1.The influence of disease uncertainty on dependency of remedy, shoulder joint movement and ability in daily life in post-operative breast cancer patients

Objective To observe the influence of disease uncertainty ( DU) to patients' compliance with rehabilitation training ( CWRT) , shoulder joint movement (SJM) , and abilities in activities of daily living ( ADL) in post-operative breast cancer (BCPO) patients. Methods Sixty-seven BCPO patients uncertain about their health was assessed using Missals' DU protocol. They were then treated according to the breast cancer rehabilitation training plan recommended by the Japanese Cancer Association. CWRT, SJM and ADL were evaluated before and after treatment. Results The scores on dependency of recovery training, systematic remedy and the ability to change their life style were distinctly higher among patients with low DU scores than among those with high DU scores. The shoulder joint mobility in two groups was similar before the remedy, but became better after treatment. ADL category grades and ADL total scores in the two groups were similar before treatment, and had increased significantly after treatment. The item scores for defecating, diet, dressing and bathing, as well as ADL total scores, were significantly lower in the high uncertainty group. Conclusion DU is one of most important factors for BCPO patients. It can influence CWRT and therapeutic effects in rehabilitation.
2.Pay more attention to the selection of proper surgical timing and method for oculoplasty in the pediatric

The pediatric population with the orbital and eyeball malformations has been an important part in ophthalmic plastic surgery.The pediatric is with great difference from adult in physiology and anatomy.Besides,visual function,orbit and face during childhood are still under development.The orbital deformityes during the childhood,especially for some congenital abnormalities,are always associated with or a cause of visual disfunction.And the congenital or acquired anophthalmia or microphthalm may lead to the retardation of orbital growth and overall of the face afterwards,so it is not only a simple cosmetic surgery for the oculoplastic surgery in the pediatric population,but also the reconstruction of function.Thus we should pay special attention for the oculoplastic surgery to the effect of development of visual function and orbit in the pediatric.Accordingly,it is crucial to select a proper surgical timing and method for ophthalmic plastic surgery in the pediatric.
3.The clinical value of growth hormone for the patients who were failed in previous IVE-ET

Objective To study the application value of growth hormone in the renewal cycle for those patients who were failed in the first IVE-ET treatment.Methods 68 patients who were failed in the previous IVE-ET treatment were divided into two groups and were given long term down-regulating protocol in the first cycle and current cycle.The study group consisted of cases with using GH (4U/d)from the day of down-regulating till the day before embryo transfer and control group consisted of those without using GH.Results The rate of pregnancy of study group and control group was 26.77% and 0, while the rate of quality embryos was 46.26% and 33.58%, respectively.There was significant difference between the two groups( X2 = 3.97,4.86, P < 0.05 ), while there was no significant effect on the rate of fertilization and cleavage, the number of M Ⅱ oocytes retrieved( all P > 0.05 ).Conclusion In the second cycle,growth hormone could improve the clinical pregnancy rate and the rate of quality embryos for those patients who were failed in the first IVE-ET treatment.
4.The comparison of the effect of UK and rt-PA intravenous thrombolytic therapy for acute myocardial infarction

Objective To compare the clinical effect of UK and rt-PA intravenous thrombolytic therapy for acute myocardial infarction.Methods By SAS statistical analysis system,72 patients with acute myocardial infarction were randomly divided into rt-PA group and UK group,each group had 36 cases.The coronary artery recanalization rate,bleeding events,adverse cardiovascular events,mortality,and cardiac troponin T(cTnT) level were recorded.Results In rt-PA group,0.5h,1 h,2h after the start of thrombolytic therapy,the coronary artery recanalization rate was significantly higher than that in the control group,the difference was statistically significant (x2 =7.32,6.12,5.81,all P < 0.05).In rt-PA group,3 cases had bleeding events,2 cases died,and in UK group,5 cases had bleeding events,3 cases died,,there was no statistically significant difference between the two groups (x2 =1.22,P > 0.05).In rt-PA group,there were 6 cases with adverse cardiovascular events,14 cases in the control group,the difference was statistically significant (x2 =4.56,P<0.05).2 cases died in rt-PA group,3 cases died in UK group,the mortality between the two groups had no statistically significant difference (P > 0.05).The cTnT level in the hospital 6h,12h was significantly higher than admission in two groups (t =6.67,6.30,4.11,4.22,4.40,all P < 0.05).In rt-PA group,cTnT level in the hospital 6h,12h was significantly lower than that in UK group (t =5.92,6.61,all P <0.05).Conclusion Compared with UK,rt-PA in the treatment of acute myocardial infarction can increase recanalization rate,decrease adverse events and myocardial injury,it is worthy of clinical application.
5.Nursing of neonates with hypoxic-ischemic encephalopathy detected by 18F-fluorodeoxyglucose positron emission tomography

Objective To explore the key points of nursing for neonates with hypoxic-ischemic encephalopathy detected by 18F-fluorodeoxyglucose positron emission tomography.Methods The features of nursing for 58 cases neonates with hypoxic-ischemic encephalopathy detected by 18F-fluorodeoxyglucose positron emission tomography were analyzed and summarized.Results 55 cases with hypoxic-ischemic encephalopathy detected by 18F-fluorodeoxyglucose positron emission tomography smoothly with pertinent nursing,only 3 cases had not completed the procedures because of change of their conditions.Conclusions Pertinent nursing was important for neonates with hypoxic-ischemic encephalopathy detected by 18F-fluorodeoxyglucose positron emission tomography because it could help them complete the procedures smoothly and make the results more accurate and satisfying so it is worthy of wide use clinically.
6.Suppressing effects of MMPs inhibitor GM6001, MMP-2/9 inhibitor Ⅰ ,MMP-2/9 inhibitor Ⅱ on migration of human lens epithelial cells

