1.Laparoscopic High Ligation of Inguinal Hernia:Report of 260 Cases

Objective To study the efficacy and safety of laparoscopic high ligation of indirect inguinal hernia in children. Methods From February 2006 to February 2008,totally 260 children with indirect inguinal hernia underwent laparoscopic high ligation in our hospital.Two trocars were used to ligate the inner circle.Results The procedure was completed successfully in all of the cases.The mean operation time was 22 minutes(range,15-45 minutes).A 2-to 23-month follow-up(mean 11.6 months) was achieved in 259 of the patients.During the period,no patient showed incisional hernia,iatrogenic cryptorchidism or atrophy of the testicles,or intestinal adhesion.One patient showed recurrent hernia in 5 months after the surgery(0.4%). ConclusionLaparoscopic high ligation of indirect inguinal hernia is a minimally invasive,effective,and safe procedure for children.
2.Expression of bFGF and BMP-2mRNA of the local femoral head in nontraumatic osteonecrosis of femoral head

[Objective]To explore the change and its significance of the expression of basic fibroblastic growth factor(bFGF) and bone morphogenetic protein-2(BMP-2) of the local femoral head in nontraumatic osteonecrosis(NONFH). [Methods]Thirty samples of femoral heads of NONFH were collected as the experimental group and 10 fresh samples of femoral heads of femoral neck fracture as control group.The specimens were collected at the time of total articular replacement arthroplasty.The bone tissues from necrosis area and healthy area were made into general sections after immobility and decalcification.Pathological changes were exexamined by optical microscopy and the expression of bFGF and BMP-2 in femoral head was determined with in-situ hybridization technique.[Results]The organization structure of experimental group was disorganized,cracked,and the bone trabecula was rarefactive and non-intact.There were a great number of empty lacune in bone trabecula.The intensity and area of positive expression of bFGF and BMP-2 in femoral head of the experimental group were obviously lower than that of control group.The result had statistical significance(P

[Objective] To explore the curative effect of synthetic surgical therapy on severe flexion deformity knees.[Methods]There were 22 patients(35 knees)with flexion deformity knees who were treated by synthetic surgical therapy during March 1999 to February 2008.Evaluation with HSS scoring system at preoperative and postoperative period were made.[Results]All patients were followed up.The mean followed-up time was 3.1 years(range 13 months to 7 years).The mean HSS score of knee joint was 23.5 points at preoperative period(range-6 to 35)and it was 79.8 points at postoperative period(range 62~91).The result showed statistical significance(P

Objective To investigate the effects of different doses of gingko flavonoid on the expression of nuclear factor-κBp65 (NF-κBp65) and tumor necrosis factor-α(TNF-α) in mice with non-alcoholic fatty liver disease(NAFLD) .Methods The mice mod-el of NAFLD was constructed by the high fat diet for 4 weeks .The mice were randomly divided into the control group ,high fat model group ,high ,middle and low doses of gingko flavonoid groups .High ,middle and low doses of gingko flavonoid groups were respectively gavaged with the different doses of gingko flavonoid .The normal control group and the high fat model group were ga-vaged with normal saline .After 12 weeks ,the mice were killed for taking liver tissue and detecting NF-κBp65 expressions ,and ser-um TNF-αand triacylglycerol(TG) levels were detected .Results Compared with the normal control group ,the levels of NF-κBp65 ,TNF-αand TG in the high fat model group were increased(P<0 .05) .Compared with the high fat model group ,the levels of NF-κBp65 ,TNF-αand TG in the high ,middle and low doses of gingko flavonoid groups were decreased (P<0 .05) ,moreover the higher the dose ,the more obvious the decrease .Conclusion Gingko flavonoid may play certain role in the treatment of NAFLD by reducing the generation of NF-κBp65 ,TNF-αand TG .
5.Clinical analysis of modified laparoscopic dismembered pyeloplasty in children with ureteropelvic junction obstruction

