2.Effect evaluation of high quality nursing service in patients with artificial joint replacement

Objective To explore the effects of high quality nursing service mode on nursing of patients with artificial joint replacement.Methods 82 patients from January to December 2010 were set as the control group,another 82 patients from January to November 2011 were named as the observation group.The observation group was given high quality nursing service mode,while the control group received conventional nursing.The satisfaction degree of patients to nursing,average hospitalization time and cost and incidence of complications were compared between two groups.Results The satisfaction degree of the observation group was 98.78%,significantly higher than 93.90% in the control group.The average hospitalization time was(14.68±5.88)days in the observation group,shorter than(19.22±6.78)days of the control group.The average hospitalization expense in the observation group was (25 665±528)yuan,less than (30 866± 732)yuan of the control group.The complication rate in the observation group was 21.95%,significantly lower than 46.34% of the control group.Conclusions Targeted high quality nursing service mode can effectively improve the quality of nursing,decrease the incidence of complications,reduce the patient's hospitalization time and cost,as well as improve the patients' satisfaction degree.
3.Clinical diagnosis and treatment analysis of ovarian tumor during pregnancy in 156 cases

Objective To explore the ovarian tumor's characteristics, diagnosis and treatment during pregnancy.Methods A retrospective analysis of 156 cases of ovarian tumor was carried out.Results About 152 cases were dignosed as benign,accounting for 97.44%.The majority of which were demoid cysts and grout cystadenomas,two cases were bordline grout papillary cystaadenoma,other 2 cases,one was some of immature teratoma, another was immature teratoma.67 cases were diagnosed by ultrasound during the first trimester, accountting for 80.72%, among 17 urgent operations,10 cases caused by torsion,7 cases caused by rupture.43 cases operated by term,among which 39 cases were tradion operations,4 cases were laparoscopic surgery.18 cases made the examination of CA125 before operation,as a result,13 cases elevated,AFP examination were made in 10 cases,7 cases elevated.Full-term birth was in 152 patients, accounting for 97.44%.Conclusion Majority ovarian tumor during prenaney were benign;Ultrasound examination combined with tumor marker were important in diagnosis and detection ovarian tumor during pregnancy; The individual operation therapy should be chosen, according to the tumor's properties, gestational weeks, and patients' will;Timely open surgery or laparoscopic surgery should not affect the pregnancy outcome.
4.Value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perforation

Objective To explore the value of gastroendoscopy check-up early after non-surgical treatment of patients with gastroduodenal perfolation.Methods The patients suspected of perforated gastroduodenal ulcer on hospital admission underwent non-surgical treatment were enrolled.After cured clinically,performed gastroendoscopy at different time periods.Results Among 133 patients underwent gastrointestinal endoscopy,129 cases(97.0%)were diagnosed gastroduodenal ulcer,3 cases(2.3%)were diagnosed gastric carcinoma,1 case (0.8%)Was confirmed duodenal diverticulum,no compilcations occurred due to endoscopy.The 6 cases that had surgical indications had be implemented the early surgical treatment. Conclusions After non-surgical treatment of patient with gastroduodenal perforation,timely gsstroendoscopy can truly show pathologic changes with gastroduodenal perforation.So endoscopy is beneficial under condition appropriate time seleetion for confirmed the eliology of PGD,treatment promptly targeted etiology and understanding the mechanism healing the perforation as non-surgical treatment.
5.Analysis of Drug Use for Gastrointestinal Ulcer in Outpatient Department of Our Hospital during 2013-2015

OBJECTIVE:To provide reference for rational use of drugs for gastrointestinal ulcer. METHODS:The drug use for gastrointestinal ulcer in outpatient department of our hospital during 2013-2015 was analyzed retrospectively in respects of drug name,consumption sum,DDDs,DDC,single/multiple diseases prescription,therapeutic efficacy,rationality of presoription,etc. RESULTS:A total of 8 kinds of drugs use for gastrointestinal ulcer in outpatient department of our hospital during 2013-2015, among which the consumption sum and DDDs of Omeprazole capsule and Rabeprazole capsule increased rapidly,and their DDC were the higher others;those of other drugs kept stable. The proportion of acid-inhibitory drugs in single disease prescriptions was lower than in multiple diseases prescriptions,while that of antiacid was higher than in multiple diseases prescriptions,with statisti-cal significance (P<0.05). The total effective rate of patients with gastrointestinal ulcer gradually increased from 77.98% to 82.68% in outpatient department of our hospital during 2013-2015,there was statistical significance between 2015 and 2013(P<0.05);the proportion of irrational prescriptions decreased from 5.01% to 2.12% year by year,with statistical significance among those years (P<0.05). CONCLUSIONS:Omeprazole is the first choice for gastrointestinal ulcer in outpatient department of our hospital with good therapeutic efficacy. There still are some irrational drug use in prescriptions,and clinicians should pay attention to those situations.
6.Influence of insulin on myocardial microcirculation in patients with type 2 diabetes mellitus complicat-ed coronary heart disease

