Objective To explore clinical effect and life quality study of Shenyi capsule combined with docetaxel and cisplatin in treatment of advanced breast cancer.Methods 135 cases of advanced breast cancer of Huangshi Central Hospital were collected and diagnosed from July 2013 to December 2014 in accordance with the random number table,were divided into treatment group(n=69) and control group(n=66).Both groups were given regular basis treatment, control group was treated by docetaxel(75 mg/m2,twice a day)and cisplatin(25 mg/m2,once a day), and treatment group combined with Shenyi capsule(20 mg,twice a day)on the basis of control group treatment.4 weeks were 1 course, and all the cases were taken 12 weeks of continuous treatment.Observed and compared the clinical efficiency and life quality of two groups.Results The remission rate of 63.77% in treatment group was higher than control group of 40.91%, difference was statistically significant(χ2 =7.070,P=0.008).After treatment, the SF-36 scale in treatment group significantly increased from ( 54.69 ±8.78 ) to ( 63.87 ±12.08 ) , and higher than that of control group ( 57.95 ±11.95 ) , differences were statistically significant(P<0.05).The adverse reactions in treatment group was lower than control group, difference was statistically significant(Z=2.858,P =0.000).Conclusion The clinical effect in treatment of advanced breast canceris obvious by Shenyi capsule combined docetaxel and cisplatin.It can significantly improve clinical effect and life quality.

Objective To evaluate the operative results of the prolapse of lumbar intervertebral disc and segmental instability.Methods37 patients with prolapse of lumbar intervertebral disc and segmental instability were treated surgically, including nucleus gelatinosus extirpation,!lumbar fusion and transpedicular screw fixation with an average 9-month follow-up after the operation.ResultsAfter the treatment, the completely spine arthrodesis was obtained in all cases. Of 37 cases, 28 cases showed excellent clinical results, 7 cases were good, 2 cases improved, and the total fineness rate was 95.6%.ConclusionIt is an effective therapy for patients with prolapse of lumbar intervertebral disc and segmental instability that fusing and fixing the lumbar with transpedicular screw after nucleus gelatinosus extirpation.

Objective To investigate curative efficacy and safety of Epirubicin in combination with Capecitabine in the treatment of triple-negative breast cancer and its effects on level of serum vascular endothelial growth factor C ( VEGF-C).Methods 86 patients of triple-negative breast cancer who received therapy from January 2013 to December 2014 were selected as research objects.According to therapeutic schemes,those patients were divided into control group(n=42)and observation group(n=44).Control group was treated with epirubicin,while observation group was treated with Epirubicin in combination with Capecitabine.Then, the short-term curative efficacy, adverse events effects,level of VEGF-C were observed and compared.Results After the course of treatment, the short-term curative efficacy ratio in control group and observation group was 59.5% and 65.9% respectively with no statistical difference.During the treatment, the occurrence of neutropenia, hand-foot syndrome, gastrointestinal reaction, cardiac damage and liver function damage in the two groups was statistically same respectively.After post-treatment, in comparison with control group, level of VEGF-C in observation group was statistically lower ( P <0.05 ).Conclusion Epirubicin in combination with Capecitabine is effective for triple-negative breast cancer,which has high short-term curative efficacy ratio and significantly decrease level of VEGF-C with not increasing occurrence of toxic side effects.

Objective To observe the changes of plasma lipid,C-reactive protein(CRP) and fibrinogen(FIB) in patients with acute coronary syndrome(ACS)treated with low dose simvastatin.Methods One hundred and twenty patients with ACS were randomly divided into treatment group and control group with 60 cases in each.The patients in the control group were treated with conventional therapy,and those in the treatment group were treated with 20 mg simvastatin per day at the base of control group.The course of treatment was 2 months.Results In the treatment group,the levels of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),CRP and FIB were decreased significantly and the level of high-density lipoprotein cholesterol(HDL-C) was increased after the treatment(all P0.05).Conclusion The levels of plasma lipid,CRP and FIB can be effectively regulated with the treatment of low dose simvastatin in patients with ACS.

Objective To study the effect and mechanism of Gehua Jiecheng decoction (GHJCD),a tra-ditional Chinese herbal medicine compound,on cell cycle regulation factors-Cyclin D1,CDK4 and p27 KIP1 in HBV transgenic mice with hepatocellular carcinoma (HCC)precancerous lesions induced by alcohol.Methods Altogether 24 HBV transgenic mice were randomly divided into normal group,model group and GHJCD group(each n =8).All the mice were sacrificed at the end of the 22 th week,then their livers were dissected.Pathohistological changes of liver were observed by using HE staining,and the expression of CyclinD1,CDK4 and p27 KIP1 protein in hepatic tissue were detected by using immuno-histochemistrical methods.Results Compared with the normal group,the positive expressions of Cy-clinD1 and CDK4 increased and that of p27 KIP1 decreased significantly (P <0.05)in the model group. Compared with the model group,restoration of injured liver tissue was occurred in GHJCD group,and the position expressions of CyclinD1 and CDK4 decreased significantly (P <0.05 )and the expression of p27 KIP1 increased significantly (P <0.05).Conclusion The protective effects of GHJCD on liver injures and HCC precancerous lesions took effects by downregulation of genes of CyclinD1 and CDK4 and upreg-ulation of gene of p27 KIP1 .

