1.Therapeutic effect of thalidomide plus irinotecan and cisplatin for recurrent small cell lung cancer

Objective To investigate the efficacy and toxicity of thalidomide plus irinotecan and cisplatin treatment for recurrent small cell lung cancer.Methods 62 Patients with recurrent small cell lung cancer in the same period were randomly divided into observation group and control group according to the principle of minimum distribution imbalance index.The observation group patients were treated with thalidomide plus irinotecan and cisplatin chemotherapy,and the control group patients were treated with irinotecan and cisplatin chemotherapy.The efficacy and toxicity of the two groups were compared.Results The overall response rate in the group observation was 86.7% compared with 63.3% in the control group,and the difference had statistical significance (x2 =8.52,P < 0.05).The major toxicities were hematologic toxicity and gastrointestinal symptoms,and the side effects differences were not statistically significant (x2 =0.18,P > 0.05).Conclusion The treatment of recurrent small cell lung cancer with irinotecan and cisplatin in combination with thalidomide has high efficiency,and the toxicity can be tolerated.
2.Observation of cortical electrical activation during aphasic recovery with EEG approximate entropy

Objective: ①To study whether changes of cortical electrical activations in two hemispheres correlated with speech tasks could be observed with approximate entropy analysis of EEG; ②To dynamically observe the correlation of speech improvements with cortical electrical activations in two hemispheres during speech treatment. Method: Changes of cortical electrical activations were observed with EEG in a conductive aphasic 7、9 and 30 weeks after stroke. EEG was recorded in 3 different conditions: eyes closed, word repetition and non-word repetition. The EEG of a healthy man was recorded in 3 different conditions as control. Result: Compared with eye-closed condition, EEG approximate entropies(ApEn_s) of the healthy subject increased in T3, T5, C3, P3 and O1 for word and non-word repetition, and additional F7 and T6 for non-word repetition task. For the patient, ApEn of the first EEG increased in P3 for word repetition, and P4 for non-word repetition; ApEns of the second EEG increased in F3, F7, P3, C3, C4,T3, T5, O1, and decreased in F8, T4 and O2 in right hemisphere for word and non-word repetition; ApEns of the third EEG increased in T3, P3, F7, O1, O2 for word repetition,and T3, O2 for non-word repetition. The first aphasia assessment showed light impairment of semantic system and initial sound auditory identification and severely impairment of picture naming and word repetition. The semantic system and initial sound auditory identification improved significantly in the second assessment compared with the first; but the word repetition remained unchanged. The range of the activated regions in the left hemisphere decreased in the third EEG compared with the second EEG and the activated regions were less than the normal subject. Conclusion: The cortical electrical activities correlated with repetition tasks and the change of cortical electrical activities in different stages of language recovery could be observed with EEG nonlinear analysis.
3.Treatment of congenital heart disease after cardiopulmonary bypass in infants

Objective To study the treatment and outcome in infants with congenital heart disease after cardiopulmonary bypass. Methods The clinical data of 217 infants with congenital heart disease after cardiopulmonary bypass in Shengjing Hospital from Jan 2005 to Nov 2010 were retrospectively reviewed. Results Eleven infants died and the mortality was 5. 1% ( 11/217 ). Four infants died of severe low cardiac output syndrome,2 died of pulmonary hypertension crisis,2 died of low cardiac output syndrome complicated with acute renal insufficiency, 1 died of severe infection, 1 died of severe hypoxemia,and 1 died of chronic pericardial tamponade. The postoperation length of hospital stay was 7 ~ 52 d, 14 d in average. Conclusion With the improvement of surgical techniques,cardiac surgery with cardiopulmonary bypass on infants can result in good clinical effect. Active and effective treatment can reduce the perioperative mortality after cardiopulmonary bypass.
4.Clinical investigation on relationship among lipoprotein(a) ,cerebral infarction and carotid atherosclerosis

Objective To investigate the correlation among Lp (a),cerebral infarction and carotid atherosclerosis.Methods 180 cases were equally divided into 3 groups (60 cases in each group).The patients with both cerebral infarction and carotid atherosclerosis composed one group(cerebral infarction group for short) ;the patients of carotid atherosclerosis which without cerebral infarction composed another group(no cerebral infarction group for short) ;the control group composed with those who had neither cerebral infarction nor carotid atherosclerosis.The level,outlier detection rate about Lp(a) and usual risk factors of cerebral infarction were compared in this research.Results The level and detection rate of Lp(a) in the cerebral infarction group were (512 ± 156) mg/L and 46.7% ;the level and detection rate of Lp(a) in the no cerebral infarction group were (316 ± 87)mg/L and 20.0% ;the level and detection rate of Lp(a) in the control group were (199 ± 123) mg/L and 5.0%.The differences of the level and outlier detection rate of Lp(a) among the three group were significant(F=13.87,x2 =29.394,P <0.01).Cerebral infarction patients had more usual risk factors of cerebral infarction (hypertension,diabetes,hyperlipemia,drinking,smoking,obesity and lack of exercise) than those who without cerebral infarction (x2 =15.523,P < 0.01).Conclusion The abnormal of Lp(a) was an important risk factor of cerebral infarction.The general survey of Lp(a) was significant in the primary and secondary prevention of stroke.To the people whose assay of Lp(a) was abnormal,the control of governable risk factors of cerebral infarction should actively be carried out.
5.Relationship between serum levels of creatinine,urea nitrogen and lung image changes in uremic patients

