1.Bioreactor in bioartificial liver system

OBJECTIVE: To introduce the basic functions, working conditions and requirements of bioreactot, additional, to discuss the research process of bioreactor.METHODS: PubMed database (http://www.ncbi.nlm.nih.) and CNKI database (www.cnki.nat/index.htm) were retrieved by the first author with search terms of "bioartificial liver, bioreactor, membrane material" both in English and Chinese. The time was limited between January 1994 and August 2009. Inclusion criteria: ①literatures closely linked to the article; ②papers published in more authoritative journals in recent years. ③the contents of old or duplicate documents were excluded.RESULTS: Totally 150 documents were seized by primary screen, 88 irrelative papers and 32 duplicate documents were excluded, finally, 30 literature entered further analysis. Bioartificial liver system has become an effective in vitro supportive treatment for hepatic failure patients. Bioreactor is an important ingredient of bioartificial liver, which provided a platform for the hepatocytes growth metabolism, substances exchange and immune isolation. The main cellulose semipermeable bioreactor comprises fiat membrane bioreactor and hollow fiber bioreactor, and the main types of semipermeable membranes contain mixed cellulose ester membrane, cellulose acetate membrane, cuprophan membrane, PVDF membrane, as well as poly(ethylene terephthalate) membrane.CONCLUSION: As a dynamic system, the optimal of control system in bioreactor is conductive to regulating mass transfer and establishing bionic physical or chemical gradients, which can implement the construction of hepatocyte with smallest unit. The selection of membrane is the key link in bioreactor construction of bioartificial liver system
2.Analysis of Antidepressants Consumption in 34 Hospitals of Wuhan During 2011-2013

Objective To provide basis for rational drug use of clinical practice by analysis of antidepressants application in Wuhan. Methods Antidepressants were classified according to the structure and mechanism. By using the analytical method of the defined daily dose (DDD) recommended by WHO,authors statistically analyzed the relative data of antidepressants used in 34 hospitals of Wuhan from 2011 to 2013. Results The consumption of the antidepressants had been increased year by year,selective serotonin reuptake inhibitor (SSRIs) and serotonin noradrenergic reuptake inhibitors (SNaRIs) were widely used and presented higher proportion than others in the consumption of antidepressants,the average amount of the sales was 50.49% and 21.81%,respectively.The application of tricyclic antidepressant (TCAs) decreased in Wuhan,the amount of sales for 3 years accounted for 0.56%,0.37% and 0.15%. Conclusion SSRIs and SNaRIs have already become the first-choice drugs in clinic practice because of good efficacy and high safety,other new types of antidepressants have a great potential for development,TCAs are fewly used now.
3.Three-dimensional virtual simulation of cervical deep vein puncturation

BACKGROUND:Three-dimensional visual model of cervical deep vein can be used to virtual simulation of puncture. The studies on elevating deep vein puncture in the clinic are stil in the stage of exploration.
OBJECTIVE:To find the application of three-dimensional virtual puncturation simulation to cervical deep vein puncturation.
METHODS:CT cross-sectional images were obtained from healthy volunteers. Mimics software was used to semi-automatical y cut and reconstruct of various tissue of the neck. Three-dimensional model revealed cervical deep veins and its surrounding anatomic structure. Cervical deep vein puncturation was simulated, including internal jugular vein, supraclavicular vein and subclavian vein.
RESULTS AND CONCLUSION:Three kinds of cervical deep vein puncturation were successful y simulated, showing three-dimensional adjacent relation of the virtual pin with surrounding anatomic structure. Safe angle, depth and optimal path of the pin were measured. A three-dimensional virtual puncturation simulation can provide visualized morphologic data for cervical deep vein puncturation.
4.Comparison of different fluorescent quantitation polymerase chain reaction for genes detection of Streptococcus pneumonia

