1.Curative efficacy of Dezocine in treatment of receiving laparoscopic appendectomy and its effects on white blood cell count and C reactive protein

Objective To study curative efficacy of dezocine in treatment of receiving laparoscopic appendectomy and its effects on white blood cell count and c reactive protein.Methods 90 patients of laparoscopic appendectomy who received therapy from January 2015 to October 2016 in our hospital were selected as research objects,according to random number table,those patients were divided into the observation group and the control group,45 cases in each group.The control group was treated with sufentanil, while the observation group was treated with dezocine.Then operation index, T0 (preoperative),T1(extubation),T2(after extubation) mean arterial pressure(MAP),heart rate(HR), respiration rate(RR),isual analogue scale/score ( VAS) , ramsay score ( RSS) , white blood cell count and c reactive protein of two groups after treatment were compared .Results After treatment, MAP,HR in the observation group were significantly lower than control group [(78.30 ±6.20)mmHg vs.(86.08 ±6.09)mmHg,(76.45 ±5.90)mmHg vs.(80.48 ±5.80)mmHg,(90.82 ±9.50)time/min vs.(96.73 ±9.83)time /min,(87.21 ±8.15)time /min vs.(93.59 ±9.90)time /min](P<0.05); VAS, RSS score were significantly lower than the control group[(2.60 ±0.70)score vs.(5.29 ±0.83)score,(3.53 ±0.92)score vs.(6.38 ± 1.21)score](P<0.05); White blood cell count, c reactive protein were significantly lower than the control group[(7.92 ±2.01) ×109/L vs.(14.98 ±2.11) ×109/L,(7.90 ±2.30)mg/L vs(12.46 ±3.10)mg/L](P<0.05).Conclusion Dezocine is well for receiving laparoscopic appendectomy, obvious analgesic effect, can significantly reduce the white blood cell count and c reactive protein.
2.The application of ultrasonography in evaluating cervical malignant lymph nodes

Background and purpose: Metastatic carcinoma and lymphoma are usually the causes of cervical malignant lymph nodes. This paper was aimed to investigate the different appearances of cervical malignant lymph nodes by 2 dimension and color doppler flow imaging (CDFI) and discuss the role of ultrasenography in diagnosing cervical malignant lymph nodes. Methods: Ninety-nine patients and 40 healthy people were selected as our subjects, among which 54 cases were diagnased as metastatic tumor and 45 cases as lymphoma according to pathology examination. With high frequency transducers, ultrasonography was used to document the shape of cervical lymph nodes, the ratio of long/short axis (L/S), types of echo and hilus of lymph node. CDFI features were also recorded and measured in order to analyze the parameters. Results: Compared with image of normal lymph nodes, the image of cervical malignant lymph nodes showed that the shape tended to be round and the ratio of L/S decreased. In addition, the majority of echo types of malignant lymph nodes was a low level echo and easily became fused. Meanwhile, the image of hilus of lymph node tended to disappear or shift. The lymph node of cases with metastatic tumor was shown to be either calcified or liquefied. The rate of blood flow in the artery was observed to be of high velocity and high resistive index (RI). Between metastatic tumor and lymphoma tumor case, there was significant difference in terms of ultrasonographic and CDFI features. All the above results showed that the features of malignant lymph nodes were significantly different from the normal ones. Conclusion: Ultrasonography could be regarded as one method of evaluating cervical malignant lymph nodes.
3.The formula of breviscapine oral disintegrating tablets and its preparation

AIM:To optimize the formulation of breviscapine oral disintegrating tablets and preparation process. METHODS: To adopt the multifactor and multilevel uniform design to optimize the preparation prescription,disintegrating time as the assessment index. RESULTS: The tablets prepared were finer in appearance,disintegrated within 35 s,with desirable taste.The dissolution rate was faster than breviscapine ordinary tablets. CONCLUSION: Breviscapine oral disintegrating tablets achieve the goal of design and it is easy to operate.
4.Quality standard of Breviscapine Orally Disintegrating Tablets

