1.The study on the clinical effects and short-term results of time-limited rescue angioplasty following thrombolytic therapy in acute myocardial infarction

Objective To evaluate the rationality, safety and efficiency of the time-limited rescue angioplasty following thrombolytic therapy in acute myocardial infarction (AMI).Methods Among the patients within 6 hours from the onset of symptoms of AMI, forty-four cases (group A) underwent primary coronary angioplasty and fifty-eight cases (group B) underwent firstly intravenous thrombolytic therapy. According to clinical early reperfusion indication within 90 minutes following thrombolytic therapy, group B was divided into two subgroups, the patients with early reperfusion (subgroup C) underwent delayed interventional examination 7~10 days after AMI and that with non-reperfusion (subgroup D) underwent rescue angioplasty. The reperfusion rates and complications in different groups were analyzed and compared. Cardiac function (left ventricular ejection fraction, LVEF) was evaluated by echocardiograph 4 weeks after AMI. Results The results showed that the rate of reperfusion, in group A, was 95.45% (42/44),that of subgroup C was 32.76 % (19/58) within 90 minutes following thrombolytic therapy (16 of subgroup C underwent delayed interventional examination and 12 of them underwent PTCA+stent) and that of subgroup D was 97.43% (38/39); There were no serious complications that occurred in subgroups C and D. 2.Effect of therapy time from myocardial infarction onset to percutaneous coronary intervention on matrix metalloproteinases level and left ventricular remodeling in patients with acute anterior wall myocardial infarction

Objective To investigate the effect of time from myocardial infarction (MI) onset to percutaneous coronary intervention (PCI) on plasma matrix metalloproteinases (MMPs) level and left ventricle (LV) remodeling in patients with acute ST-segment elevation myocardial infarction of anterior wall,and the relationship between MMPs and left ventricular remodeling.Methods All patients with anterior wall STEMI undergoing PCI were divided into early PCI group (PCI within 18 h after MI onset) and delayed PCI group (PCI between 2 and 3 weeks after MI onset).Plasma MMP-2 and MMP-9 activities were assayed on admission,and at 2 days,1 week after admission.One-year follow-up was finished after PCI.Left ventricular end-diastolic volume (LVEDV),left ventricular end-systolic volume (LVESV) and left ventricle ejection fraction (LVEF) were measured by echocardiography at baseline and one year later to elucidate the effects of time from onset to PCI on LV remodeling and the relationship between MMP-2,-9 levels and LV remodeling.Results The MMP -9 activity at 2 days after myocardial infarction was lower in early PCI group than in delayed PCI group [(46±26)μg/L vs.(66±40) μg/L,P=0.000].The changes in LVEDV and LVESV (△LVEDV and △LVESV) were lower and the change in LVEF (△LVEF) was higher in early PCI group than in delayed PCI group [(10.9±6.2) ml vs.(15.0±6.0)ml,(-1.1±5.7) ml vs.(2.9±4.6) ml,(5.5 ±4.0) % vs.(3.8±3.4) %,P=0.000,0.000 and 0.015].MMP-9 had positive correlations with △LVEDV and △LVESV,and a negative correlation with △LVEF at admission and after 1-year follow-up (r=0.32,0.36 and-0.29,respectively,P=0.000,0.000 and 0.001).Conclusions MMP-9 activity at admission is correlated with LV remodeling and LV function.Early PCI can reduce MMP-9 activity and improve LV remodeling after myocardial infarction.
3.Clinical outcomes and safety of percutaneous coronary intervention only in single-opened vessel lesion among patients with severe left ventricular systolic dysfunction

Objective This study was conducted to investigate the clinical outcomes and safety of percutaneous coronary intervention (PCI) to the single-opened vessel lesion among patients with severe left ventricular systolic dysfunction. Methods Twenty-seven patients with severe left ventricular systolic dysfunction (ejection fraction≤35%) undergoing PCI were included. All the patients received PCI only to the single-opened vessel lesion under the conditions of: (1) There were limitations to open chronic total occlusion (CTO);(2) Single-opened vessel lesion was not calcified and tortuous. Clinical outcomes, including success rate of PCI, changes of symptoms in-hospital, brain natriuretic peptide (BNP) and left ventricular ejection fraction (LVEF) pre-and one week post-PCI, the major adverse cardiac events (MACE, including death, myocardial infarction and target vessel revascularization) at 30-days after discharged were observed. Results The success rate of PCI was obtained in all 27 patients(100%), and all the patients received drug eluting stent implantation. The symptoms improvement occurred in all patients and the NYHA class improved from grade Ⅳto grade Ⅲin 22 patients(81.5%) in-hospital. Significant differences were noted in the mean BNP and LVEF between pre-PCI and one week post-PCI, BNP[(2699.6±1104.7) pg/ml vs. (737.0 ± 261.7) pg/ml, P<0.05],LVEF[(26.9±5.7)%vs. (36.0±3.41)%, P<0.05)]. No MACE happened in-hospital and at 30-days follow up. Conclusions PCI only to the single-opened vessel lesion among patients with severe left ventricular systolic dysfunction under the condition of limitations to open CTO is safe and can significantly improve clinical outcomes in-hospital and at 30-days follow up, but it must be emphasized that single-opened vessel lesion not with obvious calcification and tortuosity.
4.A potential role for the bone marrow mesenchymal stem cell in the pathogenesis of osteoporosis by ovariectomy in rat.

