1.Pleural effusion due to paragonimiasis

A patient was admitted to Department of Pneumology of B¹ch Mai hospital because of Right pleural effusion. Results of investigation showed that there were many eggs of Paragonimus heterotremus in pleural liquid, but there were not eggs in the sputum and bronchial liquid. This is the first case of pleural effusion due to Paragonimiasis, which is found in our Department.

2.Acute Intestinal Obstruction Due to Intestinal Anisakiasis Resolved with Conservative Therapy

Intestinal anisakiasis is rarely diagnosed because it is thought to be uncommon and is poorly recognized. It produces severe abdominal pain and an inflammatory reaction often resulting in reactive intestinal obstruction, which is sometimes treated with an unnecessary laparotomy as acute abdomen or intestinal obstruction. We reported a 58-year-old female with acute intestinal obstruction caused by intestinal anisakiasis, which resulted in a self-limiting clinical course. The diagnosis was based on a history of recent ingestion of raw fish and abdominal computed tomographic findings of partial thickening of the intestinal wall accompanied by focal luminal narrowing with ascites. In spite of the severity of the abdominal pain, the bowel obstruction induced by inflammation and edema was resolved with conservative treatment after three weeks. Accordingly, intestinal anisakiasis was considered in the differential diagnosis of intestinal obstruction, which can be treated with conservative therapy.

3.A Case of Falling of Taiki from the Upper Aspect of the Body Caused by Taking a Kind of Cathartic Drug

Taiki is a kind of fundamental vital energy filling the upper aspect of the body, and it can fall because of stress, heavy labor, diarrhea, excessive intake of drugs for regulating the flow of vital energy, or extreme deficiency in vital energy.We report the case of a 39 year old female patient who complained of dyspnea, psychic enervation, and other symptoms of falling taiki from the upper aspect of the body, caused by taking a cathartic drug (mashiningan) for constipation, while being treated for agitation and emotional incontinence. Shokanto improved this state of falling taiki from the upper aspect of the body.

Based on progress in home visits for rehabilitation and health guidance since 1979, the trend after the Nursing Care Insurance Law was enforced in 2000 was investigated. In home visits for rehabilitation, cooperation with many related organizations is required, and reorganization of the network built before the enforcement of the said Law is needed. The role of each home visiter for rehabilitation is being formed and cooperation with care managers is being strengthened. The tendency that the participation of the local government may decrease can't be denied. Connection with the hospital and the local government should be maintained, and a new community-based network needs to be built.

5.Sterilisation of Lucilia cuprina Wiedemann maggots used in therapy of intractable wounds.

Three new techniques of sterilising maggots of Lucilia cuprina for the purpose of debriding intractable wounds were studied. These techniques were utilisation of ultra-violet C (UVC) and maggot sterilisation with disinfectants. The status of sterility was checked on nutrient agar and blood agar and confirmed with staining. The indicators for the effectiveness of the methods were sterility and survival rate of the eggs or larvae. Egg sterilisation with UVC had the lowest hatching rate (16+/-0.00%) while egg sterilisation with disinfectants showed high hatching rate (36.67+/-4.41%) but low maggot survival rate (31.67+/-1.67%). Sterilisation of the maggots was the most suitable, since the survival rate was the highest (88.67+/-0.88%). Complete sterility was achieved in all cases, except that Proteus mirabilis was consistently found. However, the presence of this microorganism was considered beneficial.

6.Exposure Conditions in Measurement of Visceral Adipose Tissue by CT

[Purposes] In 2005, criteria for diagnosing the metabolic syndrome was officially announced. Accumulation of fat tissue in and around the abdomen is one of the metabolic syndrome. To assess the excess fat tissue, the waist circumference is measured. Computed tomography can work out the area of the intra-abdominal fat tissue. In this study, we examined exposure conditions with a view to reducing dose and radiation risk.[Methods] A CT phantom model simulating the abdominal area at the level of the umbilicus was made for obtaining the pictures of the intra-abdominal fat tissue with the tube current ranging from 10 mA to 250 mA set at intervals of 10 mA. The average CT value for the area of mimic fat tissue obtained by each scan and standard deviations were worked out.[Results] The size of fat tissue and the mean CT value were constant with a tube current of 100 mA or over. [Conclusions] It was found possible to measure the area of intra-abdominal fat tissue with a tube current of 100 mA. Therefore, our findings showed that the radiation exposure could be reduced by 60% when compared with the 250 mA CT used at a clinical setting.

