1.Combined surgery of facial fat tissue remodel with SMAS-suspended rhytidectomy

ObjectiveTo evaluate the effect of facial fat tissue grafting and remodeling with SMAS suspension in facial rejuvenation.MethodsThe treatment process of 12 patients with facial fat tissue grafting and SMAS-suspended rhytidectomy were reviewed retrospectively,the surgical operative procedure and treatment results of facial liposuction and autologous fat grafting with SMAS-suspended rhytidectomy were analyzed and evaluated.Results12 patients underwent facial liposuction,SMAS-suspended rhytidectomy and autologous free fat tissue grafting and remodeling.All the followed-up cases obtained good results without complications.ConclusionsCombined surgery of facial fat tissue remodel with SMAS-suspended rhytidectomy not only corrects the soft tissue laxity,but also modifies the faical volume loss.It solves the aging problems in different angles through soft tissue lift and volume restoration.It is a relatively ideal surgical method of facialplasty for those aged patients.
2.Countermeasure of Orthopedic Surgery Patients Infected with Pseudomonas aeruginosa

OBJECTIVE Based on the situation of orthopedic surgery patients infected with Pseudomonas aeruginosa,to choose the reasonable treatment and apply the best surgery chance.METHODS The wound secretion from the different orthopedic surgery patients with different infection time was collected,cultured and examined the Pseudomonas aeruginosa,in order to select the antibiotic treatment according to the drug sensitive tests.RESULTS From 252 cases monitored there were 53 patients with positive P.aeruginosa strains,24 patients(45.3%) of whom were cured aften treatment,25 patients(47.2%) were ameliorated,3(5.7%) were invalid and 1(1.9%) was dead.CONCLUSIONS The patients after orthopedic surgery could have the P.aeruginosa infection,the reasonable antibiotic treatment is the guarantees for the best surgery.
3.Indication and clinical analysis of autogenous fat granule replant mammaplasty

Objective To explore the methodology of the preventing the absorption after autog-enous fat granule mammaplasty, and the main point of the operative methods.. Methods The clinical data were reviewed in 30 cases of patients with liposuction combined with fat-grafting injection for breast mammaplasty in the past four years. The tumescent technique was used in liposuction and the volume of fat tissue aspirated was between 500 ml and 3250 ml. Using appropriate way we prepared pure fat granules and then evenly injected into the layer beneath the pectoralis major muscle, submam-mary layer and subcutaneous layer of the breast, respectively. Results The effect of the method was satisfying because sucking area was significantly decreased. From six months to 10 months follow-up, grafts showed no obvious absorption. The breast looked symmetry. Conclusion Fat grafting injection is a practicable method for breast mammaplasty. The key point is to use appropriate way to get pure fat granule and to prevent infection. It deserves to be recommended.
4.Surgical treatment of eyelid divided nevus.

OBJECTIVETo investigate the surgical techniques and therapeutic effect of eyelid divided nevus.
METHODSFrom January 2000 to January 2014, 27 cases with 27 eyelid divided nevi were treated by staged excision (2 cases), or full-thickness skin graft (20 cases) or frontal and facial expanded flaps (3 cases), or combined expanded flaps with skin grafts (2 cases) for large lesions. One case with skin graft underwent secondary treatment with expanded flap due to obvious scar.
RESULTSExcept for one case with residue lesion (0. 5 cm x 0. 5 cm), all the other cases underwent successful treatment with primary healing. All the patients were followed up for 3-48 months (average, 7. 4 months). Except for one case with secondary expanded flap treatment, all the other patients were satisfied with aesthetic and functional results. No occurrence happened.
CONCLUSIONSStaged excision and full-thickness skin grafts are simple and effective method for eyelid divided nevus. For large lesions, expanded flap, or combined with skin graft should be considered.

5.Reconstruction of the lower face and the anterior neck with a bipedicled expanded flap.

OBJECTIVETo explain a method for lower face and anterior neck reconstruction.
METHODSTissue expanders were implanted subcutaneously beside the defect. When the skin was properly expanded, the bipedicled expanded skin flap was created and transferred to the lower face and the anterior neck. If the defect can not be repaired by one stage, the expanded skin was re-expanded for the rest.
RESULTS11 patients have been treated with this method since 1990. All flaps survived and all the defects have been successfully repaired with satisfactory results.
CONCLUSIONThe bipedicled expanded flap, which provided good blood supply, improved the survival rate of the flap and reduced the incision scar on the face, is an effective surgical procedure.

6.Experimental study on enhancement of tissue expansion with papaverine cream.

