Objective This study was designed to assess the clinical effects and safety of betalol and enalapril on the left ventricular diastolic heart failure. Methods 82 patients of left ventricular diastolic heart failure were randomly divided into two groups, In the theraputic group of 42 patients, we took metoprolol and enalapril curing left ventricular diastolic heart failure on the founda-tion of usual treatment. There were 40 patients who received usual treatment in the control group. The clinical effects and safety were assessed after 6 months. 2.Expression and significance of PTEN, P-ERK and P-AKT in the middle ear cholesteatoma.

OBJECTIVE:
Detect the expressions of the protein tyrosine phosphatase gene (PTEN), phosphorylated protein kinase B (P-AKT) and phosphorylated extracellular signal-regulated kinase (P-ERK) in human middle ear cholesteatoma tissue and its correlation to explore their important role in the mechanism of the formation of cholesteatoma.
METHOD:
Use immunohistochemical SABC method (SABC immunohistochemical method) to detect the expressions and location of PTEN, P-AKT and P-ERK proteins in 40 cases of middle ear cholesteatoma tissue samples and 15 cases of normal ear skin specimens. Use Western blot to detect the expression levels of PTEN, P-AKT, P-ERK proteins and the internal reference GAPDH in 20 cases of cholesteatoma tissue and 10 cases in nor mal ear skin specimens.
RESULT:
(1) Immunohistochemistry showed coloring of PTEN both in the nucleus and cytoplasm of cholesteatoma and normal skin . Nuclear PTEN positive expression rates in the cholesteatoma was significantly lower than normal skin, and the difference was significant (P < 0.01); cytoplasm PTEN positive expression rates in the cholesteatoma was significantly lower than normal skin, and the difference was significant (P < 0.01); P-AKT mainly expresses in the cytoplasm of cholesteatoma and normal skin. The positive expression rates in the cholesteatoma was significantly higher than normal skin,and the difference was significant (P < 0.01); the P-ERK mainly colors in cholesteatoma and normal skin cell nucleus. the positive expression rates in the cholesteatoma was significantly higher than normal skin, and the difference was significant (P < 0.01). In cholesteatoma specimens, there was a significantly negative relationship (P < 0.01) between PTEN, respectively, and P-AKT, P-ERK protein. (2) Western blot discovered: the expression of PTEN in cholesteatoma was significantly less than the amount of expression in normal skin; P-AKT and P-ERK expression in cholesteatoma was significantly more than the level in normal skin.
CONCLUSION
Abnormal expression of PTEN, P-AKT and P-ERK protein in cholesteatoma may be closely related to antiapoptosis and high degree of proliferation in cholesteatoma. Expression of PTEN deletion leads to the weakening of the inhibition. Excessive expression of P-AKT gives rise to cholesteatoma epithelial cell apoptosis inhibited; excessive PERK expression result to cholesteatoma epithelial cell proliferation strengthened.

3.Effects of granulocyte colony-stimulating factor plus stem cell factor on the cardiac function of rats with acute myocardial infarction

BACKGROUND: Mobilizing bone stem cell is popular because of no hurling, no immunological rejection and no extraction,cultivation and amplification of stem cells.OBJECTIVE: To investigate the effects of granulocyte colony-stimulating factor (G-CSF) and stem cell factor (SCF) on treating acute myocardial infarction (AMI) of rats.DESIGN: Randomized controlled study.SETTING: Laboratory of Pharmacology, Jinzhou Medical College.MATERIALS: A total of 32 healthy SD rats weighing (200+20) g were provided by the Animal Center of Jinzhou Medical College. Recombination human G-CSF (rhG-CSF) was provided by Shuanglu medicine factory, Beijing, and recombination human SCF (rhSCF) was provided by Diao Company, Chengdu.METHODS: The experiment was carried out in Jinzhou Medical Pharmacological Laboratory from July to November 2005.① Totally, 32 male SD rats were injected with 5 mg/kg ISO in left abdomen. After 3 hours, the rats were randomly divided into 4 groups with 8 in each group. Treatment group: The rats were injected with 1 mg/kg rhG-CSF and 1 mg/kg rhSCF under epidermis for 5 days. The concentration was 10 mg/kg. G-CSF group: The rats were injected with 1 mg/kg rhG-CSF and 1 mg/kg saline for 5 days. The concentration was 10 mg/kg. SCF group: The rats were injected with 1 mg/kg rhSCF and 1 mg/kg saline for 5 days. The concentration was 10 mg/kg. Control group: 2 mg/kg saline was injected under epidermis. ② After 14 days and 28 days from injecting ISO, one subgroup was weighted from every group.The rats were narcotized with 200 g/L urethane. After the heart's exposed, a tube filled with heparin sodium was inserted to left ventricle. Then tracked record the left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP), maximal rising and descent velocity of left ventricular pressure (±dp/dr) using Medlab-u/Sc and kept the heart rate (HR) records. Tracheal cannula was pushed out; heart was cut off immediately; auricula and right ventricle were cut off; tissue was weighed with microbalance; left ventricle mass/body mass was calculated; myocardial remodeling was evaluated. Tissues were cut into sections, and pathological form and infarction area were observed under optic microscope. Infarction area in each group was analyzed with cellular image analyzer.MAIN OUTCOME MEASURES: Left ventricular function, myocardial infarction area, heart rate, ventricular remodeling and pathology of the cardiac muscle.RESULTS: All 32 rats were involved in the final analysis without any loss. ① Comparison of the left ventricular function:At 14 days after modeling, LVSP, LVEDP and ±dp/dr were higher in treatment group than those in other 3 groups (P <0.05); 28 days later, those indexes in the 4 groups were higher than those in the 4 groups at 14 days after modeling,and there was significant difference (P < 0.05). Those indexes in G-CSF group were higher than those in SCF group and control group at the two time points (P < 0.05). ② Comparison of infarction area, HR and the ventricular remodeling: At 14 and 28 days after modeling, infarction area and left ventricle weight were smaller and lighter in treatment group than those in other 3 groups (P < 0.05); 28 days later, infarction area in the 4 groups was smaller than those in the 4 groups at 14 days after modeling (P < 0.05). Those indexes in G-CSF group were less than those in SCF group and control group at the two time points. ③ Pathology results: At 14 and 28 days after modeling, there was no significant infarction car in treatment group and G-CSF group, and new capillary vessel and infiltration of fibroblast were found in the two groups. The density of capillary vessel in G-CSF group was less than that in treatment group. There were infarction scars and much lymphocyte in SCF group and control group. The nucleus was small, the color was deep, and plasma was less. New granulation tissue was not significant.CONCLUSION: Combining G-CSF with SCF can protect the ischemic cardial muscle and improve the heart function in rats. The function of combining G-CSF and SCF is better than singlly using G-CSF or SCF.
4.Cellular compatibility of PCL-b-PLLA scaffolds and chondrocytal compound in vitro