Objective:To study influence of insulin on myocardial microcirculation of patients with coronary heart disease (CHD)complicated type 2 diabetes mellitus (T2DM)after percutaneous coronary intervention (PCI).Meth-ods:A total of 65 patients,who was confirmed as CHD by coronary angiography and fractional flow reserve (FFR)<0.75 measured by pressure guidewire,were enrolled.After drug-eluting stent implantation,index of microcircula-tion resistance (IMR)was measured.According to insulin usage or not,they were divided into insulin therapy group (insulin group,n=30)and pure oral hypoglycemic agent therapy group (pure hypoglycemic agent group,n=35). After six-month follow up,IMR was reviewed,and IMR was compared between two groups.Results:There was no significant difference in incidence rate of major adverse cardiovascular events (MACEs)between two groups,P =0.07. On six months after PCI,IMR in insulin group was significantly lower than that of pure hypoglycemic agent group [(24.5±3.1)U vs.(30.2±4.3)U,P <0.05].Conclusion:Insulin therapy can improve myocardial micro-circulation in patients with coronary heart disease complicated type 2 diabetes mellitus after PCI.
7.The water separator rectum guided vaginal sacral spine ligament fixation treatment for pelvic organ prolapse

Objective To explore the clinical efficacy and surgical techniques of vaginal sacrospinous ligament fixation,which was used for treatment of uterine prolapse and vaginal prolapse of the top.Methods 25 patients with uterine prolapse and vaginal vault prolapse were treated with VSSLF,and its feasibility and effiency was analyzed.Results All patients were successfully conducted the surgical indexing method based on POP-Q,25 cases of uterine prolapse,vaginal anterior and posterior wall prolapse and urinary incontinence and other symptoms all had been corrected.There was no intraoperative and postoperative bleeding or pelvic organ injury in surgry.25 patients recovered well after operation,postoperative morbidity,not observed in other complications associated with surgery.All the patients were followed up for 24 months,patients'cervix 4 cm from the hymen edge,related symptoms disappeared or improved significantly.Conclusion Vaginal sacrospinous ligament fixation is a safe,simple,effective and minimally invasive surgery for uterine prolapse and vaginal prolapse of the top.
8.Influence of percutaneous coronary intervention on brain natriuretic peptide and ventricular remodeling ;in patients with acute ST elevation myocardial infarction

Objective:To explore influence of percutaneous coronary intervention (PCI)on brain natriuretic peptide (BNP)level and ventricular remodeling in patients With acute ST elevation myocardial infarction (STEMI).Meth-ods:According to their therapeutic methods,a total of 151 patients diagnosed as STEMI Were divided into group A (n=52,received emergency PCI treatment),group B (n=49,received selective PCI treatment)and group C (n=50,received standard medication).BNP level and left ventricular remodeling Were compared among three groups before treatment and after treatment.Results:There Was no significant difference in BNP level among three groups before treatment (F=0.01,P>0.05),BNP level significantly decreased among three groups after treatment,com-pared With group B and C,there Was significant decrease in BNP level [(466.2±113.5)ng/L vs.(601.4±126.7) ng/L vs.(242.7±101.3)ng/L]in group A (P<0.01 all).There Were no significant difference in left ventricular end-diastolic volume (LVEDV),left ventricular end-systolic volume (LVESV),left ventricular ejection fraction (LVEF)and left ventricular fractional shortening (LVFS)among three groups before treatment (P>0.05 all);af-ter treatment,there Were significant increase in above ventricular indexes in three groups (P<0.05 all),compared With group C,there Were significant increase in LVFS [(22.6±6.9)% vs.(28.9±7.6)% vs.(27.5±7.3)%]in group A and B,compared With group B and C,there Was significant increase in LVEF [(53.1±6.5)% vs.(50.9± 5.7)% vs.(57.9±6.2)%]in group A (P<0.01 all).Conclusion:PCI could effectively improve ventricular re-modeling and decrease BNP level in STEMI patients,and emergency PCI shoWs more significant therapeutic effects than selective PCI.
9.Changes of serum nesfatin-1 levels in type 2 diabetes patients with nonalcoholic fatty liver

