目的观察金钠多结合早期康复治疗急性缺血性脑卒中的临床疗效。方法将80例缺血性脑卒中患者随机分为金钠多加早期康复组和低分子右旋糖酐加早期康复组(对照组)。观察两组患者治疗前后血脂及血液流变学、神经功能缺损、肢体运动功能及日常生活活动能力评分的变化。结果金钠多加早期康复组治疗后血脂、红细胞压积、纤维蛋白原均降低,神经功能缺损评分减少值、肢体运动功能及日常生活能力增加值均高于对照组(P<0.05～0.01)。结论金钠多结合早期康复能减轻急性缺血性脑卒中患者的神经功能缺损,提高患者独立生活能力,改善功能结局,提高康复效率。

ObjectiveTo explore the effect of neuromuscular facilitation combined with faradism on limbs motor function in hemiplegic patient with early stroke.Methods108 patients were randomly divided into the therapy group(who accepted neuromuscular facilitation combined with faradism) and control group (who accepted neuromuscular facilitation only). The limbs motor function was evaluated with Fugl-Meyer Assessment (FMA) before and after 10 weeks by the same doctor. ResultsThere were not significant difference of scores of FMA between the two groups before rehabilitation(P>0.05), but there were significant difference 10 weeks after rehabilitation(P<0.01). ADL in therapy group was significantly higher than that in control group(P<0.01).ConclusionFaradism can significantly improve the effect of neuromuscular facilitation on limbs motor function and ADL.

Objective To investigate the clinical viability of pre-eclampsia by measuring the level of ?-HCG in blood,blood calcium,hematocrit(HCT)and the mean arterial blood pressure(mABP).The preventive value of calcium supplement was also evaluated. Methods 356 volunteers'(16th～20th gestational weeks)were measured ?-HCG in blood,blood calcium,HCT,meanwhile mABP from the same patients.If blood ?-HCG≥50 632 IU/L,blood calcium≤2.18 mmol/L,HCT≥0.35 and mABP≥85 mm Hg(1 mm Hg=0.133 kPa),a positive conclusion was made.Women with positive results were randomly divided into test group and control group. Results 71 cases turned out to be positive in this serial test.The positive predictive value was 81.82%,negative predictive value was(97.30%),the sensitivity and specificity were 84.38% and 96.77% respectively.In the study group,the incidence of pre-eclampsia was significantly decreased as compared with the control group(P

Objective To discuss the diagnostic value of CT and MRI for primary anorectal malignant melanoma(AMM).Methods 5 patients(2 males and 3 females)with AMM confirmed pathologically were analyzed retrospectively,which were examined by CT(n=4)and 1.5T MRI(n=1).Results AMM presented as bulky intraluminal fungating masses without intestinal obstruction(n=4)and irregular rectal wall thickening(n=1).2 cases had perirectal fat infiltration and extended to the presacral space in one case,4 cases had perirectal enlarged lymph nodes and larger than 3 cm in diameter in one case.Nonenhanced CT showed a hypodense lesion and contrast-enhanced CT showed slight enhancement(n=1)and mild enhancement(n=3).The tumor scanned by MRI depicted high intensity on T1WI and low intensity on T2WI,so did the enlarged lymph nodes and bone metastasis.Fat-saturation T1WI significantly improved the extention of tumor and detection of metastasis.Conclusion Bulky intraluminal fungating masses without intestinal obstruction,perirectal fat infiltration and enlarged lymph nodes may be helpful to diagnose AMM.MRI is especially useful in differentiating melanotic AMM from other rectal tumors.

Retinol binding protein 4 (RBP4)was a class of secreting protein,mainly synthesized by the liver,widely distribu-ted in the human body blood,urine and other body fluids.It plays an important role in assisting the physiological function of vitamin A[1].Recent research shows that RBP4 was a new kind of adipocytokine,participated in insulin resistance and occur-rence of type 2 diabetes,and had a closed relationship with diabetic nephropathy,nutritional disease.This article describes the function of RBP4,review the relationship between RBP4 and nutritional or other type of diseases,and new clinical detec-tion method with RBP4.