AIM:To establish quality control method for Breviscapine(Orally) Disintegrating Tablets. METHODS: Its main ingredient scutellarin was identified by TLC, the scutellarin in the tablets was determined by HPLC.And the disintegration, dissolution rate and tablet weights were determined. RESULTS: Breviscapine(Orally) Disintegrating Tablets could be disintegrated in 30 seconds,and the tablet weight was fit for the criterion.And the dissolution rate was more quick than the original Breviscapine tablets in the market.A good linearity was obtained within the range of 0.081 6 ?g-0.408 0 ?g with the correlation coefficient(0.999 9.) The recovery was(99.67%) and RSD was 0.47%. CONCLUSION: The method for identifying and determing Breviscapine Orally Disintegrating Tablets is simple,reproducible and practical.
5.Therapeutic effect of acupoint injection and rehabilitation training three-level program on upper limb function and daily life activities of patients with shoulder-hand syndrome after cerebral infarction

ObjectiveTo investigate the effect of acup point injection and rehabilitation techniques on recovery of upper limb function and activities of daily life (ADL) in patients with shoulder-hand syndrome (SHS) after cerebral infarction.Methods Ninety-six patients with SHS after cerebral infarction admitted to Jiuquan City Traditional Chinese Medicine Hospital and Jiuquan City People's Hospital from January 2014 to February 2016 were enrolled, and they were randomly assigned to observation group (56 cases) and control group (40 cases). The patients in the two groups were given the same routine comprehensive cerebral infarction treatments and rehabilitation technological training. In the control group, additionally intermediate frequency physiotherapy was used, while in the observation group, acupuncture and acupoint injection of salvia miltiorrhiza and ligustrazine were added. The patients in two groups were treated consecutively for one month, afterwards, the upper limb motor function, ADL, shoulder joint pain, degree of swelling and range of motion were assessed, and the clinical effects were observed.Results After treatment, the modified Fugl-Meyer motor function score (FMA), modified barthel index (MBI) and the ranges of voluntary activities of shoulder abduction, adduction, flexion, extension, internal rotation and external rotation in patients of two groups were significantly increased than those before treatment; the pain visual analog scale (VAS) and the edema volume difference in observation group were lower than those in the control group, and after treatment, the changes of above indexes were more significant in the observation group than those in the control group [FMA: 70.10±6.38 vs. 41.76±18.69, VAS: 2.00±1.37 vs. 5.00±1.25, MBI: 75.17±3.51 vs. 51.56±2.12, edema volume difference (cm3): 2.13±2.05 vs. 5.75±1.17, flexion: (153.1±6.7)° vs. (56.0±10.9)°, extension: (39.6±5.8)° vs. (17.2±6.7)°, abduction:(168.1±9.6)° vs. (52.1±8.9)°, adduction: (52.1±3.6)° vs. (21.8±4.3)°, external rotation: (49.8±7.6)° vs. (23.2±5.2)°, internal rotation: (107.0±8.5)° vs. (51.2±6.1)°], the total effect in observation group was significantly higher than that in the control group [91.1% (52/56) vs. 70.0% (28/40),P < 0.05].Conclusions Acupuncture and acupoint injection combined with three-level therapeutic regimen of rehabilitation technological training for treatment of shoulder-hand syndrome after cerebral infarction can reduce the patient's pain and edema, and improve upper limb motor function and activities of daily living.
6.Expression of FcγRⅡb on peripheral B cells and its clinical significance in patients with primary Sj(o)gren's syndrome

Objective To investigate the expression of FcγRⅡb on peripheral B cells and its clinical significance in primary Sj(o)gren's syndrome (pSS).Methods FcγRⅡb expression on peripheral B cells from 19 pSS patients and 15 healthy controls was examined by flow cytometry.The levels of serum anti-SSA and SSB antibodies were determined using enzyme-linked immunosorbent assay (ELISA).Data were analyzed with t test,one-way ANOVA,SNK-q test and Pearson's correlation test.Results The percentage of memory CD19+CD27+ B cell subpopulation was significantly lower in pSS patients [ (20.8±2.7)%] when compared to normal controls [(37.8±2.2)% ](t=-4.002,P<0.01).The level of expression of FcγRⅡb in active pSS memory CD19+CD27+ B cells [ MFI(74±8)] was significantly reduced when compared to inactive [ MFI( 132±11)] and normal controls [ MFI (139±12)] (F=10.699,P<0.01).The level of expression of FcγRⅡb on memory CD19+CD27+ B cells from pSS patients was inversely correlated with Sj(o)gren's syndrome disease activity index (SSDAI) (r=-0.744,P=0.0003 ).pSS patients with the serum anti-SSA/SSB antibodies positive group [ MFI(75+3),(48±7)] displayed a lower expression of FcγRⅡb on memory CD19+CD27+ B cell than in patients with the serum anti-SSA/SSB antibodies negative group [MFI( 122±11),(108±9)] (t=-4.336 and -3.776 respectively,the P value of both tests were less than 0.01).The level of expression of FcγRⅡb in the memory CD19+CD27+ B cells of patients with active pSS was inversely correlated with anti-SSA antibody titers (r=-0.685,P=0.014).Conclusion The expression of FcγRⅡb on peripheral memory B cells from active pSS patients is inversely correlated with SSDAI and is also inversely associated with anti-SSA antibody levels.Decreased expression of FcγRⅡb might play an important role in the pathogenesis of pSS.
7.Effects of Yiqi-Fumai injection and Shenshao capsule on vascular endothelial cell function in patients with angina pectoris of coronary heart disease and serum myeloperoxidase