7.Fundamental Study of Reduced Radiation Exposure of Virtual Endoscopy by 16 DAS MDCT

Our aim was to study the scan protocol of 16 DAS (Data Acquisition System) multidetector-row CT (MDCT) and to ascertain whether we could reduce radiation exposure and scanning time, while maintaing image quality in the Virtual endoscopy (VE).We made a mimic gastrointestinal tract and examined the result by changing a scan protocol of 16 DAS MDCT.We found that a reduction of radiation exposure and scanning time depended on slice width, angle of the table, rotation time, table speed, mAs/slice and resolution.The study confirmed that it was possible to use the scan protocol of MDCT with a radiographic dose of 40% and scanning time of 50%, without changing image quality in the VE.

8.Combined with Shakuyakukanzoto at the Time of Embryo Transfer in is vitro Fertilization : A Randomized, Retrospective Study

In vitro fertilization-embryo transfer (IVF-ET) therapy for subfertile couples has become widespread. However, it has been reported that high frequency uterine contractions at the time of embryo transfer influence adversely IVF-ET outcome. This article focuses on the methods of administration of shakuyakukanzoto combined with ET for subfertile women. 186 patients who underwent IVF-ET were recruited. 94 women undergoing IVF were randomly assigned to ET with shakuyakukanzoto for 4 days or 92 with no treatment. Background characteristics (age, number of embryo transferred, quality, and luteal function) were similar in the two groups. Pregnancy rate was 33.0% in the shakuyakukanzoto group as compared with 20.7% in the nontreated group. The increased pregnancy rate with shakuyakukanzoto compared with no treated was significant. However, the precise mechanism to explain for the results of this investigation remains to be unclear. Therefore, further research might be necessary to evaluate the effect of shakuyakukanzoto, at the time of ET.

9.GnRH agonist and GnRH antagonist in intracytoplasmic injection cycles.

The long agonistic protocol for controlled ovarian hyperstimulation (COH) is effective and used most often, thus is considered the gold standard. Therefore any new regimen has to be compared in its results with those obtained with the long protocol. This report compares the efficacy of GnRH agonist and antagonist in a retrospective study of IVF/ICSI carried out in a tertiary teaching hospital from 2003 to 2006. Only the first COH cycle followed by IVF-ICSI from 200 couples (agonist = 120 and antagonist = 80) were analysed. The end points studied included the number of oocytes recovered, number of mature (MII) oocytes, fertilization, cleavage, morphology based embryo quality, pregnancy rate, quantity and cost of gonadotrophin. The average age of female subjects was 35.1 +/- 4.7 years with 50% being 35 years and above. Major infertility factors were tubal blockage, male factor and endometriosis altogether comprising 68%. GnRH agonist and antagonist cycle parameters were comparable except lesser amount of gonadotrophin was used with lower resultant costs (both p < 0.0005) in antagonistic regime. Antagonist regime produce somewhat more good quality embryos (p = 0.065), an insignificant difference. A clinical pregnancy rate per embryo transfer of 16.3% in agonist and 20.6% in antagonist regime was achieved respectively. In conclusion, GnRH antagonist protocol produced a COH response, embryonic development and pregnancy rates on par to GnRH agonist regime. Moreover GnRH antagonist protocol required a shorter stimulation period plus fewer complications. Hence GnRH antagonist regime provided means for a friendlier, convenient and cost effective protocol for patients.

10.A retrospective review comparing the use of Gonal-F and Metrodin-HP for in-vitro fertilisation (IVF).

All cycles of IVF with pituitary down-regulation (n = 57) done at the Damansara Fertility Centre in the year 2000 were studied. All the 57 patients had controlled ovarian hyperstimulation, either using Metrodin HP (n = 27) or Gonal-F (n = 30). Of these, 53 patients reached oocyte pick-up, 26 patients in Metrodin HP group and 27 patients in Gonal-F group. Gonal-F resulted in a higher clinical pregnancy rate of 66.6% compared to Metrodin HP 38.5% (p < 0.05). The live birth rate tends to be higher in Gonal-F group (40.7%) compared to Metrodin HP (30.8%), (p > 0.05).
