1.Research progresses in the effects of folate metabolism on ovarian function

As one of important B vitamins,folate is an essential nutrient for the body and involves in various biochemical and metabolic reactions.Studies have shown that as an important methyl donor in the carbon metabolism cycle,folate has an important impact on pregnancy,pregnancy complications,and birth defects.Further studies have shown that in the folate metabolic pathway,the gene polymorphism and folic acid metabolism of a key enzyme,5,10-methylenetetetrahydrofolate reductase (MTHFR),play an important role in ovarian function.Gene polymorphism and the high homocysteine levels caused by it can lead to the damage of reproductive function and endocrine function,including follicular development,embryonic development,and hormone secretion.Gene polymorphism is also associated with the occurrence and development of ovarian disease and its response to treatment.With the deep understanding of folate metabolism,MTHFR gene polymorphism may become a new genetic marker for predicting the risk of disease and a new target for related gene therapies.
2.Clinical study of glutamine in the prevention of necrotizing enterocolitis in preterm infants

Objective To assess the role of glutamine in the prevention of necrotiziting enterocolitis (NEC) in preterm infants. Methods Two thousand seven hundred and seventeen preterm infants hospitalized in our NICU from Oct 2007 to Mar 2010 were assigned in either receiving prophylactic use of glutamine (prevention group, n = 1389) or without glutamine supplementation(control group, n = 1328). The incidence of NEC was compared between the two groups. Results There were no significant differences in sex,gestation age, birth weight, neonatal asphyxia, pneumonia, septicemia and cerebral hemorrhage between the two groups (P > 0. 05). Sixty-eight infants occurred NEC in the control group(5. 12%) ,but only 35 infants occurred NEC in the prevention group(2. 52%) (x2 = 12. 590,P <0. 01). Conclusion The prophylactic use of glutamine may reduce the incidence of NEC in preterm infants.
3.Effects of Shoutaiwan on the STAT Signal Transduction Pathway of CD_4~+ T Cells in Maternal-fetal Interface of Mice with Recurrent Spontaneous Abortion

Objective To study the effects of Shoutaiwan on the STAT signal transduction pathway of CD4+ T cells in maternal-fetal interface of mice with recurrent spontaneous abortion(RSA).Methods CBA/J?BALB/C mating combination and CBA/J?DBA/2 mating combination were used as normal pregnancy and RSA model respectively.Mice with RSA were randomly assigned to model group and Shoutaiwan group.After 14 days of administration,the STAT signal transduction pathway of CD4+ T cells in mice decidual and placental tissues were detected by Springbio 720 antibody microarrays.Results Shoutaiwan could increase the phosphorylation level of STAT3 and STAT6 protein in the STAT signal transduction pathway.At the same time,Shoutaiwan could decrease the phosphorylation level of STAT1 protein.Conclusion The activation of STAT3 and STAT6 signal transduction pathway might play a important role in the therapy of RSA by Shoutaiwan.
4.In Vitro Antibacterial Activitey of Moxifloxacin Combined with Cefoperazone/Sulbactam Against Multidrug Resistant Bacteria

OBJECTIVE:To evaluate the antibiotic activity of Moxifloxacin (MFX) combined with Cefoperazone/Sulbactam (CPZ/SBT) against clinical common resistant bacteria for clinical reference of rational use of antibacterials.METHODS: The MIC of two antibiotics used alone or in combination on staphylococcus aureus,Klebsiella pneumoniae,Pseudomonas aeruginosas(50 strains,respectively)and Escherichia coli,enterobacter cloacae(20 strains,respectively)were detected respectively by Vitek-32 Model (Full Automated bacterium Detection Device) and the fractional inhibitory concentration(FIC) index was calculated. RESULTS: The combination of two antibiotics significantly reduced MIC on 190 common resistant pathogenic strains and enhanced antibiotic action. Their antibacterial action in vitro was characterized by synergism and additive action. CONCLUSION: The study can present reference for hospital treatment of infection induced by common drug-resistant bacteria.

