1.Essence of professor SHI Xin-de's experience in application of persicae ramulus-associated prescriptions.

Persicae Ramulus decoction, as the first prescription in Treatise on febrile Diseases, has been recommended by physicians of successive generations. It is also the general prescription for harmonizing yingfen and weifen, yin and yang, qi and blood. Although it only consists of five herbal medicines, it has a wider range of application and more categorized formulas than other prescriptions. Though Persicae Ramulus decoction was originally formulated to treat taiyang apoplexy, it has functions beyond the treatment of exopathic diseases. This formula is also effective in treating internal diseases, surgical diseases, gynecologic diseases, paediatric diseases, etc. KE Yun-bo praised it as the No. 1 formula among ZHANG Zhong-jin's formulas as well as the general prescription for harmonizing Yin and Yang, yingfen and weifen, resolving fleshy exterior and inducing perspiration. Professor SHI Xin-de has been expert at treating intractable diseases by using Persicae Ramulus-associated prescriptions, such as Xiaojianzhong decoction and Baohe pill for children's chronic eczema, Persicae Ramulus and Puerariae Lobatae Radix decoction and Yupingfeng powder for chronic nephritis, and Persicae Ramulus and Longgu Muli decoction for insomnia. Instead of being restricted to Chinese or Western disease names, he prescribed appropriate formulas according to syndromes, thereby achieving a good efficacy.

2.Contracted services of general practitioner improve management of primary hypertension in community

Two hundred and forty patients with primary hypertension in Weifang Community were randomly sampled as contract group and another 240 hypertensive patients were sampled as control group.Patients in contract group signed service contract with general practitioners (GPs),who provided individualized service for hypertension management.Patients in control group receive conventional outpatient service for hypertension management.The clinical data of patients before and one year after contracted services were documented and compared with those in control group.The results showed that in contract group the average systolic pressure decreased from 134.60 mm Hg (1 mm Hg =0.133 kPa) to 130.62 mm Hg (P =0.000),reaching-the-target-BP rate increased from 56.3% to 85.5% (P =0.000),serum total cholesterol and triglyceride decreased significantly one year after receiving contracted services (P =0.000) ; while these indicators were not significantly changed in control group (all P > 0.05).The results indicate that the contracted services of GPs can improve the effectiveness of hypertension management in community.

7.Expression and significance of TRAIL, Caspase-3 and NF-κB in carcinoma of bile duct

Objective To investigate the function of TRAIUCaspase-3 and NF-κB among the occurrence, development and malignant invasion of bile duct carcinoma. Methods The in-situ hybrization technique and immunochemistry method were adopted to detect expression of TRAIL, Caspase-3 and NF-κB. Results The positive rates of TRAIL expression in carcinoma of bile duct and paracancer tissue were 45.2 % (19/42) and 53.3 %(8/15), respectively. The differences were not significant (P >0.05), and there were no evident difference between expression of TRAIL and every clinic pathology factor(P >0.05). The positive rates of Caspase-3 expression in carcinoma of bile duct were 30.9 %(13/42), which were obviously lower than those in paracancer tissue, 60.0 %(9/15)(P<0.05). However, The positive rates of NF-κB expression in carcinoma of bile duct were 61.9 %(26/42), obviously higher than those in paracancer tissue, 26.7 %(4/15)(P <0.05). The positive rates of Caspase-3, NF-κB expression were relation to metastasis and proliferation, differentiation degree of tissue and survival time (P <0.05), but were independent of sex, ages and position of carcinoma(P > 0.05). Moreover, TRAIL and Caspase-3 manifested positive relationship (P <0.05). On the contrary , NF-κB and Caspase-3 manifested negative relationship (P <0.05). However, there was no relationship between TRAIL and NF-κB(P >0.05). Conclusion Expression of TRAIL has no selectivity of tumour, Its expression is higher or lower which couldn' t absolutely decide tumour' s metastasis and proliferation, and could not decide malignant degree and prognosis of tumour, too. Activation of NF-κB and losing action or absence of Caspase-3 possibly accelerate metastasis and proliferation of tumour, which result in bad prognosis. NF-κB interdicte TRAIL inducing apoptosis approach via control activation of NF-κB, which induce tissue to avoiding apoptosis and multiplication unboundedly, leading to tumour's occurrence, development, metastasis and diffuseness.
8.Effects of remifentanil on lipopolysaccharide-induced acute lung injury in rabbits

Objective To investigate the effects of remifentanil on lipopolysaccharide ( LPS)-induced acute lung injury (ALI) in rabbits.Methods Thirty healthy male New Zealand white rabbits weighing 2.5-3.5 kg were randomly divided into 5 groups ( n = 6 each) : group Ⅰ control (group C) ;group Ⅱ ALI;group Ⅲ, Ⅳ, Ⅴ low, median and high dose RF + LPS (group LR, MR, HR) . The animals were anesthetized with intravenous 3% pentobarbital sodium 30 mg/kg, tracheostomized and mechanically ventilated. The carotid artery and jugular vein were cannulated for MAP and HR monitoring, blood sampling, and fluid and drug administration. LPS 0.5 mg/kg in 10 ml of normal saline (NS) was infused over 30 min in group Ⅱ-Ⅴ. Remifentanil 0.2, 0.4 or 0.8 μg·kg~(-1)·min~(-1) was infused starting from 15 min before LPS administration until the death of the animals. MAP, HR, peak airway pressure (P_(peak) ), PaO_2 and plasma intercellular adhesion molecule 1 (ICAM-1) concentration were measured immediately before LPS infusion (T_0, baseline) and at 1, 2.5 and 5.5 h after the end of LPS infusion. The animals were killed and the lungs were immediately removed for microscopic examination and determination of W/D lung weight ratio. Results MAP, HR and PaO_2 were significantly decreased while W/D ratio and P_(peak) were significantly increased after iv LPS infusion as compared with control group. LPS significantly increased plasma ICAM-1 concentration and damaged the structure of lung tissue. Remifentanil infusion significantly attenuated the LPS-induced changes in a dose-dependent manner. Conclusion RF has protective effect against LPS-induced ALI and inhibition of ICAM-1 expression is involved in the mechanism.
9.Surveys into the medical needs of outpatients

Objective To improve outpatient medical care. Methods A sample survey was conducted into 1716 outpatients during 6 days in October of 2004 using questionnaires that covered such items as the patients' choice of hospitals, their choice of doctors, their medical needs and their needs for continuous services. Results The medical needs of the patients, regardless of gender, age, and form of payment (personal payment, medical insurance or treatment at public expenses), are, in terms of urgency, disease relief, safety, respect, service, and the right to be informed. Ranking first in the needs for continuous medical services is medical appointments, with telephone follow-ups coming next. Conclusion Outpatients have diversified medical needs and hospitals ought to attach importance to relieving patients from disease, ensuring medical safety, respecting patients, improving services, bettering continuous medical services, and satisfying the needs of patients at different levels.
