1.China's social non-profit medical institutions:Motivations, challenges and countermeasures

Social forces participating in non-profit medical institutions will help to insist on the commonweal of public medical and health care and realize the goal of access to basic medical and health services for everyone .At present , social forces participating in non-profit medical institutions have made some prominent improvements under a series of policy support .However , due to the absolute dominance of public hospitals , social forces participating in non-profit medical institutions are still potting a lot of restrictions on access threshold of basic medical services , devel-opment space and policy implementation .In order to promote the healthy development of social forces participating in non-profit medical institutions , we should first recognize its significance and guarantee its development potential , tal-ents construction and development space through implementing preferential policies , improving supervision mecha-nisms, exploring multi-sited license, optimizing allocation of resources and so on .
2.Retrospective analysis on neuroradiological features of central neurocytoma

Objective To evaluate the neuroradiological features of intraventricular central neurocytoma in ten patients.Methods Ten patients with central neurocytoma were imaged using contrast-enhanced and non-enhanced MRI scans,including 4 were also scanned with non-enhanced CT.The imaging data were studied with regard to location,size,margin,signal intensity,enhancement characteristics and presence of calcifications.Clinical data(i.e.presenting signs and symptoms,physical findings and medical history) were collected and histopathological and immunohistochemical studies were performed.Results There were 4 male,6 female in our group.All lesions were located in the lateral ventricles.Three tumors were confined to the left side,three to the right side and four involved both lateral ventricles.The growth of central neurocytoma was of close spatial relation to the septum pellucidum.In MRI,most of the cases showed a heterogeneous hypointensity on T1WI and hyperintensity on T2WI or FLAIR with a well-defined margin.The presence of cystic components,necroses and calcifications had been the cause of these internal heterogeneities.After intravenous administration of gadolinium(Gd-DTPA) all tumors showed a heterogeneous enhancement.CT provided additional information by distinguishing intratumoral calcifications in all four evaluated cases.Conclusion Central neurocytoma should be considered when young patients with lesions in the lateral ventricle,which contain calcifications and show some enhancement.This is especially applicable for tumors involving both lateral ventricles with symmetrical growth around the center of septum pellucidum or for unilateral ventricular tumors with a wide base attachment to the septum pellucidum.
3.Assay of Hydrochlorothiazide Tablets by RP-HPLC

OBJECTIVE:To establish a RP-HPLC method for determination of the content of hydrochlorothiazide tablets.METHODS:Samples were determined by RP-HPLC on a ZORBAX SB-C 18 column with a mixture of phosphate buffer solution(pH=4.0)-methanol(80∶20)as mobile phase and UV detection at226nm.RESULTS:The calibration curves of hydrochlorothiazide was linear from0.006to0.303mg/ml(r=1.0000,n=5),the average recovery and relative standard deviation were100.1%and0.43%(n=9)respectively.CONCLUSION:The method is rapid,simple,accurate and precise.It can be used for assay of hydrochlorothiazide.
4.The relationship between homocysteine and cerebral infaction in patients with type 2 diabetes mellitus

Objective To explore the correlation between serum homocysteine (Hcy) level and diabetes mellitus combined with cerebral infarction (DMCI) in patients. Methods A total of 144 patients with type 2 diabetes mellitus (T2DM) were selected and divided into two groups, patients without cerebral infaction (group A, n=64) and patients with cerebral infaction (group B, n=80). Thirty healthy people were used as control group (group C). The serum Hcy level was detected by enzymatic cycling assay in three groups. The serum levels of cholesterol (TC) and triglyceride cholesterol (TG) were detected by enzymatic determination. The serum levels of high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were detected by homogeneous phase method. The serum level of creatinine (Cr) was detected by creatine oxidase method. The level of uric acid (UA) was detected by urinary enzyme peroxidase coupling method. Glycosylated hemoglobin (HbA1c) was detected by high performance liquid chromatography. The carotid artery intima media thickness (IMT) was examined by color Doppler ultrasound, and patients were divided into mild and no stenosis group, moderate stenosis group and severe stenosis group according to the
results. The levels of Hcy were compared between all groups. The correlation of Hcy level and other indicators was analyzed. Results The levels of Hcy and HbA1c were group C

