2.Pancreatectomy combined with portal vein/superior mesenteric vein resection: the present condition, controversy and expectation

Pancreatic cancer frequently infiltrates the portal vein system because of the close anatomical proximity between the head of the pancreas and the portal/superior mesenteric vein confluence,and the biological character of invasion. In these cases, pancreatectomy combined with portal vein/superior mesenteric vein resection is a potential treatment, but the outcome remains controversial. So far there is no significant evidence have shown that the pancreatectomy combined with portal vein/superior mesenteric vein resection improves the prognosis of pancreatic cancer patients. In order to establish clinical practice guidelines, as well as standardize operational indication and operational method, it is necessary to conduct large-scale, multicenter, randomized and controlled prospective clinical trials,and collect long-term follow up information.

The yeast one hybrid system is the new type system deriving from the yeast two hybrid system to study the interaction between DNA and protein.The principle and the technical line of this system was briefly introduced,and the progress of this system on the research of plant transcription factor gene resistant to osmotic stress was particularly summarized,such as cloning the transcription factor genes of osmosis resistance type;confirming the interaction between known DNA and protein;making sure the localization of the confirmed DNA binding domain with the interaction;also validating the transcription activation of the transcription factor gene.Furthermore,the problems existing in this system on the research of plant transcription factor gene resistant to osmotic stress was analyzed,and the development prospect according to the research was forecasted.
4.Pathogenic bacteria causing lower respiratory tract infections in the elderly orthopedics patients after general anesthesia and analysis of the risk factors

Objective To explore the incidence rates of lower respiratory tract infections after general anes-thesia in the elderly orthopedics patients and to analyze the related risk factors.Methods A total of 45 patients with lower respiratory tract infections in 479 elderly orthope -dics patients who received general anesthesia were enrolled in the study.The bacterial identification and drug susceptibility testing were preformed after the collection of sputum specimens.The univariate factor analysis and multivariate logistic regression analysis were performed to identify the risk factors for the lower respiratory tract infections after general anesthesia,including the age,gender,smoking histo-ry,chronic respiratory diseases history,basic cardiovascular disease,diabetes,mental disease,and endotracheal tube type,anesthesia ventilation time,postoperative extubation time,and postoperative analgesia.Results Smoking history (Wald =5.058,OR =4.618),chronic respiratory diseases history(Wald =37.950,OR =20.507),mental disease (Wald =26.303,OR =75.644),endotrac heal tube type(Wald =9.035,OR =11.758),anesthesia(Wald =15.035, OR =20.163)and postoperative extubation time(Wald =14.243,OR =12.613)were identified as the risk factors for lower respiratory tract infections after general anesthesia in the elderly orthopedics patients (all P <0.05 ). Conclusion Risk factors should be intervened in elderly orthopedics patients who receive general anesthesia,and the antibiotics should be chosen on the basia of the drug susceptibility testing.
5.Efficacy observation on tuina for children with cough

Objective:To observe the therapeutic effect of ‘Three Steps to Regulate Qi Method’ tuina for infantile cough.
Methods:Fifty-one children with cough were enrolled and received ‘Three Steps to Regulate Qi Method’ tuina, once a day for 5 d. The effects were observed after 5 treatments.
Results:There was no dropout case during the process. Among the 51 infantile patients, 37 cases were cured, 14 cases were improved, and the total effective rate was 100%.
Conclusion:'Three Steps to Regulate Qi Method’ tuina is an effective, simple and convenient method for infatile cough and worth spreading in clinical application.
6.Investigation on Toxoplasma gondii infection in reproductive women in Shandong Province

Objective To investigate the Toxoplasma gondii infection situation and relative factors in reproductive women in Shandong area,so as to provide the evidences for establishing strategies for reducing the birth defects and improving the quality of population. Methods A total of 5 386 reproductive women who visited hospitals at county and city levels in Shandong region from January 2013 to December 2015 were treated as the research objects,and their venous blood samples were collected to de?tect the antibodies(IgG and IgM)against T. Gondii in serum. Meanwhile,the related risk factors of T. gondii infection were sur?veyed by questionnaires. Results Among the 5 386 reproductive women,623 ones were positive for IgG antibody and the posi?tive rate was 11.56%,and 328 ones were positive for IgM antibody and the positive rate was 6.21%. The single factor analysis showed that the pregnant status(χ2 =13.12,P<0.01),whether the residences were in downtowns or counties(χ2 =6.27, P<0.05),whether having animal contact history(χ2=10.46,P<0.01),and whether eating half?baked foods(χ2=21.19, P < 0.01)were related to T. gondii infection. Conclusions The T. gondii infection rate of reproductive women in Shandong Province is high;the related risk factors include pregnant status,residence,degree of intimate contact with animal,and wheth?er eating half?baked foods. It suggests that the detection of T. gondii infection and health education in reproductive women should be strengthened.
7.Effect of dexmedetomidine combined with sufentanil on postoperative analgesia and the vascular crisis in patients undergoing free flap transplantation