Objective To evaluate prognostic indicators in severe traumatic brain injury and intracania hamatomas with hernia and analyze which is the most important indicator.Methods Data of 84 cases with severe traumatic brain injury were retrospectively analyze .Age,GCS,pupil reflex,midline shift,compression of the cisteme,decompression time and complex injury were considered as possible prognostic indicators.SPSS13.0 was employed to analyze the data,Logistic regression(Forward conditional)analysis was done to confirm which are the most important prognostic indicators for severe traumatic brain injury and to evaluate the practical value in predicting prognosis(X2= 22.92,P＜ 0.01).Results 48 patients died and 36 survived.56(67%)patients had a bad prognosis(GCS≤3)and 28(33%)had a good prognosis(GCS ＞3).Those who had a shorter decompression time(≤3h)had a better prognosis than those who had a longer decompression time(＞3h),with the mortality rate of 11% and 67% respectively.Lesser cisteme compression predicts better outcome.GCS,pupil reflex,midline shift,and complex injury didn' t enter the logistic regression equation.According to compression time,90.5% of the cases' prognosis was accurately predicted,according to compression time and cisterne compression,95.2% was accurately predicted.Conclusion The mortality rate of severe traumatic brain injury with hernia was high and surgical intervention was effective.Decompression time and cisteme compression were the most important factors affecting prognosis in severe traumatic brain injury and they could predict prognosis of most cases correctly.

Objective To observe the curative effect of cervical traction combined with low-cycle or intermediate frequency electrical current in cervical vertebrae.Methods 171 patients with cervical vertebrae were randomly divided into the therapy group and control group,86 cases in therapy group were performed the curative measure of cervical traction and low-cycle,85 cases in control group underwent the curative measure of cervical traction and intermediate frequency electrical current.Both of the two groups carried out continuous therapy for 10 days.Results There was significant difference before and after treatment within each group in CASCS score(P ＜ 0.01)and there was also significant difference after treatment between the two groups(P ＜ 0.05),the effective rate of therapy groupwas significant higher than that of control group(P ＜ 0.05).Conclusion The methods of cervical traction and low cycle or intermediate frequency electrical current has better curative effect in cervical vertebrae and cervical traction and low-cycle do much benefit in cervical vertebrae treatment.

Objective To observe and analyse the efficacy of retrograde gonarthritis with interference current and drug-iontophoresis direct current. Methods 185 patients with retrograde gonarthritis were randomly divided into treatment group 92 cases and control group 93 cases. The patients in treatment group underwent interference current and drug-iontophoresis direct current and patients in control group performed modulated medium frequency drug-ionto-phoresis direct current for two treatment courses. Results The comprehensive score of various kind of symptom such as pain、swelling、flexion、walking ability, up-down stair ability in treatment group significantly improved than that of control group (P < 0.01). Conclusion The interference current and drug-iontophoresis direct current is one of effec-tive and safe measurement in treating arthritis with main manifestation of pain and soft tissue disease.

Objectives To investigate the clinical profile and prognosis of hemophagocytic syndrome (HPS). Methods A retrospective study was carried out to analyze the clinical features and laboratory findings in 28 children with HPS. Fisher's exact probability method and Logistic multivariate regression were used to explore the prognostic risk factors.. Results HPS was clinically characterized by prolonged fever (100%), hepatomegaly (64.29%),and other minor features including respiratory symptoms (53.57%), splenomegaly (50%), hydrops of multiple serous cavity (42.86%), lymphadenectasis (32.14%), jaundice (17.85%), skin rash (14.29%), central nervous system involvement (14.29%), and alimentary tract hemorrhage (10.71%). Labo-ratory data showed that 1iver dysfunction, pancytopenia, coagulation abnormalities, disseminated intravascular coagulation, hy-pertriglyceridemia, decreased number of natural killer cells and hyponatremia were prominent. The etiological analysis indicated that infection associated hemophagocytic syndrome was most common (60.71%), in which EB virus associated HPS was pre-dominant, accounting for 64.71%. Significant difference was observed in the difference of albumin,blood urea nitrogen and acti-vated partial thromboplastin time between death and survival cases (P<0.05). The Logistic regression multivariate analysis showed that hypoalbuminemia was an independent prognostic factor. Conclusions There are various underlying diseases and clinical manifestations for HPS. The lower level of serum albumin is an independent prognostic factor. A prompt diagnosis and treatment is very important for HPS prognosis due to the rapid progression and high mortality.