6.Application of clinical pathway in the management of single disease cost

Objective To explore the application of clinical pathway in the management of single disease cost. Methods Inpatients with breast cancer from June 2006 to June 2007 were divided into the test group and the control group according to different payment models.Patients in the test group had medical insurance while patients in the control group did not.The general data in the two groups had no statistical difference.The test group were given nursing intervention by clinical pathway while the control group received routine nursing measures. Results The mean inhospital days,mean days waiting for operations and mean inhospital expenses in the test group were shorter or lower than those in the control group (P < 0.05).The mastering degree of health knowledge and the satisfaction degree of patients in the test group were higher than those in the control group (P < 0.05). Conclusions The application of chnical pathway in the single disease cost patients with medical insurance could effectively control the medical expenses and guarantee the quality.So we consider that the application of clinical pathway in single disease cost is applicable.
7.Identification and characterization of ovarian cancer stem-like cells from primary tumor

Objective To study whether the primary ovarian cancer cells containing cancer stem cells and its characterization in serum-free culture condition.Methods The primary cancer cells were isolated from one stage Ⅲ, grade 2 serous adenocareinoma tissue.Cells were cultured in serum-free culture system supplemented with epidermal growth factor,basic fibroblast growth factor,leukemia inhibitory factor and insulin.or standard serum-containing system.Methyl thiazolyl tetrazolium assay,quantitative PCR analysis,flow-cytometric analysis and xenograft experiments in vivo were performed.Results The primary cancer cells could maintain and forill cell sphere in serum-free culture system.These cells had the properties of self-renewal,overexpression of stem cell marker genes Nanog,Oct-4,Sox-2,nestin,ABCG2,CD_(133) and CD_(117) By contrast with the difierentiated cells under standard serum-containing culture conditions.these sphere-forming cells were more resistant to cisplatin and paclitaxel after treated 48 and 72 hours(61% vs.31%,73% vs.29%,P<0.05).With Hoechst 33342 exclusion assay,only 21.83%of sphere-forming cells were positive with the dye,compared with 83.04% positive cells in differentiated cells (P<0.01).Only 500 sphere-forming cells resulted subcutaneous xenograft tumors.All of these xenografts were categorized as serous adenocareinomas, overexpression of CA_(125) and cytokeratin-7 which were original tumor phenotype of ovarian cancer. Conclusion The sphere-forming cells isolated from primary ovarian cancer tissues have the characterization of cancer stem cell and may be a more reliable model system for understanding the biology of primary human tumors.
8.Development and evaluation of a DNA microarray for Listeria monocytogenes detection

Objective To develop a rapid and sensitive DNA microarray for Listeria monocytogenes detection.Methods A DNA microarray was developed using gyrB,ISR,16S rRNA,23S rRNA,hlyA,iap and prfA as the target genes and tested against 18 different species of known reference for repeatability,sensitivity,and specificity to verify the effectiveness of the chip.Results After testing of samples by the LM array,results show that the 70 mer Oligos synthesized by IDT are superior to the Oligos synthesized by Sagon with respect to both probe spotting or samples detection.The comparison of 3 spotting probe concentrations of 10 μmol/L,40 μmol/L and 80 μmol/L demonstrated that the 10 pmol/L probes result in good detection signals equivalent to the 40 μmol/L and 80 μmol/L probes.The repeatability and sensitivity evaluated by sample testing on the LM array revealed that the chips developed in this study have good repeatability and the lower limit of sample detection is 0.9 ng DNA.The LM array can distinguish clearly and definitively between Listeria and non-Listeria bacteria in the sample.Conclusion The microarray is able to rapidly detect and identify Listeria monocytogenes.
9.Association between cancer related fatigue and social support in patients after breast cancer chemotherapy in Changsha.

OBJECTIVE:
To explore the association between the cancer related fatigue and social support in breast cancer patients after chemotherapy.
METHODS:
According to the uniform inclusive and exclusive criteria, 396 breast cancer patients after chemotherapy were sampled randomly from 4 hospitals in Changsha and investigated on the spot by cancer fatigue scale and social support scale.
RESULTS:
The levels of social support, subjective support, Objective support, and utilization of support of the breast cancer patients after chemotherapy were 36.63±7.80, 21.05±4.67, 8.45±3.06, and 7.13±2.10, respectively, which were lower than the normal level, with significant difference(P<0.001). There was a negative correlation between each fatigue dimension and social support, subjective support, Objective support, and utilization of support (P<0.05 or P<0.001).
CONCLUSION
The social support system in patients with breast cancer after chemotherapy is poor. There is a negative correlation between the social support system and cancer related fatigue.

10.Analysis of a case with Brucellosis misdiagnosed as osteoarthrosis.

Brucellosis is far more frequent in a pasturing area in the northern part of our country and it has many clinical manifestations. It may cause multiple organ damage and its features lack specificity. It is rare in the south, so it is extremely easy to be misdiagnosed or overlooked. The retrospective analysis of a case with Brucellosis misdiagnosed as osteoarthrosis provides a guide for clinical doctors to understand Brucellosis, so that early diagnosis would be accessible, and prognosis could be improved.