OBJECTIVETo evaluate the effect of external use of papaverine cream on enhancement of tissue expansion.
METHODS8 mini pigs were randomly divided into 4 groups. During inflation, papaverine was given externally in group A, through a delivery system in group B, though intramuscular injection in group C. Papaverine was not given in group D. The time of inflation, the expansion rate, the surviving length of expanded flaps and the papaverine concentration in the fibrous capsule in each group were examined. Histologic and electroscope examinations of the expanded tissue were made.
RESULTSThe average inflation time was 28.9 +/- 4.5 days in group A, 34.0 +/- 2.6 days in group B, 37.6 +/- 4.8 days in group C and 38.5 +/- 3.5 days in group D. There was significant difference between each group(P < 0.05). The average surviving length of expanded flaps in group A was 13.67 +/- 1.28 cm, 11.07 +/- 0.88 cm in group B, 10.79 +/- 0.49 cm in group C, and 9.49 +/- 0.77 cm in group D. There was significant difference between each group (P < 0.01). Histologic examinations of the expanded tissue suggested that there were no significant differences in thickness of epidermis, derma and fibrous capsule among the four groups. Electroscope examinations of the myofibroblast in fibrous capsule showed that there were smaller nuclei, fewer microfilaments and fewer mitochondrias and rougher endoplastic reticulume in group A than those in other groups. The concentration of papaverine in fibrous capsule of group A was 3.5 +/- 1.2 micrograms/g. Papaverine was detected neither in the skin of other groups nor in the blood of all groups.
CONCLUSIONS1. Papaverine cream can permeate through skin and maintain high concentration and continuous effect in local tissue. 2. External use of papaverine cream can inhabit the function of myofibroblast in fibrous capsule, increase inflation rate, accelerate expansion process, and improve microcirculation of the expanded skin with a result of prolongation of surviving length of expanded flaps. 3. The technique of enhancing tissue expansion with external use of papaverine has the advantages of convenient delivery, cheapness, no more injury and infection, and no side effects etc. It is a safe, effective, simple, and reliable method for accelerating tissue expansion.

7.Clinical efficacy of modified central short incision double eyelid surgery

Objective:To investigate the clinical efficacy of the modified central short incision minimally invasive double eyelid surgery.Methods:A retrospective analysis was conducted on patients who underwent the modified procedure at Chengdu Badachu Medical Aesthetic Hospital from September 2018 to January 2023. The modification involved extending the traditional central short incision to retain the outer fascial membrane of the orbicularis oculi muscle through shallow layer dissection. At five marked points, the skin and muscle layers of both the upper and lower eyelids were fixed to the pre-tarsal fascia. The upper eyelid orbital septum was also repositioned, postoperative evaluations were performed by doctors, scoring the upper eyelid (0-8 points, with higher scores indicating greater satisfaction) and the incision was intermittently sutured. Patient satisfaction was investigated 3 months after surgery by outpatient questionnaires or telephone follow-up.Results:Ninety-four patients (147 eyelids, including 31 with ptosis) were included. There were 28 males (49 eyelids) and 66 females (98 eyelids), aged 20-41 years, with an average age of 29.7 years. Follow-up ranged from 6 to 36 months, averaging 14 months. All incisions healed without infection, scar hyperplasia, or corneal injuries. Two cases required revision surgery for shallow folds. The remaining patients had smooth, full eyelid folds with hidden, flat incisions. Doctors evaluated the postoperative effect: 141 cases were satisfied, 4 cases were general and 2 cases were dissatisfied. Patient satisfaction: 142 cases were satisfied and above, 3 cases were generally satisfied and 2 cases were dissatisfied.Conclusion:The modified central short incision minimally invasive double eyelid surgery fully exposes the surgical field, corrects ptosis, expands indications and is simple, minimally traumatic, quick to recover from, with minimal scarring and natural result.It is an optimal method for minimally invasive double eyelid surgery.

Objective To explore the feasibility of the extensive application of expanded forehead flaps in faciocervicalreconstructionin differenct aesthetic units and the flap selection based on the location of defect..Methods 251 cases using expanded forehead flaps for cervicofacial reconstruction in our center from 2000 to 2016 were systematically reviewed .Expanded frontal flap was categorized according to its blood supply .Flap application was enumerated based on the aesthetic units in face and neck .Results Considering blood supply of the flaps , expanded forehead flaps were categorized into four types .Type I was a pre-expanded local flap , which was used for repairing defects of the partial forehead unit , subunits of the periorbital unit , or partial involvement of the two adjacent units .Type II was an expanded paramedian forehead flap, which was used for resurfacing the nose , orbital unit, upper cheek unit, and partial involvement of these adjacent units .Type III was a bilateral pedicled expandedforehead flap for the reconstruction of the lower face and anterior neck .Type IV, a unilateral pedicled expanded forehead flap , which is based on the superficial temporal vessels , was used to reconstruct the ipsilateral part of the middle face.Conclusions Expanded forehead flaps could be used for the reconstruction of different faciocervical units.The selection of these flaps can be designed preoperatively based on the location and size of the defects.