Objective To investigate the therapeutic effect of modified transperitoneal laparoscopic dismembered pyeloplasty in children with with ureteropelvic junction obstruction (UPJO).Methods The clinical data of 27 children with UPJO treated with transperitoneal laparoscopic dismembered pyeloplasty were analyzed retrospectively.The age ranged from 5 to 104 months (mean,37 months).All cases were diagnosed by ultrasonography,IVU,CT and/or renal radionuclide scanning.The antero-posterior pelvic diameter was more than 3 cm by ultrasound in all cases.Indications of surgery were as followings:symptoms of upper abdominal pain or low back pain and/or split renal function <40% and/or progressive dilatation.Modified transperitoneal laparoscopic dismembered pyeloplasty was performed.Double hitch stitches were transfixed at the top of pelvis and ureter to be anastomosed,and a 6 F urethral catheter was inserted as a ureteral stent percutaneously through the puncture hole and was removed 7-9 days postoperatively without double-J stent.Results All operations were completed laparoscopically without conversion to open surgery.The mean operative time was 118 min (range,85 to 176),the mean blood loss was 16 ml (range,10 to 30) and the mean postoperative hospital stay was 10.5 days (range 9 to 13).The perinephric urine drainage occurred in 2 patients with about 200 ml/d,and reduced to 6 ml/d and 4 ml/d 5 to 6 days after operation.During the follow-up period for 12 to 36 months(mean,22 months) in 27 cases,there was no stricture at UPJ and the hydronephrosis reduced significantly or disappeared.Conclusion Laparoscopic dismembered pyeloplasty with double hitch stitches and an ordinary urethral catheter as an ureteral stent is an easy method with high successful rate and less operative time,which avoids reoperation to remove the double-J stent and is worthy of clinical popularization.
6.Effective dose and safety of adenosine triphosphate in inducing dual atrioventricular nodal pathways phenomenon

Objective To investigate the effective dose and safety of adenosine triphosphate(ATP) in inducing dual atrioventricular nodal pathways(DAVNP)phenomenon.Methods The ATP(the initial dose was 0.15 mg/kg,incremented 0.10 mg/kg each time)was injected to 40 patients who had slow-fast atrioventricular nodal reentrant tachycardia(AVNRT)through a femoral vein until signs of DAVNP,second-or third-degrec atrioventricular block(AVB)appared.The surface and intracardiac electrocardiogram was recorded consecutively.Results Forty patients of the slow-fast AVNRT had AVB after injecting ATP (16.1±5.8)mg(9-22 mg),29 patients(72.5%,29/40)occurred DAVNP phenomenon,of which 11 (37.9%,11/29)revealed DAVNP phenomenon after injecting ATP 0.15 mg/kg.Seventeen patients had side-effects of dyspnea,dizziness,cough,cardiopalmus and so on,but these side-effects were generally minor,short duration.Forty patients had transient cardiac arrhythmia.Conclusions The incremental low-dose method in inducing DAVNP has higher positive rate.The side-effects of ATP are mostly light and short duration,and can not cause serious consequence.
7.Application of indwelling renal pelvis drainage and ureteral stent in laparoscopic pyeloplasty

Objective To assess the feasibility and efficacy of renal pelvis drainage and ureteral stent of laparoscopic pyeloplasty in treatment of ureteropelvic junction obstruction.Methods Eighteen patients(14 boys and 4girls) with ureteropelvic junction obstruction,the average patient age was 66 months (range 3-182 months),who underwent transabdominal laparoscopic Anderson-Hynes pyeloplasty in Capital Institute of Pediatrics from Aug.2011 to Oct.2012 were selected.The ureteral stent and renal pelvis drainage were installed during the performance of surgery.They were removed in 7 days and 9 days after operation respectively.Results Eighteen cases successfully underwent laparoscopic pyeloplasty,without conversion to open surgery.The mean operating time was 102.8 minutes (ranging between 90 and 150 minutes).Two cases had complications on postoperative day 3,one patients ureteral stent was inadver-tentely pulled out,another was removed because of blood clots.No patient had postoperative urinary leakage or anastomotic stenosis.Postoperative follow-up time was 6 months.The hydronephrosis vanished in 13 patients,reduced obvious ly in 5 patients as revealed by ultrasound examination.Conclusions Intraoperatively,renal pelvis drainage and ureteral stent for postoperative drainage is effective,and it is worthy of application in a large scale.Patients can avoid further anesthesia for removing stents and the complications of long-term indwelling stent tubes,and have an improved quality of life.