Background The primary pathologic mechanism of posterior capsular opacification (PCO) is proliferation,migration and epithelial-mesenchymal transformation of residuary lens epithelial cells (LECs) following cataract surgery.Matrix metalloproteinases (MMPs) play a role during the migration of LECs.Researches showed that GM6001,a broad inhibitor of MMPs,can arrest the migration of LECs,but as specific inhibitors of MMPs,the efficacy and safety of MMP-2/9 inhibitor Ⅰ and Ⅱ on LECs migration remain unclear.Objective This study was to determine and compare the inhibitory efficacy among GM6001,MMP-2/9 inhibitor Ⅰ and Ⅱ on human LECs and search the clinical medication to prevent PCO.Methods Human LECs were cultured and passaged in vitro,and the cells of 3-4 generation were incubated in 6-well plates.Then the cells of 70% confluent monolayer were cultured in DMEM without fetal bovine serum for 12 hours.GM6001,MMP-2/9 inhibitor Ⅰ and Ⅱ at different concentrations (0.25,0.50,1.00,2.00,4.00,8.00,16.00,32.00,64.00,128.00 μmol/L) were added into the culture medium for 24 hours separately,and regularly cultured cells served as the control group.A bare area was made by a 200 μl sterile spear on the cell layer,and the migrated distance and inhibitory rate were calculated.The second or third generation of cells were incubated in 96-well plates at a density of 5×105/ml (200 μl/well).GM6001 (128.00 μmol/L),MMP-2/9 inhibitor Ⅰ (64.00 μmol/L) and Ⅱ (32.00 μmol/L) were added into the culture medium for 24 hours,and the cell viability was assayed by using MTT assay.Results Cultured cells grew well with irregular arrangement and presented the polygon in shape.The migrated distance was gradually reduced as the increase of concentrations of GM6001,MMP-2/9 inhibitor Ⅰ and Ⅱ,showing significant differences among the various concentration groups (GM6001:F=248.647,P<0.05;MMP-2/9 inhibitor Ⅰ:F=357.125,P<0.05;MP2/9 inhibitor Ⅱ:F=396.374,P< 0.05).The cell migrated distance in the control group was set to 1,the relative migrated distances were 0.478 ± 0.091,0.294±0.088 and 0.191 ±0.081 in the GM6001 group,MMP-2/9 inhibitor Ⅰ group and MMP-2/9 inhibitor Ⅱ group at the concentrations of 32.00 μmol/L,respectively,showing a significant difference among the groups (F =116.031,P<0.01),and cell migrated distance was obviously shorter in the MMP-2/9 inhibitor Ⅱ group than that in the GM6001 group or MMP-2/9 inhibitor Ⅰ group (all at P<0.01).The A values were 0.607±0.016,0.567±0.015,0.583±0.010 and 0.595 ±0.0138 in the control group,GM6001 group (128.00 μmol/L),MMP-2/9 inhibitor Ⅰ group (64.00 μmol/L) and MMP-2/9 inhibitor Ⅱ group (32.00 μmol/L),respectively,without significant difference among the groups (F=1.403,P>0.05).Conclusions GM6001,MMP-2/9 inhibitor Ⅰ and Ⅱ reduce the mobility of human LECs effectively but do not affect the viability of the cells in vitro.MMP-2/9 inhibitor Ⅱ appears to be most dominant in inhibiting migration of human LECs.
7.Effect of Changle (Rare Earth Complex) on Kidney Antioxidation in Rats

Objective To explore the effect of low dose Changle (rare earth complex) on the antioxidation in rats kidney. Methods The activities of Cu-Zn SOD,CAT,LPO and GSH-Px in the rat kidney were determined by biochemical methods after the rats (Wistar rats) were given different doses of Changle by gavage for 6 months respectively. Results The activity of Cu-Zn SOD and CAT in the 10.0 and 20.0 mg/kg groups reduced significantly,but the activity of LPO and GSH-Px increased. The activity of Cu-Zn SOD and CAT in the 2.0,0.2 and 0.1 mg/kg groups increased,but LPO reduced. Conclusion High exposure to Changle may enhance the peroxidation,but low exposure to Changle may enhance the antioxidation in rat kidney.

9.Value of QRS duration predicting response of cardiac resynchronization therapy.

QRS wave duration is a main method of deciding cardiac asynchrony in patients with Cardiac resynchronization therapy(CRT).But cardiac mechanical dyssynchrony was not always consistent with electrical dyssynchrony.A few patients with widened QRS wave duration had not the cardiac mechanical asynchrony,while some patients with normal QRS wave duration had cardiac mechanical asynchrony.If the cardiac mechanical dyssynchrony was determined,it would be possible to reduce the non-response of CRT.We review the role of QRS duration to predicting the prognosis of CRT and debate the effect of QRS duration to CRT.
10.Three Methods of Nosocomial Infection Surveillance and Control:Their Comparison

OBJECTIVE To search an applicable software in order to achieve real time monitoring and processing management for nosocomial infection which may improve the medical service efficiently through comparing with three different methods.METHODS To analyze and compare the three surveillance methods:the manual summed-up statistical methods,the National Computer Management System for Nosocomial Infection Surveillance Network and the Beijing Information System for Nosocomial Infection Surveillance and Control(Beijing System).RESULTS Beijing System is much more pragmatic and multipurpose than the two others.CONCLUSIONS Beijing System has the feature of wide coverage and convenience manipulating.It is confirmed practical and universal.And it is superexcellent and available for different kinds of hospitals.So Beijing System is worth for being popularized.