Objective To investigate the effect of dexmedetomidine combined with sufentanil on postoperative analgesia and the vascular crisis in patients undergoing free flap transplantation. Methods Forty patients with ASA physical statusⅠ-Ⅱhaving underwent free flap transplantation were divided into groupⅠand groupⅡaccording to the random digits table method with 20 cases each. The drugs of patient controlled intravenous analgesia (PCIA) in groupⅠwere sufentanil 1.0μg/(kg·d)+tropisetron 5 mg/100 ml, and the drugs of PCIA in group Ⅱ were sufentanil 1.0 μg/(kg · d) +dexmedetomidine 1.0 μg/(kg · d) + tropisetron 5 mg/100 ml. The PCIA pump was set in 2 groups as follows: continuous perfusion rate 2 ml/h, single bolus 2 ml, lockout time 15 min. The visual analogue score (VAS), Ramsay sedation score, press number of PCIA pump within 48 h after operation, satisfaction score after operation, untoward reaction and vascular crisis were recorded. Results The VAS at 6, 12, 24 and 48 h after operation in group Ⅱ were significantly lower than those in group Ⅰ: (2.18 ± 0.41) scores vs. (3.00 ± 0.63) scores, (2.64 ± 0.51) scores vs. (3.82 ± 0.60) scores, (2.55 ± 0.52) scores vs. (3.36 ± 0.51) scores and (2.27 ± 0.47) scores vs. (3.09 ± 0.70) scores, the Ramsay sedation scores were significantly higher than those in group Ⅰ: (2.42 ± 0.51) scores vs. (2.00 ± 0.58) scores, (2.74 ± 0.45) scores vs. (2.11 ± 0.57) scores, (2.58 ± 0.51) scores vs. (2.05 ± 0.52) scores and (2.53 ± 0.51) scores vs. (2.00 ± 0.47) scores, and there were statistical differences (P<0.05). The press number of PCIA pump within 48 h after operation in groupⅡwas significantly lower than that in groupⅠ:(3.75 ± 1.45) times vs. (8.25 ± 2.61) times, the satisfaction score after operation was significantly higher than that in groupⅠ:(2.47 ± 0.51) scores vs. (1.60 ± 0.50) scores, and there were statistical differences (P<0.05). There were no statistical differences in the incidences of untoward reaction and vascular crisis (P>0.05). Conclusions Dexmedetomidine can relieve anxiety and nervousness, reduce postoperative consumption of sufentanil, and decrease the incidence of nausea and vomiting. In addition, dexmedetomidine does not affect the incidence of vascular crisis.
8.Influence of preoperative biliary drainage on morbidity rate of severely obstructive jaundice patients after pancreaticoduodenectomy

Objective:To investigate the influence of preoperative biliary drainage (PBD) on morbidity of severely obstructive jaundice patients after pancreaticoduodenectomy (PD).Methods:A total of 98 severely obstructive jaundice(Serum total bilirubin>300 μ mool/L) patients underwent PD between February 2010 and October 2015 were enrolled in the study.The patients were divided into two groups based on undergoing PBD or not.The no-PBD group comprised 52 patients and the PBD group comprised another 46 patients.Perioperatives parameters,including operative time,intraoperative blood loss,postoperative mortality and morbidity and postoperative hospital stay were compared between the two groups.Results:The demographics,preoperative examinations and pathological results were similar between the two groups (P>0.05).Operative time of the no-PBD group was statistically longer than the PBD group (379.44 ± 88.57min vs 346.98 ± 57.17 min,P<0.05).Besides,intraoperative blood loss of the no-PBD group were much more than the PBD group (365.00 ± 187.07mL vs 297.83 ± 139.57 mL,P<0.05).There was no statistical difference of mortality rate between the no-PBD group and the PBD group(3.85% vs 2.17%,P>0.05).The overall morbidity rate of the 2 groups were similar (53.85% vs 43.48%,P>0.05),but the pancreatic fistula rate of no-PBD group was significantly higher than the PBD group (30.77% vs 13.04%,P<0.05).Conclusion:PBD could reduce operative time,intraoperative blood loss and pancreatic fistula rate after PD.Meanwhile,the mortality and overall morbidity rates were similar between the two groups.PBD should be considered for severely obstructive jaundice patients.
9. Expressions of SENP1,SENP2 and SENP6 proteins in human glioma tissue and cells and their mechanisms

Objective: To observe the expressions of SENP1, SENP2 and SENP6 proteins in human malignant glioma tissue and cells∗ and to elucidate the their effects in the development of malignant glioma. Methods: The samples of normal human brain tissue and malignant glioma tissue were obtained and used as normal control group and malignant glioma group, respectively. The Cos7 cells and the malignant glioma LN443 and U343 cells were cultured; the Cos7 cells were used as normal cell control group, and the LN443 and U343 cells as malignant glioma cell group. Western blotting method was used to detect the expression levels of SENP1, SENP2 and SENP6 proteins in human malignant glioma tissue and cells. Results: In brain tissue, the expression levels of SENP1,SENP2 and SENP6 proteins in malignant glioma group were higher than those in normal control group ( P<0. 05). Compared with normal cell control group, the expression levels of SENP1,SENP2 and SENP6 proteins in the LN443 and U343 cells in malignant glioma cell group were significantly increased (P<0. 05). Conclusion: SENP1, SENP2 and SENP6 proteins highly express in the malignant glioma tissue and cells,and they may play an important role in promoting the occurrence of malignant glioma.

Posterior vitreous detachment (PVD), retinal breaks, and lattice degeneration are common problems in ophthalmic clinical practice, which not only cause disturbance to patients' life-quality, but also increase the risk of retinal detachment and vitreoretinal traction.In September 2019, the American Academy of Ophthalmology published Posterior Vitreous Detachment, Retinal Breaks, and Lattice Degeneration Preferred Practice Pattern (PPP). Based on clinical evidence, this PPP provides authoritative guidance for the definition, epidemiological background, diagnosis and treatment of these diseases.This PPP also gives definite solution for treatment and follow-up of different sub-types.This article provides introduction and interpretation of this PPP.
