1.A clinical observation on compound trabeculectomy in treatment of refractory glaucoma

Objective To investigate the therapeutic efficacy of refractory glaucoma with compound trabeculectomy.Methods 26 eyes (24 cases ) of refractory glaucoma were treated with compound trabeculectomy.Results Followed up 1 year.There were functional filtering blebs in 20 eyes ;and the IOP was below 21mmHg in 16 eyes without any antiglaucoma drug and in 8 eyes with 1-2 categories of lower IOP drugs after 12 months.The mean IOP at 1 week,1 month,3 months,6months and 12months was (7.9 ± 3.1 ) mm Hg,( 13.5 ± 3.7 ) mm Hg,( 18.2 ±3.8)mm Hg,( 18.1 ±3.5)mm Hg,( 19.1 ±4.8)mm Hg,significantly higher than the IOP preoperatively( 38.2 ±5.6)mm Hg( P <0.01 ).Conclusion It was safe and effective of treating refractory glaucoma with compound trabeculectomy.
2.Study on GC Fingerprint of Lingyang Ganmao Tablets

OBJECTIVE:To establish the GC fingerpriont for Lingyang ganmao tablets.METHODS:The determination was performed on DB-5 ms capillary column.Flame ionization detector was adopted with temperature of 250 ℃ (temperature programming).The temperature of injector was 240 ℃.Carrier gas was nitrogen with flow rate of 1.0 mL/min;the sample size was 1 μL by split sampling with split ratio of 20 ∶ 1.Using peppermint ketone as reference,Similarity Evaluation Software for Chromatographic Fingerprint of Traditional Chinese Medicine (2012 edition) was used for the similarity analysis of 10 batches of Lingyang ganmao tablets.RESULTS:There were 14 common peaks in the batches of Lingyang ganmao tablets,similarity degrees were higher than 0.98.It was proved that the GC profiles and control fingerprint of 10 batches of samples had good consistency.CONCLUSIONS:The established fingerprint can provide reference for the identification and quality evaluation of Lingyang ganmao tablets.

4.Antidepressant-like effects of piperine and its neuroprotective mechanism.

OBJECTIVETo observe the antidepressant effect of piperine and its neuroprotective mechanism.
METHODThe behavioral studies were performed in forced swimming test (FST) and tail suspension test (TST). To further explore the mechanisms underlying their antidepressant-like activities, CORT-induced neuroblastoma SH-SY5Y cells and isolated and cultured neural progenitor cells. By using MTT assay, the effect of piperine on neural cells proliferation was observed.
RESULTThe research results indicated that after a week of administration, piperine (10, 20 mg x kg(-1)) could significantly reduce the duration of immobility in both FST and TST. Piperine has the protective effect on neuroblastoma cells and increased proliferation of hippocampus neural progenitor cells.
CONCLUSIONIn the present study, we demonstrated that the antidepressant-like effects of piperine and its mechanisms might be involved by up-regulation of the progenitor cell proliferation of hippocampus and cytoprotective activity.

5.The study of acoustically evoked short latency negative responses in normal guinea pigs

Objective To compare acoustically evoked short latency negative responses (ASNR) elicited from normal and profound hearing loss guinea pig ears and to confirm their vestibular nuclei origination.Methods Forty healthy guinea pigs were employed in the experiment, which were randomly divided into the control group (8 subjects, 16 ears), the masking group (16 subjects, 32 ears) and the deafened group (16 subjects, 32 ears).Air conductive white noise was chosen for masking.Masking dilemma was avoidable by an appropriate 15 dB gap between stimulus and masking sound.Both the nasking group and the deafened group were further divided into ASNR group and non-ASNR group based on the presence of ASNR.Electrolytic lesion was conducted to the vestibular nuclei, followed by ABR/ASNR recording.The lesioned brainstem slices were microscopically verified.Results In the masking group, ASNR were present in 24 ears (75.0% , 24/32) and 12 ears (46.2% , 12/26) in deafened group, showing statistically higher presence rate for masking group (x2 =5.07, P =0.024).There were no significant differences for the ASNR threshold and latency between the masking ASNR group and the deafened ASNR group.For the two ASNR groups, electrolytic destruction to the vestibular nuclei subsequently eliminated the ASNR.Brainstem slice proved the accurate sites of electrolytic lesion.Conclusion Moderate white noise masking prevents hearing system potential overlap without affecting vestibular system, therefore, ASNR is successfully elicited in normal guinea pigs.Both ASNRs from normal and deafened guinea pigs are of similar natures and origination from, the vestibular nuclei.
6.Research progress of gut microbiota dysbiosis after cholecystectomy