Objective To observe the therapeutic effect ofYiqi-Fumai injection andShenshao capsule in the treatment of angina pectoris and the influence on vascular endothelial cell function and serum MPO. Methods 90 cases of angina pectoris of coronary heart disease were randomly divided into a treatment group and a control group. 50 cases in the treatment group were treated withYiqi-Fumai injection and Shenshao capsule; 40 cases in the control group were treated with Plavix and metoprolol; clinical symptoms were observed before and after treatment, the indexes of endothelial cell function and electrocardiogram blood serum nitric oxide (NO), endothelin (ET), NOS and serum MPO, SOD, GSH changes were compared.Results The content of nitric oxide (NO), 6-keto-PGFLa and NOS of the treatment group were increased than the control group, while ET was reduced. In the treatment group, NO was (78.09 ± 88.71)μmol/L 6-keto-PGFL was 32.04 ± 25.82 pg/ml and NOS was 27.88 ± 9.08 U/MI before the treatment,; after the treatment, the NO was 127.11 ± 103.60μmol/L, 6-keto-PGFLa was 50.03 ± 60.87 pg/ml and NOS was 35.12 ± 22.18 U/MI. In the control group, NO was 96.98 ± 82.45μmol/L, 6-keto-PGFLa was 40.31 ± 51.92 pg/ml and NOS was 28.12 ± 5.11 U/MI before the treatment; After the treatment, NO was 95.62 ± 6.31μmol/L, 6-keto-PGFLa was 29.09 ± 42.01 pg/ml and NOS was 30.11 ± 7.51 U/MI. In the treatment group, ET was 58.81 ± 33.18 pg/ml before the treatment and 42.47 ± 40.5 pg/ml. after the treatment. In the control group, ET was 56.81 ± 32.76 pg/ml before the treatment and 60.67 ± 12.81 pg/ml after the treatment. In terms of resistance to oxidative stress and MPO, the SOD ,GSH and MPO improvement rate of the treatment group was significantly better than the control group (P<0.05). In the treatment group, SOD was 48.36 ± 17.38 U/MI, GSH was 126.68 ± 19.93 g/L and MPO was 57.33 ± 8.34 mmol/L before the treatment, after the treatment, SOD was 62.45 ± 15.76 U/MI, GSH was 135.98 ± 28.99 g/L and MPO was 17.11 ± 3.60 mmol/L. In the control group, SOD was 48.44 ± 19.37 U/MI, GSH was 29.44 ± 21.66 g/L and MPO was 56.35 ± 14.44 mmol/L before the treatment, after the treatment, SOD was 57.35 ± 14.44 U/MI, GSH was 28.56 ± 24.06 g/L and MPO was 48.62 ± 6.31 mmol/L.ConclusionYiqi-Fumai injection combined with Shenshao capsule has curative effect on angina pectoris of coronary heart disease, being worth of clinical use.
8.Flushing methods, temperature and flushing time of sodium hypochlorite affect the clearance effect on the smear layer of root canal