0.05). The scaffolds produced no effect on the cell proliferation and had a low cytotoxicity, with chondrocytes tiled and lots of microspikes and matrixes secreted on the surface. Conclusion: The PCL-b-PLLA scaffold has a good compatibility with cells and is suitable for cartilage tissue engineering.
5.Effect of long session hemodialysis on the quality of life in maintenance hemodialysis patients

Objective To study the effect of long session hemodialysis (LSHD) on the quality of life in maintenance hemodialysis (MHD) patients.Methods A total of 40 MHD patients in our dialysis center were enrolled in the study.Quality of life was investigated by SF-36 table.Sleep questionnaire survey concluded the Athens insomnia scale (AIS),Pittsburgh sleep quality index (PSQI) and Epworth sleepiness scale (ESS).Clinical data were collected.Forty MHD patients were equally divided into HD and LSHD groups according to clinical data and sleep quality score for prospective study.Hemodialysis dose of HD group was 4 h thrice weekly,and of LSHD group was 8 h thrice weekly.The trial lasted for 6 months.Changes of life quality were compared between two groups.Results As compared to HD group,LSHD group had significant higher Kt/V (1.73±0.36 vs 1.41±0.23,P<0.05),higher levels of serum hemoglobin [(124.67±9.08) vs (110.55±9.01) g/L,P<0.01] and albumin [(45.01±2.66) vs (39.28±2.63) g/L,P<0.01].better sleep quality score (16/20 vs 5/20,P=0.001) and higher blood pressure control proportion (14/20 vs 5/20,P=0.010),higher score of SF-36 (P<0.05).Conclusion LSHD can improve the life quality of MHD patients by increasing sleep quality and nutrition level.
6.The clinical study on ankle joint fracture treated by open reduction and internal fixation

Objective To explore the surgical treatment of ankle fracture.Methods 326 cases with ankle joint fracture in our hospital treated by surgical treatment were followed up.There were 182 males and 43 females,aged from 7 ~ 75 years.The patients with medial malleolus fracture were cut through postero-medial process,and the lagged screw,absorbed screw,or tension band wire were used.Patients with bimalleolus and trimalleolus fracture were cut through medial and lateral two incision.When the fracture line of the fibula is high,the triangular needle,kirschner wire,plate and the screw,remember alloy were used,when the fracture line of the fibula was high,1 ~ 2 cortical or cancellous bone lagged screw were used.The patients with diastasis of inferior tibiofibular syndesmosis were cut through lateral process,1 lagged screw was used through 3 layer cortical bone.After the operation,the plaster or brace was used according to the stability of the fracture,then systemic functional exercises.Results All the patients were followed up from 4 ~ 120 months,according to the ankle-hind foot score of American Orthopedic Foot and Ankle Society Score(AOFAS),their clinical treatment results were assessed.In 225 cases,excellent and good rate was 86.7%.Connclusion Proper approach,anatomy reduction,articular surface recovery to smooth,stable internal fixation,necessary external fixation and systemic rehabilitation were the key aspects to the surgical treatment.Good result of most of the patients with fracture of the ankle joint can be obtained.
7.The analysis on bronchial asthma children allergen test results in different districts

