Objective To identify mutations site and clinical characteristics of a familial hypercholesterolemia(FH) proband diagnosed clinically through DNA sequencing and family analysis in the proband and his family members of 3 generations.Methods Blood samples and clinical data of the kindred of total 29 from 3 generations members were collected.Proband had a physical examination electrocar-diogrom and vascular ultrasound.The proband and his family members took routine clinical exams,and genomic DNA was isolated.The promoter region and the 18 exons of low density liporotein receptor(LDLR) gene were screened by Touch down polymerase chain reaction -single strand conformation polymorphism(PCR-SSCP) and DNA sequencing.The result of sequencing were matched gene sequence published in the BLAST database.Results 1.Increased intima-media thickness and plaque were detected in the common carotid artery,right subclavian artery of the proband.Aortic valve regurgitation was found by echocardiography.2.No mutation R3500Q of ApoB100 was observed.3.Two heterozygous mutations in exon 10 and 13 of LDLR gene (W462X and A606T) were identified.The proband and 5 members of paternal relatives showed W462X heterozygosis mutation in exon 10 of LDLR gene which introduced the change from tryptophone to a new stop codon.The proband's mother and grandmother harboured A606T heterozygous mutation in exon 13 of LDLR gene due to a single base pair substitution of G for A in the codon for residue 1 879.Conclusions Disease causing mutations of proband are W462X and A606T compound heterozygosis mutation in exon 10 and 13 of LDLR gene inherited from mother and father.Proband shows homozyous phenotype though the genotype analysis indicates heterozygous mutations.
5.Effects of 1.8mm coaxial micro incision phacoemulsification on corneal endothelial injury and postoperative visual acuity

AIM:To investigate the effects of 1.8mm coaxial micro incision phacoemulsification on corneal endothelial injury and postoperative visual acuity.METHODS: Totally 145 eyes in 120 patients underwent phacoemulsification from July 2013 to July 2015 were randomly divided into observation group 60 cases (73 eyes) and control group 60 cases (72 eyes).The observation group 60 cases were given 1.8mm coaxial micro incision cataract phacoemulsification operation,while the control group were given traditional 3.2mm coaxial micro incision cataract surgery.The uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),corneal thickness of incision area,incision width,incision length,macular retinal thickness,surgically induced astigmatism,corneal endothelial cell counts and complications of the two groups were compared.RESULTS: The UCVA and BCVA on 1wk after surgery of the observation group were significantly higher than the control group (t=3.604,7.109;P<0.05);the width of incision on 1wk and 1mo after surgery of the observation group were significantly less than the control group (t=205.3,225.2;P<0.05).The length of incision in observation group was significantly greater than the control group (t=3.926,5.009;P<0.05).Macular retinal thickness 1wk after surgery of the observation group was significantly less than the control group (t=2.817,P<0.05).The surgically induced astigmatism was significantly less than the control group (t=19.43,22.16;P<0.01);the difference of corneal edema between the two groups was not significant (8.22% vs 11.11%) (x2=0.348,P>0.05).CONCLUSION: The 1.8mm micro incision phacoemulsification is helpful to improve the visual acuity of patients with cataract phacoemulsification,which may be related to the reduction of corneal cell injury,enhancement of corneal closure and decrease post-operation corneal original astigmatism.
6.Efficacy and safety of intracoronary recombinant human pro-urokinase or Tirofiban injection during primary percutaneous coronary intervention in patients with ST-segment elevation myocardial infarction:a Meta analysis