The purpose of this paper is to investigate the osteogenesis and adipogenesis in bone marrow mesenchymal stem cells (MSCs) isolated from normal rats and osteoporotic rats by ovariectomy. Osteoporotic animal model was established in 3 month-old and 6 month-old female Sprague-Dawley (SD) rats by ovariectomy. Animal experiments were divided into 4 groups: 1) control-3 group; 2) ovx-3 group; 3) control-6 group and 4) ovx-6 group. MSCs were isolated by means of the density-gradient centrifugation method from each group, respectively. Colony-forming unit-fibroblast (CFU-Fs ) number, CFU-Fs size distribution and cell density in CFU-Fs of primary passage MSCs were measured at the inverted phase contrast microscope. The cell cycle and proliferation index (PI) as well as apoptosis idex (AI) of MSCs were studied by (FCM). After osteogenic induction (OSI), calcium nodes of MSCs were marked by alizarin red staining (ARS); The expression level of alkaline phosphatase(ALP) was detected by dynamics method with substrate of phosphoric acid para-Nitro benzene and the content of osteocalcin (OCN) was detected with the isotope labelling method. After adipogenic induction (ADI), lipid droplet in MSCs were detected by oil red O staining and the mRNA level of lipoprotein lipase (LPL) was measured by RT-PCR. The results showed that CFU-Fs and PI are obviously decresed and AI are increased of MSCs in OVX-3 and OVX-6 groups (P<0.05). The secretory volume of ALP and BGP of MSCs and the content of calcium nods of MSCs are lower in OVX-3 and OVX-6 groups than that in control-3 and control-6 groups after osteogenic induction (P<0.05). The number of lipid droplet and the expression level of LPL mRNA are higher in OVX-3 and OVX-6 groups than that in control-3 and control-6 (P<0.05). The result in our study suggested that depress of osteogenesis and the up-regulation of adipogenesis of MSCs in osteoporotic rats by ovariectomy may be relate close to the pathogenesis of osteoporosis.

5.Adipogenic potential of bone marrow mesenchymal stem cells derived from ovariectomized rats.

In order to study the effect of ovariectomy on the capacity of rat MSCs' differentiation into adipocytes, we established the animal models of osteoporosis by performing ovariectomy on the 3-months-old and 6-month-old female SD rats. Then MSCs were isolated from these rats by means of the density-gradient centrifugation method. After adipogenic induction (ADI), lipid droplets were detected by oil red O staining, and the mRNA level of lipoprotein lipase(LPL) was measured by RT-PCR assay. The experiments included 8 groups: (1) MSCs from 3-month-old rats (MSCs 3control); (2) MSCs from 3-month-old ovariectomized rats (MSCs 3ovx); (3) MSCs from 6-month-old rats (MSCs 6control); (4) MSCs from 6-month-old ovariectomized rats (MSCs 6ovx); (5) MSCs from 3-month-old rats with ADI (MSCs ADI 3control); (6) MSCs from 3-month-old ovariectomized rats with ADI (MSCs ADI 3ovx); (7) MSCs from 6-month-old rats with ADI (MSCs ADI 6control); 8) MSCs from 6-month-old ovariectomied rats with ADI (MSCs ADI 6ovx). The results showed that the number of lipid droplets and expression level of LPL mRNA in MSCs ADI 3ovx and MSCs ADI 6ovx were higher than those in MSCs ADI 3control and MSCs ADI 6control, respectively; that the number of lipid droplets and expression level of LPL mRNA in MSCs ADI 6control were higher than those in MSCs ADI 3control respectively. The results of this study suggested that the adipogenic potential of MSCs from both aged and ovariectomied osteoporotic rats were augmented.

6.Risk factors of acute kidney injury in patients with multiple wasp stings

Objective:To evaluate the risk factors of acute kidney injury (AKI) in patients with multiple wasp stings.Methods:Patients with multiple wasp stings were retrospectively enrolled in Hanzhong Central Hospital from September 2010 to November 2020. Based on whether the patients developed AKI, the patients were divided into AKI group and non-AKI group. The general characteristics and laboratory examinations between the two groups were compared. The logistic regression model was used to analyze the risk factors of AKI.Results:A total of 356 patients with multiple wasp stings were recruited in this study, with 196 males (55.1%). The age was 56.0(45.0, 64.0) years old. There were 59 patients (16.6%) with hypertension and 13 patients (3.6%) with diabetes. There were 51 patients (14.3%) in the AKI group and 305 patients (85.7%) in the non-AKI group. Baseline data and biochemical examinations indicated that the two groups showed significant differences in gender, age, sting sites (systemic or local), sting needles, proportions of gross hematuria, leukocyte count, hemoglobin, creatine kinase, alanine transaminase, aspartate aminotransferase, total bilirubin, proportions of urinary protein, and proportions of urine occult blood (all P<0.05). The multivariate logistic regression analysis results showed that the increasing number of sting needles (every 10 needles increase, OR=1.866, 95% CI 1.289-2.071, P=0.001), gross hematuria ( OR=9.770, 95% CI 2.586-36.910, P=0.001), decreasing hemoglobin (every 1 g/L increase, OR=0.016, 95% CI 0.001-0.355, P=0.009), increasing aspartate aminotransferase (every 100 U/L increase, OR=1.311, 95% CI 1.144-1.502, P<0.001), and increasing total bilirubin (every 10 μmol/L increase, OR=1.200, 95% CI 1.008-1.430, P=0.041) were independent influencing factors of AKI. Conclusions:The increasing number of sting needles, gross hematuria, decreasing hemoglobin, increasing aspartate aminotransferase, and increasing total bilirubin are independent risk factors of AKI in patients with multiple wasp stings.