6.Chromatographic fingerprints of Andrographis paniculata (Burm.f.) Nees.collected from different places

AIM: To study the HPLC fingerprints and establish a sensitive and specific method for the quality control of Andrographis paniculata(Burm.f.) Nees. METHODS: All 23 samples of Andrographis paniculata(Burm.f.) Nees collected from 10 different places were determined by RP-HPLC.Shimadzu-ODS column was used with maxtures of 0.1% formic acid-acetonitrile and 0.2% formic acid-water as mobile phase in gradient mode.The flow rate was set at 1.0 mL/min.The column temperature was set at 25℃ and detecting wavelength at 254 nm.Hierarchical clustering,nonlinear mapping and similarity criteria were applied to evaluate the fingerprints. RESULTS: The simple and specific method with good repeatability was established.There were different contents of each component contained in habitat samples produced in individual area. CONCLUSION: The proceeding is suitable to differentiate Andrographis paniculata(Burm.f.) Nees from different places conveniently and can be used for the quality control of this herb.
7.Quality standard of Siwei Jianghuangtang Powder

AIM:To establish the quality standard for Siwei Jianghuangtang Powder(Rhizoma Curcumae longae,Cortex Berberidis,Fructus Phyllanthi,Fructus Tribuli). METHODS: Rhizoma Curcumae longae,Cortex Berberidis,Fructus Phyllanthi,Fructus Tribuli were identified by TLC,and the content of curcumin was determined by HPLC. RESULTS: The TLC for the identification of Rhizoma curcumae longae,Cortex Berberidis,Fructus Phyllanthi,Fructus Tribuli in the powder was accurate.While the content of curcumin was determined by HPLC,curcumin showed good linear relationship at a range of 0.022 4-0.179 ?g,r=0.999 7.The average recovery of curcumin was 99.4%(n=9) and RSD was 1.46% by HPLC. CONCLUSION: The method is specific,reliable and accurate.It can be used for the quality control of Siwei Jianghuangtang Powder effectively.
8.Recent advances in microglia activation and intervention mechanism in diabetic retinopathy

Diabetic retinopathy (DR) is one of the most significant microvascular complications of diabetes,and its incidence increases with the prolongation of DM;meanwhile,it can lead to severe vision loss.Recently,the pathogenesis of DR has been one of the focuses both at home and abroad,but the basic mechanisms are not fully elucidated.More and more scholars believe that DR may be a chronic and low degree of inflammatory response.Moreover,it is well established that microglia is a monocyte-macrophage,with antigen-presenting function.Especially in the retina,as microglia is activated,followed by the release of a large number of inflammatory factors,microglia involves in DR inflammatory response.Understanding the activation mechanism of microglia can provide a new perspective for the prevention and treatment of DR.Therefore,recent advances in activation of microglia in diabetic retinopathy and its intervention mechanism will be reviewed in the article.
9.Propofol intervention on mesenchymal stem cell transplantation on brain injury in rats between Human amniotic

