1.Paraneoplastic syndromes of hepatocellular carcinoma

Objective To investigate the clinical characteristics, pathological and biochemical featur es of paraneoplastic syndromes (PNS) of hepatocellular carcinoma (HCC). Methods The clinical data of 624 HCC patients (the ratio of male to female 5.3∶1) ident ified by pathology were retrospectively reviewed. Results Among the patients with HCC, 32.2% (201/624) had PNS, the ratio of male to femal e was 4.6∶1. PNS mainly manifested as six clinical abnormalities: hyperchol esterol emia (15.9%), hypoglycemia(8.2%), thrombocytosis(7.7%), leucocytosis(4.8%), eryt hrocytosis(3.4%), hypercalcemia (1.0%). The carcinoma of PNS originated in multi si tes, they mainly located at the right lobe of liver, whilst 47.5% of patients wi th PNS were involved in two lobes. 2.Effects of Qishenhuoxue granule on serum inflammatory reaction in children with pneumonia complicated by sepsis

Objective To study the effect of Qishenhuoxue granule on serum inflammatory response in children with pneumonia complicated with sepsis. Methods A total of 138 children with pneumonia complicated with sepsis who were admitted to our hospital from July 2014 to June 2016 were enrolled in this study. Children were randomly divided into observation group and control group. The patients in both groups were treated with conventional Western medicine symptomatic support. The observation group was treated with Qishenhuoxue granule. The curative effect was determined after treatment. Interleukin-6 (IL-6), interleukin-10 (IL-10), tumour necrosis factor-α (TNF-α), serum procalcitonin (PCT), hypersensitive C-reactive protein (hs-CRP), CD4+CD25+regulatory T cell (Treg cells) ratio value, creatine kinase-MB (CK-MB), creatinine (Scr), aspartic transaminase (AST) and brain natriuretic peptide (BNP) were measured before and 7 days after the treatment. Data of mortality and duration of pediatric intensive care unit (PICU) stay during treatment and adverse events were recorded during treatment. Results The total effective rate was 89.86% in the observation group, which was significantly higher than that of control group (71.01%, P<0.01). The levels of IL-6, IL-10, TNF-α, PCT, hs-CRP and Treg cell ratio were significantly lower after 7-day treatment in two groups than those before treatment (P<0.05), and levels of IL-6, IL-10, TNF-α, PCT, hs-CRP and Treg cell ratio were significantly lower in observation group than those in control group ( P<0.05). The CK-MB, Scr, AST and BNP were reduced after 7-day treatment in two groups, and those data were significantly lower in observation group than those of the control group (P<0.05). The 28-day mortality and the number of PICU days were significantly lower in observation group than those in control group (P<0.05). No adverse reactions were found in the both groups. Conclusion Qishenhuoxue granule can decrease the inflammatory response in children with pneumonia complicated with sepsis and improve the therapeutic effect. It is safe and effective and worthy of clinical recommendation.
3.Construction and identification of Hepatitis B Virus (genotype C) recombinant baculovirus

Objective To construct Hepatitis B Virus (HBV) Chinese-strain wild type and Lamivudine-resistant variant recombinant baculovirus. Methods 1.2-unit length HBV construct (adr serotype, C genotype) was cloned into the multiple cloning region of the pFastBac Ⅰ vector and then transformed into DH10Bac, in which homologous recombination occurred between the pFastBac vector and the inside bacmid with the help of helper plasmid. So HBV recombinant bacmid was produced and transfected into sf9 cells to generate a Recombinant HBV baculovirus called vAcHBVc. The viral titers were detected by immune plaque assay and TCID 50. Lamivudine resistance mutation (YVDD) was introduced by site-directed mutagenesis using continuous PCR. HBV YVDD recombinant baculovirus (vAcHBVcYVDD) was generated and titered as above. Meanwhile, the recombinant HBV baculovirus containing GFP reporter was constructed for evaluating the efficacy of transfection and titration of virus. Results Recombinant wild and YMDD variant HBV baculovirus were successfully developed and the titer was between 10~6~10~8pfu/ml through plaque assay. Conclusion Different kinds of recombinant HBV baculovirus could be generated rapidly and efficiently with Bac to Bac expression system. The developed recombinant HBV baculovirus could be further used in the study of anti-viral resistances.

