2.Effect of cardiac resynchronization therapy on heart failure patients with different morphologies of left bundle branch block in ECG

Objective To compare the efficacy of cardiac resynchronization therapy (CRT) on chronic heart failure (CHF) patients with different left bundle branch block (LBBB) morphologies. Methods Patients(n=45)who were treated with CRT were enrolled. According to the intrinsic ECG morphologies, patients were divided into 1)genuineLBBB group (n=32) who present negative dominant V1 and V2 lead wave (QS or rS);mid-QRS notching or slurring in at least 2 leads of Vl, V2, V5, V6, I and aVL as well as QRS duration≥140 ms in male or≥130 ms in female and 2)falseLBBB group (n=10) who meet traditional standards but fail to meet“genuine”LBBB diagnostic standard. The QRS duration, echocardiographic indi?ces and New York Heart Association (NYHA) Functional Classification were evaluated at the 12 months follow-up. CRT re?sponder was defined as patient with≥1 decrease in NYHA class and/or with≥15%reduction in left ventricular end-systolic volume (LVESV). CRT super-responder was defined as patient with≥30%reduction in LVESV. Results There was no dif?ference in basic characteristics of patients between groups. At the 12 months follow-up, 20 patients in genuine LBBB group and 6 patients infalseLBBB group were identified as responders (P>0.05). Compared with those infalseLBBB group, the responders ingenuineLBBB group showed better improvement in left ventricular ejection fraction and left ven?tricular end diastolic diameter (LVEDD) (both P<0.05). Conclusion Left bundle branch block morphology is less predic?tive for the efficacy of CRT. However, patients who show response to CRT withgenuineLBBB profile may get more bene?fits from CRT treatment than the patients withfalseLBBB profile.
3.Dynamic analysis of myocardial enzymes, serum calcium, and electrocardiogram in patients with hypoparathyrodism

24 cases of hypoparathyroidism, 24 cases of normal control were included. The levels of enzymes, concentration of Ca were measured in all cases. The electrocardiogram was carried out in the hypoparathyroidism group carries. The levels of CK, CK-MB, BHDH, LDH and AST were significantly higher in patients with hypoparathyroidism than in normal control cases(P<0.05), especially the levels of CK and CK-MB (P<0.01). After the treatment, the electrocardiogram and enzymes basically returned to normal. The changes of enzymes in patients with hypoparathyroidism may play a benchmark about the severity and treatment.
4.Repairment of pancreas mesenchymal stem cells on injured pancreas tissues

Objective To study the repairment of pancreas mesenchymal stem cells(MSCs) on injured pancreas of rats,and find a new source of cells for treatment of diabetes. Methods ① Pancreases were taken out of three-day-old Wistar rats under bioclean condition,and cells were obtained by V-collagenase digestion. Generations were passed by conventional culture,cells were purified by adherence screening method,cell morphous was observed by Giemsa staining. The expressions of surface marker CD34 and CD44 were determined by FCM,and compared with bone marrow MSCs(BM-MSCs),the differences in character and function were observed. ②The experimental rats were divided into two groups randomly. Pancreas ischemic necrosis model was made by deligation,then the purified pancreas MSCs were marked with DAPI and then were transplantated partially. After two weeks,the survival rate was measured and histopathological detection was performed. Results ① The cells had concordant morphous gradually with vigorous generation. There was no significant difference in morphology and surface antigen compared with BM-MSCs.② The survival rate in experimental group was 75% ,the necrotic tissue had basi-rebounded. 5.Value of combined detection of serum DCD, CA199, CA724, and CEA in diagnosis of colorectal cancer

Objective To investigate the possibility of early diagnosis of colorectal cancer by combined detec-tion of tumor markers dermcidin(DCD),CA199,CA724 and CEA. Methods We selected 81 colorectal cancer patients and 30 healthy individuals. Serum levels of DCD were detected by ELISA,and serum levels of CA199, CA724 and CEA were detected by electrochemical luminescent immunoassay,and the expression of DCD in colorectal cancer tissue and para-carcinoma tissue was measured by Western blot. Results Serum levels of CA199,CA724,and CEA were significantly higher in colorectal cancer patients than in healthy individuals(P<0.001). Combined detection of DCD,CA199,CA724 and CEA had a greater AUC(0.942)for identifying colorec-tal cancer from the controls. The expression of DCD was significantly higher in colorectal tissue than in para-carci-noma tissue(P<0.05). Conclusions Level of DCD expression was significantly higher in colorectal tissue than in para-carcinoma tissue. Expressions of serum DCD,CA199,CA724,and CEA may be a marker in diagnosis of can-cer,with higher clinical value for the adjuvant diagnosis of colorectal cancer.
6.Moderate cyclic compressive stress accelerates anabolism of articular chondrocytes by affecting cytoskeleton