Gut microbiota is involved in the physiological,biochemical and pathological processes of the body.It could participate in the immunoregulation and the development of enteric nervous system,prevent the invasion of pathogens,maintain normal bowel movement and inhibit the occurrence of tumor.Gut microbiota dysbiosis may also cause many diseases,such as inflammatory bowel disease,obesity and diabetes.A variety of diseases can affect gut microbiota with different ways.In addition,remote organs play a counteractive rule to gut microbiota.Maintaining the balance of the bacteria sets different requirements in different disease processes.This paper reviewed the advanced progression of gut microbiota dysbiosis after cholecystectomy and provided a new therapeutic target for gallbladder diseases.
7.Genetic analysis of a case with 11β hydroxylase deficiency caused by CYP11B2/CYP11B1 chimeric gene.

OBJECTIVE:
To analyze a child with 11β hydroxylase deficiency (11β-OHD) due to CYP11B2/CYP11B1 chimeric gene.
METHODS:
Clinical data of the child who was admitted to Henan Children's Hospital on August 24, 2020 were retrospectively analyzed. Peripheral blood samples of the child and his parents were collected and subjected to whole exome sequencing (WES). Candidate variant was verified by Sanger sequencing. RT-PCR and Long-PCR were carried out to verify the presence of chimeric gene.
RESULTS:
The patient, a 5-year-old male, had featured premature development of secondary sex characteristics and accelerated growth, and was diagnosed with 21 hydroxylase deficiency (21-OHD). WES revealed that he has harbored a heterozygous c.1385T>C (p.L462P) variant of the CYP11B1 gene, in addition to a 37.02 kb deletion on 8q24.3. Based on the guidelines from the American College of Medical Genetics and Genomics (ACMG), the c.1385T>C (p.L462P) was rated as a likely pathogenic variant (PM2_Supporting+PP3_Moderate+PM3+PP4). The results of RT-PCR and Long-PCR suggested that CYP11B1 and CYP11B2 genes have recombined to form a CYP11B2 exon 1~7/CYP11B1 exon 7~9 chimeric gene. The patient was diagnosed as 11β-OHD and effectively treated with hydrocortisone and triptorelin. A healthy fetus was delivered following genetic counseling and prenatal diagnosis.
CONCLUSION
11β-OHD may be misdiagnosed as 21-OHD due to the potential CYP11B2/CYP11B1 chimeric gene, which will require multiple methods for the detection.

Objective This study established a model of acoustically evoked short latency negative response (ASNR) in guinea pigs.Stereotaxic coordinate guided electrolytic lesion was applied to animal brainstem nuclei,the vestibular nucleus and the cochlear nucleus,to define the neural origin of ASNR.Methods Twenty four guinea pigs with normal hearing were randomly divided into the control group (8 subjects,16 ears) and the deafened group (16 subjects,32 ears).Each animal experienced the auditory brainstem response (ABR) test.According to the presence of ASNR,the deafened group was further divided into ASNR group and non-ASNR group.Electrolytic lesion was conducted to the vestibular nucleus and cochlear nucleus respectively,followed by ABR test.The lesion structures were verified by brainstem slice and microscope.Results In deafened group,the ASNR was elicited in 10 ears (31.3%).The ASNR was eliminated due to the electrolytic destruction to the vestibular nucleus,but it remained unchanged after the same procedure to the cochlear nucleus.Conclusion It is clear that the ASNR is originated from the vestibular nucleus,but not the cochlear nucleus.
9.Expression of m6 A-modified methyltransferase-like 14 in the hippocampus of depression-like mice