The abnormal activation of phosphatidylinositol-3-kinase (PI3K)-proiein kinase B (Akt) pathway has already been confirmed to be closely related to the origin and development of various tumors.PI3KAkt pathway plays an important role in development of colon cancer,it can regulate cell survival,proliferation,anti-apoptosis,cell cycle progression,etc.Furthermore,inhibition of this pathway may play a potential roll in oncotherapy.
5.A porous tantalum uncemented acetabular cup in acetabular revision arthroplasty

BACKGROUND: Biological fixation refers to the treatment of coarse or porous prosthetic surface. It is favorable to "bone ingrowth" prosthesis to achieve long-term stability. Porous tantalum is the latest scientific product that appears suitable to prosthetic surface owing to its porous feature and has been attracting a great deal attention.OBJECTIVE: To investigate the therapeutic efficacy of a porous tantalum uncemented acetabular cup in acetabular revision arthroplasty.DESIGN, TIME AND SETTING: A retrospective case analysis was performed at the Department of Orthopedics, Changhai Hospital, Second Military Medical University of Chinese PLA between April and November 2006.PARTICIPANTS/MATERIALS: Sixteen patients (16 hips), 7 males and 9 females, aged 54-81 years old, who received treatment at the Department of Orthopedics, Changhai Hospital, Second Military Medical University of Chinese PLA were included in this study. Of these patients, 2 suffered from femoral neck fracture, 6 from aseptic femoral head necrosis, and 8 from osteoarthritis. Trabecular metal cup (Zimmer, Warsaw, Indiana) provided a titanium alloy bottom layer and porous tantalum-coated surface, which was realized by technical combination of bone trabecular tantalum and titanium alloy. The prosthesis contained an ultrahigh modulus polyethylene lining.METHODS: All patients underwent acetabular revision with modular porous tantalum uncemented acetabular cup. Prior to replacement, acetabular defects and femoral prosthesis were evaluated. A posterolateral approach of hip joint was made. Following acatabular prosthesis removal, bone cement was cleared with osteotome, simultaneously scar tissue, granulation tissue, and fibrous limiting membrane in the acetabular fossa were erased with curette to avoid further damage to sclerotin. Bacteriological examination and susceptibility test with respect to intraarticular tissue fluid confirmed aseptic loosening. Antibiotics were used in each patient to prevent infection. Antibiotics and low molecular heparin were used afterwards to prevent infection and thromboembolism, respectively. Patients could not load within 4 weeks but could walk with the aid of two canes within subsequent 6 weeks. One week after surgery, plain X-ray films were taken in the lateral plane.MAIN OUTCOME MEASURES: Prior to and 6, 12, and 24 months after surgery, Harris hip score and University of California Los Angeles (UCLA) activity score were assessed. The position of acetabular prosthesis was observed through an anteropostedor pelvic radiograph.RESULTS: The average follow-up time was 24 months (range 18-25 months). Harris hip score significantly increased from 32.6 points (range 21-54 points) prior to revision to 87.5 points (range 56-90 points) after revision (P < 0.05). UCLA activity score significantly increased from 3.8 points (2-5 points) prior to revision to 8.2 points (range 6-9 points) after revision (P<0.05). The anteropostedor pelvic radiographs taken in the last follow-up displayed no phenomena regarding non-stability including prosthesis aversion and subsidence and revealed that the in-growth of sclerotin surrounding the bone trabecular tantalum acetabular cup. Postoperative complications were found in none of 16 patients. By the last follow-up, there had been 14 cases who did not complain of pains while walking and 2 cases that had slight pain and slight limp while walking. Deep venous thrombosis or nerve injury did not appear afterwards. No cases needed another acetabular revision arthroplasty.CONCLUSION: For cases of failed primary fixation of artificial acetabular cup, application of porous tantalum uncemented acetabular cup can produces favorable results in acetabular revision if no bone defects exist.
6.Correlation of psychological status of nurses experiencing negative events with coping styles and social support