Objective To explore the relationship between serum creatinine ( Scr ) , blood urea nitrogen ( BUN) and lung image changes in uremic patients .Methods According to the chest CT ,X-ray and related findings , 256 patients with uremia were divided into 84 cases of uremia ( uremic lung group ) and non-uremic lung 172 cases ( non-uremic lung group ) .The chest CT and X-ray examination ,Scr and BUN levels were observed .Results Uremic lung group had five types of image findings,including alveolar edema (67.86%),pulmonary interstitial edema (16.67%),pulmonary congestion syndrome(8.33%),pleural effusion(4.76%) and pulmonary interstitial fibrosis (2.38%).The Scr[(1 145.6 ±236.4)μmol/L]and BUN [(38.9 ±6.1)mmol/L]levels in uremic lung group were significantly higher than those in the non-uremic lung group[(905.3 ±138.9)μmol/L,(23.8 ±5.6)mmol/L](t=8.128,4.782,all P<0.05).Conclusion The image of uremic lung is mainly pulmonary edema ,and radiological and pathological changes are closely related with Scr and BUN levels .
6.The expression of OCT4 and P27kip1 in renal hyaline cell carcinoma and their correlations

Objective To investigate the expressions of OCT4 and P27kip1 and their correlations in renal hyaline cell carcinoma. Methods A total of 24 samples of renal hyaline cell carcinoma were detected by immunohistochemistry. The rate of cells with positive labelling of OCT4 and P27kip1 and their intra cellular distribution were observed in renal hyaline cell carcinoma. The expression levels of OCT4 and P27kip1 were compared between different gender, age (<60y and≥60y) and cancer cell metastasis groups. Results The rates of cells with positive OCT4 and P27kip1 expressions were 66.7%and 75% in renal hyaline cell carcinoma respectively. The ratio of cells with high, middle and low expression of OCT 4 were 33.3%, 20.5%and 12.5%in renal hyaline cell carcinoma tissues. OCT4 was mainly expressed in cytoplasm and nuclei with a few was expressed in the cell membrane. The ratio of cells with high, middle and low expression of P27kip1 were 12.5%, 25%and 37.5%in renal hyaline cell carcinoma tissues respectively. The positive staining of P27kip1 was in the cytoplasm and cell membrane, and a small number of nuclei were expressed. There were no significant differences in OCT4 and P27kip1 between different age and gender groups. There were significant differences in OCT4 and P27kip1 expressions between patients with metastasis and patients without metastasis (P < 0.05). A negative correlation was found between OCT4 and P27kip1 expres?sions in renal hyaline cell carcinoma (rs=-0.408, P<0.05). Conclusion The expression of OCT4 is negatively correlated with the expression of P27kip1. Inhibiting expressions of OCT4 or P27kip1, especially knocking down P27kip1 in cytoplasm can be used as a new diagnosis and treatment of renal cancer gene therapy.
8.Clinical features and influencing factors for prematurity in both dichorionic and monochorionic diamniotic twins

ObjectiveTo investigate the risk factors of preterm birth, as well as the clinical characteristics in dichorionic diamniotic (DCDA) twins and monochorionic diamniotic (MCDA) twins. MethodsA retrospective study was conducted on 290 premature cases out of 363 twin pregnancies who delivered alive babies in the First Affiliated Hospital, Sun Yat-sen University from September 2012 to March 2015. The selected cases, including 219 cases of DCDA and 71 cases of MCDA,were divided into three groups according to their gestational age at delivery: 28-31+6, 32-33+6 and 34-36+6 weeks. The clinical features, causes and risk factors were described between these three groups. Analysis of variance,Chi-square test and multi-variant Logistic regression were used for statistical analysis.ResultsThe incidence of premature delivery in twin pregnancies was 79.9% (290/363), while this figure was lower in DCDA twins than in MCDA [76.3%(219/287) vs 93.4%(71/76),χ2=10.955,P=0.001]. The three leading causes of preterm birth in DCDA twins were gestational age≥36 weeks (33.8%, 74/219), preterm labor (30.6%, 67/219) and preterm premature rupture of membrane (PPROM) (8.7%, 19/219), while in MCDA twins were preterm labor (31.0%, 22/71), selective intrauterine growth restriction (21.1%, 15/71) and gestational age≥36 weeks (19.7%, 14/71). Logistic regression analysis showed that the independent risk factors of preterm birth in twins at 28-31+6 weeks was PPROM (OR=2.390, 95%CI: 1.006-5.872,P=0.043), and for those twins at 32-33+6 weeks, the independent risk factors were MCDA (OR=2.758, 95%CI: 1.243-6.118,P=0.013), preeclampsia (OR=12.176, 95%CI:4.685-31.642,P=0.000), PPROM (OR=5.348, 95%CI: 2.151-13.294,P=0.000) and preterm labor (OR=3.274, 95%CI:1.453-7.375,P=0.004). MCDA (OR=3.666, 95%CI: 1.364-9.585,P=0.010) and preeclampsia (OR=8.086, 95%CI:1.044-62.617,P=0.045) were the risk factors in the group of 34-36+6 weeks.ConclusionsAlthough preterm birth in MCDA and DCDA twins is due to different reasons, the former has a higher incidence than the latter. The risk factors of premature delivery at different gestations are also different.

10.Treatment and Mechanism of Surface Electrical Stimulation for Severe Dysphagia Caused by Lower Brainstem Infarction

Objective To study the potential mechanism of recovery from dysphagia after surface electrical stimulation. Methods 3 cases recovery from dysphagia caused by lower brainstem infarction after surface electrical stimulation of lower mandible and trigeminal nerves were analyzed. Results After 3~16 weeks surface electrical stimulation, the swallow assessment scores reached from 0 to 6 in all the 3 patients. Conclusion Surface electrical stimulation can facilitate the recovery of swallow function, which may be involved with the sensory input, especially the integration of nucleus of the solitary tract (NTS).