BACKGROUND:At present,the biological functions and molecular changes of bone marrow mesenchymal stem cells in the tumor microenvironment of acute myeloid leukemia are still unclear. OBJECTIVE:To explore the changes in the biological function of bone marrow mesenchymal stem cells in acute myeloid leukemia and the role of acute myeloid leukemia conditioned medium by bioinformatics and experiment. METHODS:Differential genes were screened from GEO data sets,and enrichment analysis was performed.The protein-protein interaction network was constructed and the Hub gene was obtained.Bone marrow mesenchymal stem cells from patients with acute myeloid leukemia and healthy donors were cultured.Bone marrow mesenchymal stem cells from healthy donors were treated with acute myeloid leukemia conditioned culture solution.Each group was subjected to the adipogenic differentiation,osteogenic differentiation,staining of β-galactosidase,detection of the cell cycle,and validation of Hub genes. RESULTS AND CONCLUSION:(1)Gene expression data of bone marrow mesenchymal stem cells from acute myeloid leukemia patients and healthy donors were obtained from GSE84881,and 184 up-regulated genes and 140 down-regulated genes were screened.(2)The biological functions of enrichment mainly include cell cycle,adipocyte differentiation,cell metabolism,and MYC pathway.According to the Degree algorithm,10 up-regulated Hub genes and 10 down-regulated Hub genes were selected.(3)The cell in vitro experiment found that:compared with the control group,the surface antigen of acute myeloid leukemia mesenchymal stem cells did not change,but it showed enhanced lipid differentiation ability,weakened osteogenic differentiation ability,increased β-galactosidase positive cell number,altered cell morphology,arrested cell cycle,increased LGALS3 expression,and decreased MYC expression.Mesenchymal stem cells from healthy donors showed similar changes after being cultured in acute myeloid leukemia conditioned medium.(4)The results show that biological function of mesenchymal stem cells is altered in the acute myeloid leukemia microenvironment,which provides new insights into the interaction between mesenchymal stem cells and tumor cells.

There is a certain correlation and interaction between circulating tumor cells and recurrence-metastasis of colorectal cancer. The circulating tumor cells hidden in the human body can lead to the recurrence and metastasis of colorectal cancer， and the recurrence and metastasis of colorectal cancer can in turn cause a sharp increase in the number of circulating tumor cells， which is consistent with the characteristics of latent pathogens in traditional Chinese medicine leading to diseases. Starting from the theory of latent pathogens， this article summarized the etiology and disease mechanism of recurrence-metastasis of colorectal cancer as harmed healthy qi， latent pathogenic qi， prolonged latent pathogens， and triggering healthy qi deficiency. This article also proposes four major treatment methods as reinforcing healthy qi， assisting yang， opening the exterior， and expelling pathogen， in order to provide ideas and methods for the prevention and treatment of recurrence and metastasis of colorectal cancer.

A 68-year-old male was admitted due to fatigue and poor appetite and diagnosed pathologically as pancreatic adenocarcinoma with liver metastasis. The tumor marker carbohydrate antigen 199 (CA199) level was 2003.4 U/mL. The patient received two cycles of modified FOLFIRINOX plus immune checkpoint inhibitor (penpulimab). However, the tumor did not shrink and CA199 level was even higher. Anlotinib was added from the 3rd cycle, and the size of primary tumor and metastatic lesions were significantly reduced. Laparoscopic distal pancreatectomy and splenectomy as well as liver metastasis resection was performed. Three cycles of combined therapy were adopted after surgery followed by maintenance therapy with anlotinib plus penpulimab. There was no evidence of tumor recurrence during the follow-up (nearly 19 months since diagnosis).

OBJECTIVE：To study the mechanism of improvement effects of Fupi rougan granule （FRG）on hepatic fibrosis model rats. METHODS ：The rats were randomly divided into blank group ，model group ，Colchicine tablet group （chemical positive control ，0.2 mg/kg），Fuzheng huayu capsule group （TCM positive control ，0.415 g/kg），FRG low-dose ，medium-dose and high-dose groups （20，40，80 g/kg），with 10 rats in each group ，except for 11 rats in blank group and model group （one rat was used to judge whether the modeling was successful ）. Except for blank group ，other groups were given intraperitoneal injection of 50％ CCl4 olive oil solution and intragastric administration of 30％ ethanol to induce hepatic fibrosis model. After modeling ， administration groups were given relevant medicine intragastrically ；blank group and model group were given constant volume of normal saline intragastrically ，once a day ，for consecutive 4 weeks. After last administration ，morphology changes of liver tissue in rats were observed. The serum levels of HA ，LN，PCⅢ and Col Ⅳ in rats were detected ，and protein expression of Beclin- 1 and LC3-Ⅱin liver tissue were also determined. mRNA and protein expression of Akt ，AMPK，mTOR，p70S6K were detected in liver tissues of rats. RESULTS ：Compared with blank group ，the structure of hepatic lobules in the model group was disordered ，the proliferation of fibrous tissue was obvious ，and some pseudolobules were formed ；the serum levels of HA ，LN，PCⅢ and Col Ⅳ， the protein expression of Beclin- 1 and LC 3-Ⅱ in liver tissue as well as mRNA and protein expression of Akt ，AMPK，mTOR and p70S6K were increased significantly （P＜0.01）. Compared with model group ，the liver injury of rats in FRG groups was significantly relieved ，and the levels of the above indexes in serum and liver tissue （except for LN and PC Ⅲ in FRG low-dose group） were significantly reduced （P＜0.05 or P＜0.01）. CONCLUSIONS ：FRG can improve hepatic fibrosis in rats ，the mechanism of which may be associated with down-regulating the expression of autophagy associated protein and Akt/AMPK/mTOR/ p70S6K signaling pathway related protein.
