1.Clinical analysis of prognosis and recurrence of borderline epithelial ovarian tumors in aged women

Objective To evaluate the effect of clinic-pathological variables on prognosis and recurrence of the aged women suffered from borderline epithelial ovarian tumors. Methods The clinical and follow-up data of 102 cases were analyzed retrospectively. Univariate and multivariate logistic regression analysis were used to assess independent prognosis and recurrence factors.Results The postoperative accessory treatment, peritoneal implantation and lymph nodes metastasis could influence the 5-year survival rate(χ2=19.311, 11.054, 18. 559, all P<0.01). The surgery procedure, peritoneal implantation, lymph nodes metastasis and microinvasion could influence the recurrence rate(χ2=23.606, 23.583, 25.774, 23.583, all P<0.01). The logistic regression model showed that the postoperative accessory treatment and peritoneal implantation were the two independent prognosis factors (OR=0.060, 35.319, all P<0.01). The surgery procedure,peritoneal implantation and microinvasion were the three independent factors affecting recurrence(OR =0.001, P<0.05;OR=60.952, P<0.05;OR=88.874, P<0.01). Conclusions The early standard treatment on borderline epithelial ovarian tumors of the aged women have an excellent prognosis, the risk of recurrence remains in some patients. We emphasize that these patients with highrisk factors should be main surveillance population.
2.The Investigation of present situation of Human papilloma virus infection of women cervixes in parts of Gnizhou Province

Objective To investigate the distribution of different Human papilloma virus(HPV) subtype infection of women cervixes in parts of Guizhou Province and the high risk factors of HPV infection,in order to offer some evidences for the use of vaccine of cervix eaneers in the region, and offer a group of reliable data for the screen-ing statistics of cervix eaneer in the world. Methods A total of 2339 women having sexual life were checked for 21 HPV genotypes by DNA flow-through hybridization technique and liquid-hased pap test(LPT) in their exfoliated cer-vical cells. Cervix tissues were taken in some HPV positive women and (or) LPT positive women among them for bi-opsy by the vaginoscope. The high risk factors of HPV infection and the distribution feature of HPV subtype infection in the different cervix diseases were anlayzed. Results The total positive rate of HPV infection was 30.31% (709/ 2339 ) in the 2339 women. There were 20 HPV subtypes to be detected. HPV subtype infection from high to low were HPVI6,52,18,58 and 11. The rates of HPV subtype infection were 9.87% (231/2339) ,5.64% ( 132/2339), 4.95% (116/2339 ) ,4.19% (98/2339)and 2.99 % (70/2339). The peak age of HPV infection ranged from 31 to 35 years old. The earlier the first sexual life,the higher HPV infeetion (tendeney X2 test,P <0.05). 366 CIN pa-tients and 181 patients of invasive cervix carcinoma( ICC)were diagnosed through pathology examination. The rate of HPV infection were 34.26% (74/216)in CIN Ⅰ ,77.78% (56/72)in CIN Ⅱ ,82.05% (64/78)in CIN Ⅲ. Among the 181 ICC,the rate of HPV infection was 84.52% (131/155)in squamous carcinoma of the cervix (SCC) and 38.46% (10/26) in adenocareinoma of uterine cervix (AUC). The infeetion rate of HPV16 was 61.83% ( 81/131 ) in the SCC of HPV positive. The infection rate of HPV18 was 60% (6/10)of the AUC of HPV positive. HPV16 was related closely with SCC ( X2 = 11. 948 ,P < 0.001 , ORSCC/AUC = 5. 946). There was no significant difference be-tween HPV 18 in SCC and AUC( P > 0.05). Conclusions The major HPV subtypes are 16,52,18,58 in Guizhou province. There is a close correlation between HPV infection and cervix diseases. HPV infection often occurs in young and middle-aged women. The first sexual life is a high factor of HPV infection. High risk HPV infecting would cause cervix cancer. CIN and cervix cancers are caused by high risk HPV infection. HPV16 is related closely with SCC. There is no significant difference between HPV18 in SCC and AUC. HPV16/18 vaccine could be considered to re-duce the incidence of cervical carcinoma in Guizhou province.
3.Research of effect of Compound Rehmannia on Transgenic Mice with SAM-P/8 Alzheimer′s Disease

