1.Investigation of bacteria in brain abscesses

A retrospective study involved 110 patients (72 males, 38 females, aged from 1 - 76 years) who underwent surgery for brain abscesses at ViÖt -§øc Hospital from January 1990 to December 1993. 79 patients with brain abscesses were found bacteria in culture. The bacteria were not identified in 31 patients. Results: Rate of bacterial isolation in brain abscesses is quite high. Among these, streptococci make up 37.7%, staphylococci 23.8%, and anaerobic bacteria 17.5%. Streptococci and staphylococci are cause of more than 90% of secondary brain abscesses due to localized infection. Postoperative antibiotics reduced significantly the bacterial isolation rate, comparison with that rate of 87.5% on patients who have not used antibiotics before operation. Rate of recurrence is 7.6% in the patients who were isolated bacteria and 25.8% in the patients who were not identified bacteria.

2.Results of treatment of brain abcesses by 2 methods: needle aspiration and capsula removal

The records of 75 patients with brain abscesses treated over 7 years were analysed. Congenital cardiopathies were the most frequent causes of abscess. Organisms were identified in 74.07% of the cultures from abcesses speciment. The patients were grouped according to the treatment received: excision (n=42) aspiration (n=33). There were no significant differences is the mortality rates between the two groups. Patients treated by aspiration had a significantly less morbidity than patients treated by excision. Early and aggeressive treatment can result in an excellent outcome in the large majority of patients.

3.Surgical treatment of brain abscess by abscess ablation

An analysis of 26 patients with brain abscesses treated by abscess ablation has shown that the morbidity rate, complications, relapsed brain abscess, repeated surgery and duration of antibiotic using. The bacteria were isolated in 42% of samples. The abscess ablation was indicated for cases of brain abscess with foreign bodies, brain abscess occurred after cerebrocranial surgery or cerebrocranial injuries. The needle aspiration indicated for cases of brain multiabscess, patients with thalamic perception impairment received needle aspiration in the first stage and abscess ablation in the second stage.

4.Diagnosis and treatment of brain multiabscess

A study on the clinical and paraclinical features and results of treatment of brain multiabscess among 190 patients in Vietduc Hospital during 1992-2002 has shown that the brain multiabscess accounted for 30%. This rate did not depend on the species and bacterial resources. The frequent clinical features were headache, vomiting, fever, hemiplexia and leukocytosis. The effective treatment involved the needle aspiration. The removal operation indicated for located abscesses and meninggeal adjacent abscess.

6. MRI and SPECT in evaluating myocardial ischemia and necrosis in patients with coronary artery disease: A comparative study

Objective: To assess the value of MRI and single photon emission computed tomography (SPECT) in detecting myocardial ischemia and necrosis. Methods: Coronary angiography (CAG), MRI and SPECT were performed in 28 patients with coronary artery disease one week before revascularization operation. The stenosis criterion was a more than 70% stenosis by CAG; the sensitivity, specificity and accuracy of both MRI and SPECT were calculated according to the criterion. Results: CAG revealed 39 positive arteries and 45 negative arteries. According to the CAG results, the sensitivity, specificity and accuracy of MRI for detecting myocardial ischemia were 92.31%, 80.00% and 85.71%, respectively; and for detecting myocardial necrosis were 66.67%, 91.11%, and 79.76%, respectively. The sensitivity, specificity and accuracy of SPECT for detecting myocardial ischemia were 94.87%, 82.22% and 88.10%, respectively; and for detecting myocardial necrosis were 61.54%, 86.67%, and 75.00%, respectively. Conclusion: MRI and SPECT have high accuracy in detecting myocardial ischemia and necrosis and the results of the 2 methods accord well with each other.

9.Normal hips and the hips covered with protective brace A finite-element comparative analysis

BACKGROUND: Finite element method has become an important means for biomechanical study against sideways falls because of its unlimited sample size, small experimental error and good repeatability.OBJECTIVE: To establish three-dimensional normal pelvis and brace-covered pelvis models and then to analyze the stress, strain and displacement distribution during sideways fall and to verify the effectiveness of hip brace.METHODS: Digital human Abaqus 6.51 software was used to build three-dimensional normal pelvis and brace-covered pelvis models. The rigid constraint surface was fixed. The whole pelvic model was loaded with 2 m/s loading. The stress, strain and displacement variation with time and contours were detected prior to and after wearing the hip brace. RESULTS AND CONCLUSION: Compared with the normal model, the contact force between the pelvis and the ground, the maximal compression strain of the cancellous bone when the maximal contact force between the pelvis and the ground generated, the maximal strain of the greater trochanter and femoral neck, the maximal von-Mises stress of the greater trochanter and femoral neck, and the mean stress of the greater trochanter and femoral neck were significantly reduced in the brace-covered pelvis model during sideways falls. These findings indicate that the hip brace can play a protective role in the greater trochanter, and effectively reduce the incidence of intertrochanteric fractures during sideways fall.
