1.ANTIINFLAMMATORY, ANALGESIC AND ANTIPYRETIC EFFECTS OF DEOXYACONITINE

Deoxyaconitine (DAC), an extract from the tuber of Aconitum flavum Hand-Mazz, was assayed comparatively with the other previously studied alkaloid 3-Acetylaconitine(3AAC)with respect to their antiinflammatory, analgesic and anti-pyretic effects in laboratory animals. DACO.Smg /kg ip markedly inhibited the croton oil induced ear edema in micei 0.2 mg/kg ip or sc showed significant inhibiton on the hind-paw edema in rats induced by carrageenin or formalin,and on the increase of vascular permeability induced by intradermic injection of histam-ine in rats or acetic acid ip in mice. With the model of carrageenin induced hind-paw edema in rats, the antiinflammatory therapeutic index of DAC ip (6.38) was slightly higher than that of 3AAC (5.92 ) . The sc analgesic ED50 of DAC measured with acetic acid twisting test in mice was 0.22+ 0.06mg/kg and that measured with hotplate method by ip was 0.41+0.10 mg/kg.In the latter case, the analgesic therapeutic index of DAC (6.37) was higher than that of 3-AAC(4.60) . DAC 0.24mg/kg ip showed marked anti-pyretic effect on the rabbit fever induced by typho-paratyphoid vaccine.
2.Effect of electro-scalp acupuncture on metabolism of glucose in brain functional region of patients with depression

BACKGROUND:It is suggested that glucose metabolism of frontal lobes,cingulate gyri and caudate nuclei in cerebral functional area of patients with depression is low,and the blood flow in cerebellum decreases.OBJECTIVE:To observe the effect of scalp acupuncture on cerebral glucose metabolism in depression by positron emission tomography.DESIGN:Randomly controlled and verifying experiment.SETTING:Department of Acupuncture and Aoxibustion in Nanfang Hospital and Center of Positron Emission Tomography in a Guangdong People's hospital.PARTICIPANTS:Twelve patients of primary depression,from Nanfang Hospital,Zhujiang Hospital and Center of Positron Emission Tomography in Guangdong People's Hospital from October 2002 to October 2003,were selected randomly with the diagnostic criteria of [F32] in the Third Edition of the Chinese Classification and Diagnostic Criteria of Mental Disease.Among them,3 were male and 9 were female.Their ages were from 36 to 66 years,with the average of 48.METHODS:Giving scalp acupuncture in middle line of vertex(MS5),middle line of forehead(MS1) and bilateral lateral line 1 of forehead(MS2).All the patients received positron emission tomography examination before and after acupuncture treatment for 6 weeks.Then cerebral Regions of Interest,bilateral frontal lobes,bilateral caudate nuclei,bilateral cingulate gyri and bilateral cerebella,were chosen and their average glucose metabolisms were counted by half-quantifying analysis.MAIN OUTCOME MEASURES:Glucose metabolism of bilateral frontal lobes,bilateral caudate nuclei,bilateral cingulate gyri and bilateral cerebella.RESULTS:According to intension,all 12 patients with primary depression sults of right frontal lobes(2.91 ±0.34),left frontal lobes(2.96±0.26),right cingulate gyri(2.68±0.20),right caudate nuclei (2.61±0.39) and left cerebellum(2.55±0.18)were obviously higher than those of right frontal lobes(2.53±0.31),left frontal lobes(2.53±0.36),right cingulate gyri(2.34±0.31),right caudate nuclei(2.40±0.33) and left cerebellum(2.35±0.25)before treatment(P< 0.05-0.01).However, the results of left cingulate gyri (2.69 ±0.38),left caudate nuclei(2.66±0.01) and right cerebellum(2.50±0.27) were similar to those of left cingulate gyri(2.43±0.38),left caudate nuclei(2.45±0.21) and right cerebellum(2.30±0.34) before treatment(P > 0.05).CONCLUSION:Scalp acupuncture on middle line of vertex,middle line of forehead and bilateral lateral line 1 of forehead could improve the glucose metabolism in depressive patients of certain cerebral regions,and it might be a mechanism related to improvement of frontal lobes,caudate nuclei,cingulate gyri and cerebellum.
3.Proteomics study in ischemic postconditioning after prolonged ischemia for lessening the ischemia/reperfusion injury of intestinal in rats

Objective To investigate the changes of proteins expressions in intestinal mucosa of rats after is chemic postconditioning (IPo) against intestinal ischemic/reperfusion (Ⅱ/R) injury of intestine in order to elucidate its potential mechanisms of protective role. Methods Sixteen SD rats were randomly divided into Ⅱ/R group and IPo group ( n = 8). Rats of both groups received an episode of ischemic/reperfusion insult to intstine that was made by occlusion of the superior mesenteric artery (SMA) for 60 minutes. Rats of IPo group underwent three additional episodes of clamping SMA on for 30 seconds and off for 30 seconds successively after prolonged reperfusion/reperfusion of intestine. The intestinal mucosa was taken by scratching immediately after reperfusion in both groups, and total proteins were separated by immobilized pH gradient (IPG) based two-dimensional gel electrophoresis (2-DE). The differentially expressed proteins were analyzed using Image Master 2D Elite 5.0 image analysis software, and the proteins were cut out from the gel and then identified using MALDI-TOF-MS. The biological information of these proteins was looked for in the database of these peptide mass finger-printing (PMF) .Results Ten differentially expressed proteins were found, of which 6 were up-regulated and 4 were down-regulated in IPo group. Nine proteins were identified and characterized by their bioelements including aldose reductase and aldehyde dehydrogenase that were related to anti-oxidative stress and inhibition of cell apoptosis. Conclusions The well-reproducible 2-DE profiles of intestinal mucosa in II/R and IPo groups were established. The potentially protective effects of IPo may be attributed to up-regulating protein expressions of aldose reductase and aldehyde dehydrogenase, and thereby suppressing oxidative stress and cell apoptosis.
4.Effectiveness of cultured Cordyceps sinensis combined with glucocorticosteroid on pulmonary fibrosis induced by bleomycin in rats.