BACKGROUND:Different mechanical stimulations may have an effect on the level of metabolism of chondrocytes, but the effect is not clear.
OBJECTIVE:To investigate expression level changes in metabolic genes that participate in cartilage cel decomposition and synthesis under compressive stress and tensile stress conditions.
METHODS:We obtained articular chondrocytes from 2-week-old Sprague-Dawley rats. Primary cultured chondrocytes were identified. Passage one chondrocytes received cyclic tensile stress and cyclic compressive stress of 3%and 7%, respectively, so as to measure articular changes in chondrocytes-related genes.
RESULTS AND CONCLUSION:When chondrocytes were subjected to cyclic tensile stress of 3%, synthetic metabolic gene col agen types I and II and proteoglycan mRNA expression levels were decreased. If 3%cyclic compressive stress was applied, proteoglycan mRNA expression levels were increased, and type I col agen mRNA expression levels were decreased (P<0.001), and matrix metal oproteinase-13 mRNA expression levels were reduced (P<0.01). When strain reached 7%, cyclic tensile stress and compressive stress could lead to a general decrease in anabolism-related genes. The former could also make matrix metal oproteinase-13 mRNA expression levels increased (P<0.05). 3%cyclic compression ratio and 3%cyclic stretch made cytoskeleton become oval. These results indicated that in vitro, proper cyclic compressive stress is beneficial to maintain the growth characteristics of articular chondrocytes in rats. Smal tensile stress can decrease the synthesis ability of chondrocytes. The effect of stress may be caused by changing the cytoskeleton.
7.Optimization of Extraction Technology for the Total Saponins from the Root of Thladiantha Dubia Bunge

Objective:To optimize the extraction process for the total saponins from the root of Thladiantha dubia Bunge. Meth-ods:The extraction technology was optimized by orthogonal experiment with the dissolution content of total saponins as the index and the extraction times, extraction duration and ratio of solid to liquid as the influencing factors. Results:The optimum extraction condi-tions for the total saponins from the root of Thladiantha dubia Bunge were as follows:the reflux extraction was conducted twice(1. 5 h per time) with 70% ethanol as the solvent, and the ratio of solid to liquid was 1 ∶6. Conclusion:The optimum extraction technology is simple, reproducible and stable.
8.Influence of multidrug resistance gene 1 C3435T genetic polymorphism on the eradication of gastric ulcer with Helicobacter pylori infection

Objective To study the influence of multidrug resistance gene (MDR)1 C3435T genetic polymorphism on the eradication of gastric ulcer with Helicobacter pylori (Hp) infection.Methods A total of 106 gastric ulcer patients with positive Hp were randomly divided into two groups by lot with 53 cases each.One group was assigned with 20 mg esomeprazole,0.5 g clarithromycin,1.0 g amoxicillin twice one day(EAC group),and the other group was assigned with 20 mg omerprazole,0.5 g clarithromycin,1.0 g amoxicillin twice one day (OAC group).The therapy of two groups was one week.Hp was detected at least 4 weeks after the end of treatment.MDR1 C3435T genetic polymorphism was determined by polymerase chain reaction-restriction fragment length polymorphism assay.The influence of MDR1 C3435T genetic polymorphism on the eradication of Hp was recorded and analyzed.Results The Hp eradication rate was 84.9% (45/53) and 77.4% (41/53) in EAC group and OAC group,and there was no significant difference between two groups (P > 0.05).There was no significant difference in the Hp eradication rate in patients with different MDR1 C3435T genotypes in two groups (P > 0.05).The Hp eradication rate was 66.7%(16/24),86.3%(44/51),83.9%(26/31) in TT,CT,CC genotype,and there was significant difference(P< 0.05).The Hp eradication rate in patients with TT genotype was lower than that in patients with CT,CC genotype,and there was significant difference (P< 0.05).Conclusion There is significant relationship between the effect of gastric ulcer with Hp eradication and MDR1 C3435T genetic polymorphism,and the Hp eradication rates of patients with TT genotype are more lower.
9.Changes of Hydromechanics Using Vector Flow Mapping in Patients with Chronic Heart Failure

