1.THE CHOICE OF THE OPERATIVE APPROACHES FOR CERVICAL SPONDYLOPATHY

Different types of cervical spondylosis treated by a vartiety of operative techniques were compared with the aim at improving quality of treatment for cervical spondylosis. 1780 operations including laminectomy, open door laminoplasty, subtotoal vertebrectomy, Cloward's technique, Smith's technique, "keyhole" decompression of the intervertebral foramina, discectomy with intervertebral fusion, intervertebral fixation with titanium cage, and fixation with titanium plate. The results showed:(1)Myelopathy in 363 cases: laminectomy on 4; subtotoal vertebrectomy on 136; Cloward's technique on 201; Smith's technique on 4; intervertebral fixed with titanium on 4; discectomy with fusion on 14. 104 cases were followed up, 82 7% of them showed good and excellent recovery. (2)Myeloradiculopathy in 1106 cases: laminectomy on 3; laminoplasty on 143; subtotoal vertebrectomy on 687; Cloward's technique on 263; Smith's technique on 2; discectomy with fusion on 8. 396 cases were fouowed up, 85 6% of them showed good and excellent recovery. (3)Radiculopathy in 35 cases: posterior approach key hole decompression of the intervertebral foramina on 19; lateral approach decompression of the intervertebral foramina on 9; anterior approach decompression of the intervertebral foramina on 7. Good and excellent results reached 100%. (4)Vertebral artery compression in 9 cases: resection of the osteophyte of the uncinate processes on 5; opening of the transverse foramen on 4. The results were good. (5)Esophagus compression in 18: after removing directly the osteophyte, the symptoms disappeared. (6)Ossification of the posterior longitudinal ligament in 93 cases: subtotal vertebrectomy on 39; laminoplasty on 45; laminectomy on 9. 41 cases were followed up, good and excellent recovery was seen in 85 3%. (7)Cervical stenosis in 156 cases: laminoplasty on 149 and laminectomy on 7. Good and excellent recovery was observed in 97 5%. All the results indicated that a variety of operative techniques could be selected depending on the site of the osteophytes. Direct resection of the lesion which produced compression and fixation or fusion of vertebral bodies were both important to achieve a satisfactory result
2.CERVICAL INTRAMEDULLARY SPINAL TERATOMA: A REPORT OF 2 CASES AND A LITERATURE REVIEW

To discuss the clinical situation and treatment of cervical intramedullary spinal teratoma, two cases of it in our department were presented, and the related literatures were reviewed. A concomitant anomaly of the cervical vertebral canal was found to be accompanied by cervical intramedullary spinal teratoma. MRI showed the tumors with mixed signal intensity. During operation the tumor was found to have both cysts and hard bony nodules. It was very difficult to remove the total tumor. It is concluded that the plain film of cervical vertebra and cervical spine MRI are sensitive to differentiate teratomas from other tumors. Surgery should be the first choice for treatment.
3.Insulin-like growth factor-1 improves diabetic cardiomyopathy through antioxidative and anti-inflammatory processes along with modulation of Akt/GSK-3β signaling in rats.

Diabetic cardiomyopathy (DCM), a serious complication of diabetes mellitus, is associated with changes in myocardial structure and function. This study sought to explore the ability of insulin-like growth factor-1 (IGF-1) to modulate DCM and its related mechanisms. Twenty-four male Wistar rats were injected with streptozotocin (STZ, 60 mg/kg) to mimic diabetes mellitus. Myocardial fibrosis and apoptosis were evaluated by histopathologic analyses, and relevant proteins were analyzed by Western blotting. Inflammatory factors were assessed by ELISA. Markers of oxidative stress were tested by colorimetric analysis. Rats with DCM displayed decreased body weight, metabolic abnormalities, elevated apoptosis (as assessed by the bcl-2/bax ratio and TUNEL assays), increased fibrosis, increased markers of oxidative stress (MDA and SOD) and inflammatory factors (TNF-α and IL-1β), and decreased phosphorylation of Akt and glycogen synthase kinase (GSK-3β). IGF-1 treatment, however, attenuated the metabolic abnormalities and myocardial apoptosis, interstitial fibrosis, oxidative stress and inflammation seen in diabetic rats, while also increasing the phosphorylation levels of Akt and GSK-3β. These findings suggest that IGF-1 ameliorates the pathophysiological progress of DCM along with an activation of the Akt/GSK-3β signaling pathway. Our findings suggest that IGF-1 could be a potential therapeutic choice for controlling DCM.

4.Simultaneous Determination of 5-FU and Its Metabolites 5-FUH2 in Human Plasma by UPLC-MS/MS and Appli- cation

OBJECTIVE:To establish the method for simultaneous determination of 5-fluorouracil(5-FU)and its metabolites 5-fluoro-5,6-dihydrouracil (5-FUH2) in human plasma ,and apply it in the clinic. METHODS :After plasma samples were processed twice by ethyl acetate ,UPLC-MS/MS method was adopted using 5-bromouracil (5-Bru) as internal standard. The determination was performed on Acquity UPLC HSS T 3 column with mobile phase consisted of methanol-water (30 ∶ 70,V/V)at the flow rate of 0.3 mL/min. The column temperature was 20 ℃,and sample size was 5 μL. An electrospray ion source was used to carry out negative ion scanning with multiple reaction monitoring. The capillary voltage was 1.5 kV;the taper hole voltage was 20 V;the desolvent temperature was 450 ℃;the desolvent air flow was 850 L/h;the cone hole gas velocity was 50 L/h. The ion transitions for quantitative analysis were m/z 129.00→41.90(5-FU),m/z 130.87→82.92(5-FUH2),m/z 189.00→42.10(5-Bru), respectively. From Aug. to Oct. 2020,10 patients with advanced colorectal cancer were treated with continuous intravenous drip of 5-FU for 46 hours were collected from Harbin Medicinal University Cancer Hospital. Steady-state plasma concentration of 5-FU and plasma concentration of 5-FUH2 were determined at 18-30 h of continuous intravenous drip. The area under the curve (AUC)for 5-FU and concentration ratio of 5-FUH2/5-FU were calculated. RESULTS :The linear range of 5-FU and 5-FUH2 were 20 to 1 000 μg/mL(R 2>0.990). The quantification limits were 20 ng/mL. RSDs of precision test were all lower than 20%,and relative error ranged ±10%. The extraction recovery and matrix effects didn ’t affect the determination of substance to be measured. Among 10 patients with advanced colorectal cancer ,the steady-state concentration of 5-FU were 180.04-622.83 ng/mL,and AUC of 5-FU ranged from 8.28 to 28.65 mg·h/L. The concentration of 5-FUH2 ranged 336.48-948.43 ng/mL,and concentration ratio of 5-FUH2/ 5-FU ranged 0.93-4.21. AUC of 5-FU in 10 patients had about 3-4 fold individual differences. CONCLUSIONS :The established method has good precision and accuracy ,high sensitivity ,and simple operation. It can be used for plasma monitoring of 5-FU in patients with advanced colorectal cancer.