Objective To observe the effect of compound rehmannia on SOD and ATP enzyme in the brain of SAM-P/8 dementia mice and the change of apoptosis neuron cells.Methods SAM-P/8 mice were divided randomly into three groups:experimental group,treatment group and model group.Mice in experimental group were lavaged by compound rehmannia(2ml/day,individual).Mice in treatment group were lavaged by donepezil hydrochloride(400mg/kg,individual),and mice in normal group and model contrast group were lavaged by equal sodium chloride.After 3 weeks,we observed mice′s behavior in every group and investigated the changes of SOD,Na+-K+-ATP enzyme by immunohistochemistry and investigated the changes of apoptosis cells peak by FCM.Results Compound rehmannia could improve learning and memory ability of the SAM-P/8 mice,strengthen SOD,Na+-K+-ATP activity and reduce apoptosis neurons in brain.Conclusion Compound rehmannia can significantly improve the Alzheimer's mice learning and memory ability,strengthen the energy of brain cells,and increase resistance to free radical antioxidant capacity,reduce the neuron apoptosis and has a good relatively protection and care to SAM-P/8 mice brain neurons.
4.Application of informatized blood glucose management system to diabetes patient outside hospital

Objectivde To explore the efficacy of i-GMS applied to the diabetes patient outside the hospital.Methods Totally 80 type 2 diabetes mellitus patients were divided into an experiment group (Group A) and a control group (Group B).Group A executed blood glucose monitoring by Glucose Management App and intelligent blood glucose meter (personal version),and Group B completed monitoring by some portable blood glucose meter.Telephone follow-up was performed once a week,and the two groups were compared on glycated albumin (GA) value 3 weeks after discharging,values of HbA1c,FPG and PG2h 3 months after,the times of blood glucose monitoring and hypoglycaemia as well as the patient satisfaction over the glucose management.Results Group A had the values of GA,HbA1c,FPG and PG2h significantly lower than those of Group B (P< 0.05).The times of glucose monitoring and patient satisfaction in Group A were statistically higher than those of Group B,while the incidence rate of hypoglycaemia of the former was lower than that of the latter (P<0.05).Conclusion I-GMS proves efficient when used for glucose self monitoring and management of the type 2 diabetes mellitus patient.
5.Community-acquired infections and risk factors in patients with diabetes:A Meta-analysis

Objective To analyze the types and risk factors of community-acquired infections (CAI)in diabetic patients by system analysis method of evidence-based medicine.Methods China National Knowledge Infrastructure (CNKI),Wanfang database,VIP database were searched by computer,domestic published researches on CAI and related risk factors in dia-betic patients were aggregated,Meta-analysis was conducted by stata 1 1 .0 software.Results A total of 1 2 literatures were included in the study .The average rate of CAI in diabetic patients was 39.55% (22.12%-55.86%).The major infec-tions were respiratory system infection(40.74%),urinary tract infection(27.35%),tuberculosis(10.80%),skin and soft tissue infection(9.19%),and hepatobiliary system infection (5.57%).Stratified analysis on risk factors revealed that OR and OR95%CI of chronic complication,age,disease course,glycemic control,gender,type of diabetes,subtype of ketoac-idosis was 1.63(1.45,1.82),1.30(1.19,1.42),1.47(1.35,1.61),0.68(0.61,0.76),0.69(0.64,0.75),1.37 (1.13,1.66 )and 0.87(0.62,1.23),respectively.There was no publication bias and combined results were stable. Conclusion The main CAI in diabetic patients are respiratory system infection,urinary tract infection,tuberculo-sis,skin and soft tissue infection,and so on ;several factors,such as female,older age,long-term disease course, poor glycemic control,and complication,can contribute to the increase of CAI in diabetic patients.
6.Difference in periprosthetic acetabular bone mineral density between osteonecrosis and osteoarthritis patients after primary total hip arthroplasty

BACKGROUND:Failure rate is higher in patients with osteonecrosis of femoral head than in osteoarthritis patients after primary total hip arthroplasty, especial y acetabular prosthesis. Bone mineral density around the prosthesis is a key factor for quality of life of the prosthesis. OBJECTIVE:To observe the difference in periprosthetic bone mineral density between patients with osteonecrosis of the femoral head and osteoarthritis patients and the exact location of differences after primary total hip arthroplasty. METHODS:Fifty patients with osteonecrosis of femoral head and fifty osteoarthritis patients were enrol ed in this study. Dual energy X-ray absorptiometry examination was used to compare bone mineral density 5 years after total hip arthroplasty. In accordance with De Lee’s and Charnley’s methods, images of acetabulum were divided. The bone mineral density of different areas of the acetabulum was compared between the two groups. RESULTS AND CONCLUSION:(1) Periprosthetic bone mineral density:Bone mineral density in the lower part and upper part of the acetabulum was significantly lower in the osteonecrosis group than in the osteoarthritis group (P<0.05). No significant difference in bone mineral density in the middle of the acetabulum was detected between the two groups. (2) Results confirmed that the bone mineral density in the lower and the upper parts of the acetabular component was significantly lower in patients with osteonecrosis of femoral head than in patients with osteoarthritis.
7.The Application of Stereotactic Mammography Aspiration Biopsy in the Diagnosis of Breast

