1.Official Development Assistance (ODA) in the health sector from 1991 - 2007

This paper has highlighted some key findings as follows: from 1991 - June 2007, there are 258 projects/programs in the health sector with the total amount of 1.543,07 million USD, in which the Ministry of Health directly manages 137 projects with a total amount of 1.114.93 million USD, accounting for 72.4% of the total ODA for the health sector. The grant aid funding accounts for 57%, loan accounts for 25.6% and mix aid modality account for 17.6%. ODA from bilateral agencies is 43%, from international banks account for 37% and from UN agencies is 20%. Grant aid is declining while the loan is on the rise. As compared with the government budget, ODA accounts for 12.09% -27.78% of government budget in different periods. Priority was given to preventive medicine areas rather than treatment areas. (31.8% compared with 24.5%), in which ODA for HIV/AIDS control is 27.8% in the total amount of ODA for preventive medicine. Some important area has not received anything due to the attention of donors, such as environmental health, tropical diseases control, non communicable diseases. ODA allocation is also not adequate in terms of regional distribution. Disadvantage areas received less than other regions.

2.Study the high effect process of collecting hematopoietic stem cell from umbilical cord blood

Background: Umbilical cord blood - Stem cell CD34 are attracted to study and apply to cure many serious diseases. But the quantity of the cell is small, so it only used to children. Therefore, studying to find out the high effect collected process is necessary. Objective: studying to find out the high effect process of collecting hematopoietic stem cell from umbilical cord blood. Subjects and methods: the study included 112 umbilical cord blood specimens collected from healthy mother delivering at National Hospital of Obstetric and Gynecology.Results: The condition of the mother was healthy, with no risk factors of infection or genetic disease. The average age of mothers was 28 \xb1 5 years old, average weight: 63 \xb1 5 kg. Age of fetus was from 39-41 weeks, average weight: \ufffd?3000g. Collection technique: cutting the umbilical cord after delivery 6-10 seconds, cutting the umbilical cord far from neonatal 6 cm, blood collection continued after the collection of placenta, all processes must be implemented rapidly in the 4 -5 minutes in aseptic condition. With the above process it can be obtained with high effect blood: volume was 90 - 120ml (48%). CD34 cell count: 1.6 \xb1 0.3x106, the number of eukaryotic cells: 13.53 \xb1 1.7x108/unit of the umbilical cord. 3.Investigation of hemostatic disorders in patients with congenital heart diseases undergoing open heart surgery at Hue Central Hospital

Background: Open heart surgery with cardiopulmonary bypass (CPB) causes haemostatic abnormalities which result in postoperative excessive bleeding. Objectives: To investigate haemostatic disorders before - after CPB and postoperative bleeding. Subjects and methods: Sixty congenital cardiac patients with and without cyanosis were recruited consecutively. Hematology and coagulation tests were done 1 day before operation, 15 minutes after protamine administration, 2 and 6 hours after the operation in the intensive care unit (lCU). Mediastinal chest tube drainage (MCTO) was measured for the first 6h in the ICU. Results: Significant differences between 2 groups could be found for red blood cells, hemoglobin, hematocrit, fibrinogen, D-dimer, fibrinogen degradation products (FOP), von Kaulla and platelet aggregation to epinephrine before operation (p < 0.05). There were not significant differences in platelets, PT, APTT, platelet aggregation to adenosine diphosphate (AOP) in the acyanotic and cyanotic patients (p > 0.05). Most hematology parameters were decreased significantly and hemostasis measurements were prolonged after operation in both groups (p < 0.05). There was also the significant difference in MCTO between 2 groups (p < 0.05). 4.Man power and organization of provincial preventive medicine centers in the northern provinces

Background: Investment to meet the man power requirements are recognized as urgent; especially to efficiently implement the National Strategy of Preventive Medicine. To strengthen the capability of provincial preventive medicine centers, the Ministry of Health has approved Decision No 05/2006QD-BYT for functions, tasks, authorization and organizational structure of Provincial Preventive Medicine Centers (PPMCs). Objectives: The study was conducted to evaluate the manpower and organization structure of northern PPMCs and provide recommendations for policy makers. Subjects and method: Using the cross-sectional descriptive method, the study covered the preventive medicine centers of 29 northern provinces between Jan to Jun 2007. The information was collected by interviews and self-reported questionnaires. Results:23/29 PPMCs have not met the criteria of man power stated in the Circular 08/2007/TTLB-BYT-BNV. Only 9/29 PPMCs were well organized in accordance with Decision 05/2006/QD-BYT of the Ministry of Health. The average number of staffs in PPMCs was 50+15. Medical staffs accounted for 53%, out of which 21.1% had postgraduate degrees; 32.2% had graduate degrees and 23.3% had been trained in preventive care. Conclusion: To meet the requirements provided by the Decision No05/2006QD-BYT, the man power and training for staffs in PPMCs should be improved and strengthened.