Objective Intracoronary administration of Tirofiban or recombinant human pro-urokinase(rhPro-UK)during primary percutaneous coronary intervention(PPCI)has been shown to improve coronary artery microcirculation perfusion.This study aims to explore the efficacy and safety of intracoronary rhPro-UK or Tirofiban in patients with ST-segment elevation myocardial infarction(STEMI)undergoing PPCI.Methods The PubMed,Ovid-MEDLINE,Embase,Web of Science,Cochrane Library,clinical trials.gov,CNKI,WANFANG,VIP,SinoMed,databases were searched for randomized controlled trials(RCTs)or cohort researches of the Intracoronary injection of rhPro-UK or Tirofiban during PPCI in STEMI patients from inception to 27th November 2023.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.Meta-analysis of data was performed by RevMan 5.4.Results There were 12 studies involving 1 721 patients were included,with 864 in the rhPro-UK group(experimental group)and 857 in the Tirofiban group(control group).Meta-analysis showed that compared to the control group,intracoronary rhPro-UK significantly improved postoperative myocardial blood flow perfusion,including higher proportions of thrombolysis in myocardial infarction(TIMI)grade Ⅲ and TIMI myocardial perfusion grades(TMPG)grade Ⅲ(OR 1.83,95%CI 1.36-2.46,P<0.0001;OR 2.38.95%CI 1.22-4.66,P=0.010);And significantly lower corrected T1M1 frame count(cTFC)(MD-2.43,95%CI-2.94--1.92,P<0.00001);ST-segment resolution(STR)≥70%was higher than that of control group(OR 1.93,95%CI 1.01-3.68,P=0.050).There was a significant reduction in major adverse cardiovascular events(MACE)one month postoperatively(OR 0.54,95%CI 0.35-0.82,P=0.004).Improving cardiac function one month after surgery,higher left ventricular ejection fraction(LVEF)(MD 1.71,95%CI 0.04-3.38,P=0.050).Reducing the size of the myocardial infarction,the postoperative peak value of CK-MB was lower than that of control group(MD-74.16,95%CI-83.59--64.74,P<0.00001).The incidence of bleeding event was not significantly different between groups(OR 1.18,95%CI 0.64-2.16,P=0.600).Conclusions In STEMI patients undergoing PPCI,intracoronary rhPro-UK improves coronary artery microcirculation perfusion and cardiac function more effectively than Tirofiban,reducing the size of the myocardial infarction and the occurrence of MACE after PPCI 1 month,and there was no significant difference in bleeding events between the two groups.
7.Predictors for abundance of Rattus tanezumi in households of commensal rodent plague foci

Objective To explore the predictors on the abundance of Rattus (R.) tanezumi in households of commensal rodent plague foci.Methods Thirty natural villages that experienced previous plague cases in Lianghe county,Yunnan province,were selected followed by random selection of 20 households in each village through computer technique.Live traps were set in households to capture small mammals which were then identified to species in the field according to their morphological features.Data on potential factors for abundance of R.tanezumi were collected through questionnaires and field observation and were coded and computerized using EpiData software and further analyzed by hurdle regression model under R software.Results A total of 166 rodents (133 R.tanezumi and 33 Suucus murinus) were captured.Results from final multilevel hurdle regression model showed that the likelihood of R.tanezumi captures increased by 1.67-to 2.76-fold in households belonged to Dai ethnic families that stored foodstuff in metal pails,often raising dogs,and having adjacent houses.The number of R.tanezumi captures increased by 2.18-fold in the villages where over 80% of the households would raise chickens.In the villages with communal latrine,the likelihood and the number of R.tanezumi capture increased 1.93-fold and 2.38-fold,respectively.While the likelihood of R.tanezumi captures would reduce by 45%-61% in those households where there were cats and cattle being raised and maize grown in the village.The number of R.tanezumi captures would reduce by 63% in the households where there were outside toilets.Conclusion The abundance of R.tanczumi seemed to be closely related to the ecological environment factors.Programs on plague control and prevention should relate to ecological factors that influencing the abundance of R.tanezumi.
8.Detection of W462X Mutation in Low Density Lipoprotein Receptor Gene of A Familial Hypercholesterolemia Patient and Its Clinical Significance