Objective To investigate the technique of radio-stereotactic mammography aspiration biopsy(SCNB) and the clinical usage.Methods The stereotactic mammography aspiration biopsy was performed on 38 breast focuses,the results were compared with pathology.The technique and operation skills of SCNB were studied.Results In this 38 breast focuses,the accuracy of diagnostic aspiration,misplay and false negativity were 84.2%,7.89%and 7.89% respectively.No false positive was found.Conclusion In this technique,the distance between the needle tip and focus central was calculated by computer.This is a effective,easy operate and safe tool for the localization and very valuable in the diagnosis of a early cancer.
8.Physical Activity and Vascular Dilation Function in Healthy Middle-aged Individuals

Objective: Vascular dilation dysfunction has been linked with risk of cardiovascular disease. This study was undertaken to investigate the relationship between physical activity and vascular dilation function in healthy middle-aged adults to help explaining the effect of physical activity on preventing cardiovascular disease. Method: We recruited 91 healthy middle-aged adults to complete a serf-report 7-day physical activity recall questionnaire and an exam of brachial artery flow-mediated dilation(FMD) and Nitroglycerin-mediated dilation (NMD) detected by ultrasound. The relationship between physical activity level (PAL) and FMD and NMD were explored. Result: Physical activity showed a significant and positive relationship with the brachial artery FMD, even after adjustment for possible confounding factors (r=0.363, P<0.01). The group of high physical activity level had the highest FMD. The means of FMD (%) in low, moderate and high PAL groups were respectively 8.70%±3.93%,9.01%±3.22% and 12.38±5.67% with significant difference between individuals of low and high PAL and between moderate and high PAL group(P<0.01). The means of NMD (%) is 25.13%±6.52%,24.38%±5.44% and 29.50%±7.25% respectively (P> 0.05) and there was no significant difference among three groups. There was no positive relation between PAL and FMD in premenopausal females but in men and postmenopausal females. Although individuals of high PAL have the best FMD, the moderate PAL can also retard FMD decrease with ageing. Conclusion: Maintaining high physical activity level can enhance endothelial-dependent vascular dilation, and moderate or high physical activity level can prevent endothelial-dependent vaseular dilation declining with aging, which may contribute to decrease risk of cardiovascular disease in healthy middle-aged adults.
9.Evaluation of the effect of intervened measures for health behavior of lead exposed workers.

OBJECTIVETo intervene the health behavior of lead exposed workers by setting down and implementing a series of measures and evaluating the effects of these measures so that the occupational lead poisoning would be decreased.
METHODS105 lead exposed workers in a factory were chosen as subjects. Blood lead concentration and rate of cognition on relevant health knowledge were examined before and after intervened measures.
RESULTSThe cognition rate raised from 56.6% to 97.3% after training. Lead concentration in blood decreased by 17.7% (t = 4.10, P = 0.000) and delta-ALA in urine decreased by 23.9% (t = 4.248, P = 0.000) 12 months after intervention. The prevalence of occupational lead absorption and poisoning also decreased. The number of workers with blood lead content > 2.41 mumol/L was decreased from 6 to 3, and > 1.93 mumol/L from 14 to 10.
CONCLUSIONTo develope occupational health promotion, enhance health education and improve health behavior are the effective way to prevent from occupational lead poisoning.

10.Effect of implementation of essential medicine system in the primary health care institution in China.

Our primary health care institution began to implement national essential medicine system in 2009. In past fi ve years, the goal of national essential medicine system has been initially achieved. For examples, medicine price is steadily reducing, the quality of medical services is improving and residents' satisfaction is substantial increasing every year. However, at the same time, we also found some urgent problems needed to be solved. For examples, the range of national essential medicine is limited, which is difficult to guarantee the quality of essential medication. In addition, how to compensate the primary health care institution is still a question.