Objective To study the correlation of psychological status of nurses experiencing negative events with coping styles and social support.Methods The symptom checklist-90 (SCL-90),the simplified coping style questionnaire (SCSQ),the social science research solutions (SSRS) were used for the investigation among 108 nurses who experienced negative events so as to look into the correlation of their psychological status with coping style and social support.Results The total score by SCL-90 was (194.41 ±35.05).The psychological status and positive coping styles and social support were negatively correlated (P <0.05) and positively with negative coping (P <0.01).Conclusions The psychological health is at middle to low level and it is influenced by the coping styles and social support.So the administrative should attach great importance to their mental health status and influencing factors of the positive role,help them gain social supports and enhance their subjective sensation of social support.
7.Clinical research of autologous hematopoietic stem cell transplantation for hematological malignancies and solid tumors

Objective To evaluate the therapeutic effect of autologous hematopoietic stem cell transplantation(AHSCT)on hematological malignancies and solid tumors.Methods Twenty patients with age from 18 to 50 years received AHSCT.Two or 3 of following agents,Cytarabine(Ara-C)3~4 g/m2,Cyclophosphamide(CTX)4~6 g/m2,Etoposide(VP-16)0.5~1.0 g/m2,Semustine(me-CCNU)300 mg/m2,Melphalan(Mel)140 mg/m2,Thiotep a(TSPA)600 mg/m2,Carboplatin(CBP)1.0 g/m2,Busulfan 16mg/kg,were combined as conditioning regimen in all patients.Among them 2 patients with ALL accepted additional total body irradiation(TBI).Results All the patients have reconstituted bone marrow hematopoiesis after transplantation.None of them had the transplantation-related mortality.Among 20 patients with CML and malignant lymphoma,15 achieved disease-free survival(DFS)with a median survival time of 39.5(2~109)months.Conclusion AHSCT might represent an effective approach to the treatment of some patients with chemosensitive solid tumor who are complete remission or part remission.It is suggested that the patients have got a obvious survival benefit from AHSCT.

9.Viral dynamics during entecavtr therapy in patients with chronic hepatitis B

Objective To analyze the viral dynamics and clinical significance during the antiviral treatment by a mathematical model.Methods Six chronic hepatitis B patients were evaluated with a kinetic model(Neumann model)during dose of 0.5 mg/d oral entecavir.Blood samples were drawn for HBV DNA measurement at week 0,2,4,12,24.Non-linear modeling was used to fit individual patient data.Results The median effectiveness in blocking viral production was 99.970%(n=6).The median half-life of viral turn-over was 1.6 d(n=6).The median half-life of infected hepatocytes was 21.3 d(n=5).Compared with the other patients,the c(virions are cleared at a rate)、ε(effectiveness)、δ(infected cell are lost at a rate)value of one patient(eg.6)were all lower and the half-life of virus and infected cells were higher,and eg.6 developed viral break-through after 38 weeks of follow-up.Conclusions Viral load decay showed a biphasic pattern during entecavir therapy which can be described with a mathematical model.The model relates processes of viral infection and replication as well as drug efficacy to model parameters.It indicates the prediction of bio-mathematical model during antiviral treatment.
10.Inhibition of NF-?B activation by pvrrolidine dithiocarbamate increases sensitivity of HL-60 cells to cytotoxic drugs

AIM: To explore whether inhibition of NF-?B by antioxidant pvrrolidine dithiocarbamate (PDTC) sensitizes leukemia cells to cytotoxic drugs and its mechanism. METHODS: The indirect immunofluorescence method and electrophoretic mobility shift assay (EMSA) were used to measure the activation of NF-?B. The apoptotic cells were evaluated by flow cytometry (FCM) and the in vitro growth inhibitory effect was performed using a MTT assay. RESULTS: EMSA showed that NF-?B was activated by daunorubicin (DNR), VP-16 and then was inhibited by PDTC in a dose-dependent manner. NF-?B activation was further verified because of subunit RelA of NF-?B locating in the nuclei. FCM analysis showed that apoptotic index of HL-60 cells was up to (8.97?0.81)%, (16.01?1.06)%, (22.96?1.33)% from (5.34?0.62)%, (10.16?0.42)%, (17.32?1.15)% after exposure of HL-60 cells to 2.5-10 mg/L VP-16 combined with PDTC.