Objective To explore the molecular basis of familial hypercholesteraemia(FH)by analyzing the phenotype and genotype relationship through identify the low density liporotein receptor(LDL-r)gene mutation in a FH kindred.Methods A male patient of 15 years old was selected to examine the electrocardiogram,lipid.Color Doppler was used to examine heart and great vessels.The promoter region and the 18 exons of the LDL-r gene were screened by touch-down polymerase chain reaction(PCR)and DNA sequencing.Results The caro-tid intima-media thickness(IMT)was increased to 0.23 cm,while coronary flow velocity reserve(CFVR)was decreased to 1.57,and mode-rate mitral regurgitation was found in the proband.The genetic alteration G→A change at 1 448 of exon 10 causing premature stop codon(W462X).The same heterozygous nonsense mutation was also found in his father.The mutation had been reported in other Chinese patients.In vitro experiments showed that W462X mutation leads to low LDL binding and internalization ability.Conclusions The homozygous mutation(W462X)in exon 10 of the LDL-r gene were identified in the clinically heterozygous FH proband.The W462X mutation is the underl-ying cause of hypercholesterolaemia and clinical AS manifestations.W462X is recurrent mutation among Chinese FH patients.It might be a hot spot mutation in LDL-r in Chinese FH.J Appl Clin Pediatr,2009,24(1):18-20
9.Expression of cytokine IL-1α and S100β in different types of plaques in Alzheimer's disease.

OBJECTIVETo study the significance of cytokine IL-1α and S100β expression in formation and evolution of different types of plaques in Alzheimer's disease.
METHODSThirty-four autopsy cases of Alzheimer's disease encountered during the period from 1982 to 2008 were retrieved from the archival files of Department of Pathology, Beijing Hospital. Tissue blocks were taken from hippocampus for dual immunostaining for IL-1α/Aβ and S100β/Aβ.
RESULTSImmunohistochemical studied for IL-1α/Aβ and S100β/Aβ delineated four different types of senile plaques: diffuse non-neuritic plaques, diffuse neuritic plaques, dense-core neuritic plaques and dense-core non-neuritic plaques. The numbers of IL-1α-positive microglias and S100β-positive astrocytes associated with diffuse neuritic plaques were (7.29 ± 3.04) per mm(2) and (6.49 ± 2.20) per mm(2), respectively. In contrast, the numbers of IL-1α-positive microglias and S100β-positive astrocytes associated with diffuse non-neuritic plaques, dense-core neuritic plaques and dense-core non-neuritic plaques were (3.24 ± 1.53) per mm(2) and (4.14 ± 1.77) per mm(2), (2.09 ± 1.37) per mm(2) and (2.25 ± 0.83) per mm(2), and (1.38 ± 0.90) per mm(2) and (0.58 ± 0.36) per mm(2), respectively. The numbers of IL-1α-positive microglias and S100β-positive astrocytes associated with diffuse neuritic plaques were significantly higher than those of the other three types of plaques (P < 0.05).
CONCLUSIONThe IL-1α-positive microglias and S100β-positive astrocytes may be of certain significance in transformation of diffuse non-neuritic plaques to diffuse neuritic plaques in Alzheimer's disease.

10.The protective mechanism of N-acetylcysteine against ischemia/reperfusion induced gastric injury in rats.

The present study aimed to investigate the protective mechanism of N-acetylcysteine (NAC) against gastric ischemia /reperfusion (GI/R) injury in rats. After intravenous injection (IV) of NAC (150 mg/kg) into femoral vein, the rats were subjected to 30 min of ischemia induced by clamping the celiac artery followed by 60 min of reperfusion. After the gastric mucosal damage index (GMDI) had been calculated, gastric mucosal cell in situ apoptosis was detected by TUNEL method. The protein expression of p-ERK, p-JNK and NF-kappaB, and mRNA expression of TNF-alpha and Caspase-3 in gastric mucosa were evaluated by using Western-blot or RT-PCR, respectively. The results showed that NAC not only attenuated the GI-R injury, but also decreased gastric mucosal cellular apoptosis. Furthermore, NAC increased the protein expression of p-ERK, while inhibited protein expression of p-JNK, NF-kappaB in gastric mucosa. NAC also decreased the expression of TNF-alpha mRNA and Caspase-3 mRNA in gastric mucosa. Capsazepine (CPZ) (400 mg/kg, IV) reversed the protective effect of NAC against GI/R injury in rats. These results suggest that NAC can protect rats against GI/R injury. This protective effect is possibly mediated by the up-regulation of p-ERK and down-regulation of p-JNK and NF-kappaB. In addition, vanilloid receptor subtype 1 may be involved in the protective mechanism of NAC against GI/R injury.