5.Spoligotyping technique: improvement and application in classification Mycobacterium tuberculosis

Background: There are many methods used in epidemiological studies of tuberculosis (TB) bacteria but Spoligotyping method is widely used with high accuracy, simple procedure, and carried out on strains containing a little of IS6110 segment \r\n', u'Objectives: To improve Spoligotyping technique and apply this technique to classify Mycobacterium tuberculosis\r\n', u'Subjects and method: Subjects and methods: The study included 12 medical waste samples collected at Thai Binh Hospital of Tuberculosis and 19 samples obtained from the The Hanoi Institute for Tuberculosis and Lung Diseases. Spoligo model of 31 samples were analyzed based on Spotclust and SpoIDB4 database and divided up into family and subfamily.\r\n', u'Results: Spoligotyping technique has good results with the PCR product amplified 40 cycles and presented the film in 18 hours. Obtained results in 31 medical waste samples belong to 4 families: Beijing, EAI, T1 and H3-LAM9. EAI and Beijing are dominant families with 45.16% and 38.7%), respectively. T1 and H3-LAM9 are 12.9% and 3.22%, respectively. The number of samples in the study is little but the obtained rate of different spoligo models of strains are quite diversified (41.9%)\r\n', u'Conclusion: This result is relatively appropriate with previous studies on the distribution of EAI and Beijing families in Vietnam and the world. 6.Key containers of Aedes species, vectors to transmit dengue viruses in Nam Dinh province, 2007

Background: Dengue Fever/Dengue Hemorrhagic Fever (DF/DHF) has emerged as one of the most important public health concerns in Viet Nam in recent years. Key breeding containers plays an important role in driving dengue vector control in the public. Objectives: The research was conducted to discover the dengue situation, its vectors and key containers to make relevant recommendations in reducing effectively the vector population. Subjects and methods: Two hundred households in two communes of Nam Dinh province (100 in each studied commune) were selected randomly. Vectors collected from the field were identified using mosquito key. The number of dengue larvae and mosquitoes were directly counted or adjustment methods were used to estimate the true number in each type of water storage facility. Results: In Minh Thuan commune, Ae. albopictus larvae were mainly concentrated in jars (49%) and discards (59%), most of Ae. aegypti was found in cement tanks with a volume more than 500L (72%). In Trung Dong commune, larvae of Ae. albopictus concentrated in jars (40%) and discards (25%) and aquariums (15%), while most of the Ae. aegypti larvae was discovered in cement tanks with volumes more than 500L (86%). Mosquito density index of Ae. albopictus in Trung Dong and Minh Thuan communes was 0.56 and 0.38, respectively. The Aedes larvae, houses for larvae and Breteau index were nearly at the threshold of dengue epidemic occurrence for Ae. aegypti and over a certain threshold for Ae. albopictus. Some recommendations were provided to help reduce the dengue vectors. Conclusion: Investigation of key mosquito larvae in water containers was useful in driving the effective dengue vector control. Further studies are required to evaluate the impact and methods to manage water containers in the local area.

7.Evaluation of the efficacy of cynoff 25 ULV to eradicate aedes species, dengue vectors in Vietnam, 2004

Background: Dengue fever remains a major public health problem in Vietnam. It was transmitted through two main Aedes species, namely Aedes aegypti and Aedes albopictus in which Aedes aegypti played the predominant role in transmitting the dengue viruses (accounted for 94% of the Aedes genus).Different groups of insecticides have been widely used in the community. As a consequence, it increased the resistance level of Aedes vectors to the insecticides. Therefore, a new synthesized insecticide was required for future control and prevention of dengue epidemics. \r\n', u'Objectives: To evaluate the efficacy of Cynoff 25 ULV in suppressing Aedes species in Vietnam. \r\n', u'Subjects and methods: The study was conducted in the entomology laboratory and in the fields of Ha Tay province. ULV spraying was implemented in Hiengiang commune, Thuongtin district, in comparison with a control area, Van Mo commune in Ha Dong town.\r\n', u'Results and conclusions: : Cynoff 25 ULV had a high efficacy that kills over 90% of 2 dengue vector species, namely Aedes aegypti and Aedes albopicctus after 24 hours exposure at the distance of 30 meters from the brass nozzle of a STIHL SR 400 sprayer. In the field trials, Cynoff 25 ULV also had high efficacy in killing Aedes species, e.g. 100% of Aedes species were killed after spraying and the effects lasted for 3 months for Aedes aegypti and one month for Aedes albopictus species. 8.Evaluation of the efficacy of Bistar 80SC by residual application in dengue vectors control in the North of Vietnam

