1.Effects of different head positions during operation on wake-up time and quality among ankylosing spondylitis patients with thoracolumbar kyphosis following pedicle subtraction osteotomy

Objective To investigate the effects of different head positions during operation on wake-up time and quality among ankylosing spondylitis (AS) patients with thoracolumbar kyphosis following pedicle subtraction osteotomy (PSO).Methods Sixty patients were included in this study.All of them,treated with intravenous anesthesia,took the bow-prone position.The patients were randomly divided into two groups:the experiment group where the head was elevated by 10° and the control group where the head was not elevated.The two groups were compared in respect of wake-up time and quality of wake-up test.Results The wake-up time in the experiment group was significantly shorter as compared to that of the control group (1 3.7±2.0 min vs.24.2±2.7min,P<0.05).The wake-up quality was better as well.Conclusion Elevation of the head by 10° can shorten wake-up time and improve the quality of wake-up test during the procedure of PSO for AS patients with thoracolumbar kyphosis.
2.Spectral-domain OCT features of macular edema induced by central retinal vein occlusion

Background Cystoid macular edema(CME) is an important cause of visual impairment of central retinal vein occlusion(CRVO).Spectral-Domain optical coherence tomography(SD-OCT) has increased speed and higher resolution,offering a better chance of understanding the morphological changes and pathogenesis of CME. Objective This study was to survey the morphologic features of macular edema associated with CRVO by SD-OCT. Methods Clinical data of the patients with CRVO diagnosed in Department of Ophthalmology,Peking Union Medical College Hospital from March 2008 to August 2009 were retrospectively analyzed.SD-OCT features of macular edema induced by CRVO were analyzed and recorded.Results The average macular foveal thickness was(527.5±218.2) μm in macular edemas eyes.Main morphological changes included 55 cases(84.6%) of CME,15 cases of(23.1%) serous macular detachment(SMD),and 10 cases(15.4%) of simple macular edema,and these findings occurred at the same time in some eyes.Cystoid spaces in the parafoveal region were seen in the inner nuclear layer,outer plexiform layer and outer nuclear layer,and discontinuous or weak inner segment/outer segment(IS/OS) line was often seen in CME.The incidence of CME associated with incomplete posterior vitreous detachment(PVD) was 14.5%,and that of neural epithelial edema associated with incomplete PVD was 10.0%,showing an insignificant difference between them(χ2=0.000,P=1.000).The average area of SMD was 1838.4μm ×1428.1μm×190.1μm,and the incidence of partial PVD was higher(χ2=4.266,P=0.039).Conclusion SD-OCT can reveal the micro-morphological change of macular zone in macular edema eye.SD-OCT enabled visualization of its spatial extent in each retinal layer and the condition of IS/OS layer.Serous macular edema is related with partial PVD.

4.Analgesic effects of intraarticular cocktail versus intravenous parecoxib injection after total hip arthroplasty

BACKGROUND:Intraarticular cocktail analgesic injection is a popular postoperative analgesia method and can effectively control postoperative pain and relieve side effects after total hip arthroplasty.
OBJECTIVE:To compare and assess the effectiveness and safety of intraarticular analgesic injection or intravenous injection of parecoxib after total hip arthroplasty.
METHODS:A total of 60 patients undergoing total hip arthroplasty were randomly assigned to:treatment group (intraarticular cocktail analgesic injection with morphine, bupivacaine, and compound betamethasone), and control group (intravenous injection of parecoxib). Al patients received tramadol hydrochloride at 24 hours after replacement. Analgesic consumption, visual analog scale at rest and during activity, range of motion, and postoperative complication of patients in each group were recorded.
RESULTS AND CONCLUSION:Intraarticular cocktail analgesic injection significantly reduced analgesic consumption. When comparing visual analog scale scores, rest pain scores were significantly less in the treatment group at 12, 24 and 48 hours after replacement than that in the control group (P<0.05). Scores on range of motion were significantly less in the treatment group at 24 and 36 hours than that in the control group (P<0.05). No significant differences in total complications were detectable between the treatment and control groups (P>0.05). Results suggested that intraarticular cocktail analgesic injection lessened analgesic consumption after replacement, relieved early pain after replacement, and contributed to early rehabilitation of patients. Moreover, no significant adverse reactions were visible.
5.The relationship between the gastrointestinal myoelectrical activity and motilin and ursodeoxycholic acid in fasting rats

Objective To investigate the different characteristics of gastrointestinal migrating myoelectrical complex (MMC) of different origin in fasting state and the effects of motilin (MTL) and ursodeoxycholic acid (UDCA) on the MMC of the gastrointestinal tract of different origin in rats. Methods Three bipolar silver electrodes were chronically implanted on the antrum, duodenum and jejunum. Seven days later twenty-four experimental rats were divided into two groups. One group were injected with porcine MTL via sublingual vein, while the other group was perfused with UDCA into the stomach. The gastrointestinal myoelectrical activity was recorded one hour before and two hours after the test substances infusions on these conscious fasting rats. Results Among the total sixty-eight MMCs recorded in fasting rats under control, 67% started in the duodenum, and 33% started in the antrum. The MMC cycle duration and duration of phase Ⅲ of antral origin were longer than those of duodenal origin. Administration of porcine MTL could induce a premature antral phase Ⅲ of antral origin. However, perfusion into the stomach with UDCA resulted in a shorter MMC cycle duration and longer duration of phase Ⅲ of duodenal origin. Conclusion In fasting rats, MMC may originate from the antrum and duodenum, respectively. The different characteristics of MMC of different origin may contribute to the large variations within subjects. Pocrine MTL and UDCA can affect the MMC of different origin of the gastrointestinal tract in fasting state.
6.Processing Technology Research of Fermentation and Purification of SUMO Protease UlP1