Objective To test the allergen‐specific‐IgE in different districts for bronchial asthma children ,and analyze the re‐sults .Methods The blood samples of bronchial asthma children which selected from Heyuan City and Shenzhen City were detected by using German Ormond Western Blot Method reagents .Results The total positive rate of bronchial asthma children′s allergen‐specific‐IgE from Heyuan was 61 .7% .The positive rate of inhalation allergen was 43 .4% .The top three of inhalation allergens were house dust(41 .7% ) ,combination of mole(33 .3% ) ,the dog epithelium(20 .0% ) .The positive rate of food‐borne allergen was 36 .7% .The top three of food‐borne allergens were milk(33 .3% ) ,peanut(23 .3% ) ,beef(11 .7% ) .The total positive rate of bron‐chial asthma children′s allergen‐specific‐IgE from Shenzhen was 86 .7% .The positive rate of inhalation allergen was 73 .3% .The top three of inhalation allergens were combination of mole (63 .3% ) ,house dust (61 .7% ) ,The Liu/Yang/Elm combination (33 .3% ) .The positive rate of food‐borne allergen was 58 .3% .The top three of food‐borne allergens were shrimp(31 .7% ) ,crab (28 .3% ) ,the cod/lobster/scallop combination(21 .7% ) .Conclusion There were different parterns of the positive rate of bronchial asthma children′s allergen‐specific‐IgE in different districts .

9.Effects of continuous quality improvement on dietary and nutrition management in patients with chronic kidney disease

Objective To evaluate the effects of continuous quality improvement (CQI) management on nutritional status,renal function progression,and compliance of low protein diet in patients with chronic kidney disease (CKD).Methods Totally 115 CKD patients who were regularly followed up in CKD clinic services were recruited in this study.Plan,Do,Check,and Act (PDCA) method was adopted to manage the dietary of these patients for 12 months.The clinical indicators and diet compliance before and after receiving CQI management were compared.Results After receiving the CQI management,the nutritional status of patients was well maintained;meanwhile,the average hand strength and the hemoglobin,serum albumin,total cholesterol,and triglyceride levels showed no significant changes (all P > 0.05).Subjective feelings of patients were improved.The modified Subjective Global Assessment of Nutrition (mSGA) score was decreased from 7.0 (7.0,8.0) to 7.0 (7.0,7.0) (P =0.000).The estimated glomerular filtration rate (eGFR) calculated by formula of modified MDRD was decreased from (40.74 ± 14.49) to (37.94 ± 16.86) ml/(min · 1.73 m2) (P =0.000),and the average descended speed was (2.81 ±7.42) ml/(min · 1.73 m2) per year.The creatinine clearance rate had no statistical difference between pre-and post management (P =0.910),and the average descended speed was (0.19 ± 17.01) ml/min per year.The daily protein intake (DPI) and protein equivalent of nitrogen appearance rate (PNA) were both significandy descended:DPI/kg decreased from (0.79 ± 0.27) to (0.64 ± 0.15) g/ (24 h · kg) (P =0.000),and PNA/kg dropped from (1.02 ± 0.32) to (0.82 ± 0.24) g/ (24 h · kg) (P =0.000).The scores of awareness and compliance of patients on low protein diet were significantly increased after CQI management (P =0.000).Conculsion Applying CQI on dietary and nutrition management in CKD patients can maintain the good nutritional status and improve the compliance of low protein diet.
10.Application of CT/MR image fusing in gross tumor volume delineation of esophageal squamous cell carcinoma

Objective To investigate the application value of CT/MR image fusing in gross tumor volume (GTV) delineation of esophageal squamous cell carcinoma.Methods Twenty-nine patients with esophageal squamous cell carcinoma to be treated with radical surgery underwent routine CT scanning,MR T2-weighted imaging (T2WI) and diffusion weighted imaging (DWI) before surgery.Diffusion-sensitive gradient b values were taken at 400,600,and 800 s/mm2.GTVs were delineated on the CT image,CT/ MR T2WI,and CT/MR DWI respectively.The MR T2W1 image was used as the intermediary for the fusion of the CT image and MR DWI image.The length of GTVs measured under different imaging conditions were compared with the length of the resected specimen of esophagus.Results The GTV length was (44.94 ± 18.46) and (45.05 ±21.47) mm on the CT images and CT/MR T2WI images respectively.When the b values were 400,600,and 800 s/mm2,the esophageal carcinoma GTV length on CT/MR DWI images was (42.12 ± 17.79),(41.18 ± 17.17) and (39.77 ± 17.66) mm,respectively.The coefficient between the esophageal carcinoma GTV lengths on CT/MR DWI images and the pathological lesion lengths was 0.928,0.926 and 0.927 respectively.Conclusions CT/MR DWI images displays esophageal carcinoma GTV length more accurately,thus guiding the delineation of GTV effectively.