Objective To compare the sensitivity and specificity of different fluorescent quantitation polymerase chain reaction(PCR) for genes detection of Streptococcus pneumonia.Methods By designing five sets of fluorescent quantitation PCR primers targeting different genes of Streptococcus pneumonia (lytA,ply,spn9802,psaA,cspA),the sensitivity and specificity of fluorescent quantitation PCR was determined through detecting 37 strains of Streptococcus pneumonia and 28 strains of non-Streptococcus pneumonia,as well as 80 sputum specimens from suspected pneumonia cases.Results These five primers had excellent sensitivity with 90% amplification efficiency through analysis of the standard curve.The change of Ct value was less than 0.5,indicating that these five primers have good stability.By detecting 37 strains of Streptococcus pneumonia and 28 strains of non-Streptococcus pneumonia,it showed that the specificity of lytA,ply and spn9802 was better than the other two primers (positive rate and negative rate was all 100%).But the detection limit of lytA,ply was one order of magnitude than spn9802 (101 CFU/ml vs.102 CFU/ml).The specificity of psaA and cspA was worse [negative rate of psaA was 89% (25/28),positive rate of cspA was 97% (36/37)].By detecting 80 sputum specimens from suspected pneumonia cases,it showed that the specificity of fluorescent quantitation PCR were better than the bacterial culture.The specificity of lytA and ply was the best two [positive rate:92.50%(74/80),90.00%(72/80)],and next was spn9802 [86.25%(69/80)],the specificity ofpsaA and cspA was the worst two for detection [76.25%(61/80) and 78.75%(63/80)].Conclusions The sensitivity and specificity of fluorescent quantitation PCR genes used in clinic for detecting Streptococcus pneumonia is different.LytA and ply are the best primers in specificity,next is spn9802,and psaA and cspA are the worst primers for in specificity detection.
5.Effects of dexmedetomidine on activity of c-Jun N-terminal kinase during cerebral ischemia-reperfusion in rats

Objective To evaluate the effects of dexmedetomidine on the activity of c-Jun N-terminal kinase (JNK) during cerebral ischemia-reperfusion (I/R) in rats.Methods Eighty-one pathogen-free male Sprague-Dawley rats,aged 8 weeks,weighing 180-220 g,were randomly divided into 3 groups (n=27 each) using a random number table:sham operation group (group S);cerebral I/R group (group CI/R);dexmedetomidine group (group Dex).The rats were anesthetized with intraperitoneal 10% chloral hydrate 300 mg/kg.Cerebral ischemia was induced by occlusion of the middle cerebral artery for 2 h followed by 24 h of reperfusion in CI/R and Dex groups.The middle cerebral artery was only exposed but not occluded in group S.Dexmedetomidine 3 μg/kg was injected via the tail vein immediately before ischemia followed by infusion at a rate of 3 μg · kg-1 · h-1until 24 h of reperfusion in group Dex,while the equal volume of normal saline was given in S and CI/R groups.The rats were sacrificed at 24 h of reperfusion,and their brains were removed for determination of cerebral infarct size (by TTC staining),brain water content ((wet weight-dry weight)/wet weight × 100%),cell apoptosis (by TUNEL) and expression of phosphorylated JNK (p-JNK) protein (by Western blot analysis).Apoptotic index was calculated.Results Compared with group S,the brain water content,apoptotic index and cerebral infarct size were significantly increased,and the expression of p-JNK was up-regulated in CI/R and Dex groups.Compared with group CI/R,the brain water content,apoptotic index and cerebral infarct size were significantly decreased,and the expression of p-JNK was down-regulated in group Dex.Conclusion Dexmedetomidine reduces cerebral I/R injury through decreasing the activity of JNK and inhibiting cell apoptosis in rats.
6.Application of gonadotropin-releasing hormone analogues in children with central precocious puberty

Gonadotropin-releasing hormone (GnRH) dependent precocious puberty/central precocious puberty (GDPP/CPP) is one of the common diseases of the pediatric endocrine system.CPP is mainly treated with gonadotropin-releasing hormone analogues (GnRHa) internationally.They slow the progression of bone age and improve adult height in children with CPP by inhibiting the activity of the hypothalamic-pituitary-gonadal axis and the secretion of sex hormones.In clinical practice, the populations who benefit from GnRHa treatment and the best GnRHa treatment plan still need to be investigated, and the long-term efficacy and safety evidence of GnRHa should be further improved.
7.Clinicopathological significance of combined immunohistochemical analysis of PTEN, p53 and EGFR in breast carcinoma