Nowadays,small peptides are always expressed in the form of fusion protein.The expression product contains many superfluous amino acids which can affect the biological functions of small peptides even expressed by GST fusion protein expression system.SUMO protease can cut SUMO fusion protein expressed by fusion expression system without any amino acid residues left on target protein thus become a hot topic in this field.Recombinant His-UlP1/pET3c/BL21(DE3)engineering strain was constructed by genetic engineering technology and the expression conditions were optimized in shake flaks.The process of high density fermentation was explored and different purification conditions were detected by chromatography.The results showed that SUMO protease could be expressed well after inducing the engineering strain by IPTG of 1.0mmol/L at 30℃ for 6 hours.The expression level of the strain in fermentation pots could reach 24.3% analyzed by SDS-PAGE.The purity of SUMO protease was more than 98% after further purification by cation exchange chromatography.The yield was 355mg SUMO protease per liter fermentation liquid.Western blot analysis demonstrated that there were immune reactions between IlP1 and 6?His antibodies,so it has established a good foundation for large-scale industralazation in the future.

9.Expression of survivin and caspase-3 during the development process in rat cochlea.

OBJECTIVETo explore the role of some apoptosis regulators during the development in rat cochlea.
METHODSThe morphological development process of cochlea was observed in Wistar rat aged between embryo day 13 to postnatal day 14 in this experiment. Survivin and caspase-3 were respectively detected at protein and mRNA levels by immunohistochemistry and reverse transcription polymerase chain reaction (RT-PCR).
RESULTSThe expression of survivin and caspase-3 located in the bottom wall of the cochlear duct. Not only they widespread in the cell proliferation, but also they gradually enhanced in the cell differentiation. Both of them had a crest-time, and survivin was prior to caspase-3. In organ of corti during adult time, caspase-3 was not present and survivin was only expressed.
CONCLUSIONSDuring the development of the rat cochlea, both of them had similar location and trend. But they were derangement. This showed that both of them participated in the cochlear morphological development. It was suggested that the interaction between survivin and caspase-3 regulated the apoptosis, promoted the cochlear morphological development.

10.Expressions of neurotrophin factor receptor in spiral ganglion cell of cisplatin-induced ototoxicity.

OBJECTIVETo investigate the effects of high-affinity tyrosine kinase receptors TrkB, TrkC and the low-affinity neurotrophin receptor p75 in spiral ganglion cell (SGC) of cisplatin-induced ototoxicity.
METHODSThe 50 adult Wistar rats were divided randomly into 5 groups received intraperitoneal injection of cisplatin with vary dose. Control group was received equivalent volumes of saline. The group received 1 day intraperitoneal injection was cisplatin treated at a dose of 5 mg/kg and killed at next day. The group received 3 days was cisplatin treated for 3 days at same dose daily and then killed at next day. The group A received 5 days was cisplatin treated for 5 days and killed at next day. The group B received 5 days was cisplatin treated for 5 days and then were sacrificed after 7 days. The change of mRNA level of neurotrophin receptors in cochlear tissue were examined by RT-PCR. The expressing pattern of TrkB, TrkC, P75 in damaged cochlea were study by immunochemistry using antibodies against TrkB, TrkC, P75 protein.
RESULTSThe research data showed the expression of Trk B, Trk C, p75 exhibited in SGC was dynamic along with the administration lasting. The mRNA and protein level of Trk B (x(-) +/- s) at day 1 and 3 after cisplatin treatment were 0.76 +/- 0.06, 88.78 +/- 4.28, 0.82 +/- 0.09 and 91.64 +/- 4.06, with significant difference among those and other groups (P < 0.05). The mRNA and protein level of TrkC at day 1 after cisplatin treatment were 0.80 +/- 0.06 and 89.66 +/- 2.76, with significant difference among that and other groups (P < 0.05). The mRNA and protein level of p75 at the control group and cisplatin treated groups were 0.64 +/- 0.04, 55.16 +/- 3.10, 0.77 +/- 0.04, 78.46 +/- 3.86, 1.01 +/- 0.09, 105.02 +/- 6.61, 1.18 +/- 0.09, 111.10 +/- 6.08, 0.51 +/- 0.04 and 42.74 +/- 5.20, with significant difference among the control group and cisplatin treated groups (P < 0.05).
CONCLUSIONSThe expression of Trk B increased to peak at day 1 - 3 after cisplatin treatment and decreased at day 5 early and following weeks. The expression of Trk C went up to peak at day 1 after cisplatin treatment and went down during subsequently time. P75 kept a trend of continuance increased during the drug treatment and decrease at drug stopped. The expression of Trk B, Trk C and P75 may be involved in cochlear insult with cisplatin-induced. Trk B and Trk C may play an important role in the reparative process of cochlear, especially at early stage of the damage. P75 could promote SGC apoptosis in cisplatin-induced neurotoxicity.