OBJECTIVETo study the treatment effects of cultured Cordyceps sinensis combined with glucocorticosteroid on experimental pulmonary fibrosis in rats induced by bleomycin.
METHODFifty rats were randomly divided into five groups, including control group, model group, cultured C. sinensis groups, prednisone group, cultured C. sinensis combined with prednisone group. On experimental day 0, the rats were respectively intratracheally instilled with bleomycin, and rats in the control group and model group with the same volume of normal saline. One day after the injection, cultured C. sinensis and glucocorticosteroid was respectively given to rats daily by gastric gavage, while the same volume of normal saline was given to those in the control group and model group. On 28th d, bronchoalveolar lavage fluid (BALF) and lung tissue were collected. Histological changes of the lungs were evaluated by HE stain, Masson's trichrome stain. Collagen content of the lung tissue was assessed by hydroxyprolin concentration. Lung expression of CTGF protein was assessed by immunohistochemistry. The level of TGF-beta1 protein was measured by ELISA.
RESULTCompared to model group, pulmonary fibrosis were alleviated in cultured C. sinensis and prednisone group, and CTGF expression, Hydroxyproline concentrations and protein TGF-beta1 were decreased. The combination effect of C. sinensis and prednisone group is augmented compared with using C. sinensis or prednisone group alone.
CONCLUSIONThe cultured C. sinensis and prednisone alleviates pulmonary fibrosis, and the combination use of both drugs has synergia effects in anti-fibrous degeneration.

5.Screening and diagnostic efficacy evaluation of metabolomics biomarkers in patients with active tuberculosis

Objective:Based on UPLC-Q-TOF/MS, to screen a panel of plasma metabolite biomarkers for TB diagnosis and evaluate its diagnostic efficacy.Methods:102 active TB patients [49 males, 53 females, age 40.0(24.0, 48.5) years] from Shanghai Pulmonary Hospital and Shanghai East Hospital from January 2017 to January 2018, 100 TB-IGRA positive patients [55 males, 45 females, age 44.0(37.0, 52.0) years] and 96 healthy controls [55 males, 41 females, age 43.0(32.2, 52.8) years] from Shanghai East Hospital were randomly enrolled. UPLC-Q-TOF/MS technology was used to detect small molecule metabolites in plasma. Combined with multivariate statistical methods VIP and univariate statistic analysis Student's t-test, the main differential metabolites in the plasma of patients with active tuberculosis were filtered. The ROC curve was analyzed for the differential metabolites, and the AUC value, specificity, and sensitivity for diagnosis were used to screen metabolic biomarkers with diagnostic potential. Results:All the samples examined resulted in 10 266 variables, and 1 153 substances were identified by qualitative retrieval through the human metabolome database. After pairwise comparison of samples from the three groups, differential metabolites that simultaneously satisfied VIP > 1 and P<0.05 were plotted into a Venn diagram, and the resulting intersection set contained 38 major differential metabolites. The ROC curve analysis of 38 major metabolites showed that the area under the curve of lactic acid, dopamine, 9-pentadecenoic acid, and 12,13-dihydroxy octadecadienoic acid in the diagnosis of active tuberculosis were 0.92, 0.98, 0.94, and 0.94, respectively, the specificity was both more than 90% and the sensitivity was both more than 80%. The specificity and sensitivity of four metabolites in the combined diagnosis of active tuberculosis were both 94%. Conclusion:Lactic acid, dopamine, 9-pentadecenoic acid, and 12, 13-dihydroxy octadecadienoic acid can be used as potential metabolic biomarkers for tuberculosis diagnosis.
6.Clinical value of "four longitudinal, two transverse planes" method of membrane anatomy in laparoscopic radical cystectomy

【Objective】 To explore the clinical value of laparoscopic radical cystectomy based on fascia anatomy for bladder cancer treatment. 【Methods】 The clinical data of 51 patients with bladder cancer who underwent 3D laparoscopic radical cystectomy during Jan.2015 and Jun.2022 were retrospectively analyzed.The surgery was performed based on membrane anatomy technology along four longitudinal and two transverse planes to complete the radical cystectomy.The pelvic plexus was preserved for patients with normal preoperative sexual function. 【Results】 All surgeries were completed without conversion to open operation.The mean operation time was (502.52±108.99) min, mean intraoperative blood loss was (275.96±155.18) mL, mean postoperative drainage time was (4.14±2.41) d, and the mean postoperative hospital stay was (16.37±4.85) d.The mean number of lymph nodes removed was (17.98±11.48).The mean postoperative follow-up was (30.27 ±19.39) months.At the last follow-up, no Clavien ≥grade 3 complications were observed.The estimated overall survival (OS), tumor-specific survival (TSS), and recurrence-free survival (RFS) were 82.4%, 92.2%, and 88.2%, respectively.The lymph node positive patients had shorter OS and RFS (60.0%, 60.0%) than the lymph node negative patients (84.8%, 91.3%).Among the 19 male patients who underwent radical cystectomy with pre-exposure and preservation of pelvic plexus, daytime and nocturnal continence rate were 83.3% and 72.2%, respectively, and 17 patients recovered potency within 6 months postoperatively. 【Conclusion】 Laparoscopic radical cystectomy based on fascia anatomy is safe and effective in laparoscopic radical cystectomy, with standardized surgical procedure, satisfactory oncological outcomes, little hemorrhage, few complications and fast recovery.