PurposeTo explore the left ventricle flow propagation process in patients with chronic heart failure (CHF) using velocity flow mapping. Materials and Methods Patients with CHF (n=25) were selected as CHF group. Healthy subjects (n=28) were chosen as the control group. Flow reversal interval (FRI), flow reversal rate (FRR), corrected flow reversal rate (CFRI) and flow rate in peak ejection (FRPE) were measured. Parameters related to the vortex in CHF group were compared with that in the control group. Results Compared with the control group, FRI was longer [(71±13) msvs. (113±30) ms, P<0.05], FRR and FRPE were lower [(46.85±21.35) cm2/svs. (27.90±16.77) cm2/s;(74.54±9.10) cm2/svs. (41.12±17.25) cm2/s,P<0.05] in CHF group. FRI and CFRI were positively correlated with the Tei index (r=0.812 and 0.849,P<0.01). FRI was negatively correlated with LVEF, FS, SV, CO, CI (r=-0.646--0.515,P<0.05 orP<0.01). CFRI was negatively correlated with LVEF, FS, SV, CO and CI (r=-0.685--0.456,P<0.05 orP<0.01); FRR was negatively correlated with Tei index (r=-0.604,P<0.05), and was positively correlated with LVEF, FS, SV, CO and CI (r=0.503-0.812,P<0.05 orP<0.01);FRPE was negatively correlated with Tei index (r=-0.734,P<0.01), and were positively correlated with LVEF, FS, SV, CO and CI (r=0.557-0.776,P<0.05 orP<0.01).Conclusion Velocity flow mapping can demonstratecardiac hydromechanics in patients with chronic heart failure. The heart blood flow direction reversal efficiency may be a new method for diagnosis and treatment evaluation.
10.Expression of FcγRⅡb on peripheral B cells and its clinical significance in patients with primary Sj(o)gren's syndrome

Objective To investigate the expression of FcγRⅡb on peripheral B cells and its clinical significance in primary Sj(o)gren's syndrome (pSS).Methods FcγRⅡb expression on peripheral B cells from 19 pSS patients and 15 healthy controls was examined by flow cytometry.The levels of serum anti-SSA and SSB antibodies were determined using enzyme-linked immunosorbent assay (ELISA).Data were analyzed with t test,one-way ANOVA,SNK-q test and Pearson's correlation test.Results The percentage of memory CD19+CD27+ B cell subpopulation was significantly lower in pSS patients [ (20.8±2.7)%] when compared to normal controls [(37.8±2.2)% ](t=-4.002,P<0.01).The level of expression of FcγRⅡb in active pSS memory CD19+CD27+ B cells [ MFI(74±8)] was significantly reduced when compared to inactive [ MFI( 132±11)] and normal controls [ MFI (139±12)] (F=10.699,P<0.01).The level of expression of FcγRⅡb on memory CD19+CD27+ B cells from pSS patients was inversely correlated with Sj(o)gren's syndrome disease activity index (SSDAI) (r=-0.744,P=0.0003 ).pSS patients with the serum anti-SSA/SSB antibodies positive group [ MFI(75+3),(48±7)] displayed a lower expression of FcγRⅡb on memory CD19+CD27+ B cell than in patients with the serum anti-SSA/SSB antibodies negative group [MFI( 122±11),(108±9)] (t=-4.336 and -3.776 respectively,the P value of both tests were less than 0.01).The level of expression of FcγRⅡb in the memory CD19+CD27+ B cells of patients with active pSS was inversely correlated with anti-SSA antibody titers (r=-0.685,P=0.014).Conclusion The expression of FcγRⅡb on peripheral memory B cells from active pSS patients is inversely correlated with SSDAI and is also inversely associated with anti-SSA antibody levels.Decreased expression of FcγRⅡb might play an important role in the pathogenesis of pSS.