Seventy-five preschool children with hemoglobin content below 11 g/dl in kindergartens and nurseries were divided into five groups. The first, second and third groups were given 10 mg, 20 mg and 30 mg of iron in the form of ferrous sulfate syrup respectively every day for two months, while the fourth and fifth groups were given 100 mg ascorbic acid and 100 mg ascorbic acid plus 10 mg iron respectively every day for one month. The average hemoglobin content in the order of the five groups were 9.6?1.16, 10.6?1.34, 9.7?0.91, 9.7?0.85 and 11.0?1.86 g/dl respectively before treatment. 9.THE RELATIONSHIP BETWEEN THE DIET AND THE GROWTH OF INFANTS DURING THE WEANING PERIOD (6-18 MONTHS)

The growth curves and diet of 185 babies in rural area near Beijing from 6 months to 18 months old have been studied. Infants were divided into 3 groups according to their diet taken:(1) 89 babies had breast milk only;(2) 66 babies took more than 250g animal milk per day besides breast milk;(3) 30 babies who did not have breast milk and only animal milk was taken. All the babies were gradually transferred to an ordinary cereal based diet during the weaning period. To half of them a soybean and cereal based formula fortified with minerals and vitamins was supplied, which would provide 6.6g protein and 141 kcal and some other nutrients per day.The results showed that the growth curves of body weight and height of children were much better than the data obtained in 1975, but still lagged behind that of the urban children, especially for the body weight. The weight curves of the babies who took animal milk seemed higher than the other groups. Female babies who were fed with breast milk and supplemented with the weaning food have a better weight and height curves than non-supplemented ones, but this was not seen in males. The hemoglobin contents of the babies who took animal milk when they were 18 months old were lower than that of the 6 months old, but there were no differences in the breast milk groups, and the lowering of the hemoglobin level in the animal milk groups could be corrected by adding weaning food in the diet. When the babies were 6 months old, 39.3% of them had a hair zinc content below the lower limit of the normal range, and it became to 47.6% when they were 18 months old. So zinc deficiency of the babies were present in this local area, and it should be fortified into the weaning food.
10.Clinical Efficacy of Yangxin Dingji Capsule Combined with Metoprolol on Patients with Coronary Heart Disease Complicated with Rapid Arrhythmia

Objective: To study the clinical efficacy of Yangxindingji capsule combined with metoprolol in patients with coronary heart disease complicated with rapid arrhythmia. Methods: A total of 86 cases of patients with coronary heart disease complicated with tachyarrhythmia diagnosed in our hospital from January 2017 to December 2017 were selected as the subjects of this study. All the patients were divided into a control group and a study group according to the principle of the random number table, with 43 cases in each group. The patients in the control group were received oral metoprolol for treatment, and the patients in the study group were treated with oral Yangxindingji capsule combined with Metoprolol for treatment. The treatment effect and the change of heart function and blood rheology were compared between the two groups before and after the treatment. Results: The total effective rate of treatment in the study group was 83.7% (36/43), which was higher than that in the control group (67.4% ) (29/43). The difference was statistically significant (P < 0.05); After the treatment, the HR levels in both groups were lower than before treatment, and the study group was lower than the control group, with a significant difference (P < 0.05); The LVEF levels after treatment in the two groups were higher than before treatment, and the study group was higher than the control group. The difference was statistically significant (P < 0.05); The fibrinogen, high-cutting whole blood viscosity, plasma viscosity, and low-cut whole blood viscosity of the two groups after treatment were lower than before treatment, and the study group was lower than the control group. The difference was statistically significant (P < 0.05). Conclusion: Yangxindingji capsule combined with metoprolol has significant effects on coronary heart disease combined with rapid arrhythmia, which can effectively relieve clinical symptoms, improve cardiac function, regulate hemorheology, correct arrhythmia, and is worthy of clinical promotion.
