 subluxation were restored with manipulation in patients with hypo-curvature and straightened cervix, or other vertebra subluxation in multiple curvatures. Results1~4 yeared follow-up, 127 cases were excellently well being, 59 were well being, 23 were improved.ConclusionManipulation is effective on abnormalities of cervical curvature.
7.Neuroprotective effect of curcumin to Aβ of double transgenic mice with Alzheimer's disease.

OBJECTIVETo observe the changes in Aβ40, Aβ42 and ADDLs in brains of 3 month-old APPswe/PS1dE9 double transgenic mice after six-month intervention with curcumin, in order to discuss the neuroprotective effect of curcumin.
METHODAPPswe/PS1dE9dtg mice were randomly divided into the model group, the Rosiglitazone group (10 mg x kg(-1) x d(-1)) and curcumin high (400 mg x kg9-1) x d(-1)), medium (200 mg x kg(-1) x d(-1)) and low (100 mg x kg(-1) x d(-1)) dosage groups, with C57/BL6J mice of the same age and the same background in the normal control group. After 6 months, the immunohistochemical staining (IHC) and the Western blot method were used to observe the changes in positive cell of Aβ40, Aβ42 and ADDLs in hippocampal CA1 area, their distribution and protein expressions.
RESULTBoth of the immunohistochemical staining and the Western blot method showed more positive cell of Aβ40, Aβ42 and ADDLs in hippocampal CA1 area and higher protein expressions in the model group than the normal group (P < 0.01). IHC showed a lower result in the Rosiglitazone group than the model group (P < 0.05), while Western blot showed a much lower result (P < 0.01). The number of Aβ40, Aβ42 and ADDLs positive cells and the protein expressions decreased in the curcumin high group, the medium group showed a significant decrease (P < 0.01), and the low dose group also showed reductions in the protein expressions of Aβ40 and Aβ42.
CONCLUSIONThe six-month intervention with curcumin can significantly reduce the expressions of hippocampal Aβ40, Aβ42 and ADDLs in brains of APPswe/PS1dE9 double transgenic mice. Whether curcumin can impact Aβ cascade reaction by down-regulating expressions of Aβ40, Aβ42 and ADDLs and show the neuroprotective effect needs further studies.

8.Relationship between apolipoprotein AI gene rs12721026 polymorphism and cerebral hemorrhage

Objective To study the relationship between apolipoprotein AI (ApoAI) gene rs12721026 polymorphism and cerebral hemorrhage.Methods One hundred and eighty patents with cerebral hemorrhage,admitted to our hospital from May 2014 to June 2015,were enrolled as ICH group,and 100 normal subjects were chosen as control group.ApoAI gene polymorphisms and related serum lipid parameters of the patients between the two groups were compared.Results The frequency of ApoA I gene polymorphism rs 12721026 alleles G and T in the ICH group showed no significant difference as compared with that in the control group (P>0.05).The total cholesterol (TC) and triglyceride (TG) levels in the ICH group were significantly higher than those in the control group (P<0.05),and high density lipoprotein cholesterol (HDL-C) level in the ICH group was significantly lower than that in the control group (P<0.05);There was no statistically significant difference on the low density lipoprotein cholesterol (LDL-C),ApoAI and lipopolysaccharide (Lp) levels between the ICH group and control group (P>0.05).In the ICH group,the TC,TG,LDL-C,ApoAI and Lp levels showed no significant difference between TT and GG/TG rs12721026 genotypes (P>0.05);HDL-C level ofTT genotype was significantly lower than that of GG/TG genotype (P<0.05).Conclusion ApoAI gene rs12721026 polymorphism shows no obvious role in the pathogenesis of cerebral hemorrhage,but may be associated with reduced HDL-C level.
9.Aberrant methylation of CpG island in 5′promoter region of p16 gene and its role in diagnosis of pancreatic cancer

Objective:To investigate the aberrant methylation of CpG island in 5′promoter region of p16 gene in the pancreatic juice and its value in diagnosis of patients with pancreatic cancer.Methods:Pure pancreatic juice(PPJ)was collected from the pancreatic duct by a nasopancreatic tube put under endoscopic retrograde cholangiopancreatography(ERCP). Cytological examination was performed by H-E staining in pure pancreatic juice.Aberrant p16 methylation was detected using the methylation specific PCR(MSP)in the PPJ.Results:The sensitivity,specificity,positive predictive value,negative predictive value and accuracy cytological examination in diagnosis of pancreatic cancer were 40%,100%,100%,45.4% and 60.0%,respectively.The DNA sequences were successfully extracted from the PPJ of 30 patients with pancreatic diseases and were subjected to MSP.Seven(35%)of the 20 cases with pancreatic cancer showed aberrant methylation of p16 gene.No aberrant methylation was detected in the pancreatic juice samples of patients with chronic pancreatitis and mucinous cystoadenocarcinoma of pancreas.When cytological examination combined with p16 methylation detection,the sensitivity, specificity,positive predictive value,negative predictive value and accuracy for diagnosis of pancreatic cancer were 55%,100%, 100%,52.6% and 70%,respectively.Conclusion:Pancreatic juice collected by nasopancreatic drainage during ERCP can be used for molecular analysis.Detection of aberrant methylation of p16 gene in pancreatic juice combined with cytological examination is a better method for diagnosis of pancreatic cancer.
10.Cbl-b and PI3K/Akt pathway are differently involved in oxygen-glucose deprivation preconditioning in PC12 cells.

BACKGROUNDTransient sublethal ischemia is known as ischemic preconditioning, which enables cells and tissues to survive subsequent prolonged lethal ischemic injury. Ischemic preconditioning exerts neuroprotection through phosphatidylinositol 3-kinase (PI3K)/Akt pathway. Cbl-b belongs to the Casitas B-lineage lymphoma (Cbl) family, and it can regulate the cell signal transduction.The roles of ubiquitin ligase Cbl-b and PI3K/Akt pathway and the relationship between them in oxygen-glucose deprivation preconditioning (OGDPC) in PC12 cells were investigated in the present study.
METHODSOxygen and glucose deprivation (OGD) model in PC12 cells was used in the present study. The 3-(4, 5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, nuclear staining with Hoechst 33258, and Western blotting were applied to explore the roles of Cbl-b and PI3K/Akt pathway and the relationship between them in OGDPC in PC12 cells.
RESULTSCell viability was significantly changed by OGD and OGDPC. OGD significantly decreased cell viability compared with the control group (P < 0.05), and preconditioning could rescue this damage was demonstrated by the increase of cell viability (P < 0.05). The expression of Cbl-b was significantly increased after OGD treatment. However, the activation of Akt and GSK3β was greatly inhibited. Preconditioning could inhibit the increase of Cbl-b caused by OGD and increase the activation of Akt and GSK3β. LY294002, a specific inhibitor of PI3K, could effectively inhibit the increase of Akt and GSK3β after preconditioning treatment. It partly inhibited the decrease of Cbl-b expression after preconditioning treatment.
CONCLUSIONUbiquitin ligase Cbl-b and PI3K/Akt pathway are differently involved in OGDPC in PC12 cells.