Objective To investigate serum nesfatin-1 levels in type 2 diabetes (T2DM)patients with or without nonalcohol-ic fatty liver(NAFLD).Methods The hight,weight,serum GLU,lipid profiles,INS,HbA1c,ALT,nesfatin-1 levels were measured in 21 1 T2DM patients[98 without NAFLD(TD2M without NAFLD group)and 1 13 with NAFLD(TD2M with NAFLD group)] and 75 normal subjects(NGT group).BMI、HOMA-IR were also calculated.Results The serum nesfatin-1 level was significantly higher in T2DM group than those in NGT group(P <0.05),and T2DM with NAFLD group had higher nesfatin-1 level than T2DM without NAFLD group (P < 0.05 ).There was a close relation between nefastin-1 of T2DM group and course of disease (r=-0.447,P <0.05).NAFLD existed a significant correlation with BMI、TG and nesfatin-1 (P <0.05).Conclusion nesfatin-1 may partially contribute to the pathogenesis of T2DM and NAFLD.
10.Effect of Electroacupuncture on Gastrointestinal Function in Critically Ill Patients

ObjectiveTo observe the effect of early-stage intervention with acupuncture on gastrointestinal dysfunction in critically ill patients.MethodTotally 112 critically ill patients were randomized into two groups. Based on the treatment for primary disease, the conventional group additionally received Lived Combined B. Subtilis and E. Faecium Enteric-coated Capsules, plus Alanyl Glutamine injection when parenteral alimentation was required; in addition to the treatments given to the conventional group, the acupuncture group received electroacupuncture, once a day. Before treatment, and on treatment day 7 and day14, the gastrointestinal function and nutritional state (hemoglobin, albumin), nutrition type, APACHEⅡscore, incidence rate of gastrointestinal function failure, and mortality rate were evaluated and compared.ResultAfter 7-day treatment, the gastrointestinal function scores were significantly improved in both groups (P<0.05), and there was a significant difference between the acupuncture group and conventional group (P<0.05); after 14-day treatment, the gastrointestinal function scores were significantly improved in both groups compared to that before treatment (P<0.01), and there was a significant difference between the acupuncture group and conventional group (P<0.05). Respectively after 7-day and 14-day treatment, the APACHEⅡscores were significantly improved in both groups (P<0.01), and there was a significant difference between the acupuncture group and conventional group (P<0.05); after 14-day treatment, there was a significant difference between the two groups (P<0.05). After 7-day treatment, the hemoglobin levels were insignificantly changed in the two groups (P>0.05), and the difference was insignificant between the two groups (P>0.05); after 14-day treatment, the hemoglobin levels were significantly changed in both groups compared to that before treatment (P<0.05), and there was a significant difference between the two groups (P<0.05). After 7-day treatment, the albumin levels were insignificantly changed in the two groups (P>0.05), and the between-group difference was statistically insignificant (P>0.05); after 14-day treatment, the albumin levels were significantly changed in both groups compared to that before treatment (P<0.05), and there was a significant difference between the two groups (P<0.05). In the acupuncture group, 10 patients were dead, and there were 13 dead in the conventional group, and the between-group difference in death rate was statistically insignificant (P>0.05), though the death rate was comparatively lower in the acupuncture group; in the acupuncture group, 3 patients turned from complete parenteral nutrition to mixed nutrition, while 4 patients turned to mixednutrition because of intolerance to enteral nutrition and 2 cases turned to complete parenteral nutrition in the conventional group.Conclusion The two groups of treatments both can effectively improve the gastrointestinal function, decrease the incidence rate of gastrointestinal function failure, and the efficacy in the acupuncture group was more significant than that in the conventional group;the two groups of treatments both can sustain the gastrointestinal function and nutrition supply, and improve the nutrition state of the patients, and the acupuncture group was superior to the conventional group; the two methods both can effectively reduce APACHEⅡ score and improve the prognosis, while the acupuncture group was superior to the conventional group. Regardingthe death rate and enteral nutrition, the acupuncture group produced a higher comprehensive efficacy than the conventional group.
