1.Anti-adhesion effect of absorbable biomaterials during tendon reconstruction

BACKGROUND:After sports tendon injury, tendon adhesion is the main reason for the failure to repair tendon injury. So, an ideal anti-adhesion material plays an important role in the tendon reconstruction.
OBGECTIVE:To analyze the clinical effect and features of absorbable medical biomaterials in inhibiting tendon adhesion after repair of sports tendon injury.
METHODS:CNKI and Pub Med database were retrieved by the first author using computer to search relevant articles about meniscal repair and tissue engineering technology published from 1988 to 2015. The key words were “tendon repair, tendon adhesion, absorbable biomaterials” in Chinese and English, respectively.
RESULTS AND CONCLUSION:Basic and clinical studies on absorbable anti-adhesion materials have achieved remarkable results. Currently absorbable anti-adhesion materials include hyaluronic acid, polylactic acid, colagen and fibrin glue, which have good biocompatibility and can be absorbed and degradedin vivo. With simple clinical implementation, these materials can completely cover the wound surface. The main mechanisms are as folows: reducing the generation of colagen fibers, isolation barrier, accelerating hemostasis, antisepsis and anti-inflammation, thereby effectively reducing reconstructed tendon adhesion and promoting tendon healing. But they have their limitations: hyaluronic acid has short acting durationin vivoand immunogenicity; polylactic acid eventualy is decomposed into smal molecules that cancause aseptic inflammation; colagen and fibrin glue both have certain shortcomings in biocompatibility and activity.

4.Application of Early Enteral Nutrition on Treatment of Severe Acute Pancreatitis

Objective To discuss the feasibility and safety of early enteral nutrition (EN) on treatment of severe acute pancreatitis (SAP) and its influence. Methods The advancement about application of early EN on treatment of SAP in recent years were reviewed. Results In patients with SAP, early EN via catheter placed in the jejunum could protect the integrity of intestinal barrier and reduce infectious complications. But no consensus had been reached about the starting time, ingredient and infusion mode yet. Conclusion Early EN may have positive effects on treatment of SAP, but further researches are still needed.
5.Open Men iscal Repair of the Knee Joint

The meniscus has improtant functions of shock absorption, load transmission, lubrication and significant contribution to stability of the knee. Accordingly, repair rather than excision can be expectd to be satisfactory and functional teatment for knee joint. We had Repaired 56 cases of meniscal tears from March, 1987 to September of 1992, among which 42 cases were followed up for at least 12 months to 60 months at most(in average 36 months). The results were as follows; 1) In medial meniscal tears, 26 cases were acute, while five were chronic. As for the lateral meniscus, acute and chronic teats were observed in nine and two cases, respectively. 2) Eleven of the 35 acute tears were associated with anterior cruciate ligament tears, while three of seven chronic tears were associated with anterior cruciate ligament tears. 3) Second-look arthroscopic examinations were performed on nine repaired cases. Retears occurred in two cases and seven cases revealed healing at repair sites. 4) Two cases(5%) out of 42 repaired menisci were return during follow-up. One was a case of isolated meniscal tear and the other was associated with rupture of anterior cruciate ligament. 5) Two return cases were repaired in the chronic stage of more than one year after trauma. 6) The standing radiographs which were taken in full extension and 45 flexion on eight out of 13 patients, who had been followed up for more than 4 years showed intact joint space without narrowing.

6.The Effect of Histoacryl(R) for Gastric Variceal Bleeding Which Failed by Ethanolamine Oleate Injection Sclerotherapy.

Treatment modalities of gastric variceal bleeding are endoscopic injection sclerotherapy, endoscopic variceal ligation, combined above two method, operation, TIPS, and percutaneous transhepatic embolization. Recently, Histoacryl was introduced as a new sclerosant of acute variceal bleeding failed by others. So, we studied 18 cases of acute gastric variceal bleeding uncontrolled by EIS using Ethanolamine oleate. Intravariceal injection of Histoacryl is considered as a safe and effective method for hemostasis of intractable acute gastric variceal bleeding uncontrolled by other sclerosants. But, it is important that endoscopist should be in harmony with assistant at injection of Histoacryl. And its shooting speed should be fast because of clogging within the catheter. It is necessary to do long term study for evaluation of eradication effect in acute gastric variceal bleeding.

7.Removed Intraocular Lenses: A Clinical and Histopathological Study.

Twenty intraocular lenses removed after periods of use ranging from 2 months to 17 years were examined by scanning electron microscopy(SEM). The lenses were retrieved from three sites, anterior chamber, iris-fixed and posterior chamber. The reasons for removal and type of lens were compared to the pathological findings and length of wear. Scanning electron microscopy showed pigment containing epithelial cells, fibroblasts and macrophages on all three categories of lens. The pathological response was not site specific. The presence of these cells on the lens surface strongly suggests a foreign body reaction by the host to the intraocular lens regardless of the duration of implantation or type of intraocular lens.

Celiac disease (CD) is a chronic autoimmune gastrointestinal disorder induced by ingestion of gluten-containing foods.In the past CD was thought prevalent only in Western countries,whereas it has been found common throughout the world and increasing numbers of Chinese patients has been reported recently.However,there are still many deficiencies in management of patients with CD in China and either the diagnosis or the treatment of CD is lacking evidence-based clinical pathway.We report a case of a young woman with CD presenting with chronic diarrhea,abdominal distention,and weight loss,whose outcome was improved by following evidence-based guidelines of medical nutrition therapy.Diagnosis and treatment of the patient was analyzed with review of foreign and domestic literature to provide readers reference of the standardized nutritional intervention of CD.