Purpose To study clinical pathological significance of the expression of PTEN, p53 and epidermal growth factor receptor ( EGFR) in breast carcinoma and their correlation. Methods Immunohistochemical MaxVision method was used to detect the expres-sion of PTEN, p53, EGFR in 209 cases of infiltrating ductal carcinoma of breast and 40 cases of benign breast diseases. Results The over-expression rate of PTEN, p53 and EGFR protein in breast carcinoma was 57. 9%, 55. 0% and 38. 3% respectively, which was significantly different from those in benign breast diseases (P<0. 05). The expression of PTEN, p53 and EGFR in breast cancer was correlated with tumor size, histological grade, lymph node metastasis, ER status, PR status and molecular subtype (P<0. 05). There was an association between PTEN or p53 and TNM stage, PTEN or EGFR and HER-2 status. There was a negative correlation in the protein expression of PTEN vs EGFR or PTEN vs p53 (P<0. 01). There was a positive correlation between EGFR and p53 protein ex-pression (P=0. 000). The expression of PTEN or EGFR was correlated with the prognosis of breast cancer, but not independent prog-nostic factors. The survival rate of patients with PTEN- /p53 + /EGFR+ was lower than those with other combined expression of PTEN/p53/EGFR (P=0. 008). Conclusions Low or loss expression of PTEN, p53 mutation and EGFR over-expression may be coordinate-ly involved in the pathogenesis and progression of breast cancer. The combined detection of these markers may play an important role in making treatment and indicating prognosis for breast cancer patients.

9.MELD-Na score combined with serum cholesterol and endotoxin concentration on the evaluation of the decompensated liver cirrhosis prognosis

Objective To evaluate the prognosis of the decompensated liver cirrhosis by means of MELD-Na score combined with serum cholesterol and endotoxin concentration.MethodsData of 156 hospitalized patients with decompensated liver cirrhosis was retrospectively analyzed.Patients were divided into survival and death group according to follow-ups at 6 months,12 months and 24 months.MELD-Na score was made among 156 patients by detecting relevant indicators.Serum cholesterol and cholesterol levels were measured,too.The relationship between decompensated liver cirrhosis prognosis and MELD-Na score combined with serum cholesterol and endotoxin concentration was analyzed.ResultsNinteen patients died at the follow-up for 6 months.Fifty nine patients died at the followe-up for 12 months.Seventy seven patients died at the follow-up for 24 months.There was significant difference on MELD-Na score,endotoxin concentration and serum cholesterol between the survival group and the death group (t =-9.68,-9.22,11.4,-4.65,-19.60,16.20,-20.0,-18.7,17.3,respectively,P <0.05).The best critical value of MELD-Na score to predicate death in patients with decompensated liver cirrhosis was 32 points.The risk of death would rise if MELD-Na score increased.The best critical value of plasma endotoxin to predicate death in patients with decompensated liver cirrhosis was ≥ 12 ng/L.The best threshold value of serum cholesterol to predict death in patients with decompensated liver cirrhosis was ≤ 1.70 mmol/L.ConclusionMELD-Na score,serum cholesterol and serum endotoxin were of higher prognostic value to judge the prognosis of patients with decompensated liver cirrhosis.
10.3D Reconstruction for Medical Image Based on ITK and OpenGL

Obiective To develop a new medical image processing platform based on ITK and OpenGL. Methods This platform was designed by VC ++ 6.0 and MFC class library. ITK class library was wsed to fulfill reading, writing,segmentation of images and iso-surface extraction. OpenGL was wsed to realize 3D surface rendering. Results The new 3D-medical imaging reconstruction system was designed. Conclusion This system not only can provide graphic interface which can not be realized by using ITK class library, but also enhance the ability of three-dimension visualization by using OpenGL.
