Objective:To study the clinical results and the hospitalization cost of video-assisted thoracoscopic surgery (VATS) with ligation and suture for the treatment of spontaneous pneumothorax. Methods:Thirteen patients were treated by VATS with ligation and suture. The consumed time of operation, duration of the chest drainage, amount of the chest drainage, percentage of patient discontinuing anodyne within the postoperative 24 hours, the average length of hospitalization and the hospitalization cost were analysed. Results:Satisfactory therapeutic effects were found in all cases without postoperative death and complications. The average hospitalization time was 3 days.VATS with ligation and suture was preferable to transaxillary minithoracotomy (TAMT) in the clinical results. The hospitalization cost is less for VATS with ligation and suture (7372.47?871.3) than that with Endo-GIA (12524.32?2962.18) (P

Objective To study the expression of vascular endothelial growth factor-C (VEGF-C) and VEGF receptor-3 (VEGFR-3) in non-small cell lung cancer (NSCLC), and to explore its relationship with lymph node metastasis. Methods Sixty patients with NSCLC were studied. The expression of VEGF-C and VEGFR-3 was detected by immunohistochemistry in lung cancer samples, and its relationship to clinical and pathological data was analyzed. Results The positive expression rate of VEGF-C in NSCLC tissues was 68.3% (41/60), the expression rate of VEGFR-3 was 53.3% (32/60). The expression of VEGF-C was related with the degree of cell differentiation in lung cancer. Both the expressions of VEGF-C and VEGFR-3 were positively associated with lymph node metastasis. There was a positive correlation between VEGF-C and VEGFR-3 expressions (r=0.27). Conclusion The expressions of VEGF-C and VEGFR-3 are related to lymph node metastasis in NSCLC. VEGF-C may promote intratumoral lymphangiogenesis via VEGFR-3 and lymphatic metastasis.

Objective To evaluate the therapeutic effect of acupuncture-moxibustion for treatment of disease of skin and subcutaneous tissue and to provide objective evidence for classifying the evidence-based disease spectrum of acupuncture-moxibustion. Methods Retrieve PubMed,Cochrane Library,Ovid,EMBase,CBM,CNKI,WANFANG,VIP,and collect the randomized controlled trials of treatment of disease of skin and subcutaneous tissue with acupuncture and moxibustion, select those conforming with the enrolled criteria, and use Meta-analysis to evaluate the effect and grade the disease spectrum of acupuncture and moxibustion according to the evidence degree. Results Totally 1149 clinical studies in Chinese and 3 full text papers in English were acquired. Of them, there were 224 randomized controlled trials, and 56 studies conformed to the enrolled criteria, with 11 diseases involved. Based on the result of Meta-analysis and the quality of studies,4 studies of strong-evidence,10 of moderate-evidence,and 15 of weak-evidence were obtained. Conclusion Based on the present evidences, the effect of acupuncture-moxibustion is definitely beneficial in acne (cystic type),chronic urticaria, and neurodermatitis (localized); the effect of acupuncture-moxibustion is likely to be beneficial in chloasma, clavus, and pruritus; and the effect of acupuncture-moxibustion is possibly beneficial in alopecia areata, eczema, psoriasis, vitiligo, and bedsore.

BACKGROUND: Biomechanical mechanisms are complex, and previous studiers focus on the stress conduction in the carpus. However, the stress distribution and characteristics of trabecula in the carpus are rarely reported.OBJECTIVE: To investigate the stress distribution and deformation characteristics of the normal lunate through a two-dimensional sagittal finite element model.METHODS: A normal cadaveric lunate sample was scanned with Micro-CT and the central sagittal image was chosen for further finite element analysis (FEA). The chosen image was processed and imported into the finite element analysis software (Ansys 14.0). A two-dimensional sagittal finite element model of the lunate bone was established. Axial pressure was applied to the model with the wrist held in different positions, and nine regions of interests (ROIs) were identified, for which stress and displacement nephograms were created. These included the first principal stress (S1, the maximum stress in a principal plane), the third principal stress (S3, the minimal stress in a principal plane), shear stress (SXY, the component of stress coplanar with a material cross section), von Mises stress (SEQV, yielding begins when the elastic energy of distortion reaches a critical value)and displacement of each ROI (UY, displacement on the vertical plane of the lunate) which were calculated and compared.RESULTS AND CONCLUSION: (1) The stresses on ROIs located in the proximal and volar cortices of the lunate bone were much higher than those in the distal and dorsal cortices. At the proximal lunate, S1 was less than S3; however at the distal lunate, S1 was greater than S3. The ROIs of the distal and proximal ends of the lunate bone received much higher stress than the ROIs of the middle part. As for axial trabecular displacement, both distal and proximal ROIs were compressed by axial pressure. However, the dorsal and the volar parts of the proximal lunate moved in different directions at different wrist postures. Besides, the stress values and magnitudes of displacement were elevated in wrist flexion and extension compared to neutral position.Furthermore, the stress concentration zones (the proximal volar ROI, the proximal dorsal ROI, the distal volar ROI, and the distal dorsal ROI) had different directions of shear stress and displacement in different wrist postures. (2) These results suggest that when stress is loaded on a normal lunate model, four stress concentration zones, the proximal volar ROI, the proximal dorsal ROI, the distal volar ROI, and the distal dorsal ROI are found. The wrist postures can significantly affect the value and distribution of axial stress on the sagittal lunate.