Background: Two species, Aedes aegypti and Aedes albopictus are the main intermediate vectors of transmission of the dengue viruses in Vietnam. Insecticide applications by different methods that may help interrupt the spread of dengue outbreaks. Many different groups of insecticides have been used for dengue vector control. Some recent studies revealed that Aedes vectors obscuring their resistance to insecticides at different levels. Therefore, a new insecticide formula is required for effective dengue vectors control. Objective: To evaluate the efficacy of Bistar 80SC by residual application in suppression of Aedes species in a northern province of Vietnam. Subject and methods: Bistar 80 SC with a component of Bifenthrin 80g/L was evaluated by residual application in suppression of 2 Aedes species in vitro and in an intervention commune of Hien Giang, Thuong Tin district and a control commune of Van Mo, Ha Dong town, Ha Tay province from March, 2004 to June, 2004. Results: WHO Bifenthrin paper test kit was effective at the concentration of 37.5mg/m2 and over with Aedes aegypti vectors ranged from 80% to 100% mortality in the North, In other words, Aedes vector remains susceptible to Bifenthrin in the region. The mortality on wood surface after 60 minutes of exposure to Bistar 80SC at concentrations of 25mg/m2 by using residual application was higher than those on brick walls. Up to 100% Aedes aegypti was killed after 60 minutes in exposure and remains in effect until the end of the third months and one month with Aedes albopictus. No side effects, to the sprayer and humans, livestock and environment caused by Bistar 80SC were reported in the intervention area. Conclusion: Bistar 80SC is suggested as a nominated alternative to effectively control the dengue outbreak by residual application.

9.Study on mechanism of astragaloside Ⅳ in treatment of pulmonary fibrosis with epithelial-mesenchymal transition

OBJECTIVE Currently discussing the clinical treatment of pulmonary fibrosis commonly used drugs astragalus main active ingredient astragalosideⅣ(ASTⅣ)in vitro after transforming growth factor-β1 induced lung adenocarcinoma A549 epithelial cells after epithelial cell interstitial Epithelial-Mesenchymal Transition(EMT). METHODS The effect of astragalosideIV on the proliferation of A549 cells was detected by MTT assay for the first time.No significant effect of astragaloside on the prolifera-tion of A549 cells was found in the range of 1.25-20 μmol/L. A549 cells in vitro were divided into 5 groups: normal group, control group, low, medium and high experimental groups, which were treated for 72 hours,and the morphological changes of cells in each group were observed by light microscope. Real-time quantitative PCR (qPCR) and Western blotting were performed. Detection of gene and protein expression levels. RESULTS The results of real-time fluorescence quantitative PCR showed that the quantitative analysis of high-dose astragalosideⅣin the experimental group was lower than that of the control group in the α-SMA analysis,and the difference was statistically significant(P<0.05).The dose of AstragalosideⅣ in the experimental group was higher than that of the control group in the E-Cad analysis,and the difference was statistically significant(P<0.05).Western Blot results showed that the expression of α-SMA antibody in the high-dose and low-dose experimental group was lower than that in the control group, the difference was statistically significant (P<0.05). The high-dose experimental group had a significantly higher expression of E-Cad antibody than the control group, the difference was statistically significant (P<0.01). CONCLUSION This study uses A549 epithelial cells as a model, A549 cells were modeled and confirmed that Astragaloside can effectively inhibit TGF-β1-induced epithelial-mesenchymal transition(EMT)and provide a new basis for the treatment of pulmonary fibrosis.
10.Study PML/RAR alpha fusion gene on 21 patients diagnosed with acute promyelocytic leukemia (AML \u2013 M3)

Background: In Vietnam, there are a number of studies on the application of ATRA in treating acute promyelocytic leukemia (AML \u2013 M3) but they have still faced with certain difficulties. Objectives: (1). Study PML/RAR alpha fusion gene on the patients diagnosed with AML \u2013 M3. (2). Study the index of hematology of the PML/RAR alpha positive group. Subject and Method: 21 patients with acute promyelocytic leukemia (M3) were studied. All patients were examined with morphology, coagulation and cytogenetic tests and RNA were extracted from leukemic cells and PCR for PML/RAR alpha fusion transcript. Result and conclusion: PML/RAR alpha positive in 67% including 4 patients which were not discovered t(15; 17) by cytogenetic technique. Rates of three subtype (bcr1, bcr2 and bcr3) of PML/RAR alpha were 7 patients (50%), 3 patients (21,5%) and 4 patients (28,5%), respectively. WBC and bone marrow cells of PML/RAR alpha positive group were 5.08+/-3.87 and 155.82+/-106.21. D \u2013 Dimer level was 1954.89+/-1575.28; 93% of patients in the PML/RAR alpha positive group had DIC.