BACKGROUND:In root canal preparation, the clearance effect of sodium hypochlorite for root canal irrigation on the smear layer is associated with the contact time and contact area of irrigation fluid, the amount of irrigation fluid, irrigation fluid temperature, irrigation fluid concentration, flushing methods and so on. OBJECTIVE:To investigate the clearance effect of 3% sodium hypochlorite on the smear layer of root canal under different flushing methods, flushing temperature and flushing time. METHODS:Totaly 180 isolated front teeth with single root were colected and root canal preparation was conducted using motorized ProTaper. The root canal was flushed with 3% sodium hypochlorite. According to different flushing methods, these root canals were randomly and evenly divided into root canal active flushing group, passive ultrasound swished group and side opening flushing needle group. According to the flushing temperature and the flushing time after replacing a file, each group was randomly divided into six subgroups: 20℃ 30 seconds group, 20℃ 60 seconds group, 37 ℃ 30 se group. The clearance effect on the smear layer of root canal was observed using scanning electron microscope. RESULTS AND CONCLUSION:The general clearance effect on the smear layer in the root canal active flushing group was better than that in the passive ultrasound swished group and side opening flush acupuncture group (P< 0.05). At the same time, the clearance effect on the smear layer in the 60℃ group was better that that in the 37 ℃ group and 20℃group (P < 0.05). At a certain temperature, the clearance effect on the smear layer in 60 seconds group was better than that in the 30 seconds group (P < 0.05). The clearance effect on the smear layer in the 1/3 root cap zone was better than that in the 1/3 root and 1/3 apical root zone (P < 0.05). These results show that the best clearance effect on the smear layer can be achieved as folows: 3% sodium hypochlorite irrigation at 60℃ for 60 seconds.
9.Clinical research on methimazole treatment of 379 children with hyperthyroidism at a single institution

Objective To investigate the clinical efficacy and adverse events of methimazole ( MMI ) treatment for children with hyperthyroidism, and to identify the predictors of remission and relapse. Methods A total of379children(260girlsand119boys)diagnosedwithhyperthyroidismandtreatedbyMMIinGuangzhouWomenand Children's Medical Center from March, 2004 to July, 2014 were retrospectively analyzed. The average age at diagnosiswas(9.3±2.3)years(range2.0~15.9years). Results AftertreatmentwithMMIfor3and6months, the thyroid functions of 96. 3%(365/379) and 98. 9%(375/379) patients returned to normal, respectively. By the end of this study, 256(67. 5%) patients continued to use MMI treatment and 44 patients(11. 6%) dropped out. 79 patients(20. 8%) achieved remission, 35 patients (44. 3%) of whom experienced a later relapse. Children who achieved constant remission had significantly lower FT3 and FT4 levels at diagnosis compared with the relapsed children(P<0. 05 or P<0. 01). It was more likely to remain long-term remission for children turned to be euthyroid within 3 months after initiating MMI treatment(P<0. 05). The relieved patients with family history of thyroid diseases weremorelikelytoberelapsed(P<0.05). Therewerenosignificantdifferencesinage,gender,exophthalmos, initial goiter size, thyroid peroxidase autoantibody, and thyroglobulin antibody levels between the relieved and relapsed patients. The overall incidence of adverse events associated with MMI was 27. 7%, mainly elevated alanine aminotransferase, bilirubin, and neutropenia. Most(66. 7%) of adverse events occurred within the first three months of MMI treatment. Conclusion MMI has a good effect on pediatric hyperthyroidism, with low remission and high relapse rate. The low thyroid hormone concentrations at diagnosis and normalization of thyroid function within three months seem to be useful predictors of remission. Vigilance is needed concerning MMI-associated adverse events throughout the MMI treatment period, especially during the first trimester of MMI initiation.
10.Relationship between illness perceptions and health behavior of patients with lower extremity arterial disease

Objectives To explore the relationship between illness perceptions and health behavior in the subjects. Methods Revised illness perception questionnaire and health promoting lifestyle profileⅡ (HPLPⅡ) were used to measure 101 patients′illness perception and health behavior between the groups. Results The HPLPⅡscore of LEAD was(2.17 ± 0.29) with the score of dimension. The course of disease, prognosis and perception of emotion were negatively related to health behaviour; personal control and primaty congtion of drsease were postively related to health behaviour (all P<0.01). Conclusion Medical personnel should pay attention to the relationship between illness perceptions and health behavior in patients with LEAD and effective measures should be taken to improve the patients′illness perceptions, promote their choices and persistence in health behavior and improve the quality of life.