Objective To investigate the expression of m6 A methyltransferase-like protein 14(METTL14)in the hippocampus of depression-like mice,and to provide a theoretical basis for further study of the molecular mecha-nism of depression and new drug targets.Methods 1)Twenty-four male C57BL/6J mice were divided into con-trol group and depression model group.The control group was fed normally,and the model group was kept in single animal cage for 4 months and added with Chronic Unpredictable Animal Stress(CUMS)to establish the depression-like model.2)After modeling,depression-like behaviors in model group were tested using forced swimming test(FST),sucrose preference test(SPT)and tail suspension test(TST).3)Twenty-four hours after completion of behavioral test,the hippocampus tissues of mice were collected,and the expression of m6 A methyltransferase-like protein METTL14 in the hippocampus of mice was detected by molecular biology experiments.Results 1)The mice in the model group showed significant depression-like behavior;2)The expression of MET-TL14 in the hippocampus of the model group showed an increased mRNA expression with the increased m6 A modifi-cation.3)In the model group,the expression of METTL14 in hippocampal CA1,CA3 and DG districts increased and there was neuronal damage found.Conclusions m6 A methylation function has been proved to be more active in the hippocampus of depressed mice.
10.Correlation analysis of serum FOXO1 and MCL1 levels with severity and short-term prognosis in patients with acute ischemic stroke

Objective To explore the correlation between serum forkhead box protein O1(FOXO1)and myeloid cell leukemia protein 1(MCL1)in patients with acute ischemic stroke(AIS)and the severity and short-term prognosis of the disease.Methods A total of 359 AIS patients admitted to the hospital from Feb-ruary 2020 to February 2023 were regarded as the study subjects.According to the National Institutes of Health Stroke Scale(NIHSS)scores at admission,359 patients were grouped into mild group(131 cases),moderate group(156 cases),and severe group(72 cases),and the patients were separated into a good progno-sis group(263 cases)and a poor prognosis group(96 cases)based on the modified Rankin Scale(mRS)score after 90 days of onset.Real-time fluorescence quantitative PCR(qPCR)was applied to detect serum FOXO1 mRNA expression level.Enzyme linked immunosorbent assay(ELISA)was applied to detect the expression level of serum MCL1.Multivariate Logistic regression analysis method was applied to analyze the factors that affected the short-term prognosis of AIS patients.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum FOXO1 mRNA and MCL1 expression for short-term prognosis of AIS patients.Results Compared with the mild group,the serum FOXO1 mRNA level in the moderate and severe groups was obviously reduced(P<0.05),while the MCL1 level was obviously increased(P<0.05).The FOXO1 mRNA level in the severe group was lower than that in the moderate group(P<0.05),and the MCL1 level was higher than that in the moderate group(P<0.05).Compared with the good prognosis group,the serum FOXO1 mRNA level was significantly lower in the poor prognosis group,and the level of MCL1,C-reactive protein(CRP),age,NIHSS score,and proportion of diabetes were significantly higher than those in the poor prognosis group(t/χ2=11.328,7.617,5.344,2.314,16.788,4.459,all P<0.05).The re-sults of multivariate Logistic analysis showed that FOXO1 mRNA was a protective factor for the short-term prognosis of AIS patients(OR=0.726,P<0.05),while MCL1,CRP,and NIHSS score were independent risk factors affecting the short-term prognosis of AIS patients(OR=1.334,1.319,1.442,P<0.05).The results of ROC curve showed that the area under the curve(AUC)of serum FOXO1 mRNA level for predicting poor short-term prognosis of AIS patients was 0.807,with sensitivity and specificity of 69.79%and 85.93%re-spectively.The AUC of serum MCL1 level for predicting poor short-term prognosis of AIS patients was 0.824,with sensitivity and specificity of 71.87%and 84.79%,respectively.The AUC of the combination of serum FOXO1 mRNA and MCL1 for predicting poor short-term prognosis of AIS patients was 0.886,with sensitivity and specificity of 85.42%and 81.37%,respectively.Conclusion The expression of FOXO1 mRNA and MCL1 is abnormal in AIS patients,they are related to the severity of AIS and have high predictive value for the short-term prognosis of AIS.
