1.The patients satisfaction with participation in medical and nursing decision making among cancer patients: a cross-sectional study

Objective To explore the patients satisfaction with participation in medical and nursing decision making and the influencing factors among cancer patients, and to provide a basis for developing better medical care services. Methods Totally 159 cancer patients were recruited from Beijing Cancer Hospital, and then were investigated with the general information questionnaire, the patients′ expectation for participation in clinical decision making scale, the patients′ competence for participation in clinical decision making scale, and the patients′satisfaction with participation in medical and nursing decision making questionnaires. Results The total score of patients′satisfaction with participation in medical and nursing decision making were (45.92±3.91) and (34.25±3.31) respectively. Compared with female patients, male patients had higher satisfaction with medical decision making (46.45±3.24&44.87±4.84) and nursing decision making (34.68 ± 2.93&33.42 ± 3.84) (t=2.416, P=0.017;t=2.275, P=0.024);and compared with melanoma patients, lung cancer patients had higher satisfaction with medical decision making (46.51 ± 2.69&43.33 ± 7.07, P=0.002);compared withⅢstage patients, Ⅳstage patients had higher satisfaction with nursing decision making(34.97±2.24&32.40±4.56, P<0.01). Meanwhile, the satisfaction with participation in decision making was significantly positively associated with patients′ expectation and competence for participation in clinical decision making. Conclusions Patients′satisfaction for decision making may vary from different gender, tumor types, disease stages, and patients′expectation and competence. Health care providers still further strength patients′ knowledge and initiative of decision making, and then improve patients′satisfaction with participation in decision making.
2.Clinical diagnosis and treatment of splenic tumor

Objective To summarize the experience on diagnosis and treatment of splenic neoplasm. Methods Clinical data of 40 patients with primary splenic space occupying lesion treated in our hospital were retrospectively analyzed. Results Thirty four out of the 40 cases were diagnosed as primary splenic neoplasm preoperatively. The detectable rate of B-US was 94%, and that of CT was 96%. The discrimination rate of benignancy and malignancy by CT was 84%. Serum AKP and ?-GT were significanfiy increased in most of the malignancies. Seventeen among 22 cases with benignancy were treated by splenectomy, and the others underwent partial splenectomy or tumor resection. Sixteen of 18 patients with malignancy underwent splenectomy, and two did biopsy. Pathology revealed cysts in 13, angiocavernoma in 4, inflammatory pseudotumor in 3, caverous lymphangioma in 1, and cystic degeneration of liomyoma in 1; malignant lymphoma and lymphsarcoma in 9, hemangiosarcoma in 3, and fibrosarcoma, liomyosarcoma, malignant fibrous histiocytoma in 1 each. Three cases with malignancy have survived more than 5 years. Conclusions Splenic neoplasm is diagnosed mainly according to clinical manifestation and image examination. The discrimination of benignancy and malignancy depends on CT, angiography, and serous AKP and ?-GT level. Radical operation and complex treatment could improve the prognosis of splenic malignant tumor.
3.Establishment and Implementation of Central Venous Line Management Information System in Cancer Special Hospital

Objective To establish and implement a central venous linemanagement information system based on the hospital information system (HIS). Methods The central venous line information management system was developed by the infusion therapygroup and the information technology department according to the requirement of completed central venous line management and the Chinese intravenous nursing practice standard. Results ① A complete information management system was constructed for central venous line from placement, maintenance and management of complications to drawing tube, and it operated smoothly. ②Totally, 4304 cases of PICC, 14,330 cases of CVC and 123 cases of PORT were collected from January 2016 to December 2017. The information of the maintenance number, catheter retention time, complications can be counted. Conclusions The central venous vein management information management system can realize the whole-process management, and ensure the continuous evaluation, maintenance and tracking of the patient's central venous line, making detailed statistical analysis of catheter- related complications, whichcan provide reliable basis for continuous nursing quality improvement.
4.Establishing quality evaluation criteria of cancer pain nursing record