Objective Human amniotic mesenchymal stem cells (AM-MSCs) transplantation and to investigate the effect of propofol treatment, recovery of brain injury in rats. Methods 80 SD rats, male and female, weight 300-350 g. Human amniotic mesenchymal stem cells cultured in vitro and recovery (CM-Dil) before transplantation standby. The fluid percussion brain injury device, give 2.5-3.0 ATM hydraulic impact, severe fluid percussion brain injury model was made. After injury 6h give corresponding treatment which were randomly divided into 4 groups: injury group (medium injection group), AM-MSCs transplantation group and propofol group, AM-MSCs + propofol group. The number of positive cells in each group was observed under the fluorescence microscope using CM-Dil markers. 4 weeks after transplantation, 6 rats were randomly executed, and RT-PCR and Blot Western were used to detect the changes of GAP-43, AQP4 gene expression and protein synthesis in brain tissues. After transplantation, 24 h, 3 days and 1, 2, 3, 4 weeks for animal neurological deficit score, in 4 weeks after transplantation of Morris water maze (Morris Water Maze, MWM) test. They were killed after four weeks by immunohistochemistry, HE staining. Results Number of CM-DIL positive cell was the most in AM-MSCs + propofol group, which was in propofol group and AM-MSCs transplantation group were lower than that in the AM-MSCs + propofol group, which in injury group was the least, and the differences among the groups had statistically significant (P<0.05). 4 weeks after transplantation, AQP4 and its mRNA expression of brain damage surrounding tissue in injury group were higher than those in AM-MSCs transplantation group and propofol group; which in AM-MSCs transplantation group and propofol group was higher than those in AM-MSCs + propofol group (P< 0.05); GAP-43 protein and mRNA expression in injury group were lower than those in AM-MSCs transplantation group and propofol group, which in AM-MSCs transplantation group and propofol group were lower than those in AM-MSCs + propofol group (P<0.05). 1 weeks after transplantation, rat nerve dysfunction score in AM-MSCs + propofol group was lower than that in propofol group and AM-MSCs transplantation group; which in propofol group and AM-MSCs transplantation group was lower than that in the control group (P<0.05). The times of crossing platform in AM-MSCs + propofol group was higher than that in propofol group and AM-MSCs transplantation group, significantly higher than that in the injury group (P<0.01). The pathological section was observed 4 weeks after injury, there was no axonal passage in the injury group. In propofol group and AM-MSCs transplantation group, a few axonal like structures were observed, more axonal like structures were observed in AM-MSCs + propofol group. Conclusion Propofol combined with human amniotic mesenchymal stem cells transplantation can significantly improve the neurological function of rats with brain injury.
10.Evaluation of Intervention Effects of Standardized Pharmaceutical Care on Clinical Treatment of AECO-PD Patients

OBJECTIVE:To evaluate the intervention effects of standardized pharmaceutical care on clinical treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients. METHODS:A total of 127 AECOPD patients were col-lected from respiratory medicine department of our hospital during Sept. 2014-Mar. 2015,and then divided into control group(63 cases)and observation group(64 cases)according to random number table. Control group only received traditional medical service and drug therapy. Observation group was additionally given standardized pharmaceutical care provided by clinical pharmacists ac-cording to AECOPD clinical pharmacy pathway on the basis of control group. The patients were followed up 1 month after dis-charge. The use of antibiotics during hospitalization,medication adherence,the occurrence of ADE,patient satisfaction and medica-tion bias during hospitalization and after discharge,the rates of ADR awareness and return visit after discharge were compared be-tween 2 groups. RESULTS:In both groups,1 patient withdrew from the study during hospitalitation;33 patients didn't complete return visit after discharge. During hospitalization,the rate of medication adherence and the total satisfaction of traditional medical procedures and medication guidance in observation group were significantly higher than control group;the utilization ratio of antibi-otics and combined use of antibiotics,AUD,the incidence of ADE,the proportion of medication bias were significantly lower than control group,with statistical significance(P<0.05). After discharge,the rate of ADR awareness and the total satisfaction of medi-cation guidance in observation group were significantly higher than control group,and the incidence of ADR was significantly low-er than control group,with statistical significance(P<0.05). There was no statistical significance in the rate of medication compli-ance,the rate of return visit,the proportion of medication bias between 2 groups after discharge(P>0.05). CONCLUSIONS:The standardized pharmaceutical care can significantly improve medication compliance and satisfaction of inpatients,contribute to ratio-nal use of antibiotics,reduce the incidence of ADE and medication bias,but have no significant effect on medication compliance, the rate of return visit and medication bias of discharged patients.