Objective? To construct the quality evaluation criteria of cancer pain nursing record. Methods? Based on the requirements for Standard for Diagnosis and Treatment of Cancer Pain and Beijing Expert Consensus Document on Cancer Pain issued by the National Health Commission of the People's Republic of China, the indexes and scoring criteria for evaluating the quality of nursing documents writing for pain nursing were drawn up,and the reliability and validity of the criteria were tested through expert consultation, pre-investigation and scorer reliability. Results? The nursing record quality evaluation criteria of cancer pain consisted of 4 first-level indicators, 14 second-level indicators. The I-CVI value of each item was 0.88-1.00, S-CVI/Ave was 0.97, and the scorer reliability coefficient of each item was 87.5%-100%, and 0.84 for the scorers of the whole scale. Conclusions? The quality evaluation criteria of cancer pain nursing records are scientific, comprehensive, reasonable and have good reliability and validity. It can provide clear guidelines for clinical nurses to write cancer pain nursing records, and provide scientific quality evaluation tools for managers to supervise cancer pain nursing quality. It is worth popularizing and applying.
5.Status and influencing factors of tumor patients' participation in medication safety behavior

Objective:To explore the status of tumor inpatients' participation in medication safety behavior, and analyze its influencing factors.Methods:From October to November 2020, convenience sampling was used to select 502 patients with malignant tumor in Peking University Cancer Hospital. A survey was carried out on the patients by using the General Information Questionnaire and the Inpatients Involvement in Patient Medication Safety Scale. Multiple stepwise regression was used to analyze the influencing factors of medication safety behaviors in tumor inpatients.Results:The total score of 502 patients' participation in medication safety behavior was (88.86±16.79) . The scoring rate of the 3 dimension items from high to low were decision-making involvement, care involvement, appealing involvement. Multiple stepwise regression showed that educational level, the importance of conscious participation in medication safety, knowledge of disease treatment, medication process and participation methods were the influencing factors for tumor inpatients' participation in medication safety behavior ( P<0.05) . Conclusions:Tumor patients' participation in medication safety behavior needs to be further improved. It is necessary to improve the patient's awareness of participating in medication safety and explore feasible ways of patient participation to improve patient participation in medication safety behavior and further promote patient medication safety.
6.Effects of electroacupuncture at "Weizhong" (BL 40) on morphology and expression of CK and IL-17 in rats with bupivacaine-induced multifidus muscle injury.

OBJECTIVETo observe effects of electroacupuncture (EA) at "Weizhong" (BL 40) on morphology and expression of creatine kinase (CK) and interleukin-17 (IL-17) in rats with bupivacaine-induced multifidus muscle injury.
METHODSA total of 32 male SD rats were randomly divided into a control group, a model group, a Weizhong group and a Shenshu group, 8 rats in each one. The rats in the model group, Weizhong group and Shenshu group were treated with intramuscular injection of 0.5% bupivacaine to establish the model of multifidus muscle injury; the rats in the control group were injected with 0.9% sodium chloride solution. The rats in the Weizhong group and Shenshu group were treated with EA (2 Hz/10 Hz in frequency, 1~2 mA in intensity) at "Weizhong" (BL 40) and "Shenshu" (BL 23), 20 min per treatment. No treatment was given in the control group and model group. After 14-day treatment of EA, the inflammatory cell count, scar tissues area and muscle fiber cross sectional area of multifidus muscle were observed with HE and Masson staining method. The activity of CK and serum content of IL-17 were test with enzyme-linked immunosorbent assay (ELISA) method; the expression of IL-17 in multifidus muscle was measured with immunohistochcmical method.
RESULTSAfter intervention, the inflammatory cell count and scar tissues area in the model group, Weizhong group and Shenshu group were higher than those in the control group (all<0.01), but the muscle fiber cross sectional area was significantly reduced (all<0.01); the inflammatory cell count and scar tissues area in the Weizhong group and Shenshu group were lower than those in the model group (all<0.01), and the muscle fiber cross sectional area was significantly increased (<0.01,<0.05). After intervention, the expression of IL-17 in multifidus muscle, serum content of IL-7 and activity of CK in the model group, Weizhong group and Shenshu group were higher than those in the control group (all<0.01); the expression of IL-17 in multifidus muscle, serum content of IL-7 and activity of CK in the Weizhong group and Shenshu group were lower than those in the model group (<0.01,<0.05); compared with the Shenshu group, the down-regulation of IL-17 was more obvisous in the Weizhong group (<0.01).
CONCLUSIONEA at "Weizhong" (BL 40) can down-regulate the overexpression of serum CK and IL-17, alleviate inflammation reaction and improve the repair of multifidus muscle.
